1.Empirical Analysis on Price Policy Compensation Status in 4 Years after the Drug Zero Profit Policy

Objective:Retrospective analysis was conducted on the status of drug profit decreasing and pricing compensation profits increasing after the implementation of drug zero profit policy.The ratio of pricing policy compensation in place and the reform effects were evaluated to provide references for poliey making on implementing drug zero profit policy.Methods:The total usage of western medicine and Chinese patent medicine,the item of medical charging costs and item times before and after the implementation of drug zero profit,the decreasing of drug profits by outpatient and inpatient and the increasing of medical items were selected from hospital information system from 2013 to 2016.Results:From 2013 to 2016,the price of compensation policy in place rates were 94.59%,89.51%,95.18%,95.64%;4 year total compensation policy in place rate was 93.67%;outpatient compensation mainly by inspecting fee (75.22%),the cost of treatment (11.18%),surgery (5.91%);hospital compensation mainly surgical fees,nursing fees (36.40%)(19.09%),(19.08%) medical expenses and treatment expenses (13.53%);a good compensation effect of price policy.Conclusion:The compensation of price policy was in accordance with the expectation of reform,and the sharing of information with the competent department of price could help to establish a scientific and reasonable price compensation policy.It needed to further promote the reform of drug and comsumable usage control.
2.Preparation and Characterization of monoclonal antibodies to rabies virus CVS-11 strain

Objective To get the high specific and sensitive mononclonal antibodies against RV. Methods The myeloma cell line SP2/0 fused with the spleen cell of 6 - 8 weeks old BALB/c mice immunized with the CVS-11 virus antigen. The hybridized fusing cells were chosen by indirect ELISA detection and the positive hybridizing cells were amplified through mouse abdomen injection and the mouse McAbs ascites was purified by Protein A Sepharose 4 Fast Flow(Pharmacia Company). The specificity and sensitivity of the McAbs was identified by indirect ELISA and indirect DFA test. Results The cell fusion rate reachs 100% and the indirect ELISA results showed that the McAbs ascites titer were 1 ×104,1 ×105, 1×104 and 1×105;The immunoglobulin G type McAbs show no cross reaction with other related viruses. Conclusion The high specific and sensitive mononclonal antibodies of RV can be used for rapid RV diagnosis.

Objective To discuss the optimal clinical diagnosis and treatment of ectopic ACTH syndrome with occult tumors. Methods Clinical features, imaging examinations and treatment of 17 patients with ectopic ACTH syndrome were described and compared. Results All patients illustrated the typical clinical features of Cushing’s syndrome. They had hypokalemic alkalosis, elevated serum cortisol and plasma ACTH levels. In the high-dose dexamethasone suppression tests, most patients failed to suppress serum cortisol and 24-hour urinary cortisol. CT and MRI are useful imaging modalities to localize the ACTH-secreting tumor in patients with ectopic ACTH syndrome. The patients with overt ACTH-secreting tumors had surgical curative resection soon after diagnosis. Among patients with occult ACTH-secreting tumors, three underwent subtotal bilateral adrenalectomy, two underwent right adrenalectomy, four received inhibitor of steroidogenesis aminoglutethimide. Their hypercortisolism was controlled. Conclusion Surgical curative resection is the optimal treatment of ectopic ACTH syndrome with overt ACTH-secreting tumor. Bilateral adrenalectomy, right adrenal ectomy or chemotherapy to control hypercortisolism is an available treatment of ectopic ACTH syndrome with occult ACTH-secreting tumors.
4.Diagnosis and management of Crohn disease complicated with gastrointestinal fistulae.

OBJECTIVETo investigate the diagnosis and treatment of patients with Crohn disease (CD) complicated with gastrointestinal fistulae.
METHODSClinical data of sixty-two cases with CD complicated with gastrointestinal fistula e from 1978 to 2004 were analyzed.
RESULTSThese were 68 external fistulae in 6 2 patients including recurrent fistulae in 6 cases, internal fistulae in 8 cases . Twenty- seven fistulae were located in the terminal ileum and 21 fistulae wer e located in ileocolic anastomosis site. The main surgery included 14 ileocecal resections with primary anastomosis and 26 resections of original ileocolic anastomosis with fistula and re-anastomosis. The incidence of recurrence was lower (15.4% ) in patients with postoperative medication including sulfasalazine and immunomodulator than that (34.8% ) in patients without postoperative immunomodulator,but the recurrence time was longer [(40+/- 17) months] in patients with postoperative medication than that [(8+/- 3)months] in the patients without postoperative specific medication.
CONCLUSIONSMost CD fistulae are external fistulae,most of the external fistulae are treated by resection of the fistula and anastomosis. Specific medication including sulfasalazine,mesalamine and immunomodulators should be used to prevent postoperative complications and CD recurrence.

5.Role of miR-126 in effect of biological behaviour and epithelial to mesenchymal transition (EMT) in human colon cancer cells

Objective:To investigate the expression of miR-126 in human colon cancer cell lines with different metastatic potential and its effect on the proliferation, invasion and metastasis of colon cancer cells, and to explore the possible mechanism.Methods:The expression of miR-126 in human colon cancer cell lines(SW480,SW620 and HCT116) was determined by Real-time fluorescence quantitative PCR.miR-126 mimics were transiently overexpressed in SW620 cells throuIgh liposome transfection and the negative control group was set up.The proliferation ability of cells was detected by CCK8 method and the mobility of cells was detected by wound healing assay and Transwell migration and invasion assay.The expression of E-cadherin,Vimentin was determined by Western blot.Results:The expression of miR-126 was decreased in SW620 and HCT116 cells with high metastatic potential compared SW480 cells with low metastatic potential.The overexpression of miR-126 significantly inhibited the proliferation,migration and invasion ability of SW620 cells and Western blot indicated that miR-126 overexpression increased the expression of E-cadherin and decreased the expression of Vimentin in SW620 cells,which was significantly different from that of negative control(P<0.05).Conclusion:Low expression of miR-126 is closely related to metastasis of colon cancer and the effect of miR-126 on the biological behavior of colon cancer cells may be mediated by the regulation of the EMT process.
6.Study of PAK4 induces epithelial-mesenchymal transition of breast cancer cells

[Abstartc] Objetcive: To analyze the effect of p21 protein activated kinase 4( PAK4) on epithelial-mesenchymal transition of breast cancer cells.Met hods: The mRNA and protein expression of PAK4,E-Cadherin,N-Cadherin and Vimentin were detected by qRTP-CR and Western blot in breast cancer cells transfected with pcDNA -PAK4 or siRNA PAK4;The biology behaviors of MCF-7 cells and MDA-MB-231 cells transfected with pcDNA-PAK4 or siRNA PAK4 were analysed by cell migration assay ,invasion assay.Results:Overexpressing PAK4 could significant increase in the migration and invasion compared with vector-infected cells, decrease the expression of epithelial marker E-cadherin and upregulate the expression of mesenchymal markers N-cadherin and Vimentin in detached MCF7 cells.Silenced PAK4 gene in detached MDA-MB-231 cells could suppress the migration and invasion ,decrease the levels of the mesenchymal markers and increased the levels of the epithelial markers at both mRNA and protein levels .Conclusion:PAK4 plays a key role in EMT of breast cancer cells ,and its promoting EMT effect associated with upregulating the expression of Slug .
7.Study on mortality, incidence and risk factors of stroke in a cohort of elderly in Xi'an, China.

OBJECTIVETo prospectively study the relationship between risk factors and stroke incidence and mortality in the Chinese elderly.
METHODSAn analytic study in a cohort population of 1,268 male retired cadres in a military setting. A health-screening program was carried out for all cadres aged 55 years or older in Xi'an in February 1987. Baseline data on smoking, cholesterol, triglyceride, blood pressure, body mass index (BMI), histories of hypertension, hyperlipidemia and coronary heart disease (CHD), stroke, diabetes and family histories of cardiovascular disease were investigated. Main outcome measures were stroke incidence, stroke and all-cause mortality.
RESULTSThe cohort was followed up until June 30, 2001 and a total number of follow-up person-year was 15,546. During the follow-up period, there were 113 new stroke cases identified and the adjusted incidence was 727 per 100,000 person-year. Forty-five deaths were due to stroke and the adjusted mortality was 289 per 100,000 person-year. Using Cox model analysis, after adjustment on age, total cholesterol, triglyceride, smoking, drinking and physical exercise, we noticed that the systolic pressure, BMI, history of CHD and hyperlipidemia were independent risk factors for stroke incidence and morality.
CONCLUSIONThe incidence and mortality of stroke in this cohort were lower than those in the same age group of general population. Monitoring and controlling body mass index and blood pressure level seemed to be important factors for the prevention of stroke in the elderly.

8.Establishment of an artificial neural network model for analysis of the influence of climate factors on the density of Aedes albopictus.

OBJECTIVETo establish a model for predicting the density of Aedes albopictus based on the climate factors.
METHODSThe data of Aedes albopictus density and climate changes from 1995 to 2001 in Guangzhou were collected and analyzed. The predicting model for Aedes albopictus density was established using the Artificial Neural Network Toolbox of Matlab 7.0 software package. The climate factors used to establish the model included the average monthly pressure, evaporation capacity, relative humidity, sunshine hour, temperature, wind speed, and precipitation, and the established model was tested and verified.
RESULTSThe BP network model was established according to data of mosquito density and climate factors. After training the neural network for 25 times, the error of performance decreased from 0.305 539 to 2.937 51x10(-14). Verification of the model with the data of mosquito density showed a concordance rate of prediction of 80%.
CONCLUSIONThe neural network model based on the climate factors is effective for predicting Aedes albopictus density.

9.Perpetual impact of pregnancy-induced hypertension on blood pressure

Objective To explore the perpetual impact of pragnancy-induced hypertension on blood pressure. Methods This retrospective cohort study included 782 cases of pregnant women who hospitalized at Kailuan Linxi hospital between October 1976 and August 2001. Patients were divided to with pregnancy induced hypertension(PIH, n=77) group and non pregnancy induced hypertension (NPIH, n = 705) group. Patients were followuped for 5 to 34 years ( mean 18.8±5.3 years), the incidence of essential hypertension was obtained in July 2006 - September 2007. Results (1) The cumulative incidence of essential hypertension during follow up was significantly high in PIH group (29.87%) than that in NPIH group 18.87% (P =0.022). (2) At the final follow up, waist circumference; [(86.06±10.15) cm vs. (83.07±8.19) cm, P=0.015], BMI [(24.83±4.01) kg/m~2 vs. (23.50±3.39) kg/m~2, P = 0.006], TC[(5.11±0.88) mmol/L vs. (4.89±0.94) mmol/L, P = 0.045] and GLU [(5.57±1.78) mmol/L vs. (5.20±1.38)mmol/L, P = 0.010] were all significantly higher in PIH group than those in NPIH group. (3) After adjudgment of age and BMI, PIH was still significantly correlated with long-term systolic blood pressure levels (P = 0.048), fasting glucose level was also significantly associated with long-term systolic blood pressure. Age, BMI, white blood cell count and uric acid were also predictors for perpetual systolic and diastolic blood pressure levels. Conclusions Incidence of essential hypertension in women with PIH was higher than that in women without PIH. After edjudgment of covariates including age, BMI, and glucose, PIH was significantly associated with the level of systolic blood pressure. BMI, fasting glucose and cholesterol levels might contribute to the increase of systolic blood pressure in patients with PIH.
10.Anti-tumor effects of vascular endothelial growth factor fusion protein vaccine modified by a molecular adjuvant

Objective To investigate whether 2 repeats of mycobacterial microbial hot shock protein 70 (HSP70) 407-426 (2mHSP70407-426,M2) could act as an effective molecular adjuvant to enhance the anti-tumor efficiency of vascular endothelial growth factor (VEGF) fusion protein vaccine.Methods The gene encoding two tandem repeat sequences of HSP70407-426 was introduced to the terminus of the constructed hVEGF121 mutant 1 gene to prepare a recombined hVEGF121 mutant 2 protein vaccine by PCR technique.The anti-tumor efficacy of hVEGF121 mutant 2 protein vaccine was investigated using a H22 liver cancer subcutaneous tumor model.The 24 mice were randomly divided into three groups:model group,experiment 1 group (80 μg hVEGF121 mutant 1 protein) and experiment 2 group (80 μg hVEGF121 mutant 2 protein).The humoral and cellular immune responses were detected by ELISA and splenic lymphocyte proliferation assay.The anti-angiogenesis effect was evaluated by an intradermal tumor model.Results The tumor weight of the experimental 1 group and the experimental 2 group were (1.17± 0.25) g and (1.58 ± 0.28) g.Compared with the experimental 1 group,the tumor weight of tumor bearing mice in the experimental 2 group was lower significanfly(P <0.05).The anti-VEGF antibodies of the experimental 1 group and experimental 2 group was 0.54 ± 0.09,0.74 ± 0.1.Compared with experimental 1 group,higher titers of anti-VEGF antibody were detected in immune serum of the experimental 2 group with statistically significant(P <0.05).The proliferative activity of spleen lymphocytes in the experimental 1 group and the experimental 2 group was 0.26 ±0.03,0.36 ±0.04.Compared with the experimental 1 group,the vaccine immunization in experimental 2 group can stimulate the proliferation of spleen lymphocytes more effectively with statistically significant (P < 0.05).Conclusion M2 could act as an effective adjuvant to help VEGF fusion protein vaccine to elicit a strong anti-tumor efficacy.
