1.Endoscopic nasal reshaping of the structure in structural rhinitis

OBJECTIVE To explore the methods and effects of endoscopic nasal structure reshaping for treatment of chronic rhinosinusitis(CRS). METHODS One hundred and twenty-three cases of CRS patients with abnormal anatomy in nasal cavity and sinus were treated by the Messerklinger approach. At the same time varieties of anatomical abnormalities were corrected,including:①submucous resection of nasal septum,②submusous middle turbinate resection,③submusous inferior turbinate resection,④Uncinate process resection ⑤deal with frontal recess and its adjacent cells. RESULTS All patients were followedup for 1 to 2 years. One hundred and six cases were successful,14 satisfied and 3 inefficient. The rate of full recovery was 97.56%. CONCLUSION Reshaping nasal structure during endoscopic surgery is a method worth recommendation for structural rhinitis.

4.Expression of phosphorylated cyclic adenosine monophosphate response element-binding protein in rat brain striatum during learning and memory training

BACKGROUND: The marginal division, a new sub-region in rat brain striatum discovered in recent decades, has been found to closely relate to learning and memory function of the brain. The immediate-early genes such as c-fos and c-jun participate in the signal transduction of learning and memory in the marginal division. But what other intermediate events are initiated in the marginal division in the process of learning and memory?Cyclic adenosine monophosphate (cAMP) response element-binding protein (CREBP) is an molecule essential for the formation of long-term memory,and investigation of the expression and distribution of phosphorylated CREBP in the striatum may help understand the signal transduction mechanism in the striatum during learning and memory at the molecular level.OBJECTIVE: To investigate the expression of phosphorylated CREBP in rat brain stratum during learning and memory process.DESIGN: Completely randomized controlled study.SETTING: Institute of Neurosciences, Zhujiang Hospital of the First Military Medical University of Chinese PLA.MATERIALS: This experiment was conducted in the Institute of Neurosciences, Zhujiang Hospital of the First Military Medical University of Chinese PLA between April and August 2003. Totally 48 normal male adult SD rats were provided from the Experimental Animal Center of First Military Medical University, and after two Y-maze tests, 40 rats were selected for this study (MG-2 type, Sanshengxing electricity company).METHODS: The 40 SD rats were randomized into 4 equal groups. The rats in the first group were subjected to training to acquire dark avoidance reflex in a Y maze, those in the second group underwent sham training with only light stimulation in the Y maze without electricity on the floor.The rats in the third group were trained in the Y maze with electricity on the floor but not light stimulations, with the rest 10 rats serving as the control group subjected to sham training in the Y maze without either electric or light stimulations. After the training in the Y maze, the rats were sacrificed immunohistochemical detection of phosphorylated CREBP expression in the brain striatum.MAIN OUTCOME MEASURES: Expression of phosphorylated CREBPin rat brain striatum.RESULTS: All the 40 rats enrolled in this study were examined for phosphorylated CREBP expression. Positive expression of phosphorylated CREBP was observed in the medial marginal division of the brain striatum after the rats were trained in the Y maze with electric stimulation, but no obvious expression was seen in rats in the sham training or control groups.Massive expression of phosphorylated CREBP could be observed, typically,in the hippocampus, front prefrontal lobe cortex and cingulate gyrus of the rat brain.CONCLUSION: The transcriptional factor phosphorylated CREBP in the marginal division of the striatum participates in the signal transduction for learning and memory in rats receiving Y maze training to acquire dark avoidance reflex.
5.Analysis of Risk Factors on Coronary Artery Lesions Secondary to Kawasaki Disease

Objective To investigate the risk factors on children with coronary artery lesions (CAL) secondary to Kawasaki di-sease(KD). Methods One hundred and forty - five patients with KD from January 1999 to December 2001 were collected. Among them, 93 cases without CAL and 52 cases with CAL. The test results, therapeutic methods and prognosis were analyzed in two groups. Results The duration of fever was longer and mean value of erythrocyte sedimentation rate (ESR) higher in group with CAL than those m the group without CAL(P0.05).The incidence of CAL was 18.8% (18/96) in patients of IVIG treatment within 10 days from onset, and 69.4% (34/49) in patients without IVIG treatment (P
7.Endoscopic surgery in nasal-skull base tumor

OBJECTIVE To investigate the efficacy and feasibility of endoscopic surgery to manage the tumors involving naso-ethmoidal complex and anterior skull base.METHODS 19 cases of tumors involving nasal cavity,ethmoid sinus and anterior skull base were summarized retrospectively, including 3 cases of ethmoidal squamous carcinoma, 3 cases of esthesioneuroblastoma,3 cases of melanoma,1 case of plasmocytoma,2 cases of ethmoidal adenocarcinoma,4 cases of adenoid cystic carcinoma,2 cases of meningioma.RESULTS Tumors were totally removed in 18 cases and subtotally removed in 1 case,all the cases were followed up for 1 to 3 years.Of the 3 cases of melanoma,1 case died of brain metastases 1 year after surgery,1 case recurred 8 months after surgery,1 case of adenoid cystic carcinoma recurred 17 months after surgery.No recurrence were found in the rest 16 cases during 1-3 years of following-up.CONCLUSION Endoscopic surgery is a effective method for malignant tumors localized in nasal cavity and ethmoid sinus and benign anterior skull base tumors involving nasal cavity and sinuses.Indication must be carefully selected for malignant tumors and postoperative radiotherapy should be advised.
8.Effects of atorvastatin on blood pressure and left ventricular remodeling in patients with both essential hypertension and hypercholesterolemia

Objective To compare the changes in serum levels of high sensitive C-reactive protein (hs-CRP)and uric acid(UA)before and after atorvastatin treatment for the patients with both essential hypertension and hypercholesterolemia,and to evaluate its effects on blood pressure and left ventricular remodeling.Methods One hundred and twenty-six hypertensive patients complicated with hypercholesterolemia were randomized into group A with amlodipine 10 mg/d(n = 65)and group B with amlodipine 10 mg/d plus atorvastatin 20 mg/d(n = 61),for three months continuously.Serum levels of total cholesterol(TC),low-density lipoprotein-cholesterol(LDL-C),high-density lipoprotein-cholesterol (HDL-C),triglyeerides(TG),hs-CRP and UA,as well as blood pressure,were determined for both groups before and after treatment.Left ventricular posterior wall thickness(LPWT)and interventricular septum thickness(IVST)were measured by echocardiography and left ventricle mass index(LVMI)was calculated.Results Serum levels of TC,LDL-C,HDL-C,TG,hs-CRP and UA decreased significantly in gr6up B after three-month treatment with atorvastatin,while serum level of HDL-C increased significantly. 9.Use of iodixanol in coronary intervention for patients with diabetic renal insufficiency

Objective To evaluate the safety of coronary intervention using iodixanol in patients with diabetic renal insufficiency.Methods Clinical data of 97 patients with diabetic renal insufficiency undergoing coronary intervention during June 2004 to June 2006 were retrospectively analyzed,50 of them with iodixanol,an iso-osmolar contrast medium (iodixanol group),and 47 with iopromide,a hypotanic contrast medium (iopromide group).Judkin's coronary angiography showed 167 diseased vessels in the patients,65 in anterior descending branches,44 in circumflex branches,and 58 in right coronary arteries. Levels of serum creatinine and blood urea nitrogen were determined before percutaneous coronary intervention (PCI),on the day of the procedure,the 3rd and 7th days after PCI,respectively,as well as radiocontrast media-induced nephropathy (CIN) was observed.Results Totally,192 drug-eluting stents were successfully implanted in 167 diseased vessels,and all patients' angina pectoris symptom disappeared soon after the procedure,with a full success.No acute or subacute stent thrombosis and major adverse cardiac events (MACE) occurred.Two patients (4%) in iodixanol group and 10 (21%) in iopromide group got CIN,with a statistical significance (P
10.Analysis of surveillance results of drinking-water-borne endemic fluorosis in Jiangsu Province in 2009

Objective To investigate the development trend of drinking-water-borne endemic fluorosis in Jiangsu Province,and to provide the basis for further prevention and treatment of the disease.Methods In 2009,eight major counties were chosen,and in each county all diseased villages were classified into light,moderate and severe disease types according to water fluorine content based on historical data,and one village was chosen from each type.In monitoring villages with improved water,one source water and three tap water samples were collected,respectively.Five water samples were collected in water unimproved monitoring villages according to water well locations of the east,the west,the south,the north and the center.The fluorine content in water was determined according to the Standard Testing Methods for Drinking Water (GB/T 5750-2006).Children aged 8 to 12 were examined for dental fluorosis by Dean method.Residents over the age of 16 were examined for clinical osteofluorosis,and two monitoring counties were chosen,then one village was respectively chosen in each county,and clinically diagnosed patients with skeletal fluorosis were examined again by X-ray.Both clinical and X-ray diagnosis were on the basis of Diagnostic Criteria of Endemic Skeletal Fluorosis (WS 192-2008).Urine samples of 30 children aged 8 to 12 and of 20 adults over the age of 16 were randomly collected and urinary fluoride was determined by F-ion selective electrode method(WS/T 89-1996).Results Of all the 24 villages of 8 counties,20 villages were water improved,and water-improvedprojects ran normally in 18 villages,while scrapped in the rest 2 villages.One hundred and two samples were tested,and the mean of water fluoride in water-improved villages was 1.10 mg/L,while in water unimproved villages and villages with water improved projects scrapped was 1.90 mg/L.The prevalence of dental fluorosis was 42.51%(854/2009).The prevalence of clinical skeletal fluorosis was 23.23% (2024/8713) and the X-ray detection rate was 32.00% (24/75).Six hundred and sixty-four urine samples of children were determined,and geometric mean of urinary fluorine was 1.59 mg/L,while 370 adult urine samples were determined,and geometric mean of urinary fluorine was 2.20 mg/L.Conclusions Endemic fluorosis in Jiangsu Province has not been fully controlled and there are signs of recovery.We must pay attention to water improvement measures to reduce fluoride and the management and maintenance of water improvement projects,and further strengthen the prevention and control of endemic fluorosis.
