1.Obsoervation of Low Molecular Weight Heparin in Treatment of Diabetic Foot

Objective To observe the effect of low molecular weight heparin(LMWH) in treatment of diabetic foot.Methods Fifty-four patients were divided into test group and control group randomly.Both two groups were given routine therapy including mediating blood glucose with Insulin,anti-infection and changing dressings,while the test group were givon LMWH 5000IU once datly every 12 hours for 15 days besides the routine treatment.Then the endothelin(ET),thromboxaneB2(TXB2),hemorheology,ankle-brachial index(ABI),ulcer healing,fibrinogen(FIB),prothrombin time(PT) and avtivated parital thromboplastin time(APTT) were observed.Results Compared to that of before treatment,there are ET,TXB2,hemorheology and better ulcer heals after treatment,The improvement in test group patients was superior to that of control group(P0.05) after treatment in two groups.No hemorrhage and other complications occurred.Conclusion LMWH is effective and safe for treatment of diabetic foot.

5.Relationship Between Diabetic Foot (DF) and the Change of Blood Fluidity

Objective To study the relationship between diabetic foot(DF)and the change of blood fluidity.Methods The blood viscosity、plasma viscosity、hematocrit、erythrocyte sedimentation rate、fibrinogen were determined in 126 type 2 diabetes cases.They were divided into DF group and non-diabetic foot(NDF)group.Results ①The incidence of DF is higher in older and longer course of diabetes cases with renalpathy、neuropathy、peripheral artery disease and retinopathy.② The blood viscosity、plasma viscosity、hematocrit、erythrocyte sedimentation rate and fibrinogen are significantly higher in DF group.③ The indexes of blood fluidity are significantly higher in Ankle Brachial Index(ABI)abnormal group than normal group.Conclusions To have the blood fluidity under control effectively is very important for prevention and treatment of DF.

7.Adolescent idiopathic scoliosis and melatonin: a survey on scoliosis in 8 198 adolescents from over 10 middle and primary schools in Haikou City

BACKGROUD: It is not consistent on whether melatonin correlates with the adolescent idiopathic scoliosis(AIS) demonstrated by the domestic and foreign scholars.OBJECTIVE: To study the pathogenesis of AIS and its correlation with melatonin.DESIGN: A nonrandomized age-matched controlled study screened with two tests.SETTING: The Second Department of Orthopaedics, Haikou People' s Hospital.PARTICIPANTS: The experiment was completed in the Second Department of Orthopaedics, Haikou People' s Hospital. Totally 8 198 in-school students.from over 10 schools of towns in Haifu District, 4 423 males and 3 775 females, aged 7 to 16 years were surveyed.METHODS: Forty-two adolescents who have been diagnosed with AIS were selected and 50 healthy age-matched adolescents were selected as controls. Melatonin was assayed with the radioimmunology and the data were processed statistically.MAIN OUTCOME MEASURES: The levels of serum melatonin in the two groups.RESULTS: The serum melatonin of the preadolescents below 10 years old in AIS group, especially of females, was less than that of adolescents in the control group, with significant differencein statistics.CONCLUSION: The serum melatonin correlates with pathogenesis of AIS.
8.Application value of tissue dispersion quantitative analysis technique in different stages of SAT

Objective To investigate the application value of the ultrasonic elastic tissue dispersion quantitative analysis technique in different stages of subacute thyroiditis (SAT).Methods One hundred and forty-four SAT lesions detected from 81 patients were enrolled in the patient group.They were further divided into three subgroups,including acute group (group Ⅰ),medium group (group Ⅱ) and recovery group (group Ⅲ).Another 59 healthy volunteers were collected as control group.All the participants accepted conventional ultrasound and elastographic examinations.Eleven parameters were obtained by the tissue dispersion quantitative analysis software.These parameters were compared between groups and among subgroups by ANOVA.The correlation between all the parameters and the course of SAT were analyzed by Spearman and Multiple linear regression methods.Results Between groups and among subgroups,the complexity (COMP) and correlation (CORR) were not statistically different(all P >0.05).Differences of kurtosis (KURT) and angular secon moment (ASM) among the three subgroups were not significant (all P >0.05).Differences between groups and among subgroups were significantly different among the value of all the other seven indexes (all P <0.01).Moreover,they were all correlated with the clinical staging,with the highest coefficient in area ration of low-strain region (% AREA)(r =-0.881).Regression model was constructed and only % AREA was selected into the regression equation.ROC curves were constructed to estimate the clinic value of % AREA in staging patients of SAT,the areas under ROC curves were0.986(group Ⅰ vs group Ⅱ-Ⅲ) and 0.988 (group Ⅰ-Ⅱ vs group Ⅲ[) for %AREA,respectively.Conclusions The tissue dispersion quantitative analysis technique is helpful in estimating the stiffness of thyroid in patients with SAT.
9.Clinicopathologic study of juvenile granulosa cell tumor of ovary.

OBJECTIVETo study the clinicopathologic features, diagnostic criteria and prognostic parameters of juvenile granulosa cell tumor of ovary.
METHODSThe clinical and pathologic findings of 7 cases of juvenile granulosa cell tumor were retrospectively reviewed. Immunohistochemical study was carried out in 6 of these cases. The follow-up data were also analyzed.
RESULTSThe mean age of the patients was 24 years (range=6 to 53 years). Four patients presented with hormonal disturbance, while 3 patients presented with abdominal pain or swelling. Six patients underwent unilateral salpingo-oophorectomy. Six cases were in stage IA and the remaining case in stage IC. Follow-up information was available in 6 patients and the duration of follow up ranged from 1 to 10 years (mean=4.3 years). Five patients remained healthy, with no evidence of tumor recurrence. One patient died of tumor metastasis one year after the diagnosis. Gross examination showed that the tumor masses ranged from 7 to 20 cm in the greatest dimension (average=13.4 cm). Four of the 7 tumors were mixed solid-cystic in appearance and 2 cases were unilocular cystic in nature. Microscopic examination showed diffuse atypical follicular structures formed by granulosa cells. The granulosa cells contained round hyperchromatic nuclei, without nuclear grooves or Call-Exner body formation (6/7). In one of the cases studied, minor foci resembling adult granulosa cell tumor were also demonstrated. The degree of cellular atypia varied (3 cases with severe atypia, 1 case with moderate atypia and 3 cases with mild atypia). The mitotic count ranged from 1 to more than 5 per 10 high-power fields. Immunohistochemical study showed diffuse positivity for vimentin (6/6). The staining for cytokeratin (AE1/AE3) and calretinin was negative. Four cases expressed CD99 and 1 case was positive for inhibin.
CONCLUSIONSJuvenile granulosa cell tumor is characterized by the presence of diffuse atypical follicular structures formed by small round cells, without nuclear grooves or Call-Exner bodies. Rare cases contain minor foci of adult granulosa cell tumor. They can be unilocular cystic in nature. The degree of nuclear atypia, mitotic activity and size of the tumor vary and do not correlate with the risk of recurrence and aggressive biologic behavior.

10.Clinical study:endovascular embolization of intracranial aneurysm with coils

Objective To summarize the technique and managements of complications in endovascular embolization on intracranial aneurysm with Guglielmi detachable coils(GDC),and to evaluate the effect of the treatment.Methods One hundred and thirty six cases with Aneurizym were treated using GDC to embolize the aneurismal sac via femoral artery approach.Results One hundred and thirty six aneurysms were cured.Of them 132 cases recovered clinically,4 patients died.The mortality was 2.9%. The sac of 123 aneurysms were embolizied at 100%,8 cases with 95% embolization,5 with 90% embolization.3 aneurysms reptured during the embolization,cerebral vasospasm happened in 7 eases. microcoil escaped in 2 case.Three recurring cases were cured after second GDC embolization.The technique-related complications occured in 13 cases.No re-bleeding occurred during the 6 to 54-month follow-up.Conclusion Endovascular embolization of intracranial aneurysm with coils is a safe and effective treatment for intracranial aneurysm.Advances in Techniques and treating the complications correctly would decrease the complications and improve future outcomes.
