1.Complications and preventions of pediatric fiberoptic bronchoscopy

With the wide applications of flexible bronchoscopy in pediatric clinical affairs,the rate of adverse reactions and complications have been increased synchronously.In order to safely make correct diagnosis and improve the therapeutic effects,lower the incidence of complications,it is very important to be familiar with various complications and the ways of management about them as well as indications,contraindications and operating procedures.It's also necessary to negotiate with children's parents on health and fortune including anaesthesia so as to decrease the risks of unexpected affairs and adverse effects.
2. Renal papillary calcareous deposits in patients with calcium oxalate kidney stones: Characteristics and formation mechanism

Objective: To investigate the characteristics of renal papillary calcareous deposits and their role in the formation of calcium oxalate kidney stone, and to explore the formation mechanism of renal calcareous deposits. Methods: A total of 21 patients with calcium oxalate kidney stones were included in the present study. The components of the calculi were detected by Fourier-transform infrared spectroscopy. The calcific plaques were observed and the renal papilla biopsy specimens were obtained during PCNL. The specimens were then subjected to alizarin bordeaux staining and light microscopic examination. The expression of osteopontin, BMP-2, and type II collagen in the kidneys was examined by immunohistochemistry. Seven resected renal specimens from patients with non-urolithiasis served as control. Results: Renal papillary calcific plaques were found in all the 21 patients with renal calculi. Local calcareous deposits were found in the renal interstitium around tubular basement membrane and extended toward the mucous membrane in the renal papillas. Microscopically, once the calcareous deposits pierced into the collection system, tiny stones could be seen growing on the deposits. Immunohistochemistristry showed the renal tissues of renal stone patients and normal renal tissues both expressed osteopontin, but not BMP-2 or type II collagen. Conclusion: Renal papillary calcareous deposits may be one of the initiating nidi for kidney stone formation and they may not be an osteoblastic reaction resembling arteriosteogenesis.
3.Analysis of the distribution and drug resistance of pathogens in patients with acute exacerbation of chronic obstructive pulmonary disease

Objective To observe the distribution and drug resistance of pathogens cultured from the sputum of hospitalized patients with lower respiratory infection in acute exacerbation of chronic obstructive pulmonary disease (AECOPD) .Methods To i‐dentify the germiculture and test the drug susceptibility of the sputum or respiratory secretion isolated from the bronchial brush of 262 hospitalized AECOPD patients in People′s Hospital of Jiangxi Province from Janurary 2013 to December 2014 and analyze the results .Results Among all the AECOPD patients ,215 cases with positive sputum culture ,281 sputum pathogens were isolated . Gram‐negative bacilli were found in 190(67 .6% ) .Gram‐positive aureus were detected in 76(27 .1% ) .Fungus pathogens occurred in 15(5 .3% ) .The top six pathogenic bacteria were acinetobacter baumannii ,escherichia coli ,klebsiella pneumonia ,pseudomonas aeruginosa ,staphylococcus aureus ,streptococcus pneumonia .Drug susceptibility results showed that the drug resistance of acineto‐bacter baumannii was the strongest .Except that the drug resistance rate of cefoperazone/sulbactam and levofloxacin were less than 50 .0% ,the others were no less than 75 .0% .The drug resistance rate of escherichia coli and klebsiella pneumoniae to ampicillin , ampicillin sulbactam ,cefazolin ,ceftriaxone ,cefotetan ,gentamycin ,ofloxacin ,ciprofloxacin ,and compound sulfamethoxazole trime‐thoprim were no less than 70 .0% .The drug resistance rate of staphylococcus aureus to penicillin G ,oxacillin ,erythromycin ,clinda‐mycin were 100% .The drug resistance rate of streptococcus pneumoniae to erythromycin ,clindamycin ,tetracycline ,sulfamethox‐azole trimethoprim were greater than 75 .0% .Conclusion Gram‐negative bacilli are the main pathogenic bacterium in the AECOPD patients with lower respiratory infection .The key of treatment is to pay more attention to the bacterial culture and drug sensitive test ,use antibiotics reasonably according to the results of drug sensitive experiment .
4.The Curative Effect of MIPPO Technology United LCP Treatment for Humeral Fractures

Objective To discuss the clinical methods and curative effect of minimally invasive percutaneous plate osteosynthesis ( MIPPO) technique applied in the treatment of humeral shaft fractures. Methods A retrospective analysis was applied on 14 patients with humeral fracture underwent the MIPPO operation in our department from April 2012 to March 2013. There were 8 males and 6 females, with their ages ranging from 28 to 64 years. Results Fourteen cases were followed up for 2 months to 12 months (an average of 6 months) . Their incisions got primary healing. The fracture segments got satisfactory reduction with good apposition and alignment radiologically, and the radial never function recovered well. UCLA score concluded as excellent in 13 cases and good in 1 case. Conclusion MIPPO is a safe and effective treatment for the humeral fracture with the benefits of less invasion, fewer complications and higher union rate.
5.Expression of hypoxia-inducible factor-1?,vascular endothelial growth factor and sFlt-1 in preeclampsia placenta

Objective To investigate the expression and correlation of hypoxia-inducible factor-1?(HIF-1?),vascular endothelial growth factor(VEGF)and sFlt-1 in the preeclampsia placenta,and discuss their significance in the pathogenesis of preeclampsia.Methods Placentas were collected from 20 pregnant women with preeclampsia as study group and 15 normal pregnant women as control group.The expressions of HIF-1?,VEGF and sFlt-1 protein were semi-quantitatively analyzed with immunohistochemical assay and mRNA level was determined using reverse transcription polymerasc chain reaction(RT-PCR)technique. Results(1)the expression of HIF-1?and sFlt-1 protein in preeclampsia group obviously increased.Strong (+++)positive expression was observed in 9 and 11 cases respectively,significantly higher than in control group(2 and 3 cases)(P<0.05),however,VEGF expression obviously reduced in preeclampsia group(P<0.01).(2)the level of HIF-1?and sFlt-1 mRNA in preeclamptic placenta was 0.604?0.013, 0.898?0.041,significantly higher than 0.208?0.007 and 0.559?0.244 in normal placenta(P<0.05). Although the level of VEGF mRNA increased in preeclampsia placenta,it was not significantly different from that in normal placenta(P>0.05).The ratio of VEGF mRNA/sFlt-1 mRNA obviously reduced in preeclampsia group and was significantly lower than in control group(P<0.05).(3)in preeclampsia group,HIF-1?mRNA expression was positively correlated with the expression of sFlt-1 mRNA(r=0.577, P<0.05),and negatively correlated with the ratio of VEGF mRNA/sFlt-1 mRNA(r=-0.376,P<0.05).Conclusion Abnormal high HIF-1?expression in preeclampsia placenta indicates that HIF-1?might play an important role in the pathogenesis of preeclampsia,possibly through affecting the cytotrophoblastic invasion and placental vascular reconstruction via the modulation of VEGF and sFlt-1 gene transcription.
6.Significance of changes in inositol requiring enzyme 1, apoptosis signal regulating kinase 1 and c-Jun N-terminal kinase levels in peripheral blood CD4+ T cells of children with acute paraquat poisoning

Objective To explore the changes in inositol requiring enzyme 1 (IRE1),apoptosis signal regulating kinase 1 (ASK1) and c-Jun N-terminal kinase (JNK) mRNA levels in peripheral blood CD4+ T cells of children with acute paraquat (PQ) poisoning.Methods Blood samples of 30 cases of acute PQ poisoning (PQ group),who visited Tianjin Children's Hospital from June 2014 to June 2016,with 18 male and 12 female,aged from 2 to 14 years old,were collected,and the clinical and laboratory data were documented.Peripheral venous blood samples were collected after paraquat was taken.Thirty healthy children at the same age and of the same sex were selected as a healthy control group,18 male and 12 female,aged from 2 to 14 years old.CD4+ T cells in the peripheral blood were separated,and IRE1,ASK1 and JNK mRNA levels in peripheral blood CD4+ T cells were measured by real time polymerase chain reaction (Real-time PCR) method.Specificity of PCR products was validated through agarose gel electrophoresis.The data were statistically analyzed by SPSS 13.0 software.Results All of the 30 children had mucosal lesions,nausea,vomiting and abdomen pain,19 cases with oliguria and anuria,16 cases with alimentary tract bleeding,12 cases with headache and dizziness,11 cases with short of breath,dyspnea and difficult breathing,8 cases with convulsion,5 cases with jaundice.The IRE1,ASK1 and JNK mRNA levels in PQ group were significantly higher than those in healthy control group (1.70 ± 0.16 vs.1.02 ± 0.18,3.56 ± 0.85 vs.1.05 ± 0.31,5.22 ± 0.87 vs.1.01 ± 0.33,t =15.26,15.21,24.78,all P < 0.01).Conclusions PQ increased the expressions of IRE1,ASK1 and JNK in peripheral blood CD4+ T cells,which may be related to PQ-induced oxidative stress and immune activation and lead to a complex cytokine network via endoplasmic reticulum stress and CD4+ T cell apoptosis and then results in the occurrence and development of multiple organ failure.
7.Determination of Cinnamaldehyde in Zegui Longshuang Capsules by HPLC

Objective To establish a method for determining cinnamaldehyde in Zegui Longshuang Capsules. Methods A C18 column was used with Acetonitrile-Water (30∶70) as the mobile phase,and the detection wavelength was at 290 nm. Results The calibration curve of cinnamaldehyde was linear from 0.010 ?g to 0.507 ?g,and r=0.999 9. The average recovery was 98.55 %with RSD being 2.06 %. Conclusion This method is proved to be accurate and reliable,and can be used to control the quality of Zegui Longshuang Capsules.
8.Delay in Diagnosis of Spontaneous Dual Arteriovenous Fistulas : Correlative Factors and Influence on Outcome

[Objective] To study the factors relative to the delayed diagnosis of spontaneous dual arteriovenous fistulas (DAVF) and its influence on the prognosis.[Methods] We included 102 continuous patients diagnosed DAVF in the First Affiliated Hospital of Sun Yat-sen University,and analyzed the correlative factors and impact on outcome of diagnostic delay.Outcome was whether symptoms were non-improvement,improvement or restoration at discharge.[Results] Median delay from onset to diagnosis was 3 months (interquartile range,1 to 6).Compared with patients diagnosed carlier(diagnose time≤3 months),patients diagnosed later (diagnose time > 3 months) had a lower frequency of headache (P =0.012),ptosis (P =0.035) and parenchymal lesions (P =0.001),a higher frequency of conjunctival congestion (P =0.004),tinnitus (P =0.021),visual dysfunction (P < 0.001),isolated visual dysfunction (P =0.007) and delayed imaging scan (P < 0.001),a higher frequency of endovascular treatment,and a lower frequency of improvement or restoration at discharge (P =0.033),in which patients with visual dysfunction had a lower frequency of improvement or restoration than those without visual dysfunction (P =0.023).Compared to those with visual dysfunction and other symptoms,patients with isolated visual dysfunction had a higher frequency of onset with paroxysmal blurring or blinding (P < 0.001),two eyes involved (P < 0.001) and more severe visual loss (P =0.057),a higher frequency of draining into transversesigmoid sinus (P < 0.001) instead of cavernous sinus (P < 0.001),and suffered intracranial hypertension all (median intracranial pressure,405 mmH2O;interquartile range,370 ~ 512 mmH2O).However,no statistically significant differences were found in the frequency of improvement or restoration at discharge between two groups (P =0.739).[Conclusion] Diagnostic delay was considerable in this cohort and was associated with outcome,especially in patients with visual dysfunction.
9.Progress in the study of tubulin inhibitors.

Microtubule is one of the key components of the cytoskeleton and plays an important role in the maintenance of cell shape and the process of signal transduction and mitosis. Due to the extreme importance of microtubule in the process of mitosis, tubulin becomes one of the most important targets for development of new anticancer drugs and tubulin inhibitors are used for the treatment of cancer nowadays. These inhibitors have antitumor activity by inhibiting or promoting the assembly of tubulin to microtubules and interfering the process of cell mitosis. This review summarized the research progress of the tubulin inhibitors, especially the introduction of the tubulin inhibitors of pharmacological activities and the progress of clinical research. Also, the development trend of these inhibitors is discussed.
