2.The effect of enteral nutrition support with low carbohydrate formula in critically ill patients with hyperglycemia

Objectives:To study the effect of low carbohydrate formula's enteral nutrition in critically ill patients with hyperglycemia.Methods:52 patients were randomly divided into control group and study group.The control group was given Nutrison and the study group was given Diasip.Blood glucose,blood lipid,serum protein,PaCO_2 and PaO_2 were measured before and after therapy.Results:The fasting blood glucose was significantly increased in patients having received Nutrison and did need insulin treatment.The fluctuation of blood glucose was less in patients who had used Diasip and didn′t need insulin.The changes of serum protein were significant after therapy and were not significant between groups.The changes of serum lipid were significant after therapy and were not significant between groups.Conclusion:Diasip can control blood glucose and improve nutritional status and respiratory failure in critically ill patients with hyperglycemia.

4.Anti-thyroid drugs—first choice for Graves' disease

Graves' disease,the most common disorder of hyperthyroidism,is an autoimmune disease related with autoantibody (TRAb),with the specific mechanism remaining incompletely understood.The therapies for Graves disease include three conventional methods:anti-thyroid drugs,radioiodine and surgery.Only treatment with anti-thyroid drugs is aimed at the immune factor;they can eliminate TRAb and lead to recovery ultimately.Literatures have drumed in that treatment with anti-thyroid drugs was a basic therapy for Graves' diseases,and they were the first choice to cure hyperthyroidism all over the world (except for the U.S.A).The drugs do not destroy the structure of thyroid follicle,and are safe and efficient;moreover,their effect is reversible and therefore do not cause permanent damage to the thyroid.Anti-thyroid drugs not only can be used alone,it also can be used before radioiodine or surgery.ATA chairman professor Cooper(NEJM,2005) propounded a therapy path for Graves' disease,which is helpful for clinical practice.
5.Anesthesia selection for senile total hip arthroplasty

Total hip arthroplasty is common joint replacement method in senile patients,which is characterized by large wound,large amount of bleeding,and great stimulation.Anesthesia methods during total hip arthroplasty include epidural anesthesia,epidural and lumbar anesthesia,and general anesthesia.The anesthesia method is dependent on patient condition.Appropriate anesthesia based on disease condition can reduce anesthesia risk.The key of anesthesia is to control anesthesia plane,supplement blood and monitor toxic reaction during cement application.Because senile patients are always complicated by respiratory,circulatory and endocrine system diseases,comprehensive treatment before surgery to improve body symptoms can increase anesthesia and surgery tolerance and better guarantee patient safety during surgery.
6.The experimental studys on the effect of glutamine in the intestinal barrier function on rats

Objective: To study the effect and mechanism of glutamine(Gln) on the intestinal barrier function in rats. Methods: Forty-five Sprague-Dawley rats were randomly divided into three groups(groupⅠ,groupⅡand groupⅢ) of fifteen animals each:.Rats of groupⅠ,groupⅡwere fed on normal diet and group Ⅲ was fed on 3% Gln-supplemented diet.The stress model was established in rats of groupⅡand Ⅲ.3 weeks later,the D-lactate,Gln levels in plasma and the contents of corticotropic releasing hormone(CRH),neurotensin(NT) in brain were measured.The morphology of small intestinal protein,DNA contents and its mucosa were observed. Results: Compared with control group,stress increased D-lactate level in plasma and reduced plasma glutamine level and the content of protein and DNA in small intestinal mucosa respectively.The small intestine demonstrated evident pathological characteristics under stress.Compared with stress group,D-lactate level in plasma was reduced and DNA content in small intestinal was increased in group Ⅲ.The CRH content in brain tissue was reduced and NT content in brain tissues was increased after Gln-supplemented.The pathological picture of small intestinal mucosa under stress was ameliorated after Gln-supplemented.Conclusion: The intestinal barrier function of rats may be protected after Gln-supplementation.Gln-derived neurotransmitters could alleviate the stressful damage of intestinal mucosa.
7.Preparation and identification of monoclonal antibody against angiopoietin-related protein 2

Objective:To prepare a monoclonal antibody against human angiopoietin-related protein 2(ARP2).Methods: Human spleen ARP2 gene was obtained by RT-PCR.A pET32a-ARP2 plasmid was constructed and was incorporated into(E.coli.) The products were purified and were used to immunize 6-week-old BALB/c female mice.Hybridoma secreting anti-ARP2 monoclonal antibody was obtained by standard procedure.Mass production was carried out after specificity identification with Western blotting.Results: The fusion protein obtained by pET32a system had a relative molecular weight of about 570 000,which was in accordance with the theoretical value.The purity of the protein was more than 90% after purification.The antibody titer was 110~(4)in the hybridoma culture supernatant and 110~(7)-10~(8) in the ascites.The IgG2a type antibody had a relative molecular weight of about 570 000 by Western blot analysis.Immunohistochemistry method showed that the antibody bond with human ARP2.Conclusion: The prepared anti-human ARP2 monoclonal antibody in this study can be used for identification of ARP2 protein.
8.THE ORIGIN AND TERMINATION OF THE RETICULOSPINAL TRACT IN THE CAT——HRP METHOD

The origin and termination of the reticulospinal tract were studied with HRP technique in ten cats by injecting HRP into the spinai dorsal horn or gray matter on one side of the cervical or lumbar enlargement, It was discovered that the reticulospinal neurons were located not only in the medial part of the medullopontal tegmentum, but also in its lateral part. There were also a few scattered labeled neurons in the reticular formation of midbrain. The reticulospinal tract ended in the spinal dorsal horn as well as in the ventral horn. The reticulospinal tract mainly originated from the ventromedial part of the medulla and pons. In this region the labeled cells were most numerous in the nucleus gigantocellularis, less in the nucleus medulla oblongatae centralis subnucleus ventralis and the nucleus pontis centralis caudalis, and much less in the nucleus pontis centralis oralis, the nucleus paragigantocellularis laterlris and the paramedium reticularis subnucleus ventralis. Occasionally a few scattered abeled cells could be seen in the nucleus cuneiformis. The reticulospinal fibers from these nuclei projected bilaterally to both cervical and lumbar enlargments of the spinal cord except that the nucleus cuneiformis projected bilaterally to the cervical enlargement only.Two groups of labeled cells were found in the ventrolateral part of the medullary and pontal reticular formation, corresponding to the position of group A 1 and A 7 of noradrenergic neurons respectively.The reticulospinal tract derived from the medial part of the brain stem terminated predominately in the gray matter ventral to the spinal dorsal horn, but a few of them ended in the dorsal horn. These connections provide direct pathways for the control of motor and sensory functions of the spinal cord by medial reticular formation.It has been proved by Brodal with chromatolysis techenique that every cell in the nucleus paramedium reticularis sends its efferent fiber to the spinal area of cerebellum, but in this study labeled cells were found in e nucleus paramedium reticularis subnucleus ventralis. We suppose that the axons of these labeled neurons may be divided into two branches, one projecting to the cerebellum and the other to the spinal cord.It is noteworthy that we also found some labeled neurons in the nucleus medullae oblongatae centralis subnucleus dorsalis and nucleus parvocellularis, the so called "receptive region" of the reticular formation. The fibers originating from this region terminated in the spinal dorsal horn mainly, suggesting that this tract probably influences the sensory function of the spinal cord. It seems that the conception of the lateral reticular formation of the brain stem as a pure "receptive area" of the reticular formation may have be modified.
9.THE PROJECTION FROM EDINGER-WESTPHAL NUCLEUS TO THE SPINAL CORD IN THE RABBIT——HRP METHOD

The projection from Edinger-Westphal (E—W) nucleus to the spinal cord was studied in fifteen rabbits by injecting HRP or WGA—HRP into the cervical, thoracic and lumbar segments of the spinal cord. The normal cytoarchitecture of E—W nucleus was also studied with normal Nissl stained sections.The E—W nucleus of the rabbit is a midline unpaired structure except for the most rostral part which is paired. The shape of the whole nucleus looks like a band with its long axis lying rostrocaudally. It is mainly consisted of medium and small fusiform neurons with their long axis lying dorsoventrally. Neurons of the rostral part of the E—W nucleus, which is called the anterior median nucleus in this paper, are longer and darker than those of the caudal part of the E—W nucleus, or the proper E—W nucleus.Many labelled cells were found in AM and E—W nuclei in all the injected cases. The neurons, projecting to the cervical, thoracic and lumbar segments, were distributed diffusely in E—W nucleus without evident somatotopic organization. In the lumbar injected cases, there were a few labelled cells in the rostral end of AM nucleus where the nucleus was divided into two parts bilaterally.
10.Clinical validation of multi-formula to estimate the weight of fetal macrosomia by ultrasonic measurement

Objective:To derive a formula of higher veracity in estimating the weight of fetal macrosomia by ultrasonic measurement.Methods:The relationship between ultrasonic measurement of fetal abdominal circumference(AC),biparietal diameter(BPD),femur length(FL),humeral soft tissue thinkness(HSTT) and the weight of the neonate respectively in 361 cases,and formula of multiple regression was established.Then a clinical validation was carried out in 75 cases by this new formula.A comparison with other two formulas estimating fetal macrosomia was also made.Results:In comparison with the monoparameter formula and another multiparameter formula,the checking-out rate of fetal macrosomia by this new formula is higher,about 64 percent ( P
