1.Clinical analysis of excision and primary suture and Limberg transposition flap for the treatment of pilonidal sinus

Objective To compare the clinical therapeutic efficacy of excision and primary suture after the hydrogen peroxide and methylene blue treatment, and Limberg transposition flap for the treatment of pilonidal sinus. Methods The clinical data of 55 patients with sacrococcygeal pilonidal sinus were analyzed retrospectively. Thirty-six patients were treated by excision and primary suture after the hydrogen peroxide and methylene blue treatment (suture group), and 19 patients were treated by Limberg transposition flap (flap group). The operation time, length of hospital stay, incision healing time, incidence of complications and recurrence rate were compared between 2 groups. Results There were no significant differences in the length of hospital stay, incision healing time, incidence of complications and recurrence rate between 2 groups (P>0.05). The operation time in suture group was significantly shorter than that in flap group: (51.25 ± 7.02) min vs. (71.42 ± 12.29) min, and there was statistical difference (P = 0.000). Conclusions Excision and primary suture after the hydrogen peroxide and methylene blue treatment is a good method for the treatment of pilonidal sinus. The operation time is short. It is easy to operate, and could achieve good results.
2.Anterolateral thigh perforator free flaps for reconstruction of soft tissue defects around the ankle

Objective To investigate the methods and therapeutic effects of anterolateral thigh perforator free flaps for reconstruction of soft tissue defects around the ankle.Methods A retrospective case series study was made on 30 patients treated with anterolateral thigh perforator free flaps for traumatic soft tissue defects around the tibia between January 2010 and June 2016.There were 22 males and eight females,with the age range of 16-58 years [(41.5 ± 1.2) years].Dimension of the soft tissue defect ranged from 9 cm ×4 cm to 23 cm × 12 cm.Flap stabilization time,flap survival rate,postoperative complications,second plastic surgery rate,and ankle joint function by Baird-Jackson score were recorded.Results All patients were followed up for (13.0 ± 5.4) months (range,6-24 months).Time for flap stabilization was (6.5 ± 1.1) days.All flaps survived except in one patients with flap loss due to postoperative arterial thrombosis.Postoperatively,one patient had arterial crisis,one had venous thrombosis,and one presented sinus formation due to infection.No second plastic surgery was carried out.According to the Baird-Jackson score,the results were excellent in 25 patients and good in five.Conclusion Anterolateral thigh perforator free flaps for reconstruction of traumatic soft tissue defects around the tibia has advantages of easy harvesting and survival,less postoperative complications,less second surgery,and good function recovery.
3.Comparison of detection rates of women’s subclinical thyroid diseases between iodine-deficient regions with iodine supplying and water-borne iodine excess regions

Objective By comparing the detection rates of subclinical thyroid diseases in women(pregnant,lactating and child-bearing age)between iodine deficient regions (supplied iodized salt)and water-borne iodine excess regions (consumed non-iodized salt),and to find the different prevalence of subclinical thyroid disease between two regions under their different iodine source and iodine levels, and to provide reference for screening susceptible population with iodine-related thyroid diseases.Methods The iodine deficiency but salt iodine surpplying regions were selected from six provinces in our country,the local people who were pregnant women,lactating women and 18 to 45 years old women of child-bearing age,a total of 991 cases were investigated.The iodine nutrition levels of the pregnant women were grouped by <150,150-249,and ≥500μg·L-1;the iodine nutrition levels of the lactating women were grouped by <100 and ≥100μg·L-1 .The high water-iodine regions in Shanxi Province were selected,and according to the water-iodine levels 50-99,100-149,149-299,and more than 300μg·L-1 four regions were selected;20 cases of three kinds of people mentioned above were selected in each region,a total of 241 cases.The blood and urine samples were collected, and the serological thyroid function indexes were detected by chemiluminescence immunoassay method or radioimmunity method, and the urine-iodine was detected with cerium catalytic spectrophotometric method. Results In iodine deficient regions and water-borne iodine excess regions, the concurrence rates of subclinical hypothyroidism(hypothyroidism for short)and thyroid antibody positive of women were 2.32% and 4.98%,respectively;accounting to about 1/3 to 1/2 of those subclinical hypothyroidism population.The prevalence rates of subclinical thyroid diseases in the women population between the two regions were 27.55% and 34.85%,respectively;nearly accounting for 1/3 of the women population.The subclinical hypothyroidism detection rates of the three populations in high water iodine regions were significantly higher than those in iodine deficient regions(P<0.05).The lactating women’s detection rates of thyroid antibody positive and subclinical hypothyroidism with antibody positive in high water iodine regions were significantly higher than those in iodine deficient regions(P<0.05).With different iodine sources,when took appropriate iodine,there was no statistical difference of the detection rates of subclinical thyroid diseases among three kinds of women(P>0.05). With the increase of iodine exposure levels,the prevalence of women who suffered from subclinical hypothyroidism and thyroid antibody positive was increased,the coincidence rate was also increased.The detection rates of low T4 concentration and total subclinical thyroid disease of pregnant women in iodine nutrition<100μg·L-1 group were significantly higher than those in iodine nutrition 250-499μg·L-1 group (P<0.05).The detection rates of low T4 concentration and total subclinical thyroid disease of lactating women in iodine nutrition <100μg· L-1 group were significantly higher than those in iodine nutrition >100μg·L-1 group (P<0.05).Conclusion When the iodine intake is appropriate, iodine intakes from salt or from water have no effect on subclinical thyroid diseases. When the iodine intake increases,the prevalence of subclinical thyroid diseases will increase too.
4.A comparison of four correction methods for measuring thyroid volume of children aged 8-10

Objectives To analyze and compare four correction methods for measuring thyroid volume,and to find out the best one for thyroid volume correction of children aged 8-10.Methods In 2012 and 2013,the project was carried out in iodine deficient rural areas (water iodine below 10 μg/L) of Jilin and Gansu Provinces,10 villages in each province were chosen as survey sites,and 40 children of 8 to 10 years old were chosen from each village,half boys and half girls.Utrasonography was adopted to determine the thyroid volume,and height and weight were measured.The thyroid volume was corrected by Body Mass Indicator(BMI),weight and height,body surface,and height,separately.Body Mass Indicator corrected Volume (BMIV),Weight and Height corrected Volume Indicator(WHVI),Body Surface Area corrected Volume(BSAV) and Height corrected Volume Indicator(HVI) were calculated and compared with their original thyroid volumes to find out the best one.Results Datas of 450 and 408 children were collected from Jilin and Gansu Provinces,separately.According to their original thyroid volumes,goiter rates of Jilin and Gansu were 10.4% (47/450) and 0.5% (2/408),respectively.All these four methods could decrease the difference between provinces and sexes.Among the four methods,WHIV and BSAV were abnormal for correction of different ages,heights and weights.HVI had the most normalized distribution and stronger correlation with the original thyroid volumes than others(r =0.961).The HVI corrected results had similar distribution with the original thyroid volume but the data were different.After being adjusted by 1.7 × HVI,the results were close to the original thyroid volumes.Goiters in Jilin and Gansu Provinces,were 9.1%(41/450) and 0.5%(2/408),respectively,after correction.Conclusion The 1.7 × HVI is the most suitable method for correcting the thyroid volume of children aged 8-10.
5.Investigation of thyroid function change and urinary iodine standard of pregnant women

Objectives To study the changes of thyroid function under different urinary iodine levels and pregnant trimesters,and to investigate the urinary iodine standard of pregnant women,taken thyroid function as a golden standard.Methods Early,mid and late trimester pregnant women in the iodine deficient rural areas of Gansu and Jilin Provinces were selected,at least 70 cases were included for each trimester in each province.Pregnant women included were physically healthy;cases should be excluded if their immediate relatives had a thyroid disease history.Urinary iodine,free triiodothyronine (FT3),free thyroxine (FT4),thyroid stimulating hormone (TSH),thyroid peroxidase antibody (TPOAb),thyroglobulin antibody (TgAb),and edible salt iodine from pregnant women's home were tested.The urinary iodine was measured with the method for determination of iodine in urine by As3+-Ce4+ catalytic spectrophotometry (WS/T 107-2006).Thyroid function was tested by chemiluminescence method,salt iodine was determined by direct titration method (GB/T 13025.7-1999).Results Totally 439 pregnant women were investigated in the two provinces.Median of urine iodine was 178.6 μg/L;for the 215 women in Gansu,the value was 156.4 μg/L;for the 224 women in Jilin,the value was 206.4 μg/L.Along with the increase of urinary iodine,the FT3 of Gansu decreased from 5.77 pmol/L to 5.34 pmol/L,TSH increased from the lowest 2.63 mU/L to 3.75 mU/L;the FT3 of Jilin increased from 4.41 pmol/L to 4.45 pmol/L,TSH increased from the lowest 0.92 mU/L to 1.50 mU/L.Five indicators of thyroid function of more than 97% pregnant women were in normal range and FT3 was the most stable in the three trimesters.FT3 of the three triimesters in Gansu was 6.27,5.76,5.28 pmol/L,respectively,and that in Jilin was 4.50,4.42,4.32 pmol/L,respectively.The 5% and 95% quantile were 55.24,581.27 μg/L of urinary iodine,of pregnant women with normal thyroid function.Conclusions Under different urinary iodine levels,pregnant women's thyroid function is relatively stable.TSH has showed an increasing trend with increasing iodine level.Along with the progress of pregnancy,the urinary iodine is different among the three pregnancy periods,but the thyroid function is stable.The pregnant women with normal thyroid function have a urinary iodine level between 50-600 μg/L.
6.Modified Stoppa approach for treatment of anterior pelvic ring fractures

Objective To investigate the feasibility and effects of modified Stoppa approach in treatment of anterior pelvic ring fractures.Methods The therapeutic effects of 21 patients with anterior pelvic ring fractures treated through the modified Stoppa approach from June 2008 to August 2011 were summarized and analyzed.According to Tile classification,there were seven patients with type A2 fractures,six with type B2 fractures,five type B3 fractures and three type C2 fractures.Surgical treatment through the modified Stoppa approach included anterior pelvic ring plates for 14 patients,anterior pelvic ring plates and external fixators for four and anterior pelvic ring plates and sacroiliac joint compressed cannulated screws for three.The fracture reduction and post-operative function were assessed by Matta criteria and Majeed system respectively.Results The incision length of the modified Stoppa approach ranged from 10 to 12 cm ( average,11 cm).The operation time ranged from 70 to 120 minutes ( average,95 minutes).The intra-operative blood loss ranged from 400 to 1 000 ml (average,700 ml ).According to the Matta criteria,all 21 patients obtained sound reduction,of whom 15 patients had bony union after 8-20 months follow-up post-operatively.The Majeed score in all patients showed a gradual increase postoperatively.Conclusions The modified Stoppa approach is characterized by convenience and directness in making incisions,clear operation field,easy reduction,few complications and fast recovery and hence is an ideal choice in surgical treatment of anterior pelvic ring fractures.
7.Clinical application of the 3D heart printing technology: hope or hype?

3D printing technology has been used in the diagnosis and treatment of cardiovascular diseases worldwide,i.e.,preoperative assessment of complex congenital heart disease,comprehensive assessment of novel clinical techniques,management of postoperative complications,and clinical teaching and training.Chinese hospitals introduced cardiovascular 3D printing technology from 2014.Preliminary clinical application showed that 3D printing had a positive significance,howeverit was expensive and caused inevitableloss of image information.Scholars should have an open but critical mind,actively try this technology and improve it.It can be reasonably believed as a good supplement of existing imaging tools in future.
8.R & D of medicine and vaccine for Ebola hemorrhagic fever

The outbreak of Ebola hemorrhagic fever in West Africa in February 2014 is the worst one in the history of the disease.Medicines and vaccines for Ebola have attracted special attention .This study utilizes Cortellis database of Thom-son Reuters and knowledge about Ebola virus to analyze current R&D of medicines and vaccines for Ebola in order to offer references for research and development .Study findings shows that there are mainly 5 important categories:vaccines, neu-tralizing antibodies , small-molecule anti-Ebola drugs , RNAi drugs and nucleotide drugs .In each category , there are prom-ising products under development that are worthy of attention .
9.Changes in the number and distribution of myoepithelial cells during atrophy of rat parotid gland.

OBJECTIVETo investigate the changes in the number and distribution of myoepithelial cells during atrophy of the rat parotid gland.
METHODSAtrophy of the right parotid was induced by ligating the right stensen duct of rats, histological changes of parotid glands were examined by hematoxylin-eosin (HE) staining during each step of glandular atrophy at the time of 0 (control), 1, 3, 5, 7, 14, 21, 30, 60, 100, and 150 days after ligation. Immunohistochemical labelling was performed to study the changes in number and distribution of myoepithelial cells during atrophy of the rat parotid gland.
RESULTSHistological analysis showed disappearance of the acini at 5 d and gradual decrease and fibrosis of the glandular lobules accompanied by the occurrence of duct-like structures. Quantitative analysis of myoepithelial cells showed significant increase in number up to day 5 after ligation, then followed by gradual increases at a low level, at last it was followed by a rapid decrease after the total number reached the peak in 100 days. In addition, the acini and intercalated ducts were covered by myoepithelial cells ranged from the shape of spindle to stellate during the early phase of atrophy, while spindle-shaped myoepithelial cells were located at the periphery of duct-like structures in the later phase of atrophy.
CONCLUSIONMyoepithelial cells proliferated rapidly up to day 5 after ligation, then followed by gradual increase at a low level, at last it was followed by a rapid decrease after the total number reached the peak in 100 days.

10.Detection Rates and Influencing Factors of Atrial Septal Defect and Ventricular Septal Defect Among Live Births in Two Cities of East China

Objective: To investigate the detection rates and inlfuencing factors of atrial septal defect (ASD) and ventricular septal defect (VSD) among neonates in two cities of East China and to provide scientiifc basis for the prevention, diagnosis, treatment and monitor of ASD and VSD. Methods: 2100 newborns with gestational age of at least 28 weeks were recruited consecutively from each city between 2013-09 and 2014-11. Data related to ASD and VSD were collected by questionnaires and echocardiographic screening was conducted within 7 days after birth. Results: A total of 4152 neonateswere examined with gestational age of (39.03 ± 1.29) weeks, among whom 2189 were male infants (52.72%), and age of mother was (26.32 ± 4.10) years old. Detection rates of ASD and VSD were 60.5‰ and 12.8‰ respectively, showing no significant difference between genders (P>0.05). Multivariate logistic regression analysis showed that maternal pre-pregnancy BMI and home decoration were the inlfuencing factors of ASD and maternal drug use in early pregnancy was the inlfuencing factor of VSD among newborns. Conclusions: Detection rates of ASD and VSD among neonates were relatively high in two cities of East China. Early screening is importtant to reduce the incidence of ASD and VSD and improve the prognosis.
