1.The feasibility of using International Classification of Functioning, Disability and Health core set for Chinesebreast cancer patients

Objective To determine the reliability, validity and feasibility of using International Classifica-tion of Functioning, Disability and Health (ICF) core set for breast cancer in Chinese patients. Methods Quality of life (QOL) of 110 patients with breast cancer were investigated using ICF breast cancer core set and SF-36 scale. Statistical analysis tested their reliability, validity and feasibility. Results It was shown that the ICF core set for breast cancer had good reliability, validity and feasibility. Conclusions The ICF core set for breast cancer is a re-liable and valid instrument for Chinese breast cancer patients.
2.Kidney cancer stem cells and epigenetic regulation

Targeted therapeutic strategy for cancer stem cells (CSCs) is the key to prevent tumor relapse and metastasis. The im-portant roles of epigenetic regulation on the development of stem cells and gene reprogram of somatic cells suggest that this process may remarkably affect the occurrence and development of CSCs. The epigenome, which comprises DNA methylation, histone modifica-tions, chromatin structures, and non-coding RNAs, controls gene expression patterns. In renal cell carcinoma (RCC), aberrant changes occur in the epigenome. To date, cells with CSC properties from RCC have been successfully isolated using different methods, such as sorting using the Hoechst 33342 side population, forming tumor spheroid, and sorting CD105 cell surface biomarker. According to the progress in genetic studies on RCC, in addition to DNA sequence, the abnormality in the regulatory mechanism has considerable func-tions in tumor progression. Epigenetic changes may be integral to the behavior of cancer progenitor cells and their progeny. Knowledge on epigenetics in renal tumorigenesis process is beneficial in the development of new therapeutic modalities and may deliver new prog-nostic and early diagnostic markers. This paper reviews the latest development in the study of RCC stem cells and the underlying mech-anisms of epigenetic regulation on the development of CSCs in RCC.
3.Health Economic Evaluation of Neonatal Phenylketonuria(PKU)Screening in Dongguan/

Objective:To acquire the implementation effect of the neonatal Phenylketonuria(PKU)Screening Project in Dong-guan,and evaluate its economic effectiveness,so as to provide references for the prevention and control policies of genetic defects in newborns.Methods:Choosing PKU children born in Dongguan from January 2012 to December 2021(excluding BH4 deficien-cy)as the subjects,conduct the cost-benefit and cost-effectiveness analysis on the prevention and treatment of PKU.The relevant data was gained from questionnaire surveys,statistical data,medical service prices of non-profit medical institutions in Dongguan,and relevant literature materials.Results:In the past 10 years,1 245 243 newboms were screened for PKU in Dongguan,the average screening rate is 93.32%.Among them,29 were diagnosed with PKU,with a positive rate of 1:42939.During continuous follow-up,23 cases were diagnosed.Among the 23 follow-up cases,only 1 had intelligence at the critical value,while the rest were normal.The total cost of screening is 29.728 7 million,and the total benefit is 14.239 9 million.The cost-effectiveness ratio is 102.51:1,the cost-benefit ratio is 1:4.79.Conclusion:Neonatal phenylketonuria(PKU)screening in Dongguan has significant social and economic benefits.It's necessary to continue and improve the screening work,especially the policy which can benefit the PKU's full life.
4.Association of stress hyperglycemia with occurrence and prognosis of sepsis-associated encephalopathy

Objective:To investigate whether stress hyperglycemia (SH) is an independent risk factor for the occurrence and mortality of sepsis-associated encephalopathy (SAE).Methods:From August 2016 to October 2021, sepsis patients admitted to the ICU of Guangdong Provincial People's Hospital were selected as the study subjects. According to whether they developed to SH (RBG>11.1 mmol/L) within 7 days of enrollment, the pat ients were divided into the SH group and the non-SH group for analysis. Logistic regression was used to analyze whether SH was an independent risk factor for SAE occurrence, and ROC curve was used to analyze the predictive value of SH to SAE. Kaplan-Meier curve was used to compare the 90-day survival of SAE patients with or without SH. Cox regression analysis was used to analyze the risk factors of 28-day and 90-day death in SAE patients.Results:A total of 183 sepsis patients were included, including 62 patients in the SH group and 121 in the non-SH group. Logistic regression analysis demonstrated that SH was an independent risk factor for SAE ( OR=4.452, 95% CI: 2.021-9.808, P <0.001). ROC curve demonstrated that SH could accurately predict SAE (AUC=0.831; Sensitivity=78.4%; Specificity=76.8%; and Yoden index=0.553). Kaplan-Meier curve demonstrated that the 90-day survival of SAE patients with SH significantly declined (log-rank test: P<0.01). Cox regression analysis suggested that SH was a risk factor for death at day 28 and day 90 in SAE patients (28 d, HR=2.272, 95% CI: 1.212-4.260, P=0.010; 90 d, HR=2.456, 95% CI: 1.400-4.306, P<0.01). Conclusions:SH is an independent risk factor for SAE and can predict SAE occurrence. SH significantly reduces 90-day survival and increase mortality at 28 and 90 days in SAE patients.
5.Early-warning value of PCT/PLT ratio on sepsis-induced myocardial injury

Objective:To investigate the diagnostic and early-warning value of laboratory test indicators for sepsis-induced myocardial injury (SIMD).Methods:The clinical data of 183 patients with sepsis admitted to the Department of Emergency and Critical Care Medicine of Guangdong Provincial People's Hospital from August 2016 to October 2020 were collected. The patient's age, gender, past medical history, vital signs and pathogen culture results were extracted. Cardiac function, blood routine, liver function, renal function, inflammatory factors, coagulation function, APACHE Ⅱ and SOFA scores were recorded at enrollment and 72 h after admission. SIMD was defined as cTnT ≥300 pg/mL and NT-proBNP ≥1243 pg/mL twice in 72 h intervals between enrolled cases, and the early-warning factors of patients with SIMD were analyzed. The differences in various indicators between the two groups were compared, and Logistic regression analysis was used to explore the diagnostic efficacy of cTnT and NT-proBNP combined for SIMD, and the correlation between PCT/PLT ratio and the occurrence of SIMD.Results:Among 250 patients, 67 patients were excluded for lack of the main indicators, and 183 patients (including 62 patients with history of cardiac disease) were enrolled finally. Among 183 patients with sepsis, 105 patients (57.38%) with cTNT ≥300 pg/mL and NT-proBNP ≥1 243 pg/mL, were diagnosed as myocardial injury; after excluding 62 patients with history of cardiac disease, 59 patients (48.76%) with cTNT ≥300 pg/mL and NT-proBNP ≥1 243 pg/mL were diagnosed as myocardial injury. Logistic regression analysis showed that increased PCT/PLT ratio ( OR=1.585, 95% CI: 1.124-2.237, P=0.009) was an independent risk factor for early-warning of SIMD. The PCT/PLT ratio ( OR= 1.850, 95% CI: 1.103-3.102, P=0.020) could stably predict the occurrence of SIMD in patients without previous history of heart disease. ROC curve analysis showed that PCT/PLT ratio could effectively predict the occurrence of SIMD (AUC=0.693, 95% CI: 0.617-0.769, P<0.001), the optimal cut-off value was 0.177 (sensitivity: 65.7%, specificity: 66.7%). The PCT/PLT ratio was still effective in predicting the occurrence of SIMD after excluding patients with previous history of heart disease (AUC=0.733, 95% CI: 0.643-0.823, P<0.001), and the optimal cut-off value was 0.429 (sensitivity: 55.9%, specificity: 83.9%). Conclusions:The combination of cTnT and NT-proBNP has certain diagnostic value for SIMD, and the PCT/PLT ratio could warn the occurrence of SIMD.
6.Clinical observation of intermittently repeated application of levosimendan in the treatment of right heart failure of pulmonary hypertension

OBJECTIVE To observe th e efficacy and safety of intermittently repeated application of levosimendan in the treatment of right heart failure of pulmonary hypertension. METHODS A total of 70 cases of right heart failure of pulmonary hypertension were collected from the First Affiliated Hospital of Guangxi Medical University during Jan. 2019-May 2021,and then randomly divided into control group and observation group ,with 35 cases in each group.In control group ,the patients with moderate risk of classification of pulmonary hypertension were given Tadalafil tablets and Ambrisentan tablets ,while the patients with high risk were given Tadalafil tablets ,Ambrisentan tablets and Treprostinil injection ;both were given anti-right heart failure drugs at the same time. On the basis of treatment in the control group ,patients in the observation group were additionally intravenously pumped with levosimendan injection 12.5 mg,once a month ,at a rate of 0.05-0.1 μg/(kg·min),3 months in total. Response rate of therapy ,improvement of risk stratification of pulmonary hypertension after treatment ,and average pulmonary artery pressure ,six-minute walk distance (6MWD),serum level of N-terminal pro brain natriuretic peptide (NT-proBNP),right atrial pressure (RAP),oxygen saturation in mixed venous blood (SvO2),cardiac index (CI),right ventricular ejection fraction (RVEF),early diastolic tricuspid inflow velocity/early diastolic tricuspid annular velocity (E/Ea),tricuspid annular plane systolic excursion(TAPSE),right ventricular end-diastolic diameter (RVEDD),right ventricular end-systolic diameter (RVESD)before and after treatment ,and the occurrence of adverse reactions were observed in 2 groups. RESULTS The total response rate of observation group was significantly higher than control group after treatment (P<0.05). After treatment ,average pulmonary artery pressure,NT-proBNP level ,RAP,E/Ea,RVEDD and RVESD of 2 groups were significantly lower than before treatment ,while 6MWD,SvO2, CI, TAPSE and RVEF were significantly higher than before treatment ;NT-proBNP level and E/Ea of observation group were significantly lower than control group , while 6MWD,CI,TAPSE and RVEF were significantly higher than control group (P<0.05 or P<0.01). There was no significant difference in average pulmonary artery pressure , risk stratification improvement cases of pulmonary hypertension , RAP, SvO2, RVEDD, RVESD and the incidence of asymptomatic hypotension between 2 groups(P>0.05). CONCLUSIONS Intermittently repeated application of levosimendan can improve the function of right heart of pulmonary hypertension complicated with right heart failure and has good safety.
7.Study on the improvement of quality standard for Tibetan medicine Qinjiaohua

OBJECTIVE To improve the quality standard of T ibetan medicine of Qinjiaohua ,and to provide scientific basis for comprehensive quality evaluation. METHODS The qualitative analysis of 16 batches of Qinjiaohua with different producing areas and different origins was carried out by microscopic and TLC identification. According to the method stated in 2020 edition of Chinese Pharmacopoeia ,water content ,total ash content ,acid-insoluble ash content and alcohol-soluble extract content were determined. HPLC method was used to determine the contents of 5 components (loganic acid ,swertiamarin,gentiopicrin, swertionolin,isoorientin) in Qinjiaohua. RESULTS The medicinal powder of Qinjiaohua was light brown-yellow ,and the microscopic features of the powder were clear ,and pollen grains ,ducts,non-glandular hairs ,corolla epidermal cells and calyx epidermal cells were all found. The results of TLC indentification showed that there were fluorescent spots of the same color in the chromatogram of the tested product and the corresponding position of substance control (isoorientin). The content ranges of water content,total ash content ,acid-insoluble ash content and alcohol-soluble extract were 5.40%-8.87%,3.76%-6.40%,0.27%-0.58%, 26.81%-42.51%,respectively. The results of content determination methodology met the requirements of pharmacopoeia ;the content ranges of loganic acid ,swertiamarin,gentiopicrin,swertionolin and isoorientin in 16 batches of Qinjiaohua were 3.13-9.36,1.26-22.39,13.80-74.60,1.24-12.22,2.58-14.96 mg/g,respectively. CONCLUSIONS On the basis of the original quality standard of Qinjiaohua ,microscopic identification ,TLC identification ,content determination and examination items of water,total ash ,acid-insoluble ash and alcohol-soluble extract are added. It is preliminarily proposed that water content ,total ash content and acid-insoluble ash content should not exceed 9.0%,6.5% and 0.6%,while the contents of ethanol-soluble extract and gentiopicrin should not be less than 26.0% and 13.8 mg/g,respectively.
