4.Management of traumatic periprosthetic femoral fractures following hip replaceme nt

Objective To explore the management alternatives to tr aumatic periprosthetic femoral fractures after hip replacement. Metho ds A retrospective analysis was done on five cases of traumatic perip rosthetic femoral fractures posterior to hip replacement admitted in from June 1 998 to June 2003. There were two males and three females, with age range of 52- 71 years (average 59 years). One case suffered from traffic accident and the oth er four from slipping. Three cases had received hip replacement for femoral head aseptic necrosis, the other two for femoral neck non-union. Total hip replacem ent was carried out in four cases (five hips) and femoral head replacement in on e. These traumatic periprosthetic fractures occurred from one month to 17 years after hip replacement. According to classification to periprosthetic fracture of Vancouver (1995), there were 3 cases of type B and 2 type C. Two cases (one cas e of type B and one type C) were treated with traction and conservative treatmen ts, two (type B) with long shaft prosthesis revision combined with shape memory saw tooth embracing fixator or wires and one (type C) with anatomical plate inte rnal fixation. Results All 5 cases were followed up for f rom two months to five years and attained fracture union with no complications s uch as infection, nonunion or fixation breakage. Conclusions Traumatic periprosthetic femoral fractures after hip replacement are hard to deal with. The management alternatives differ according to fracture site, prost hesis loosening, bone quality and general status of patients. Classification cri teria and corresponding managements of periprosthetic fractures of Vancouver are proved to be suitable guidelines.

Objective To investigate the effect of arsenic(As2O3) on spermatogenic cell apoptosis and the expression of telomerase activity in adult rats.Methods 40 healthy male Sprague-Dawle rats were randomly divided into four groups and they were treated with As2O3 at doses of 0(control group),0.375,0.75 and 1.5 mg/kg body weight respectively through gavage for 16 consecutive weeks.The numbers of testicular sperm head were counted and the coefficient of testicular viscera,daily sperm production(DSP) were calculated in every group.The apoptosis of germ cell was assessed by in situ terminal deoxynucleotityl transferase mediated dTUP nick end labeling(TUNEL) technique.The activity of telomerase in the spermatogenic cells was determined by immunohistochemistry.Results In the testes of As2O3-treated rats,the coefficient of testicular viscera,the number of testicular sperm head and DSP in moderate and high dose groups decreased significantly than that of control(P

[Objective]To investigate the pathogenesis of osteoarthritis by increasing stiffness of subchondral bone to simulate subchondral bone sclerosis to induce the osteoarthritis.[Methods]The proportion between polymethylmethacrylate(PMMA),methylmethacrylate(MMA),hydroxyapatite(HA) and distilled water was adjusted to keep the maximum reaction temperature under 40℃.The composite material PMMA/HA was obtained and compressed on a mechanical material machine to determine its ultimate strength and stiffness,which were compared with those of subchondral bone.The subchondral bone of medial tibial plateau was scraped,where the composite material PMMA /HA was implanted into.Rabbits were sacrificed at 3,6,9 and 12 weeks,respectively.The articular cartilages of the medial tibial plateau were harvested for histological observation.Immunohistochemical analysis was adopted to record the expression and distribution of collagen type Ⅱ and MMP-1 in cartilage at protein level,which were compared with the blank and control groups.Transmission electron microscope was used to examine the ultramicroscopic changes of chondrocytes in blank and 6,12-week-postsurgery groups.[Results]With the time going,the experimental groups gradually presented degeneration,and had increased histological Mankin score.TEM demonstrated degenerative changes of chondrocytes.Collagen type Ⅱ expressed mainly in transition zone and upper deep zone,and MMP-1 expressed predominantly in the superficial and upper intermediate layers of cartilage.Both expressions steadily increased with time passing.[Conclusion]The increased stiffness of subchondral bone induced osteoarthritis in rabbits,which suggests that subcondral bone sclerosis may induce cartilage damage,and that subcondral bone sclerosis is one of the initial pathogenesis of osteoarthritis.
7.The effect of ischemia-reperfusion injury on concentrations of free amino acids in rat myocardium and taurine's protective role

By using the model of ischemia-reperfusion injury in the isolated working rat heart,the change of free amino acid concentrations in myocardium and the effect of taurine on the heart were investegated. It was found that concentrations of 17 free amino acids in myocardium, except Cys,decreased,and that 20mM taurine was able to ameliorate the ischemia-reperfusion injury of the heart ;e. g. , the improvement of recovery of cardiac mechanical functions, the decrease in the leakage of LDHand CK from myocardium,in the accumulation of lipid peroxides and calcium,and in the loss of free amino acids in myocardium,as well as the decrease in the occurence rate of ventricular arrhythmias during the reperfusion phase. So, it may be suggested that exogenous taurine has some protective effects against the myocardial is-chemia-reperfuaion injury.

9.Protective Effects of Polydatin on Acute Lung Injury Following Endotoxic Shock in Rats

OBJECTIVE:To investigate the protective effect of Polydatin on acute lung injury following endotoxic shock in rats.METHODS:Using a rat endotoxic shock model,animals were randomly divided into4groups∶sham operation group,en?dotoxic shock group,Polydatin treatment group and Polydatin pretreatment group.MAPs(mean artery pressures)were mea?sured at given time points.At the end of the experiment,the serum,the lung tissue,and the bronchoalveolar lavage fluid(BALF)were collected.The levels of lung coefficient,lung penetrating index(LPI),the protein concentration of BALF and the content of NOS in the lung tissue were measured.Furthermore,the histologic changes of the lung were observed under light micro?scope.RESULTS:In addition to prohibiting the dropping of MAP,Polydatin could mitigate the acute lung injury induced by endotoxin which increased the levels of lung coefficient,LPI,the protein concentration of BALF and the content of NOS in lung tissue.Enough morphological evidence could be found in pathological sections.Especially,the protective effects were more ob?vious in Polydatin pretreatment group.CONCLUSION:Polydatin has prophylactic and therapeutical effects on acute lung injury following endotoxic shock and the prophylactic effect is more marked than the therapeutic one.
10.Alexithymia of Patients with Essential Hypertension or Type 2 Diabetes Mellitus

Objective: To investigate factors related to alexithymia of patients with essential hypertension or type 2 diabetes mellitus. Method: 42 patients wi th essential hypertension, 40 with diabetes mellitus and 45 healthy control were assessed by TAS (Toronto Alexithymia Scale), SCL-90 and EPQ (Eysenck Personalit y Questionnaire). Multiple correlation analysis and multiple stepwise regressio n were used in data analysis. Result: Compared with normal contr ol, patients with hypertension or diabetes showed alexithymia. They were deficient in the abilit y of describing emotion, recognizing and distinguishing between emotion and body feeling, and used to extroversion thought. Besides these, patients with diabete s showed fancy-lacking either. There was close relation between personality and a lexithymia. The more introversive of the hypertensives, the more deficient they were in describing emotion. The more introversive of the diabetics, the more de f icient they were in recognizing and making distinguish between emotion and body feeling. Hostility ideation and neuroticism of hypertension patients were major factors related to alexithymia. Paranoid ideation and psychoticism had similar e ffects in diabetics. Conclusion: Patients with essential hypert ension or diabete s have alexithymia relating to their personality traits and psychological state .
