4.OBSERVATION ON HYPNOZOITE OF DIFFERENT ISOLATES OF PLASMODIUM VIVAX IN CULTURED MATERIALS

The hypnozoites of different isolates of P. vivax from Shenzhen and Shixin, Guangdong Province, Changsha, Hunan Province and Yincheng, Hubei Province of China in cultured materials were observed employing immunoperoxidase staining method. The percentages of hypnozoites among exoerythrocytic stages were 40.1, 43. 5, 50. 6 and 57. 1 %, respectively,indicating that the percentage of hypnozoites and the latitude are in positive correlation. The equation is y= - 6. 68+ 2. 05x. When the dlfference in latitude of Plasmodium vivax isolate source was more than 5?, a significant difference in the percentage of hypnozoites in various geographic isolates was found. However, the average diameters of hypnozoites and schizonts among various geographic isolates showed no significant difference. The results showed that ln the regions north to the Yangtze Rlver, the proportion of hypnozoite in the liver stage of P.vivax was larger than those in the southern China, being consistent with the clinical manifestations.
5.Perioperative nursing for patients receiving second stent implantation due to malignant airway stenosis

Objective To discuss the perioperative nursing measures and their effectiveness for patients with malignant airway stenosis who are receiving airway stent implantation once more due to airway re-stenosis.Methods The clinical data of 7 patients,who received airway stent implantation once more due to airway re-stenosis during the period from May 2014 to July 2016,were retrospectively analyzed.The effectiveness of nursing intervention for stent implantation was assessed.Results The symptoms of dyspnea were relieved immediately after the stent re-placement procedure in all the 7 patients,and no severe complications,such as massive hemorrhage,respiratory failure or asphyxia occurred.Conclusion The key points to ensure a successful second airway stent implantation include following measures and preparations:cooperation mode of medical care integration,preoperative individualized psychological intervention,guiding the patients to cooperate with the treatment,ready for in-operation emergency rescue,rapid and effective assistant cooperation for stent placement,and postoperative close observation for the occurrence of respiratory tract infection,bleeding and other complications.
6.The value of hyperenhancement sign on ultrasound, CT and their combination in diagnosis of thyroid benign and malignant nodules

Objective To investigate the diagnostic value of hyperenhancement sign on ultrasound,CT and their combination in diagnosis of thyroid benign and malignant nodules.Methods The contrast-enhanced ultrasound and enhanced CT datas of 172 thyroid nodules in 144 cases confirmed by operation and pathology were retrospectively analyzed,including 97 benign nodules and 75 malignant nodules.According to the degree of enhancement,these nodules were divided into iso-or low-enhancement and hyperenhancement.The distributions of ultrasound hyperenhancement,CT hyperenhancement and both of them in thyroid benign and malignant nodules,adenomatoid lesions and nodular goiters were analyzed,followed byx2 test for statistical analysis.Results In 172 thyroid nodules,the proportions of ultrasound hyperenhancement,CT hyperenhancement and both of them in thyroid benign and malignant nodules were 53.6%(52/97) and 20.0%(15/75)(x2=20.090,P<0.05),34.0% (33/97) and 4.0% (3/75) (x2=23.033,P<0.05),31.0% (30/97) and 0% (0/75) (x2=28.096,P<0.05),respectively.Their sensitivity and specificity of diagnosing benign nodules were 53.6% (52/97) and 80.0% (60/75),34.0% (33/97) and 96% (72/75),30.9% (30/97) and 100% (75/75),respectively.The proportions of ultrasound hyperenhancement,CT hyperenhancement and both of them in adenomatoid lesions and nodular goiters were 96.9%(31/32) and 32.3%(21/65)0x2=35.946,P<0.05),65.6%(21/32) and 18.5%(12/65)(x2=21.250,P<0.05),65.6%(21/32) and 13.8% (9/65)(x2=26.912,P<0.05).Their sensitivity and specificity of diagnosing adenomatoid lesions were 96.9%(31/32) and 67.7%(44/65),65.6%(21/32) and 81.5%(53/65),65.6%(21/32) and 87.2%(56/65).Conclusions Ultrasound hyperenhancement,CT hyperenhancement for diagnosing thyroid benign nodules are of significant value.Especially for adenomatoid nodules,ultrasound hyperenhancement has a higher sensitivity,while CT hyperenhancement has a higher specificity.Their combination can further improve the diagnostic specificity,thus reducing the unnecessary surgical trauma.
7.Preparation of LEM-GM-CSF and its inhibition effect on HL60/ADM cells in vitro

Objective To prepare liposome-entrapped mitoxantrone (LEM)-GM-CSF and observe the cytotoxicity of HL-60/ADM cells treated with LEM-GM-CSF, LEM and dihydroxyanthraquinone (DHAQ) in vitro. Methods LEM was prepared by reverse phase evaporation (REV). High speed centrifugation was applied to separate LEM and dissociate DHAQ. Colorimetry was employed to determine encapsulation efficiency. The liposome structure and particle size were determined by transmission electron microscopy. GM-CSF was coupled to LEM by glutaraldehyde method. UV-spectrophotometric analysis was applied to measure the coupled efficiency. Flow cytometry was applied to determine the immunoconjugate retained efficiency. The cytotoxicity of HL60/ADM cells and interdiction efficiency of GM-CSF were investigated by MTT test. Results The encapsulation efficiency of LEM was 80%. Most liposomes were monolayer, and the particle size was 170-220 nm. Its coupled efficiency with GM-CSF was 42.3%, and the immunoconjugate retained efficiency was 74.6%. All LEM-GM-CSF, LEM and DHAQ had cytotoxicity on HL60/ADM, their cytotoxic power in decrement sequence: LEM-GM-CSF, LEM, DHAQ. After treated with LEM-GM-CSF, LEM and DHAQ for 24 h, the IC50 of HL-60/ADM was 8.73, 12.42, 27.31 ?g/ml respectively and for 48 h the IC50 were 0.62, 8.25, 12.44 ?g/ml. The inhibition rate increased in a dose-dependent manner. Conclusion The encapsulation efficiency, the coupled efficiency and the immunoconjugate retained efficiency of LEM-GM-CSF prepared by our method were satisfying. LEM-GM-CSF representing anti-leukaemia efficiency in vitro had cytotoxicity on HL60/ADM cells.
8.Career identity and satisfaction of family doctors in Pudong New Area of Shanghai municipality

Objective To survey the career identity and satisfaction of family doctors in Pndong New Area of Shanghai municipality.Methods Two hundred and thirty four family doctors were selected with random sampling method from 6 community health centers in Pudong New Area of Shanghai.The survey on career identity and satisfaction was conducted using a self-designed questionnaire.Results The results showed that 37.2% of participants were very satisfied (8.1%,19/234) or satisfied (29.1%,68/234) with their current career situation.The three lowest satisfaction factors were salary (14.1%,33/234),benefit and social insurance (27.3 %,64/234),professional reputation and social status (20.9% 49/234).Meanwhile,the three highest factors were quality of their leadeship (79.5%,186/234),interpersonal relationship(74.4%,174/234) and the institutional management (63.3%,148/234).The survey also showed that 44.5% and 30.6% of participants expected their monthly pay as 8 001-10 000 and 5 000-8 000 RMB; 39.1% thought the current performance evaluation mechanism as effective,while 25.7% thought the opposite.Conclusion The social status and salary level of family doctors need to be improved; there is also an urgent need to improve the performance evaluation mechanism and other supporting measures.
9.The value of thromboelastography in evaluating the risk of bleeding in patients with acute leukemia

Objective To investigate the role of thrombelastography(TEG) in assessing the risk of bleeding and diagnostic value in patients with acute leukemia(AL) .Methods The TEG and PLT data were counted in 127 patients(272 sets of data) who were diagnosed with AL .Those patients were divided into two groups :group 1 (including patients with bleeding) and group 2 (in‐cluding patients with no bleeding) .The indicators(R values ,K values ,α‐angle ,MA values)and PLT count were compared between two groups .Those data with PLT<30 × 109/L of these two groups also were divided and the 4 indicators of TEG were compared between the two groups .We used the ROC curve to evaluate the sensitivity and specificity in assessing the risk of bleeding .Results According to the data in total ,the K value ,R value of the group 1 were higher than those of the group 2(P<0 .05);theα‐angle and MA value ,PLT counts of group 1 were lower than those of the group 2(P<0 .05) .In those AL patients whose PLT<30 × 109/L ,the K value of the group 1 was higher than that of the group 2(P<0 .05);theα‐Angle and MA value of the group 1 were lower than those of the group 2(P<0 .05);R values and PLT count were not different between the two groups(P>0 .05);the are‐as under the ROC curve about the PLT counts ,MA value andα‐angle were more than 0 .5 (0 .750 ,0 .740 and 0 .653) .Conclusion T EG could predict the risk of bleeding in acute leukemia patients and it could be used in clinical application .

Probing into the methods and techniques of physician-patient communication is important to clinic medicine and medical education. Physician- patient shared decision is one of important aspects of physician-patient communication, 80 patients with hematologic malignancies were reviewed for classification of models of medical decision making, and the methods and techniques of physician-patient shared decision were also discussed .
