1.Clinical Observation of Qili San in the Treatment of Trauma-induced Swelling and Pain

OBJECTIVE:To observe therapeutic efficacy and ADR of oral administration and external application of Qili san in the treatment of trauma-induced swelling and pain. METHODS:70 traumatic patients were randomly divided into treatment group and control group with 35 patients in each group. Treatment group was given Qili san(1.5 g/time,2 times/d)orally,and ap-plied to affected area(blended with 75% alcohol to make paste,1 time/d);control group was only given Qili san(1.5 g/time,2 times/d) orally;a treatment course lasted for 7 d. Pain degree score,pain duration,swelling degree score and improvement rate, total effective rate and ADR were compared between 2 groups before and after treatment. RESULTS:Compared with before treat-ment,pain degree score,pain duration and swelling degree score of 2 groups were decreased after treatment;there was statistical significance(P<0.01 or P<0.05);clinical symptom of treatment group was better than that of control group;there was statistical significance(P<0.01 or P<0.05). There was statistically significant difference in the improvement rate of swelling and total effec-tive rate between 2 groups after treatment(P<0.01 or P<0.05). No obvious ADR was found during treatment. CONCLUSIONS:Oral administration and external application of Qili san is safe and effective for trauma-induced swelling and pain,and its therapeu-tic efficacy is better than only oral administration.
2.Terminal surgical reconstruction of complex and old midfacial fractures

Objective To study the operation design and surgical methods for terminal surgical reconstruction of severe post-midfacial fracture deformities. Methods From July 1997 to December 2002,11 cases of severe and complex post-midfacial deformities were reconstructed. There were four cases with Le Fort Ⅰ, Ⅱ and Ⅲ fractures,five with Le Fort Ⅰ and Ⅲ fractures and two with Le Fort Ⅰ and Ⅱ fractures combined with right orbital-zygomatic fractures. Typical bicoronal and subcilliary incisions and intra-oral approach were employed to expose all the fractured sites. The displaced orbito-zygomatic bone fragments were repositioned firstly in order to reconstruct the outer midfacial framework. Then, the malunited maxilla was reduced to its proper position after osteotomy of Le Fort Ⅰ fractures under the guidance of mandible through inter-maxilla fixation. The depressed naso-orbital region were reconstructed using autogeneous outer cranial table. Meanwhile, nasal framework reconstruction, medial canthal tendon reapproximation and plasty, and fractured orbital walls repairing were performed to correct the enophathalmos. Results All the cases recovered well and the post-operative facial appearance and occlusal function were improved obviously. Conclusions Complex midfacial fractures, usually involving orbital-zygomatic bone, naso-orbit and maxillary bone, can be well improved through osteotomy and reduction, internal rigid fixation with mini-plates and screws, autogenenous bone grafting and framework reconstruction.
5.CT feature of microcarcinoma of thyroid

Objective To evaluate the imaging feature of microcarcinoma of thyroid with CT.Methods CT findings of 50 lesions in 47 patients with microcarcinoma of thyroid ( dimeter,0.5 to 1.0 cm) were retrospectively analyzed.All of the patients had pathological diagnosis.Results Of the 50 lesions,38 lesions showed homogeneous low density on non-enhanced CT and various degree of enhancement on postcontrast CT. Thirty-three lesions showed discontinuous edge of the thyroids on non-enhanced CT. The boundary of 30 lesions became unclear on post-contrast CT relative to non-enhanced CT and the extension of low density of lesions decreased. Theshape of 31 lesions were irregular. Fifteen lesions showed calcifications,with granular calcifications in 13 lesions.Ten lesions were complicated with thyroiditis.Conclusions Irregular shape,discontinuous edge of the thyroids,the shrinkage of low density of lesions on post-contrast CT relative to non-enhanced CT,granular calcifications and multiple small lymph node around lesions indicate the diagnosis of microcarcinoma of the thyroid.It should be noted that thyroiditis can cover up microcarcinoma of thyroid.
6.Efficacy on perennial allergic rhinitis treated with acupuncture at three nasal poinits and the acupoints selected by syndrome differentiation.

OBJECTIVETo compare the difference in the clinical efficacy on perennial allergic rhinitis between three nasal points acupuncture therapy and the oral administration of loratadine so as to provide the better acupuncture program in clinical treatment.
METHODSSixty cases were randomized into an acupuncture group (30 cases) and a medication group (30 cases). In the acupuncture group, acupuncture was applied to three nasal points [Yingxiang (LI 20), Yintang (EX-HN 3), Bitong (Extra)] and acupoints selected by syndrome differentiation. Acupuncture was given once every two days, three times a week, for 4 weeks totally. In the medication group, loratadine was prescribed for oral administration, 10 mg every day, for 4 weeks. The symptom and physical sign scores before and after treatment, as well the short-term and long-term efficacy were compared between the two groups.
RESULTSThe total effective rate was 96.7% (29/30) in the acupuncture group and was 93.3% (28/30) in the medication group after treatment. The efficacy was similar between the two groups (P>0.05). In follow-up, the total effective rate was 86.7% (26/30) in the acupuncture group, which was better than 56.7% (17/30, P<0.05) in the medication group. The scores of symptoms and physical signs after treatment and in follow-up were all reduced apparently as compared with those before treatment in the patients of the two groups (all P<0.05). The scores of symptoms and physical signs were reduced more apparently in the acupuncture group as compared with those in the medication group in follow-up (all P<0.05).
CONCLUSIONThe acupuncture at three nasal points and the acupoints selected by syndrome differentiation achieves the similar short-term efficacy on perennial allergic rhinitis as compared with the oral administration of loratadine. The acupuncture therapy presents the obvious advantages on long-term efficacy.

7.Relationship between leptin level and glycometabolism in pregnancy with abnormal glucose challenge test

Objective To investigate the relationship between serum leptin(LP) level and glycometabolism in pregnancy with abnormal glucose challenge test(GCT) and the corresponding outcome of pregnancy. Methods Serum LP level,fasting serum insulin(FINS) and fasting blood glucose(FBG) levels were measured by radioimmunoassay in 23 patients with abnormal GCT but normal oral glucose tolerance test(OGTT),20 with impaired glucose tolerance(IGT),20 with gestational diabetes mellitus(GDM)(studied groups above) and 30 normal pregnant women(control group).LP,FINS,FBG, insulin resistance index(HOMA-IR) and ?-cell function index(HOMA-IS) were compared among the groups and the outcomes of pregnancy were observed. Results ①There were significant differences in LP,FINS,FBG,HOMA-IR and HOMA-IS between the studied groups and the control group(P0.05).③There were signi-ficant differences in the gestational weeks of delivery,cesarean section rate and neonatal weight between the studied groups and the control group(P0.05). Conclusion Both insulin secretion and insulin resistance are elevated in pregnancy with abnormal glycometabolism,and are closely correlated to the increased serum level of LP.There existed abnormal glycometabolism in patients with abnormal GCT but normal OGTT,which is correlated to the outcome of pregnancy.
8.Influence factors of knowledge, attitude and practice of pelvic floor dysfunction during pregnancy and its intervention measures

Objective To investigate the level of knowledge,attitude and practice(KAP)of pelvic floor dysfunction(PFD)and its influencing factors in pregnant women and provide reference for further health education for pregnant women. Methods Totally 637 pregnant women were investigated by Knowledge-Attitude-Practice (KAP)scale for PFD. Results The average scores of KAP in pregnant women were(31.68 ± 7.73),(16.41 ± 4.01) and (10.57 ± 3.00) respectively. About 85.6% of the pregnant women had interest in knowledge of PFD. Educational background,willing to understand knowledge of PFD,and gestational age were dominant factors affecting KAP of women with PFD. Conclusions The awareness rate of PFD and compliance rate of pelvic floor muscle function exercise in pregnant women are low. Effective measures should be taken to prevent PFD for pregnant women.

10.The value of CT annular calcification in identifying benign and malignant thyroid nodules

Objective To evaluate the value of annular calcification in CT in the diagnosis and differential diagnosis for benign and malignant thyroid nodules.Methods CT findings of 67 nodules in 67 patients pathologically diagnosed with annular calcifications were retrospectively analyzed to identify 49 benign nodules and 18 malignant nodules.The interior or boundaries of annular calcification before and after contrast-enhancement were compared,and the degree of enhancements of both interior annular calcification and thyroid tissues were observed.After contrast-enhancement,the numbers of lesions showing clearer boundaries and higher degree of enhancement were summarized.Statistic analysis was conducted by using x2 test.Results Among 67 nodules with annular calcifications,clearer boundaries after contrast-enhancement were observed in 61.2% (30/49) benign nodules and 16.7% (3/18) malignant nodules,showing significant statistical difference (x2 =10.457,P < 0.05).The sensitivity and specificity of clearer edge after contrast-enhancement for benign nodules were 61.2% (30/49) and 83.3% (15/18) respectively.Higher degree of enhancement were observed in 16.3% (8/49) in benign nodules and 0(0/18) malignant nodules,showing no significant statistical difference (x2 =3.337,P > 0.05).The sensitivity and specificity of higher degree of enhancement for benign nodules were 16.3% (8/49) and 100.0% (18/18) respectively.The combination of clearer boundaries and higher degree of enhancement after contrast-enhancement was observed in 77.6% (38/49) benign nodules and 16.7% (3/18) malignant nodules,showing significant statistical difference (x2 =20.549,P < 0.05).Meanwhile,the sensitivity and specificity of such combination for benign nodules were 77.6% (38/49) and 83.3% (15/18) respectively.Conclusions CT is important in the diagnosis of thyroid nodules with annular calcification.Clearer boundary after contrast-enhancement and its combination with higher degree of enhancement are helpful for the diagnosis of benign nodules.Boundaries that similar to or more obscure than that of plain scan indicate malignant nodules.
