1.International comparative study of ADR compensation system and its implications in China

In recent years , increasing use of drugs has given rise to more and more Adverse Drug Reaction ( ADR) cases in China.However, due to the absence of ADR compensation legislation in China , these victim's rights cannot be restituted.By analyzing experiences from Germany , Sweden, Japan, USA, and Taiwan, as well as the current situation and difficulties of ADR compensation in China , this paper provides suggestions for ADR compensa-tion policy in China , which is to constitute an ADR compensation system based on funds .
2.EVACUATION OF HYPERTENSIVE INTRACEREBRAL HEMATOMA BY STEREOTACTIC TECHNIQUE

Evacuation of intracerebral hematomas in 38 hypertensive patients was reported. Hematomas were found in the internal capsule in 24 patients, subcortex in 5. cerebellum in 7 and brain stem in 2. The volume of hematoma was less than 2ml (in the brain stem) in 2 patients, 20-50ml in 15, and over 100ml in 6. The fluid hematoma was to tally aspirated with stereotactic technique in 7 patients. In 17 patients, over 80% of hematoma was evacuated, and in 14 about 60-80% of hematoma was evacuated by the same technic. The results of the operation were good in 11 patients (29.0%), while in 29% there was a mild disability, and in 12 (31.5%) there was a marked disability, 4(10.5%) died.
3.Evaluation of the combination of multiple subpial transection and other techniques for treatment of intractable epilepsy.

OBJECTIVEMultiple subpial transection (MST) is one approach to the surgical treatment of intractable epilepsy with epileptogenic lesion located in functional areas. To verify the effect of MST, an experimental study was performed first, followed by clinical application.
METHODSOn the basis of the experimental study, MST was performed in 200 intractable epileptic patients from 1991 to 2000. Of them, 80 cases underwent MST only while 120 others underwent MST combined with other techniques, such as corpus callosotomy, temporal lobectomy and focus resection. A series of modifications of the surgical techniques were made.
RESULTSThe results of the experimental study indicated that MST could inhibit the formation and spreading of epileptic discharge and limit the damage to neurons in a minimal area on the epileptogenic agent injected cortex. MST does not impair major functions of the cortex. After the clinical application and modifications, 160 patients were followed up for 1 to 8 years. Complete control of seizure was obtained in 100 cases (62.5%), significant reduction (more than 75%) in 32, reduction (more than 50%) in 20 and no change in 8. The total rate of effectiveness was 95.0%, and the significant rate of effectiveness was 82.5%. No functional defects were found in any patients.
CONCLUSIONSThe results indicate that MST is an effective approach to the surgical treatment of intractable epilepsy. MST can be combined with other approaches. The outcome of the subdivision of the MST only group indicates that MST on local epileptogenic lesion without structural changes is as effective as that of the combined operation group. To evade hemispheric disturbance, MST should be done first to avoid severe complications. Hemispherectomy should be performed only on poor effected cases of MST.

4.Analysis on the relationship between cumulative high temperature exposure and steel workers' hypertension in a large steel mill

Objective:To analyze the relationship between cumulative high temperature exposure and hypertension among steel workers.Methods:We conducted a survey among 7 660 production workers (7 023 males and 637 females) in a large steel mill during February-June 2017 and March-July 2018. Basic information, blood pressure, occupational history and high temperature data in workplace were collected through questionnaire survey, physical measurement and occupational exposure detection. Restrictive cubic spline model was used to analyze the dose-response relationship between cumulative high temperature exposure and hypertension, and the correlation between cumulative high temperature exposure and workers' hypertension was analyzed by logistic regression.Results:The median number of cumulative high temperature exposure was 626.56 ℃·year and 49.20% of workers were exposed to high temperature. The hypertension prevalence rate was 25.60% among the workers. According to the results of the spline model, the cumulative high temperature exposure was divided into three sections: <292.61, 292.61~<720.13, ≥720.13 ℃·year. Taking the exposure <292.61 ℃·year group as a reference, the risk of steel workers' hypertension in the 292.61~<720.13 ℃·year group and ≥720.13 ℃·year group is 1.44 and 2.17 times that of the reference group ( P<0.05) . Conclusion:There was a nonlinear dose-response relationship between cumulative high temperature exposure and steelworkers' hypertension. With the increase of cumulative high temperature exposure, the risk of steelworkers' hypertension increases.

6.CT-guided stereotactic biopsy of deep brain lesions: report of 310 cases

Objective To evaluate the accuracy of CT-guided stereotactic biopsy in making correct pathological diagnosis and choosing corresponding management of brain tumors. Methods From 1991 to 1995, CT-guided stereotactic biopsy was performed in 310 patients with intra-cerebral lesions which were deep-seated or located in certain main functional areas. The patients were 198 men and 112 women. Their ages ranged from 4.5 to 70 years (average: 39.3 years). The lesions were located in the deep cerebrum (74 patients), the sellar area (62), the basal ganglion (51), the posterior part of the third ventricle (38), other intraventricleular area (21), the cerebellum (17) and the brain stem (9), and intracranial multiple lesions were found in 38 patients.Results Brain tumors were diagnosed pathologically in 266 patients (85.8%); inflammatory process in 25 (8.1%), other lesions in 8 (2.6%) and uncertain cases were 11 (3.6%). The overall positive rate of biopsy was 96.4% and the positive rate for brain tumor was 85.8%. Intracranial hematomas after biopsy were found in 5 patients (1.6%). There were no deaths induced by the biopsy or other serious complications.Conclusions The results suggest that CT-guided stereotactic biopsy is a reliable method for histopathological diagnosis of brain tumors and it is also of great help in selecting appropriate management.

8.Analysis of abnormal dose values from individual dose monitoring for radiation workers in medical institutions in China in 2020

Objective:To analyze distributions of dose range, occupational category, and causes of abnormal dose values from individual dose monitoring of radiation workers and to provide recommendations for improving the quality of individual monitoring data and occupational health management in medical institutions.Methods:The data of individual dose monitoring result exceeding 1.25 mSv in a single monitoring cycle, collected by Occupational Health System for Radiology Facilities’ on National Radiological Health Information Platform, was analyzed.Results:A total of 1 113 abnormal dose values were detected in 31 provinces in 2020, with an abnormal rate of 2.48‰, of which workers of diagnostic radiology and interventional radiology accounted for 68.01% and 18.78%, respectively. The dose distribution of abnormal dose values was mainly between 1.25 and 5 mSv. The abnormal dose values were mainly caused by improperly wearing or using personal dosimeters, accounting for 63.88%. Workers for whom the dose values was abnormal due to increased workload accounted for 12.32%, with an average annual individual dose of 5.14 mSv. There was no statistically significant difference in annual individual dose among radiation workers between different occupational groups( P>0.05). Conclusions:Education and training on radiation protection for radiation workers should be strengthened, and a specific and feasible system for radiation protection management, as well as a reward and punishment system, should be established in order to reduce the occurance of the improper wearing of personal dosimeters. Great attention should be paid to occupational exposure of workers in diagnostic radiology and interventional radiology, and a work shift system should be conducted to reduce individual dose levels. It is recommended that standard verification procedures for abnormal dose values from individual dose monitoring should be developed to improve the quality of individual monitoring data.
9.Research progress of the impact of the Fukushima Daiichi nuclear accident on childhood thyroid cancer

In March 2011, a nuclear accident happened and released a large amount of radionuclides into the environment in Fukushima, Japan. This review introduced the latest findings on the relationship between childhood thyroid cancer and radiation exposure after the accident in Fukushima. Many reports suggested that there was no accurate correlation between the incidence of childhood thyroid cancer and the local external dose due to the accident at the Fukushima Daiichi nuclear power, while the relationship between the two varied with method of dose calculation and statistics. Also there was no clear evidence that radiation exposure had a negative impact on children′s thyroid, and the increased incidence of thyroid cancer may be due to over-screening.
10.The investigation and analysis of eye lens dose to interventional radiology workers in China from 2019 to 2021

Objective:To investigate and analyze the eye lens dose to interventional radiology workers in China from 2019 to 2021.Methods:The monitoring data on eye lens dose to interventional radiology workers from 31 province-level units during 2019-2021 were collected through the National Radiological Health Information Platform. The eye lens dose evaluation indicator was Hp(3), with each monitoring period of no more than 3 months. Kusall-Wallis H test was used for the comparison of multiple groups and pairwise. Results:A total of 6 643 interventional radiology workers were investigated from 2019 to 2021. The average annual eye lens dose was 1.03 mSv, with the median of 0.17 mSv and the maximum of 94.88 mSv. The annual eye lens dose to 59 workers exceeded 20 mSv. It was also found that the annual eye lens dose to the doctors in 2019 and 2020 was slightly higher than that to nurses (rank mean difference=118.29, 129.71, P<0.01), and the lens dose to interventional radiology workers who performed cardiac interventions in 2019 was higher than that to workers who performed peripheral vascular interventions (rank mean difference=46.52, P<0.05). Conclusions:The lens dose to interventional radiology workers is lower than the limits given in Chinese national standard currently in effect, but exceed the latest internationally recommended limit for a few ones. In order to protect the occupational health of interventional radiology workers, the monitoring of lens dose should be strengthened.
