1.Expression and significance of PTEN and Ki-67 in thymoma

Objective To investigate the relationship of PTEN,Ki-67,type of credits of thymoma in WHO organization and Masaoka staging,and the significance of the biology's expression in thymoma.Methods The expression of PTEN and Ki-67 in 52 pieces of thymoma specimens was detected by the method of immunohistochemical SP.Results Positive expression rate of the 52 patients with thymoma was decreased gradually after WHO classification (8/10,10/14,6/12,4/11,0,0 and 10/13,12/20,6/16,0),Masaoka staging and PTEN.The difference were significant in statistics (λ2 =11.751,P =0.038 ; x2 =8.311,P =0.040).Positive expression rate of Ki-67 was gradually increased (1/10,6/14,6/12,8/11,2/3,2/2 and 3/13,9/20,10/16,3/3),the difference was also significant in statistics (x2 =11.231,P =0.047; x2 =7.904,P =0.048).Conclusions The positive expression rate of PTEN,Ki-67 are both related to clinical classification and staging closely.The detection to the positive expression rate of PTEN and Ki-67 can contribute to judging thymoma biological characteristics,thus,it will be helpful in diagnosing the thymoma,analyzing the degree of malignancy,invading and transferring ability.And then provide objective indicators for clinical diagnosis and treatment.
2.Effects of Naloxone on plasma level ?-endorphinl in rats with alcoholic fatty liver

Aim To explore the effect of naloxone on th e plasma level of ?-endorphin (?-EP) in rats with alcoholic fatty liver (AFL) and its possible mechanism. Meth ods Forty-eight Wistar male rats were divided randomly into four grou ps: Model group (n 12), low doses treatment group (n12), high doses tre atment group (n12) and normal control group (n12). By the end of the f ourth week, all the rats were weighted, narcosised, sacrificed, and got the whol e liver. The level of ?-EP in plasma and the level of superoxidized dismutase (SOD) and malondialdehyde (MDA) in plasma and liver tissue were measured.The liv er samples were analyzed for histopathology with microscope. Results Rat models of AFL were established successfully. Objective To explore the clinical efficacy and safety of modified PPH combined with partial internal anal sphincterotomy in the treatment of circular mixed hemorrhoids.Methods Patients with annular mixed hemorrhoids were divided into two groups by a completely randomized controlled method.54 patients in the experimental group were treated with modified PPH combined with partial internal anal sphincterotomy,while 51 patients in the control group were treated with conventional PPH.The postoperative indicators,perioperative and long-term complication rates of the two groups were compared,and the clinical efficacy and safety were observed.Results The operation time in the experimental group was(48.35±4.37)minutes,which was higher than that in the control group(36.42 ±6.21)minutes(P<0.05).The incidence of postoperative anastomotic stenosis in the experimental group was 1.9%,lower than 15.6% in the control group(P<0.05).Anal pain,urinary retention,first defecation time,long-term anal distention,the experimental group was significantly better than the control group,the difference was statistically significant(P<0.05);The hospitalization time in the experimental group(4.8±0.62)days was not significantly different from that in the control group(5.1±0.54)days(P>0.05).The amount of intraoperative bleeding and anastomotic bleeding in the experimental group[(17.28±2.22)ml,3.7%]were not significantly different from those in the control group[(16.75± 2.13)ml,3.9%](P>0.05).Conclusion Compared with conventional PPH,the modified PPH combined with partial internal anal sphincterotomy slightly increases the operation time,but does not increase the risk of anastomotic bleeding,the incidence of rectal fistula,the amount of surgical bleeding,and the length of hospital stay.It can significantly improve postoperative anal pain,urinary retention,long-term distention symptoms,shorten the time of first defecation,ease the difficulty of defecation,and significantly reduce postoperative anastomotic stenosis,The long-term efficacy and safety are good.
4.Pharmacological Effects of Icariin on Male Reproductive System:A Review and Literature Visualization

Icariin,which belongs to the class of flavonoids,is the main active ingredient of the traditional tonic Chinese herb Epimedii Folium.Modern studies have shown that icariin has a wide range of effects on the male reproductive system.It has various pharmacological activities such as regulating cell proliferation and apoptosis,antioxidants,promoting testosterone secretion,improving erectile function,inhibiting prostate cancer cell migration,invasion,and regulating cell cycle.It has research value and application prospects in the field of urology and assisted reproduction.Therefore,Icariin's pharmacological effects and molecular mechanisms on the male reproductive system are reviewed in this paper combined with literature visualization analysis.It is expected to provide a theoretical basis for the therapeutic value development and application of icariin in male reproductive health.
5.Significance of jellyfish sign in predicting adverse perinatal outcomes of complete placenta previa combined with placenta accreta spectrum disorders

Objective:To explore the value of jellyfish sign, an abnormal ultrasonographic sign, in predicting adverse perinatal outcomes of women with complete placenta previa combined with placenta accreta spectrum disorders (PAS).Methods:This retrospective study analyzed the ultrasound images of 72 singleton gravidas, diagnosed with complete placenta previa combined with PAS, who underwent cesarean section at the First Affiliated Hospital of Nanjing Medical University between January 2020 and February 2023. Based on the presence and absence of the jellyfish sign in ultrasound images, these gravidas were divided into the jellyfish-sign group (15 cases, 20.8%) and the non-jellyfish-sign group (57 cases, 79.2%). The clinical data and perinatal outcomes of the two groups were analyzed. The adverse perinatal outcomes encompassed conditions such as abdominal aorta balloon block, uterine artery embolism, hysterectomy, postpartum hemorrhage, and neonatal intensive care unit (NICU) admission of their neonates. Statistical analysis was performed using two independent samples t-test, the Mann-Whitney U test and the Chi-square (or Fisher's exact) test. Results:(1) The jellyfish-sign group exhibited a higher parity [(1.6±0.7) times vs (1.2±0.6) times, t=2.01] and higher prenatal scores of placenta accreta [(12.3±1.5) scores vs (8.6±2.9) scores, t=6.59] than those in the non-jellyfish-sign group (both P<0.05). Among the 57 cases in the non-jellyfish-sign group, there were 14 cases of placenta creta (24.6%), 40 cases of placenta increta (70.2%), and three cases of placenta percreta (5.3%). Among the 15 cases in the jellyfish-sign group, nine cases were diagnosed with placenta increta, six with placenta percreta, and none with placenta creta. The difference in distribution between the two groups was statistically significant (Fisher's exact test, P<0.001). (2) Intraoperative blood loss [(for those who accepted abdominal aorta balloon block, 1 973±1 057) ml vs (1 211±576) ml, t=2.55], red blood cells transfused [4.0 U (2.0-23.0 U) vs 2.5 U (0.0-11.0 U), Z=-2.53], postoperative hospitalization time [(9.7±2.4) vs (7.5±2.2) d, t=3.36], the incidence of abdominal aorta balloon block [15/15 vs 38.6% (22/57), χ2=17.92], uterine artery embolism [for those who accepted abdominal aorta balloon block, 3/15 vs 1.8% (1/57), Fisher's exact test], and requiring blood transfusion [15/15 vs 63.2% (36/57), Fisher's exact test] were higher in the jellyfish-sign group than those in the non-jellyfish-sign group. However, the non-jellyfish-sign group had lower gestational age at delivery [(33.6±1.5) weeks vs (35.2±1.8) weeks, t=-3.24], and lower neonatal Apgar score at 1 min and 5 min [1 min: 8 scores (3-10 scores) vs 9 scores (4-10 scores), Z=-2.46; 5 min: 9 scores (7-10 scores) vs 10 scores (6-10 scores), Z=-2.02] (all P<0.05). There were no significant differences in emergency surgery rate, 24 h postoperative blood loss, neonatal birth weight, and proportion of NICU admission between the two groups. Additionally, no cases of hysterectomy or death were observed in the two groups. Conclusions:Ultrasound examination revealing jellyfish signs in patients with complete placenta previa and PAS is associated with an increased likelihood of adverse perinatal outcomes. Consequently, the management of these patients should be given greater attention.
6.Quality control of Portulaca oleracea by HPLC fingerprint combined with quantitative analysis of multi-components by single-marker

OBJECTIVE To establish HPLC fingerprint of Portulaca oleracea, establish quantitative analysis of multi- components by single-marker (QAMS) method for the content determination of caffeic acid, ferulic acid, genistin and quercetin, and provide reference for quality control of the medicine. METHODS The determination was performed on Eclipse XDB-C18 column with mobile phase consisted of methanol-0.2% phosphoric acid solution (gradient elution) at the flow rate of 1.0 mL/min. The column temperature was 25 °C, and detection wavelength was set at 360 nm. The sample size was 10 μL. HPLC fingerprint of P. oleracea was established according to the above chromatographic conditions. Cluster analysis (CA) and principal component analysis (PCA) were performed for 15 batches of specimens. Using caffeic acid as internal standard, relative correction factors of other three components were calculated by QAMS, and then the component content was calculated on the basis of relative correction factors, which was compared with the external standard method. RESULTS HPLC fingerprints of 15 batches of P. oleracea were calibrated with a total of 17 common peaks, and 4 components (caffeic acid, ferulic acid, genistin, quercetin) were identified; the similarities of 15 batches of samples were greater than 0.890. The results of CA showed that S1-S10 were clustered into one category, and S11-S15 were clustered into one category. The results of PCA revealed that the accumulative contribution rate of the two main components was 92.502%, and the classification results were basically consistent with CA. The linear range of caffeic acid, ferulic acid, genistin and quercetin were 0.003 1-0.157 1, 0.003 6-0.181 7, 0.008 5-0.425 6,0.000 4-0.021 8 mg/mL (R2≥0.999 7); the results of precision, repeatability, stability (24 h) and recovery tests all complied with the requirements of Chinese Pharmacopoeia. The relative correction factors of ferulic acid, genistin and quercetin calculated by QAMS were 1.534, 5.302 and 0.174; there was no significant difference in the contents of components measured between this method and the external standard method. CONCLUSIONS The established HPLC fingerprint combined with QAMS can be used for the quality control of multiple index components in P. oleracea. The origin has a certain influence on the quality of P. oleracea, and the quality of P. oleracea produced in Sichuan is better than that produced in Anhui and Hebei.
