4.Relative factors in home safety evaluated by multilevel statistical models

Objective To understand the situations of home establishments and the related domestic behaviors that causing injuries. Methods A total of 9760 families with regular residents in a community in Shanghai were investigated. Questionnaire was designed bascd on the Guidelines for Conducting Community Surveys on Injuries and Violence and International Classification of External Causes of Injuries. Results Inside the home settings, relative factors were found on issues as fire protection and using of electricity. In terms of household settings, 14.85% of the families had smoke alarm systems in the kitchen; 40.75% had no windows set for emergence. 50% to 70% of the residents had the idea of safe behaviors, including 35.93% of the families stored cleaning products,other chemicals or medical substances in alternative containers, such as used drinking bottle. Only 1.81% of the people being investigated thought that home was also a high risk place where injury might occur and it was placed number 9 in a multiple choice questionnaire. Data from the multiple level model analysis showed that factors as number of family members, space, education, profession etc. were closely related to the situation of home settings and their resident's behaviors. Conclusion Many injury related factors were found related to home establishments and people' s daily behaviors at home which called for interventions to be taken.

6.Identification of melatonin receptor in human embryonic peripheral tissue

Objective: To determine protein binding characteri stic and signal transmission pathway of melatonin(Mel) receptor(MR) in human e mbryonic peripheral organ tissues. Methods: MR was measured by radio ligand-binding assay and the effect of GTPγS on melatonin specific bindi ng was studied. Results: Mel specific binding sites were det ermined in 16 kinds of human embryonic tissue and this binding could be inhibit ed by GTPγS, supporting the theory that MR is coupled to inhibitory G-proteins system. Conclusion: MR is measured in human embryo tissue, the se results provide experimental data for elucidating the mechanism of the effect of Mel.
7.The clinicopathological features of early renal amyloidosis.

OBJECTIVETo investigate the clinicopathological manifestations of early renal amyloidosis (AL) and its diagnostic criteria.
METHODSFifteen cases with early renal amyloidosis admitted from 1994 to 2001 were collected from the hospital, and their clinical and pathological features were reviewed. Of them, the initial diagnoses were not made by depending findings from the light microscopy (LM) and immunofluorescense (IF), but confirmed by electron microscopy (EM) afterwards. Immuno-electron microscopy (IEM) were applied for amyloidosis typing.
RESULTSMost patients of early renal AL were in the middle to old age. Nephrotic syndrome was the most prominent symptoms and signs accompanying with rare microscopic hematuria and hypertension. Most of them had a normal renal function. Pathological examinations of renal biopsies using LM and IF showed mild mesangial proliferation and mild thickening of glomerular basement membrane (GBM). Immunoglobulins and complements were negative or only scanty in certain cases, but in all cases there was a light chain protein deposition homogeously. There were 4 cases of minimal change glomerulopathy, 5 cases of mild mesangial proliferative glomerulonephritis, 5 cases of stage I membranous nephropathy, and 1 case of cast nephropathy diagnosed with LM. The amyloid fibrils (diameter 8 - 10 nm) were randomly distributed in the mesangium, along GBM and at the arteriolar wall under EM. Additionally, Congo red staining was positive. IEM demonstrated that amyloid fibrils labeled with colloid gold was combined with a kind of light chain protein which was confirmed as the light chain type of AL.
CONCLUSIONSThe diagnosis of early renal AL was occasionally neglected by depending only findings of LM and LF. However, special amyloid fibrils can be detected using EM. EM observation is an indispensable technique for the diagnosis of early renal AL and the typing of AL may further be determined by using IEM.

8.Study of clinical outcome and analysis of prognosis related factor in children with acute myeloid leukemia.

OBJECTIVETo analyse the clinical outcome and the prognostic factor of childhood acute myeloid leukemia (AML).
METHODSDisease-free survival (DFS), event-free survival (EFS) and overall survival (OS) rates were estimated by Kaplan-Meier method and prognostic factors were evaluated by Cox regression with SPSS in 141 childhood AML in our hospital from August 1995 to July 2004. The patients were divided into 2 groups: acute promyelocytic leukemia (APL) as group A and AML other than APL as group B.
RESULTSOf the 90 group B patients, 54.4% (49/90) achieved complete remission (CR) after one course chemotherapy , with a total CR rate of 76.7%. The cumulative 5 year DFS and OS rate for group B patients were (28.4 +/- 9.0)% and (35.5 +/- 6.3)%, the 51 group A patients were (94.3 +/- 4.0)% and (81.4 +/- 5.7)%, and for total 141 AML patients were (56.9 +/- 6.3)% and (53.3 +/- 4.8)% respectively. Multivariate analysis demonstrated that higher bone marrow blast cell percentage at diagnosis, CR after more than one course of chemotherapy and less than six courses of consolidation chemotherapy were risk prognostic factors in childhood AML other than APL (P < 0.05).
CONCLUSIONThe prognosis of childhood APL is better, while of childhood t(8;21) AML is no better than other FAB subtypes.

9.Investigation of GSTT1, GSTM1 and NQO1 genes polymorphisms in myelodysplastic syndromes.

OBJECTIVETo investigate the relationship between GSTM1, GSTT1 and NQO1(C609T) genotypes and myelodysplastic syndromes (MDS) susceptibility and chromosome abnormalities.
METHODSGSTT1, GSTM1 and NQO1(C609T) genotypes were detected in 52 MDS patients and 241 unrelated controls by PCR or PCR-RFLP.
RESULTSThe incidence of GSTT1 and GSTM1 null genotype was significantly increased in MDS patients as compared with controls (P = 0.001 and P < 0.001, respectively). In individuals with GSTT1 and GSTM1 null genotype, the odds ratios for MDS risk were elevated to 2.873 (95% CI: 1.491-5.537) and 3.591 (95% CI: 1.717-7.508), respectively. A significantly increased frequency of GSTT(1) null genotype among MDS patients with normal karyotype and increased frequency of GSTM1 null genotype among MDS patients with chromosome abnormalities were found as compared to controls (OR = 5.336, P = 0.005 and P = 0.003, OR = 3.740, respectively). There was no difference in the incidence of NQO1(C609T) genotypes between MDS patients and controls.
CONCLUSIONDetermination of the GSTM1 and GSTT1 genotypes may be used as a stratification marker to predicate high-risk individuals for MDS.

10.Morphological study and Ca/P ratio analysis of Er, Cr:YSGG laser irradiation on periodontal diseased root surfaces.

OBJECTIVETo observe the morphological transformation and Ca/P changes of the periodontally disease root surfaces irradiated by Er, Gr:YSGG Laser.
METHODS18 periodontally diseased teeth and 6 wisdom teeth were collected in vitro. After 18 periodontally diseased teeth were planed, 12 teeth were randomly selected as the laser treatment group, the others as the acid treatment group. The 6 wisdom teeth were selected as the healthy control. Then the evaluation for root surfaces morphological transformation was conducted by SEM. An energy spectrum analyzer was used to analyze the Ca/P ratio of root surfaces.
RESULTSThe root surfaces were clean and even in the laser treatment group. Smear layer could also be effectively eliminated in the laser treatment group and the acid treatment group, but the SEM results were different. Atom content analysis showed that the Ca/P ratio of the laser treatment group and the acid treatment group had no distinct difference.
CONCLUSIONIt is effective to remove smear layers and infected cementum of surface layers with Er, Cr:YSGG irradiating on the planed periodontal root surfaces.
