Objective To summarize the experience in application of the bovine jugular vein valved conduct in right ventricular outflow tract (ROVT) reconstruction. Methods Totally 14 patients [6 male, (6.3?5.4) years old] underwent ROVT reconstruction from February 2004 to March 2005 was recorded, and the function of the implanted bovine jugular vein valved conduit was evaluated by Doppler echocardiography one month later after operation. Results There was no perioperative morbidity related to the bovine jugular vein valved conduit. 14 patients were discharged well. One month after operation, echocardiography showed that reconstructed ROVT was patent, and only four implanted conduits had mild regurgition. Conclusion The bovine jugular vein valved conduit provides the favorable hemodynamic performance and thus could be an excellent substitute for the homograft in the reconstruction of ROVT. The mid- and long-term outcomes required further study.

ZHU Lian is the founder of international communication of acupuncture-moxibustion after establishment of New China. This article discussed ZHU Lian's great deal of pioneering work and historic contributions from the aspects of acupuncture-moxibustion treatments for foreign people, international training and communication, and the international influence of Xin Zhenjiu Xue. Our research showed that ZHU Lian's early work didn't only enlarge acupuncture's worldwide influence and spreading pace, especially in Soviet Union, India and Korea, but also accumulated precious experiences for each subject of Chinese medicine attending into international medical care and academic communication in the future.

OBJECTIVETo explore clinical characteristics and treatment of posterior Pilon fracture.

METHODSFrom January 2011 to January 2013,18 patients with posterior Pilon fracures were treated. Among them, 13 were male and 5 were female, aged from 22 to 63 years old, with an average age of 46. All the patients were closed fractures. Open reduction and internal fixation were performed after swelling subsided, lateral malleolous and posterior Pilon fracture were exposured through lateral approach on healthy side, plates were used to fixed,screws or small plates were used to fix the posterior prominence of medial malleolus after changed to supine position. AOFAS scoring were applied to evaulate clinical effects.

RESULTSAll patients were followed up with an average of 22(ranged, 12 to 48)months. All patients obtained satisfactory reset except one patient. All factures were recovered well with an average healing of 11 weeks. According to AOFAS score at the final following up, 7 cases were excellent,2 cases were moderate, and the total score was 86.8±9.2.

CONCLUSIONPosterior Pilon fracture is not rare in clinical, its mechanism of injury, traumatic anatomy, surgical procedure and prognosis are different from that of classical ankle fracture and Pilon fracture.

Objective To investigate the effect and mechanism of transplantation of marrow multi- function stem ceils in treating femoral head necrosis.Methods Sixty japanese rabbits were divided into A,B and C groups randomly.After creation of the models of hormone induced necrosis of femoral head;A group was designated as the treatment,B as the control and C as the normal groups.The bone marrow of A group was extracted and isolated and then injected into the left femoral head and the right femoral head was decompressed by drilling only.The rabbits were killed at 8 weeks after the treatment and changes in various parameters were observed,including imaging data of molybdenum target films,CT and MRI;routine pathology with HE staining and ultrastructural alteration by election microscopy.Results Eight weeks after the treatment of transplantation of marrow multifunction stem cells,the X-ray showed only a little change but the typical appearances were revealed by CT and MRI.Pathohistologic manifestation demonstrated decrease of empty bone lacuna,increase of osteoblast and new bone formation.Election microscopy displayed abundant organelles in osteoblasts with few empty bone lacuna,in addition the tansplantation of marrow multifunction stem cells could obtain better reconstraction for the involved femoral head.Conclusions The treatment of transplantation of marrow muhifunction stem cells in femoral head necrosis could accelerate the process of repairing,worthy to be acknowledged as a good and valuable management in rabbits.(J Intervent Radial,2007,16:122-126)

In this paper, the antitussive and expectorant activity of platycodin D (PD) were studied by constructing a mouse cough induced by concentrated ammonia water and a mouse trachea phenol red excretion model. The mechanism of antitussive and expectorant effect of PD was studied by metabolomics. The animal experiment was approved by the Animal Ethics Committee of Jiangxi University of Chinese Medicine (approval number: JZLLSC-20220739). Then mice were randomly divided into the normal, model, positive drug, PD low-dose, PD medium-dose and PD high-dose group. The antitussive and expectorant effects of PD were evaluated using a cough mouse model induced by concentrated ammonia water and a mouse tracheal phenol red excretion model, respectively. UHPLC-LTQ-Orbitrap-MS was used to identify the metabolites of mouse lung tissue, and multivariate statistical analysis method of orthogonal partial least squares discriminant analysis (OPLS-DA) was used for metabolites profile analysis. The differential metabolites were screened by variable projected importance value (VIP) and t-test results. Pathways for enrichment of differentiated metabolites were analyzed using the MetaboAnalyst platform. The comparative method was applied to analyze the differences in mechanisms of PD, Deapio-platycodin D (DPD) and total platycosides fraction. The results showed that PD at different concentrations could significantly prolong (P < 0.05) the incubation period of cough mice induced by ammonia water, reduce the coughs frequency, and significantly increase (P < 0.05) the amount of phenol red excretion in phenol red excretion model mice. PD could regulate 6 metabolic pathways of phenylalanine, tyrosine and tryptophan biosynthesis, linoleic acid metabolism, phenylalanine metabolism, glycerophospholipid metabolism, and tyrosine metabolism to exert antitussive effect. It could also regulate 8 metabolic pathways of linoleic acid metabolism, glyoxylic acid and dicarboxylic acid metabolism, glycerol phospholipid metabolism, citric acid cycle and arachidonic acid metabolism to exert an expectorant effect. However, only linoleic acid metabolism and glycerophospholipid metabolism could be regulated by the PD, total platycosides fraction and DPD, which may be ascribed to the structural difference of the platycosides and the interaction between platycosides and the intestinal microbiota. Functional analysis showed that these metabolic pathways are closely related to the regulatory mechanisms of anti-inflammatory response, immune function regulation, neurotransmitter release, cell signal transduction, energy metabolism and cell apoptosis. This study shows that PD possesses good antitussive and expectorant activities. In addition, the mechanism difference of PD, total platycosides fraction and DPD imply that the apiose in PD and the interaction between PD and intestinal microbiota could exert an important effect on the antitussive and expectorant mechanism of the platycosides.

OBJECTIVETo investigate the relationship of seasonal variation with pre- and post-thaw semen parameters as well as the cryosurgical of human spermatozoa.

METHODSA total of 6 414 semen samples were collected from 1 135 donors aged 22 - 32 years by Zhejiang Human Sperm Bank, and divided into spring, summer, autumn and winter groups according to the time of collection. All the samples underwent routine seminal analysis, and the sperm parameters were compared between different seasons. The sperm specimens were cryopreserved in aliquots and analyzed after thawing.

RESULTSThe semen volume was (2.92 +/- 1.17) ml in spring, significantly higher than in summer, autumn and winter ([2.71 +/- 1.07 ], [2.74 +/- 1.15] and [2.83 +/- 1.15] ml, P < 0.05). Sperm density was the highest in autumn ([105.60 +/- 39.76] x 10(6)/ml) as compared with the other three seasons ([101.18 +/- 40.16] x 10(6)/ml, [93.54 +/- 35.10] x 10(6)/ml, and [101.29 +/- 38.37] x 10(6)/ml, P < 0.05). The sperm progressive motility was the highest in spring ([58.49 +/- 10.04] %) and the cryosurgical of sperm the lowest in summer, with statistically significant differences from the other groups (P < 0.05).

CONCLUSIONSeasonal variations affect human semen quality and cryosurgical of sperm. The semen volume, the percentage of progressive motile sperm, the cryosurgical of sperm, and the post-thaw density of progressive motile sperm are higher in spring than during the rest of the year.

Objective To study the expressions of nucleostemin gene mRNA and Ki-67antigen in pituitary adenomas and investigate the role of nucleostemin gene in the tumorigenesis and development of pituitary adenomas. Methods Seventy-one samples of pituitary adenomas were collected. Semi-quantitative reverse transcriptase-polymerase chain reaction (RT-PCR) was used to detect the expression of nucleostemin gene mRNA in samples. Immunohistochemistry technique was applied to examine Ki-67 antigen expression in the paraffin sections of samples. At the same time,coherent clinical data were collected. Results Nucleostemin gene mRNA was detectable in all samples of pituitary adenomas. Between invasive and noninvasive pituitary adenomas, the difference of nucleostemin expression was extremely significant (P＜0.01), and Ki-67 labeling index (LI) was also significantly different (P＜0.05). The difference of Ki-67LI between recurrent and non-recurrent groups was significant (P＜0.05). There was a positive correlation between nucleostemin gene and Ki-67LIlevels (r=0.237, P＜0.05). Conclusions Nucleostemin gene plays an important roles in the invasion of human pituitary adenoma. Expression of nucleostemin gene is positively related to Ki-67 antigen expression, and both may be the valid clinical detection markers for assessing proliferation, invasion and recurrence of pituitary adenomas.

Objective: To explore the feasibility of DDD pacemaker implantation under EnSite system guidance with micro X-ray. Methods: DDD pacemaker implantation under EnSite system guidance with micro X-ray was performed in 5 special patients who needed cardiac pacing while should be avoid X-ray exposure. Results: All 5 patients were smoothly finished DDD pacemaker implantation, no wire dislocation or perforation occurred after the operation. The cumulative skin radiation dose was (2.20±0.45) mGy, operative X-ray exposure time was (11.60±2.07) s and the operation time was (58.40±4.04) min. Conclusion: DDD pacemaker implantation under EnSite system guidance with micro X-ray was safe and feasible.

20?10~9/L.The proteinuria decreased or disappeared.The antinuclear antibody decreased or became negative.The level of complement was increased.The following complications were ob- served:septicemia in 2 patients,cytomegalovirus infection in 2 patients,renal toxicity in 1 patient,acute left heart failure in 3 patients and cardiac arrhythmia in 3 patients.There was no transplantation related mortality. Conclusion APBSCT may improve the disease activity and the immunological markers in SLE.It is a valid therapy for refractory SLE,but the long-term effects need to be observed.

Objective To observe the effects of casein on iodine metabolism in blood,urine and thyroid of mice under excessive iodine.Methods A 2 by 3 factorial design was used in the experiment.The levels of iodine and casein were 50 and 600 μg/L in drinking water and 0( Ⅰ ),10％( Ⅱ ),20％( m ) in food,respectively.After six and twelve months,iodine content in serum,thyroid gland and urine was determined by arsenic-cerium catalyzed spectrophotometry.Results In six months,the levels of serum iodine in 50 Ⅰ,50 Ⅱ,50Ⅲ,600 Ⅰ,600 Ⅱ,and 600Ⅲ groups were (85.59 ± 8.78),(64.59 ± 9.06),(72.53 ± 11.69),(110.04 ± 9.37),(81.06 ± 9.94),(86.63 ± 19.59)μg/L,respectively; the levels of iodine content in thyroid gland were (0.21 ± 0.09),(0.29 ±0.08),(0.24 ± 0.05),(0.50 ± 0.10),(0.37 ± 0.13),(0.42 ± 0.12)g/kg,respectively; the levels of urinary iodine median were 87.5,68.1,105.5,746.5,828.3,1014.2 μg/L,respectively.The variance analysis results of factorial design showed iodine and casein significantly influenced the serum iodine level(F =27.95,18.52,all P ＜0.05),however,there was no interaction between the two(F =0.81,P ＞ 0.05); iodine significantly influenced the iodine content in thyroid gland(F =31.35,P ＜ 0.05),the presence of iodine interacted with casein(F =3.34,P ＜0.05).In twelve months,the levels of serum iodine were (88.54 ± 12.33),(72.45 ± 7.73),(72.93 ± 13.61),( 106.26 ± 12.00),(90.03 ± 7.90),( 104.88 ± 11.67)μg/L,respectively; the levels of iodine content in thyroid gland were (0.58 ± 0.12),(0.40 ± 0.14),(0.69 ± 0.16),(0.84 ± 0.13),(0.89 ± 0.13),(1.02 ± 0.11 )g/kg,respectively;the levels of urinary iodine median were 104.8,121.5,102.7,829.1,1080.8,895.2 μg/L,respectively.The variance analysis results of factorial design showed iodine and casein significantly influenced the iodine content in serum and thyroid gland (F =42.78,7.42 and 66.62,7.90,all P ＜ 0.05),however,there was no interaction between the two(F =1.93,2.31,all P ＞ 0.05).Conclusions Long-term intake of iodine can significantly increase the iodine content in serum,thyroid gland and urine,but casein may accelerate iodine excretion and partly reduce the effect.
