1.Report of 27 culture-positive fungal endophthalmitis

Objective To report the clinical features and treatment outcomes of 27 patients with fungal endophthalmitis (27 eyes)over a five year period.Methods The authors retrospectively reviewed the etiology,direct smear examination,fungal cul-ture and treatments of 27 patients with culture-proven fungal endophthalmitis at the affiliated Hospital of Medical College Qing-dao University from 2007 to 2012.Results Exogenous infection was defined in 21 patients (77.8%),including 20 associated with penetrating wound,and 1 following cataract surgery.Endogenous infection was found in 6 patients (22.2%),including 3 associated with recent use of high-dose steroids,1 after abortion,1 following pelvic fracture,and 1 with long-term use of im-munosuppressive agents.Fungal hyphae were found in 17 smears of 27 samples (63.0%)by direct microscopic examination. The fungal strains cultured from 27 samples belonged to 8 genus and 12 species.The most common isolates were Aspergillus , Fusarium and Candida species.A.flavus (22.2%)and A.fumigatus (18.5%)were the predominant Aspergillus species. F .moniliforme (14.8%)and F .oxysporum (11 .1 %)were the most predominant Fusarium species.Two eyes were eviscer-ated immediately due to the serious condition.Among the other 25 eyes,22 (88.0%)got improvement after at least one of such treatments as intravitreal injection of antifungal agent,vitrectomy or penetrating keratoplasty (PK).Conclusions Exoge-nous fungal endophthalmitis is the most common type of fungal endophthalmitis in this hospital.Penetrating wound is the main cause of such infections.Microscopic examination of hyphae and fungal culture were effective for the diagnosis of fungal endophthalmitis.Aspergillus is the predominant pathogens, followed by Fusarium.Intravitreal injection of antifungal a-gent combined with vitrectomy is an effective treatment of fungal endophthalmitis.
2.Retrospective study on relationship between expression of insulin-like growth factor receptor and prognosis of patients with osteosarcoma

Objective to observe the correlation between the expression and prognosis of osteosarcoma Insulin-like growth factor receptor(IGF1R),explore the factors affecting the prognosis of patients with osteosarcoma.Methods between January 2005 and April 2014 menstrual standardized comprehensive treatment of 60 cases of patients with stage ⅡB osteosarcoma were retrospectively analyzed.Using immunohistochemical method,the detection of osteosarcoma tissue and IGF1R expression rate in normal tissue adjacent to tumors and to analyze its correlation with survival of patients with osteosarcoma and factors affecting the prognosis.Results IGF1R in the positive expression rate of osteosarcoma tissues was 80.0% (48/60),expression in normal tissue adjacent to tumor rate was 4/8,has a statistically significant difference between (P =0.032).IGF1R positive expression in osteosarcoma tissues group median disease-free survival time and the median overall survival time were significantly lower than the negative expression group (P =0.023,P =0.040) respectively.IGF1R expression level and pathological necrosis rate of preoperative chemotherapy,the time of first visit to the lung metastasis,and the survival prognosis of patients have significant correlation (P < 0.05),and age,tumor location,tumor size,the serum LDH and ALP levels,is urgent to make a diagnosis and give treatment time,factors such as operation,preoperative chemotherapy times no significant correlation (P > 0.05).Condusion IGF1R positive expression rate in stage Ⅱ B osteosarcoma tissue significantly negatively correlated with the prognosis of patients with osteosarcoma,is the independent prognostic factors of patients with osteosarcoma.
3.Experimental study on infertility related antibody detection in female infertility

Objective To study whether the correlation exists between 6 kinds of autoimmune antibodies and female infertility . Methods The protein microarray technique was adopted to detect serum antibodies of anti-sperm antibodies(ASAb) ,anti-endome-trial(AEmAb) ,anti-zona pellucida(AZpAb) ,anti-ovarian(AVoAb) ,anti-hCG(AhCG Ab) and anti-trophoblastic(ATAb) in 140 ca-ses of female infertility and the correlation between these 6 kinds of antibody with female infertility was evaluated .Results Com-pared with the control group ,except AvoAb ,the positive rates of other 5 kinds of antibody had statistically significant differences (P<0 .05) .In the infertility group ,the positive rate of the combined detection of these 6 kinds of autoimmune antibody had statisti-cal difference compared with that of single item detection (P<0 .05) .The positive rate of these 6 kinds of autoimmune antibody in the primary infertility group and the secondary infertility group had statistical difference compared with the control group ;the posi-tive rate of these 6 kinds of autoimmune antibody in the secondary infertility caused by different etiologies had no statistical differ-ence (P>0 .05) .The positive rate of ANA had no statistical difference between the infertility group and the control group (P>0 .05) ,but the titers of ANA in the experimenter were ≥1∶100(6/7) and most of karyotypes were nuclear coarse (5/7) ,which was differed from the low positive titer in the control group .The positive rate of ANA had statistical difference between the infertility related antibody positive and the infertility related antibody negative in the infertility patients .Conclusion ASAb ,AZpAb AEmAb , AhCGAb and ATAb are related with female sterility .The combined detection of these 6 kinds of antibody can significantly increase the detection sensitivity .
4.Chemical constituents in leaves of Acanthopanax gracilistylus

Objective To study chemical constituents in the leaves of Acanthopanax gracilistylus.Methods The chemical components were isolated and purified by silica gel,ODS C-18,and Sephadex LH-20 column chromatogram.The chemical structures were elucidated on the basis of physicochemical properties and spectral data.Results Seventeen compounds were isolated and identified as(-)-kaur-16-en-19-oic acid(Ⅰ),quercetin(Ⅱ),kaempferol(Ⅲ),protocatechuic acid(Ⅳ),acankoreoside A(Ⅴ),acantrifoside A(Ⅵ),3?,11?-dihydroxy-20(29)-lupene-23,28-dioic acid(Ⅶ),?-sitosterol(Ⅷ),daucosterol(Ⅸ),palmitic acid(Ⅹ),rutin(Ⅺ),stigmast-5,22-dien-3-O-?-D-glucopyranoside(ⅩⅡ),acankoreagenin(ⅩⅢ),3,11-dihydroxy-23-oxo-20(29)-lupen-28-oic acid(ⅩⅣ),3-hydroxy-23-oxo-20(29)-lupen-28-oic acid(ⅩⅤ),myristin(ⅩⅥ),and acanthopanaxgric acid(ⅩⅦ).Conclusion Compounds Ⅱ-Ⅳ,Ⅶ,Ⅺ,ⅩⅡ,ⅩⅣ,and ⅩⅤ are obtained from the leaves of the plant for the first time and compounds ⅩⅦ is a new proved compound named acanthopanaxgric acid.
5.Susceptibility to AcMNPV and Expression of Recombinant Proteins by a Novel Cell Clone Derived from a Trichoplusia ni QAU-BTI-Tn9-4s Cell Line

It is well known that Tn5B1-4(commercially known as the High Five)cell line is highly susceptible to baculovirus and provides superior production of recombinant proteins when compared to other insect cell lines.But the characteristics of the cell line do not always remain stable and may change upon continuous passage.Recently an alphanodavirus,named Tn5 Cell Line Virus(or TNCL Virus),was identified in High Five cells in particular. Therefore,we established a new cell line,QB-Tn9-4s,from Trichoplusia ni,which was determined to be free of TNCL virus by RT-PCR analysis. In this paper,we describe the development of a novel cell clone,QB-CL-B,from a low passage QB-Tn9-4s cell line and report its susceptibility to ACMNPV,and the level of recombinant protein production. This cell clone was similar to its parental cells QB-Tn9-4s and Tn5B 1-4 cells in morphology and growth rate;although it also showed approximately the same responses to AcMNPV infection and production of occlusion bodies,there were higher levels of recombinant protein production in comparison to QB-Tn9-4s(parental cells)and High5 cells.
6.Analysis on staphylococcus xylosus inducing postoperative nosocomial infection

Objective To investigate the pathogens inducing a posto perative nosocomial infection. Methods Specimens was collected fro m exudates or air for bacteria culture and identification. Re sults The postoperative infection was induced by staphylococcus x ylosus. Conclusions The relevant factors affecting the po stoperative nosocomial infection include incomplete sterilization of operative r oom and operative tools. Thus strict control measures must be put into effect.
7.Protective effects of valproic acid on lipopolysaccharide-induced acute lung injury in rats

Objective To investigate the effects of valproic acid (VPA) on acute lung injury induced by Lipopolysaccharide in rats. Method The rat model of acute lung injury was made by intravenous injection of lipopolysaccharide (LPS). The pathological changes of lung were observed under light microscope and inflammatory cytokines in serum detected by using ELISA to judge whether the model was successfully done or not. All rats were divided into three groups as per the different intervention agents employed. Rats in control group were treated with intravenous injection of NS in dose of 5 ml/kg, rats in LPS group were exposed to LPS with dosage of 10 mg/kg and model rats in LPS + VPA group were treated with VPA in dose of 300 mg/kg. The rats were sacrificed 6 h after LPS or NS administration. The blood PaO2 ,A-aDO2 and blood lactic acid (Lac) were measured, the lungs were removed for observing the histopathological changes and determination of wet/dry lung weight (W/D) ratio and myeloperoxidase (MPO) activity as well as albumin concentration in broncho-alveolar lavage fluid (BALF) . Seurm was collected to determine the concentrations of tumor necrosis factor-a (TNF-α) and interleukin-1β( IL-1 β) by using LISA 6 h later. All data were presented in ((x)±s). One-way ANOVA was used for comparing differences between groups. Results Compared with acute lung injury group, the blood PaO2 (94. 50 ± 4.38 ) in rats of LPS + VPA group was higher, whereas A-aDO2 ( 13.50 ± 4.77 ) and blood lac( 2.13 ± 1. 02 ) in LPS + VPA group were lower. VPA significantly lowered W/D (5.33 ±0. 12) ratio and MPO activity (4.38 ±0. 42) in the lung. Albumin concentration ( 1. 260 ± 0. 039 ) in BALF, and the levels of TN F-α( 2 410 ±320 )and IL-1β( 1 220 ± 162 )in serum were lower in LPS + VPA group. The histological changes of lung injury were lessened by VPA. Conclusions Valproic acid has protective effects against lipopolysaccharide-induced acute lung injury in rats.
8.Significance of mediastinum drainage tube in prevention and treatment of intrathoracic anastomotic leak after esophagectomy

Objective To explore the significance of mediastinum drainage tube in prevention and treatment of intrathoracic anastomotic leak after esophagectomy. Methods The morbidity and prognosis of intrathoracic anas-tomotic leak after esophagectomy in 180 patients with esophageal or cardia carcinoma (group A) who used mediasti-num drainage tube in operation from 2006 to 2008 was studied retrospectively,and were compare with 154 patients (group B) without mediastinum drainage tube from 2004 to 2006. Results The morbidity of intrathoracic anasto-motic leak after esophagectomy was 1.67% (3/180) in group A,and 4.55% (7/154) in group B,but no difference between them(χ2=1.4807, P>0.05) were found. Compared to group B, the final diagnosis of anastomotic leak in group A was early [(6.7±0.6) vs (10.0±2.1) days after operation (t=2.62, P<0.05)] and the duration of fe-ver was short [(5.3±1.5)vs (43.0±20.9) hours (t=4.39, P<0.01)]. The prognosis of 3 cases in group A was well, who all recovered without operation and had shorten hospital days than group B [(23.7±5.9)d vs (45.3± 5.0)d,t=4.86,P<0.01)]. In 7 cases of group B,there were 5 cases underwent second operation,but only 3 cases recovered, the case fatality rate was 2.60% (4/154), higher than in group A (0, Fisher, P<0.05). Conclusions Although the mediastinum drainage tube used or not used in esophagectomy is unrelated with the occurrence of anas-tomotic leak, and the significance to use it is that it can help to discover anastomotic leak early, and control mediasti-num infection effectively without reoperation,and improve patients' prognosis.
9.Surgical treatment of invasive pulmonary fungal infections in patients with hematologic malignancies

Objective To summarise the experience of surgical treatment of invasive pulmonary fungal infection in patients with hematologic malignancies.Methods From Jan.2000 to Dec.2012,78 hematologic patients with invasive pulmonary fungal infections,including 49 males and 29 females with average age of 29.6 years,underwent surgical treatment.Preoperatively,all patients with 1 to 3 courses of chemotherapy had received average 4 weeks antifungal treatment.There were 58 cases received conventional open thoracic surgery and 20 cases received VATS including lobectomy,segmentectomy,wedgectomy.Results The procedures were successful,the average operation time w as(125.3 ± 35.7) min and intraoperative blood loss was(253.1 ± 42.8) nl.There were no severe postoperative complications.The main complications were excessive effusion (10.2%) and persistence airleak (7.7%).The hospital days was(7.8 ± 2.5) days,no reoperation and death case in 30 days after operation.The follow-up was from 6 to 24 months,38 cases(48.7%) received prophylaetic antifungal treatment,no ease with fungi spread and recurrence.Conclusion The surgical treatment is safe and useful for the hematologic patients with invasive pulmonary fungal infections,when internal medicine conservative treatment is invalid.
10.Curative efficacy of tetramethylpyrazine in combination with chemotherapy in treatment of medium and advanced liver cancer and its effects on level of BDNF

Objective To investigate curative efficacy of tetramethylpyrazine in combination with chemotherapy in treatment of medium and advanced liver cancer and its effects on level of brain-derived neurotrophic factor ( BDNF).Methods 90 patients of medium and advanced liver cancer who received therapy from January 2011 to June 2012 were selected as research objects.According to therapeutic schemes, those patients were divided into the control group (n=42) and the observation group (n=48).The control group was treated with transcatheter hepatic arterial chemoembolization ( TACE) , while the observation group was treated with tetramethylpyrazine in combination with TACE.Then, the short-term curative efficacy, long-term curative efficacy, level of BDNF and adverse reactions were compared.Results The total short-term therapeutic efficacy ratio in the observation group was statistically higher than that in the control group ( 83.3% vs 64.3%, P <0.05 ).During the three-year follow-up, the one-year and two-year survival rate in the observation group was statistically same with that in the control group respectively (75.0% vs 66.7%, 66.7% vs 59.5%), while the three-year survival rate was statistically higher than that in the control group (52.1%vs 30.9%, P<0.05).After treatment, in comparison with the control group, level of BDNF in the observation group was statistically lower(P<0.05).During treatment, incidences of liver function deterioration, abdo minal pain and diarrhea, nausea and vomiting, fever and headache in two groups were statistically same.Conclusion Tetramethylpyrazine in combination with TACE is effective for medium and advanced liver cancer, which can increase short-term and survival rate to some extent, significantly reduce level of BDNF with not increasing incidence of adverse reactions.
