1.Acid-base Disturbance in Hepatic Encephalopathy: An Analysis of 33 Cases

In order to evaluate the clinical significance of the acid-base disturbance complicating hepatic encephalopathy, arterial blood gas and electrolyte levels were determined in 33 patients with encephalopathy due to various hepatic disorders. It was found the respiratory alkalosis occurred in 31 cases(93.9%) and respiratory acidosis complicated with metabolic acidosis in 2 cases (6.06%). In the 31 cases of respiratory alkalosis, there were 4 cases(12.12%) of simple respiratory alkalosis, 18 cases(54.55%) complicated with metabolic alkalosis, 3 cases(9.09%) complicated with metabolic acidosis, and 6 cases complicated with both metabolic alkalosis and acidosis. In addition, the arterial blood pH of 29 cases (87.87%) was above the normal value of 7.45. The main type of acid-base disturbance found in hepatic encephalopathy was alkalosis. Its occurrence was closely related to hypoxemia, hyperammonemia, and elevated blood level of progesterone in the case of respiratory alkalosis and to salt-limited diet and improper administration of diuretics and alkalite drugs in the case of metabolic alkalosis. Metabolic acidosis was usually the result of severe complitions. In alkalosis, there was frequently an elevation of blood free ammonia, which could penetrate the blood-brain barrier easily and induce coma and encephalopathy. Therefore energetic correction of alkalosis would be valuable to prevent hepatic encephalopathy. Effective treatment of the primary disease, correction of hyperventilation.correction of disturbances of blood electrolytes, and administration of arginine hydrochloride and potasium chloride would be appropriate measures in dealing with alkalosis.
2.Clinical observation of under endoscopic assisted semi-laminectomy approach on the treatment of intra-spinal tumor

Objective To retrospective analysis of the clinical effect and complications of the semi-laminectomy approach on the treatment of intra-spinal tumor.To summarize the endoscopic treatment experience of intra-spinal tumor.Methods To retrospective analysis of the clinical data of 24 cases with intra-spinal tumor in Shanxi Provincial People's Hospital from January 2008 to September 2015.The diagnosis of the intra-spinal tumor was confirmed by preoperative MRI examination.Intraoperative application of endoscopic surgery in the resection of tumor.Longterm followed up of patient was implemented and observe the efficacy and complications after surgery.Results Postoperative symptoms of all patients have different degrees of improvement,the degrees of sensory disorder was reduce,the limb muscle strength was increased and alleviate the damages of nerve electrophysiology.No serious complications were found after operation.Of the 24 patients,17 cases were total resection of the tumor and 7 cases were subtoal resection of the tumor.Tethered cord release was operated in patients with tethered spinal cord.Conclusions Endoscopic resection assisted semi-laminectomy approach of intra-spinal tumor has slight damage to the surround structures and the impact to the anatomy of the spine was small.Endoscopic assisted semi-laminectomy approach may have siginficant clinical effect on intra-spinal tumor,and have good security.
3.The diagnostic value of CT bronchial sign in peripheral solitary pulmonary lesions

Objective To investigate the differential diagnostic values of CT bronchial sign for peripheral solitary pulmonary lesions(SPLs).Methods One hundred and eleven patients with peripheral SPLs were scanned using multi-slice helical CT(MSCT),and multiplanar reconstruction was performed to show the relationship between the lesion and bronchus,the diffefences between the benign and malignancy were compared by using chi-square test.Results Bronchial cutoff rate in malignant lesions(47/95,49.5%)was markedly hi er than that in benign lesions(10/42,23.8%.X12=7.896,P<0.05),the frequency of type Ⅰ and type Ⅱ air bronchogram presented in malignant lesions(10/11.8/9)was higher than benign lesions(1/11,1/9.X2=6.975,4.818,P<0.05),but type Ⅳ in benign lesions(12/17)was more common than that in malignant lesions(5/17.X2=7.390,P<0.05).No significant difference was found in bronchus ran at the periphery of the lesion and bronchus dragged by the lesion between benign(9/24.1/4)and malignant lesions(15/24,3/4.X2=0.641,0.062,P>0.05).The focal bronchial wall thickening in malignancy(21/22)was markedly higher than benign lesions(1/22.X2=4.185.P<0.05),whereas the extensive thickening in benign lesiom(4/7)was more common(3/7.X2=8.650,P<0.05).Conclusion CT bronchial sign is very important in the differentiation of benign and malignant pulmonary lesions.
4.Genotype and phenotype of CRB1 mutated Leber congenital amaurosis and early-onset retinal atrophy

Objective:To investigate the relationship between genotype and phenotype in children with CRB1 mutated Leber congenital amaurosis (LCA) and early onset retinal dystrophy (EOSRD). Methods:A retrospective clinical study. From January 2013 to December 2019, 10 children with CRB1 mutated LCA/EOSRD were enrolled in the study. The patients were identified as CRB1 mutation by the second generation targeted capture sequencing, Sanger sequencing and the family segregation analysis. All children underwent electroretinogram (ERG) and fundus examination. At the same time, 6 cases were examined by optical coherence tomography (OCT); 1 case was examined by fluorescein fundus angiography (FFA), 7 cases were examined by wide-angle laser scanning ophthalmoscope (UWF SLO). Results:There were 6 cases of LCA and 4 cases of EOSRD in 10 patients with CRB1 gene mutations. The average age of first visit was 3.61 years old. The light and dark wave of ERG was flat in 6 cases, and decreased in 4 cases. A total of 19 pathogenic mutations were detected. There were 1 homozygous mutation and 9 compound heterozygous mutations. There were 4, 2 and 1 cases of "copper-coin" like, "salt and pepper" like and "osteocyte" like pigment changes in retina, 1 case of "crystalline pigment" change and 2 cases of macular pigment scar. In 7 cases of UWF SLO examination, different degrees of para-arteriolar pigment epithelium retention (PPRPE) were found in the middle and peripheral fundus. In 6 cases examined by OCT, the outer layer of retina atrophied and the band of ellipsoid disappeared. Symmetrical cystoid macular edema, splitting cystoid macular degeneration and adhesion of epi-macular membrane to optic disc and macular area were found in 1 case, respectively, the retinal structure was rough and thickened, and the fovea became thinner in 3 cases. In FFA examination, 1 case showed uveitis-like changes with late optic disc fluorescein staining, macular fluorescence accumulation, strong fluorescence diffusing along the blood vessels in each quadrant, peripheral PPRPE of "frost-branch" like strong fluorescence. Conclusion:The relationship between genotype and phenotype of CRB1 mutation is complex, and PPRPE is a common characteristic change.
5.Effects of heroin and ephedrine on the histological structure and ChAT activity of hypothalamus and hippocampus of filial mice

Objective To explore effects of heroin and ephedrine on the histological structure and ChAT activity of hypothalamus and hippocampus of filial mice. Expression of Bcl-2 associated X protein(Bax protein) and keratinocyte growth factor(KGF) of hypothalamus and hippocampus were measured. Methods One hundred and eight filial mice were given intraperitoneal injection of heroin and ephedrine by gradually increase of doses, apoptosis and expression of Bax protein and KGF of hypothalamus and hippocampus were observed by Giemsa staining and immunohistochemistry, and the ChAT activity was detected by colorimetry. Results After administration of heroin and ephedrine at 5,10,15,20 days, the number of apoptotic cells and expression of Bax protein and KGF of hypothalamus and hippocampus were significantly increased and ChAT activity was lower than those of the control group(P<0.05 or P<0.01). There were differences between heroin group and the ephedrine group in the above-mentioned four indexes (P<0.05 or P<0.01). The number of apoptotic cells and Bax protein and KGF immunopositive neurons of hypothalamus and hippocampus increased by the increase in dose of heroin and ephedrine. Conclusions Heroin and ephedrine had great effect on the histological structure and ChAT activity of hypothalamus and hippocampus of filial mice, and this effects would be related to the cell apoptosis of hypothalamus and hippocampus.
6.Virulence factor O-acetyltransferase A inhibits Staphylococcus aureus infection-induced macrophage death

Objective:To explore the effect of the virulence factor OatA during Staphylococcus aureus infection.Methods: In vitro,wild type of Staphylococcus aureus USA300,OatA gene deletion strain or OatA gene complemented strain were used to infect mice bone marrow divided macrophages (BMDMs).Subsequently BMDMs were separated and the case of cell death were detected.In vivo,mice pulmonary infection model was constructed with nasal inhalation of Staphylococcus aureus.Alveolar macrophages were isolated and the case of cell death were detected.Results: In contrast to wild type and OatA gene complemented strain,OatA gene deletion strain induced severer macrophage death both in vitro and in vivo.Conclusion: The virulence factor OatA inhibits Staphylococcus aureus infection-induced macrophage death.
7.Value of diffusion-weighted imaging in the diagnosis of orbital lymphoma

Objective To investigate the diagnostic value of apparent diffusion coefficient (ADC)for discriminating orbital lymphomas from other orbital mass lesions.Methods In this prospective study,87 subjects (22 orbital lymphomas and 65 other orbital mass lesions) were enrolled.These patients underwent serial MR and echo-planar DW imaging examination of the orbits with b values of 0 and 700 s/mm2 at 1.5 T MR (GE Signa Excite).ADC maps were reconstructed,and the ADC values of the orbital masses were calculated.Mass ADC was also compared with that of normal-appearing white matter (ADC ratio,ADCR).The receiver operating characteristic curves (ROC) were constructed using optimal cut point of ADC and ADCR to differentiate between orbital lymphomas and other orbital mass lesions.The areas under the ROC curve for ADC and ADCR were also calculated.Results The mean ADC and ADCR of orbital lymphomas were (0.77 ± 0.17) × 10-3 mm2/s and 0.89 ± 0.21,respectively.The mean ADC and ADCR of other orbital mass lesions were (1.36 ± 0.38) × 10-3 mm2/s and 1.51 ± 0.43,respectively.Lymphomas had lower ADCs and ADCRs than other orbital mass lesions (t =-9.620,-9.003,P =0.000).The areas under the ROC curves of ADC and ADCR diagnosing lymphoma were 0.94±0.03 and 0.91 ±0.03,respectively.An ADC of less than 0.804 × 10-3 mm2/sec and ADCR of less than O.956 were optimal for predicting lymphoma (sensitivity,77.3% for both; specificity,98.4% and 92.3 %,respectively ; and accuracy,93.1% and 88.5 %,respectively).Compared with pathological results,both ADC and ADCR had high correlations (Kappa values were 0.806 and 0.696,respectively).Conclusion Diffusion-weighted imaging can be applied as a complementary tool in the detection of orbital lymphomas.
8.Investigation of rumination level and its influential factors among psychiatry nurses who go through work place violence

Objective:To investigate the level of rumination and its influential factors among Chinese psychiatry nurses who go through work place violence.Methods:In this study, 150 psychiatry nurses were recruited from Affiliated Brain Hospital of Guangzhou Medical University (specialize in psychiatry), via the combination of convenient sampling and snowball sampling. Chinese Event-related Rumination Inventory (C-ERRI) was applied in the survey.Results:The total score of C-ERRI was (22.11±9.62) points, and the scores of intrusive rumination subscale and deliberate rumination subscale were (12.99±5.58) and (9.12±6.01) points, respectively. Multiple Linear Regression Analysis show that assault frequency in recent year ( B=-3.195, P<0.01) and whether got injury in the recent assault ( B=8.591, P<0.01) were predictors of deliberate rumination, which account for 26.8% variance of the equation. Gender (male) ( B=-2.415, P<0.01), Injury frequency in recent year ( B=2.864, P<0.01) and whether got injury in the recent assault ( B=8.949, P<0.01) were predictors of intrusive rumination. They account for 36.0% variance of the equation. Conclusion:In this study, the level of rumination among Chinese psychiatry nurses was low. Their rumination style was deliberate rumination.
9.Effect observation on different transurethral resection patterns for treating patients with non-muscle invasive bladder cancer

Objective To compare the safety and efficacy of the 2μm laser and the bipolar electrotome used in transurethral re-section of bladder tumor(TURBT)for treating non-muscle invasive bladder cancer(NMIBC).Methods The clinical data in the pa-tients with NMIBC treated by TURBT in our hospital from March 2009 to May 2013 were retrospectively analyzed.All patients were divided into the 2μum laser group(n=89)and the bipolar electrotome group(n=82).The operation time,complications,post-operative hospital stay and recurrence rate were compared between the two groups.Results There were no statistically significant differences in the operation time,postoperative hospital stay and recurrence rate between the two groups(P>0.05).Compared with the 2 μm laser group,the bipolar electrotome group showed significantly higher occurrence rate of the obturator nerve reflex (20.7%vs.0,P<0.05)and the bladder perforation(7.3% vs.0,P<0.05)and longer postoperative bladder irrigation time [(3.1±0.9)d vs.(2.2±1.0)d,P<0.05],the differences between the two groups had statistical significance.Conclusion Com-pared with bipolar electrotome,the 2μm laser used in TURBT is safe and effective with few complications for treating NMIBC.
10.Investigation and demand analysis of training status of emergency personnel in primary hospitals in Sichuan Province

Objective To get well-informed of the current status of emergency training in primary hospitals in Sichuan Province and to find weaknesses in the system in order to provide scientific basis for emergency knowledge and skills training.Methods A stratified random cluster sampling method as adopted and an anonymous questionnaire survey method was conducted to investigate the basic circumstances of the current training and the training needs of 1 000 emergency medical staff in 9 regions throughout Sichuan Province.Results The scope of the survey covered emergency doctors,nurses and technicians in hospitals of Grade Ⅱ,Grade Ⅲ and below.A total of 1 000 surveys were sent out and 983 were recovered.The questionnaire completion rate was 98.3%.The respondents mainly worked for more than 10 years,most of which with Bachelor degree,junior and intermediate titles.Results of the survey showed,63.1% Most respondents have only one or two training opportunities per year.The training methods are mainly continuing education (59.0%,580 trips),training organized within the unit (58.5%,575 trips),and training organized by health administrative units (39.3%,386 trips).The main contents of the training are:basic professional skills learning (66.7%,655 trips),basic professional theoretical study (59.2%,582 trips),and new emergency technology (42.5%,418 trips),only 32.0% of the respondents believe that these methods meet the practical needs.The main factors that influence participation in the training were:not be arranged by employer (36.2%,356 trips);the training was different from actual work (31.6%,311 trips);no replacement for work so they couldn't join training (29.8%,293 trips).The survey showed that the most desirable way of training is:practical application (60.1%,591 trips) and the most desirable training content is:emergency first aid knowledge (73.7%,724 trips).There were significant differences in case discussion,technical observation,academic lectures,special study trainingand learning methods in hospitals of different levels (P<0.05),but there was no significant difference in the choice of professional practice methods (P>0.05),and the selection of training contents was in emergency first aid knowledge,general medical knowledge and prevention and treatment of chronic diseases.There was no statistical difference in the selection of training contents of knowledge,infectious disease knowledge and department management knowledge (P>0.05).There were significant differences in the choice of nursing knowledge training (P<0.05).Conclusion The current situation showed a lack of first-aid knowledge and skills training for emergency personnel in Sichuan Province.The present situation of skill training can't satisfy the need of their desire to participate in training.It is urgent to develop a standardized,systematic and scientific training mode to improve the emergency first aid ability of primary medical and emergency personnel.
