1.Clinical application of repairing donor site of abdomen flap by V-Y flap pedicled with deep circumflex iliac artery perforator musculocutaneous

Objective To research the clinical outcomes of repairing donor site of abdomen flap by V-Y flap pedicled with deep circumflex iliac artery perforator musculocutaneous.Methods Nine cases with skin defects of hand were treated with lower abdomen flap from December, 2011 to March, 2013, abdominal donor sites could not be directly sutured, and pedicled with deep circumflex iliac artery perforator musculocutaneous.The flap was 8 cm × 16 cm-12 cm × 24 cm in size.Results The average healing time of the V-Y flap pedicled with deep circumflex iliac artery perforator musculocutaneous was 17 days.The patients were followed-up for 3 months to 18 months, averaged of 9 months.The appearance and the texture of the flaps were good.Abdominal wound healing was flat, with no significant depression and navel no significant skew.Patients were satisfied with function and appearance.Conclusion This procedure is easy and effective.The treatment result is satisfactory.It is improvement for repairing donor site of tradition abdomen flap.
2.Clinical application of incision cataract extraction after manual fragmentation and emulsification of nucleus for traumatic lenssubluxation and cataract

AIM: To observe the surgical effects of the taumatic lens subluxation and cataract after manual fragmentation and emulsification of nucleus and foldable intraocular lens implantation.
METHODS: A 3. 0mm tunnel limbus incision was operated through the predicted bulbar conjunctiva and sclera on 26 cases ( 26 eyes ) with taumatic lens subluxation ( suspensory ligament rupture range less than 120 ) and cataract (Ⅰ ~ Ⅲ) . And after the manual fragmentation and emulsification of nucleus, foldable intraocular lens was implantated. Intraocular lens loop was imbedded in the middle of the lens zonular ligament breakup to reset the pouch. The surgical complications and postoperative vision changes were observed.
RESULTS:Three month after operation, 22 eyes had a intraocular lens centric position taking up 85% of the whole. Four eyes had a slightly eccentric position ( 1 ~2mm), taking up 15% of the whole. 21 eyes had their visual acuity 0. 5~0. 8, taking up 81% of the whole. Five eyes of visual acuity was 0. 2~0. 8. Within 24h intraocular pressure of 12 eyes (46%) after operation were elevated, and returned to normal after 2~7d. There was no severe complication during operation and postoperation.
CONCLUSION: The manual fragmentation and emulsification of nucleus and foldable intraocular lens implantation of the traumatic lens subluxation and the cataract through the 3. 0mm corneal sclera limbus tunnel incision is a simple and effective surgery.
3.Analysis on the epidemiologic characteristics of human rabies in all the 31 provinces in China, from 1991 to 2005.

OBJECTIVETo study the epidemic intensity and trend of human rabies from 1991 to 2005 among 31 provinces, metropoli and municipalities in China so as to increase the awareness of the disease.
METHODSContrastive analyses were performed and the annual publishing data by the Chinese Center for Disease Control and Prevention were used.
RESULTSThe total number of reported cases was 14 942 from 1991 to 2005 with an annual average mortality rate as 0.080/100000. The increase of five-years mortality ratio on relative ratio with circular base of mortality rate were--66.24% (1996-2000 to 1991-1995) and 506.13% (2001-2005 to 1996-2000). When comparing incidence rates between 2000-2005 and 1991-1995, the relative ratio with fixed base increase became 104.62%.
CONCLUSIONAmong the 31 provinces, metropolis and municipalities, 27 had reported human rabies cases. The enzootic areas mainly distributed in the drainage area along the Yangtze River. The incidence rates of Guangxi, Hunan, Guizhou, Jiangxi and Guangdong were the highest.

4.Differentiation of human embryonic stem cells into neural stem cells in vitro

Objective To explore a kind of simple and high efficient approach to differentiate human embryonic stem cells(hESCs)into neural stem cells(NSCs).Methods hESCs were cultured in bacterial culture dish filled with serum free medium to gain embryoid bodies.Then the mature embryoid bodies grew in the special medium including B27 and noggin by adherent culture to differentiate into NSCs. Results The hESCs kept floating in the bacterial culture dish and growing all the time and then formed mature embryoid bodies 7 to 10 days later.The embryoid bodies could be differentiated into highly pure (96.4%)nestin positive cells.And these cells were differentiated into all kinds of neural cells if cultured further.Conclusions This kind of method is less time-consuming,cheaper,and more efficient than those of the results in literatures reported.It affords very good source of seed cells for cell transplantation therapy in the future.
5.Study on the relationship between passive smoking and blood lipids, fibrinogen and viscosity among women who never smoke.

OBJECTIVETo determine whether blood lipids profile, fibrinogen and viscosity were associated with passive smoking (i. e. environmental tobacco smoke, ETS) in Chinese women who never smoke.
METHODSIn Xi'an, China, a case-control study was carried out on 115 cases of coronary heart disease (CHD) defined by coronary arteriography (CAG) and 208 non-CHD controls confirmed by CAG and/or exercise electrocardiography. Data on exposure to ETS, defined as exposure from cigarettes smoking husband or co-workers or both for at least 5 years, was obtained through standardized interviews. Standard laboratory methods were used and the lipid measurements were under US CDC quality control programs.
RESULTSIn the subjects defined by CAG, the levels of high density lipoprotein cholesterol (HDL-C), HDL2C, apolipoprotein (apo) A1 among passive smokers appeared lower than those in non-passive smokers,but the low density lipoprotein cholesterol (LDL-C), apoB, apoB/A1, fibrinogen, plasma and whole blood viscosity were higher than that in non-passive smokers. There were positive associations of the numbers of coronary arteriosclerosis with the levels of blood lipids,fibrinogen and viscosity. In the non-CHD controls, 81 subjects were not exposed and 127 were exposed to ETS. The P values of t-test for the adjusted (for age, body mass index, present diseases history) means between two groups were listed below: 0.06 (total cholesterol), 0.30 (triglyceride), 0.004 (HDL-C), <0.001 (HDL2-C), < 0.001 (apoA1), 0.009 (apoB), <0.001 (apoB/apoA1), <0.001 (fibrinogen), <0.001 (plasma viscosity), <0.001 and 0.004 [two measures (5.75/s and 230/s) of whole blood viscosity]. The correlation coefficients between cumulative exposure of passive smoking and HDL-C,HDL2-C,apoA1, apoB, apoB/apoA1, fibrinogen, plasma viscosity, and two measures of whole blood viscosity were -0.25, -0.27, -0.30, 0.24, 0.31, 0.32, 0.43, 0.51 and 0.36 (all P<0.01), respectively.
CONCLUSIONPassive smoking could affect blood lipid metabolism, fibrinogen and viscosity in the never smoking women which might contribute to the causation of coronary heart disease.

6.Comparison between pathologic, clinical and MRI findings of demyelinating pseudotumor in brain

Objective To analyze the MRI findings and study the correlations between pathologic, clinical and MRI findings of demyelinating pseudotumor in brain. Methods Pathologic,clinical and MRI findings in 8 cases proved pathologically with demyelinating pseudotumor in brain were retrospectively analyzed. Results The clinical manifestations comprised mostly dyskinesia and/or limb sensory disability, as well as epilepsy in few cases. MRI showed a single focal lesion mainly invading subcortical white matter in every case, and most lesions were showed round or nearly round with clear boundary, homogenous low signal on T1WI and high signal on T2WI, and marked enhancement. Pathological examination showed demyelinating, perivascular inflammatory infiltration consisting predominantly of lymphocytes, and glial cell proliferation. Conclusions MRI can well mirror the pathological and clinical changes of demyelinating pseudotumor in brain, so MRI plays an important role in its diagnosis. It is easy to make a differential diagnosis between glioma and supratentorial demyelinating pseudotumor, but it is difficult for those lesions in cerebellum. For such cases, both clinical and MRI diagnosis are necessary. Trial treatment of corticoids or even biopsy is available for those cases that are difficult to diagnose by clinical and MRI methods.
7.Expression of CD147, matrix metalloproteinases and transforming growth factor beta1 in breast cancer.

OBJECTIVETo investigate the expression of CD147, matrix metalloproteinase (MMP)-2, MMP-9, Transforming growth factor (TGFbeta1) and TGFbetaRI proteins and their relationships to breast cancer.
METHODSThe expression of CD147, MMP-2, MMP-9, TGFbeta1 and TGFbetaRI proteins was examined on tissue chips containing 160 cases of breast carcinomas by S-P immunohistochemical method.
RESULTSThe positive rates of CD147, MMP-2, MMP-9, TGFbeta1 and TGFbetaRI proteins were 87.5% (140/160), 96.9% (155/160), 95.0% (152/160), 73.7% (118/160) and 60.6% (97/160), respectively. The expression of CD147 was positively correlated with axillary lymph node metastasis, TNM staging and HER2 over expression (P < 0.01, P < 0.05 and P < 0.01, respectively), and inversely correlated with PR expression (P < 0.05). The patients' relapse-free survival was shorter in TGFbeta1-positive group than in TGFbeta1 negative group (P < 0.05). Both the expression of MMP-2 and MMP-9 were positively correlated with CD147 expression; and both the expression of TGFbeta1 and TGFbetaRI were positively correlated with CD147 expression (P < 0.01 and P < 0.05, respectively).
CONCLUSIONThe expression of CD147 is considered closely correlated with tumor invasion, metastasis and prognosis in breast cancer, and has also a close correlation with MMP-2, MMP-9, TGFbeta1 and TGFbetaRI expression.

8.Analysis of lymph node metastases in early esophageal carcinoma and treatment regimens.

OBJECTIVETo investigate the incidence of lymph node metastasis (LNM) in early esophageal carcinoma and the techniques of dissection.
METHODSStandard three-field dissection was performed in patients with small superficial esophageal carcinoma detected by endoscopy from 1993 - 2007. The lymph node metastases in different regions were identified by histopathology. The survival rate of the cases was analyzed.
RESULTSA total of 149 patients with early esophageal carcinoma were identified by postoperative pathological examination. The overall lymph node metastasis (LNM) rate was 22.8%, and the degree of LNM was 2.4% in all fields. Most lymph node metastases from upper thoracic esophageal carcinoma were found in cervical and the right upper mediastinal nodes. The LNM from middle thoracic esophageal carcinoma were approximately equal in the cervical, mediastinal, and abdominal lymph nodes, and abdominal lymph node metastasis predominated in lower thoracic esophageal carcinoma. The metastatic rate of LNM adjacent to the right recurrent laryngeaal nerve was the highest (44.1%). Significant differences were shown among the rates of LNM in relation to different macroscopic pattern, depth of invasion and differentiation of tumor (P < 0.01), but not to the longitudinal length of tumor (P > 0.05). The overall 5-year survival rate was 77.9%. It was 87.0% in patients without LNM, and 47.1% in those with LNM.
CONCLUSIONLymph node metastasis in early esophageal carcinoma is in a high frequency. Patients with tumor invasion into the mucosa or lamina propria but without lymph node metastasis may undergo a local operation such as endoscopic mucosectomy and have a good prognosis. Patients with tumor invasion into the muscularis mucosae or submucosa should be treated with radical surgery with three-field lymphadenectomy, especially, to dissect the lymph nodes adjacent to the recurrent laryngeal nerve.

9.Influence of soft-start irradiation on polymerization shrinkage stress of universal hybrid composite resins.

OBJECTIVETo compare the difference between soft-start curing mode and standard curing mode in polymerization shrinkage stress of universal hybrid composite resins and to study effect of the soft-start curing mode on the decrease of shrinkage stress.
METHODSThree universal hybrid resins (A: Charisma, B: TPH Spectrum, and C: Esthet-X) were respectively filled in cavities (4 mm in diameter) of epoxide resin disks, 16 specimens of each. Off them, eight of the specimens for each composite resin were exposed using soft-start mode and the other eight using standard mode. Polymerization contraction stress was calculated during 48 h after curing with photo-elastic stress analysis.
RESULTSThree composite resins cured with soft-start mode showed the same trend of shrinkage stress changing as that with standard curing mode and values of polymerization shrinkage stress at 24 h after curing were (3.80 +/- 0.31) MPa, (3.21 +/- 0.40) MPa, and (2.84 +/- 0.22) MPa respectively for A, B and C composite resins. The corresponding figures for the composites with standard curing mode were (4.19 +/- 0.24) MPa, (3.69 +/- 0.33) MPa, and (3.14 +/- 0.28) MPa. Three composite resins cured with soft-start mode had significantly lower polymerization shrinkage stress compared with standard curing mode at 24 h after curing (P < 0.05).
CONCLUSIONSUsing soft-start curing mold can reduce, to some extent, the polymerization shrinkage stress of universal hybrid composite resins.

10.Simultaneous quantitative analysis of four lignanoids in Schisandra chinensis by quantitative analysis of multi-components by single marker.

The aim of the study is to establish a new method of quality evaluation and validate its feasibilities by the simultaneous quantitative assay of four lignanoids in Schisandra chinensis. A new quality evaluation method, quantitative analysis of multi-components by single marker (QAMS), was established and validated with Schisandra chinensis. Four main lignanoids, schisandrin, schisantherin A, deoxyschizandrin and gamma-schizandrin, were selected as analytes and schisandrin as internal reference substance to evaluate the quality. Their contents in 13 different batches of samples, collected from different bathes, were determined by both external standard method and QAMS. The method was evaluated by comparison of the quantitative results between external standard method and QAMS. No significant differences were found in the quantitative results of four lignanoids in 13 batches of S. chinensis determined by external standard method and QAMS. QAMS is feasible for determination of four lignanoids simultaneously when some authentic standard substances were unavailable, and the developed method can be used for quality control of S. chinensis.
