Helicobacter pylori(Hp) is an important pathogenic factor in chronic gastritis,peptic ulcer and gastric cancer.With the extensive use of broad-spectrum atibiotics in recent years,the antibiotic resistence of Hp was increasing,together with many side effets and low patient compliance,so the study of new anti-Hp drugs became a research hotspot.Domestic and foreign scholars had extracted active anti-Hp ingredients from the traditional Chinese drug curcuma,they hope to open a new way for the treatment of Hp infection.This paper was to make a brief overview on the study progress of effect of curcuma on anti-Hp.
2.Analysis of 18F-FDG PET/CT imaging in infectious mononucleosis

Objective:To summarize the characteristics of 18F-fluorodeoxyglucose (FDG) PET/CT imaging in adult patients with infectious mononucleosis (IM). Methods:18F-FDG PET/CT imaging data and clinical data of 5 IM patients (all males, age 18-77 years) detected in the Department of Nuclear Medicine of the East Hospital Affiliated to Tongji University from July 2015 to July 2019 were analyzed retrospectively. Distribution of lymphatic tissues in the lesions (lymph nodes, tonsils, bone marrow, liver, etc) and 18F-FDG radioactive uptake in the lesions were analyzed semiquantitatively. Results:In 5 patients with IM, the volume of superficial lymph nodes (mainly distributed in the neck, axilla, and groin lymph nodes) and deep lymph nodes (mainly distributed in the mediastinum, hilum of lung, abdominal cavity and posterior abdominal cavity) increased to varying degrees, as well as the FDG uptake. Maximum standardized uptake value (SUV max) of superficial lymph nodes were 1.2-7.3, and those of deep lymph nodes were 3.5-9.7. All patients showed diffuse uptake of FDG in bone marrow, with SUV max of 3.0-7.9. All patients had pharyngeal tonsillar enlargement and FDG uptake increasing, with SUV max of 1.7-13.4. Compared with patients with lymphoma, IM patients had more organ involvement and relatively lower radioactive uptake. Conclusion:18F-FDG PET/CT imaging features of IM can help the diagnosis of IM.
3.Determination of verbascoside in herba of Galeobdolon Chinense by RP-HPLC.

OBJECTIVETo determine Verbascoside in Galeobdolon chinense.
METHODHPLC method was used, with the column packed with Kromasil C18(4.6 mm x 150 mm, 5 microns). The mobile phase was a mixture of acetonitrile and 1% acetic acid in a ratio of 13:87, and the ultraviolet wavelength was set at 350 nm.
RESULTThe average recovery was 97.0%, the RSD being 1.69%.
CONCLUSIONThis method is sensitive and reliable. It can be used to determine being Verbascoside in Galeobdolon chinense.

4.Effect of thymosin alpha1 on immunological function and metabolism in peritoneal sepsis rats.

OBJECTIVETo investigate the effect of thymosin alpha1 on immunological function and protein metabolism in peritoneal sepsis rats.
METHODSWe observed the effect of thymosin alpha1 on T cell subclassification, TNFalpha, IL-6, IL-10, albumin, C-reactive protein (CRP) and mortality in cecal ligation and puncture (CLP) induced septic rats.
RESULTSConcentration of TNFalpha and IL-6 in CLP induced septic rats increased significantly, and concentration of IL-10 decreased significantly compared to control group. Thymosin alpha1 significantly decreased TNFalpha, IL-6, and significantly raised concentration of plasma IL-10, percent of CD(3), CD(4), and CD(4)/CD(8) in septic group. Thymosin alpha1 reduced degressive degree of albumin. Concentration of CRP increased in both septic groups, but was less prominently in thymosin alpha1 treated group. Thymosin alpha1 reduced cumulate 7-day mortality.
CONCLUSIONThymosin alpha1 can improve immunological function, inflammation condition and protein metabolism.

5.Role of NF-kappa B in cancer cachexia.

OBJECTIVETo assess the putative involvement of NF-kappaB and pro-inflammatory cytokines in the pathogenesis of cancer cachexia and the therapeutic efficacy of indomethacin (IND) on cachexia.
METHODSThirty young male BALB/c mice were divided randomly into five groups: A, control; B, tumor-bearing plus saline; C, tumor-bearing plus IND (0.25 mg/kg); D, tumor-bearing plus IND (0.5 mg/kg); and E, tumor-bearing plus IND (2.0 mg/kg). Colon 26 adenocarcinoma cells of murine were inoculated subcutaneously to induce cachexia. Saline and IND were given intraperitoneally daily for 7 days from the onset of cachexia to sacrifice. Food intake and body composition were documented, serum TNF-alpha and IL-6 levels and activity of NF-kappaB in spleen were investigated in all animals.
RESULTSCachexia was observed in all tumor-bearing mice. No difference was found between groups in food intake (P > 0.05). By day 16, body weights of non-tumor mice were about 82.0% of healthy controls (P < 0.01), and the weight of gastrocnemius was decreased by 28.7% (P < 0.01). Gastrocnemius weight was increased markedly (P < 0.01) after treatment of IND (0.5 mg/kg). Tumor-bearing caused a significant increase in serum TNF-alpha and IL-6 levels (P < 0.01). The concentration of TNF-alpha (P < 0.05) and IL-6 (P < 0.01) in tumor-bearing mice was reduced after administration of 0.5 mg/kg IND for 7 days. NF-kappaB activation in the spleen was increased in tumor-bearing mice in comparison with controls. NF-kappaB activity was reduced in mice treated with IND. The maximal inhibition was observed at an dosage of 0.5 mg/kg (P < 0.01). Liner positive correlation was found between NF-kappaB activity and cytokine levels (r(TNF-alpha) = 0.918, P(TNF-alpha) = 0.028; r(IL-6) = 0.884, P(IL-6) = 0.046).
CONCLUSIONSCachexia induced by colon 26 adenocarcinoma cells may be partially attributed to the enhanced TNF-alpha and IL-6 levels which is controlled by NF-kappaB. IND may inhibit the activation of NF-kappaB, decrease serum TNF-alpha and IL-6 levels and thus alleviate the cachexia.

6.Effects of intraoperative hybrid procedures for the treatment of congenital heart disease.

OBJECTIVEA hybrid operation is a joint procedure involving the interventional cardiologist and the cardiac surgeon concomitantly to optimize surgical management for complex congenital heart disease. We analyzed the feasibility and efficacy of this procedure in 20 patients with congenital heart disease.
METHODSData from 20 patients who underwent intraoperative hybrid procedures from March to October 2005 were analyzed. Two groups were identified: (1) Balloon valvuloplasty group: pulmonary atresia with intact ventricular septum in neonate (n = 3) and severe pulmonary stenosis in infants (n = 4); (2) Device closure group: atrial septum defects (ASD) in child (n = 7), adult ASD (n = 3) and multiple ventricular septum defect (VSD, n = 3). Pulmonary balloon dilation under the guidance of echocardiography together with modified Blalock-Taussig shunt procedure and PDA ligation were applied to patients in balloon valvuloplasty group. For ASD patients, the occluder was placed via right atria after opening the chest either with median sternotomy (4 case) or with submaxillary minimally invasive incision (6 cases), PDA ligation was performed in 1 patient and 2 coronary artery bypass surgeries were performed in 2 adult ASD patients complicating CHD. For multiple VSD, perimembranous VSD was closed surgically and muscular VSD closed by occluder via tricuspid valve approach under cardiopulmonary bypass.
RESULTSAll operations were successful and 1 infant received another conventional valvuloplasty due to the pulmonary restenosis after initial balloon valvuloplasty and 1 patient's muscular VSD was closed by conventional procedure after unsuccessful device closure. No device-related complications were found during follow-up (1-6 months).
CONCLUSIONOur results showed that intraoperative hybrid procedure was feasible and effective in selected patients with congenital heart disease.

7.Studies on chemical constituent and quantitative determination of Mallotus apelta.

OBJECTIVETo investigate the chemical constituents and establish a quantitative method of Mallotus apelta.
METHODCompound was isolated by silica gel, Sephadex LH-20 column chromatography and Pre-HPLC chromatography. Its structure was identified by physicochemical properties and spectral evidences. The content of M. apelta was determined by HPLC. Chromatographic conditions included Inertsil ODS-3 C18 column (4.6 mm x 150 mm, 5 microm) and the mobile phase consisting of a mixture of methanol-water (24:76). The detection wavelength was set at 335 nm.
RESULTOne compound was isolated from n-butanol extract of the M. apelta and its structure was identified as vicenin II. The calibration cure was linear in the range of 0.053-10.60 microg (r = 0.9999), the average recovery was 99. 32%, RSD 1.82% (n = 6).
CONCLUSIONThe compound was isolated from this plant for the first time. The method to determine the content of vicenin II by HPLC was established for the first time. This method is simple, accurate and reliable.

8.Laparoscopy-assisted total/subtotal colectomy: a report of 150 cases.

OBJECTIVETo summarize the clinical experience and to evaluate the safety and technical characters of laparoscopy-assisted total/subtotal colectomy(LAC).
METHODSFrom June 2005 to May 2008, a total of 150 cases underwent LAC in Jinling Hospital. There were 126 cases of severe functional constipation (SFC) treated by LAC combined with modified Duhamel procedure. There were 11 cases of familial adenomatous polyposis, 8 cases of ulcer colitis and 5 cases of multiple colorectal tumors treated with LAC combined with ileum-rectum anastomosis.
RESULTSOf the 150 cases, LAC was successfully performed in 147 cases. The mean operation time of LAC was (76.0+/-23.5) min. The estimated operative blood loss was (35.4+/-10.9) ml. The length of assisted incision was (5.2+/-1.1) cm. The time to resume intestinal function was (42.5+/-12.6) h. There was no postoperative wound infection, anastomotic stoma and other complications. Within 1 week after operation, severe diarrhea occurred in 2 cases and incomplete small bowl obstruction in 2 cases, who were relieved by conservative treatment. The mean postoperative hospital stay in patients without complication was(7.2+/-1.5) d. One year later, the score of gastrointestinal index quality of life in 116 patients was (110.3+/-20.7), which was significantly elevated as compared with the pre-operation score (90.1+/-23.8) and was not significantly different with the healthy score. The constipation symptoms were released in all the 105 SFC patients with different degree diarrhea in 7 cases and recurrence of mild constipation in 4 cases. There was no sign of recurrence and metastasis in 3 cases of multiple colorectal tumors.
CONCLUSIONSLAC embodies the advantages of less invasiveness of laparoscopy. It is a reasonable choice of operation method in treating various diseases related to total colon.

9.Repairing tibial post-traumatic osteomyelitis with bone and skin defect by Ilizarov technique at stage I.

OBJECTIVETo explore clinical effects of Ilizarov technique at stage I for repairing tibial post-traumatic osteomyelitis with bone and skin defect.
METHODSFrom June 2010 to December 2013,44 patients with tibial post-traumatic osteomyelitis with bone and skin defect were treated with Ilizarov technique at stage I . Among them, there were 35 males and 9 females aged from 18 to 70 years old with an average of 42.5 years old. Bone defect ranged from 4 to 16 cm, skin defect ranged from 3 cm x 4 cm to 5 cm x 16 cm. The operation was performed debridement thoroughly, removed inflammatory bone section, osteotomy invasively, install circular external fixator by Ilizarow technique; screw nut were rotated at 1 week after operation, and prolonged 0.5 to 1.0 mm everyday. Wound surface, new born callus and bone healing were observed to evaluate clinical effects.
RESULTSAll patients were followed up from 11 to 36 months with an average of 18.5 months. Bone defect after osteotomy was from 6 to 22 cm with an average of 11.5 cm; the time of wound healing time ranged from 21 to 79 d with an average of 38 d; bone defect healing time was from 8 to 15 months with an average of 12.5 months. All patients were cured, no recurrent infection, refracture and shorten of calf deformity were occurred.
CONCLUSIONRepairing tibial post-traumatic osteomyelitis with bone and skin defect by llizarov technique at stage I has advantages of less trauma, low inflammatory recurrence rate, could avoid multiple complex operation, and receive definite curative effect.

10.Arteriovenous neoadjuvant FLEOX chemotherapy with nutritional support improves the resectable rate for advanced gastric cancer.

OBJECTIVETo summarize the resectable rate of advanced gastric cancer characterized by abdominal lymph node metastases with the arteriovenous neoadjuvant FLEOX chemotherapy plus appropriate nutritional support.
METHODSArteriovenous neoadjuvant FLEOX chemotherapy was administered to 50 patients of gastric cancer with advanced abdominal lymph node metastases. Of the 50 patients, 42 had never received any previous therapy and preoperative CT scanning revealed unresectable tumors because of advanced lymph node (station No.3, 7, 9, 12) or distant lymph node (No.14,16) metastases. The other 8 were characterized with relapse of severe lymph node metastases or with unresectable lymph node metastases demonstrated by exploratory laparotomy. Arteriovenous neoadjuvant FLEOX chemotherapy was conducted as follows: from day 1 to day 5, 5-FU 370 mg/m(2) and leukovorin 30 mg/kg intravenously, at day 6 and day 20, CDDP 70 mg/m(2) and epotoside 70 mg/m(2) intraarterially. This FLEOX regimen was repeated every five weeks for two or three courses. Out of the 50 patients,12 malnutritional cases received parenteral and/or enteral nutritional support according to the nutritional condition. Arteriovenous neoadjuvant FLEOX chemotherapy was then administered after the improvement of nutritional state. Their nutritional support methods were adapted to their chemotherapy as well.
RESULTSAll the patients' general conditions and symptoms were improved significantly. For the 50 cases, the imageological and histological response rate (CR+PR) was 84.0%, and curative resection rate was 78.0%. Thus, 39 patients underwent subtotal or total gastrectomy, even combined organ resection, with D(2)+alpha or D(3) lymphadenectomy. Despite neoadjuvant chemotherapy, all malnutritional cases had significant weight gain after nutritional support, and other nutritional indexes,such as serum albumin, also resumed to normal.
CONCLUSIONArteriovenous neoadjuvant FLEOX chemotherapy proves favorable therapeutic effect for gastric cancer with advanced abdominal lymph node metastases, and downstages inoperable metastatic lymph nodes for radical operation. This combined modality regimen and nutritional support may play an important role in the treatment of advanced gastric cancer.
