1.Discussion on Academic Features of “Yi Lin Kou Pu Liu Zhi Mi Shu”

Objective] Making a discussion on academic features ofYi Lin Kou Pu Liu Zhi Mi Shu. [Method] With the research methods of philology, we collect Qing transcript ofYi Lin Kou Pu Liu Zhi Mi Shuand analyze its academic features. [Result]Yi Lin Kou Pu Liu Zhi Mi Shuwas additionally compiled, supplemented and annotated by Zhou Sheng, a famous doctor of the Qing Dynasty, on the base of Yi Lin Kou Pu which was composed by Lu Qi. Within four volumes, the contents of the book contain Medicine, Surgery, Gynecology, Pediatrics, etc. The diagnosis and treatment based on syndrome differentiation and Chinese herbal prescription in this book have their own characteristics. That is: theories from different schools make arguments on the facts; six treatments of syndrome lucidly expose the essential points; secret prescriptions and Chinese herbs are widely collected; opinions and knowledge from his teacher grasp the essence of the problem and bring out the crucial point. [Conclusion]Yi Lin Kou Pu Liu Zhi Mi Shu is a distinctive and only existing medical book with high academic value and practical significance.
2.Detection rate analysis on neurological sign of workers exposed to different concentrations of carbon disulfide.

OBJECTIVETo study the effects of exposed to different concentrations of carbon disulfide on neurological signs of workers.
METHODSCollection the information of concentration of carbon disulfide in the workplace or workers individuals exposed of a chemical fiber industry from 2004 to 2011, a total of 3 537 workers exposed to carbon disulfide were detected muscle strength and muscle tone, knee reflex, Achilles tendon reflex, trembling limbs, sensory function, and three chatter. Chi-square test was used for statistical analysis on abnormal neurological signs of workers.
RESULTSEight hours time-weighted average concentration range of workers exposed to carbon disulfide in this chemical fiber industry was 0.2-41.0 mg/m(3), geometric mean was 2.38 mg/m(3). Concentration of carbon disulfide exposure of 1 771 workers was from 0.2 to 2.5 mg/m3( ≤ 2.5 mg/m(3)), 642 workers was 2.6-4.8 mg/m(3) (< 5.0 mg/m(3)), other 1 051 workers was from 5.1 to 41.0 mg/m(3) ( > 5.0 mg/m(3)) in all subjects. The different detection rates of knee reflex were 3.0% (31/1 045), 3.7% (21/574), 4.8% (16/331), 3.3% (10/305), 5.9% (11/187), 6.7% (68/1 022), the different detection rates of Achilles tendon reflex were 2.2% (23/1 045), 3.7% (21/574), 2.7% (9/331), 2.3% (7/305), 2.1% (4/187), 5.6% (57/1 022), the different detection rates of sensory dysfunction were 0.4% (4/1 045), 0.5% (3/574), 0.6% (2/331), 0.0% (0/305), 2.1% (4/187), 1.7% (17/1 022) in different cumulative amount of contact groups ( ≤ 10.0, 10.1-20.0, 20.1-30.0, 30.1-40.0, 40.1-50.0, >50.0 mg/m(3) per year), and the differences were statistically significant (χ(2) = 19.53, 21.27 and 15.89, all P values were <0.01) . Stratified according to age and gender, in addition to the ≤ 25 years group the difference of detection rate analysis on Achilles tendon reflex was statistically significant in the different concentration group (the ratio of on Achilles tendon reflex in the different groups of concentration of carbon disulfide exposure of 2.5, 2.6-5.0, ≥ 5.0 mg/m(3) were 0.4% (2/511), 1.0% (1/98), 2.1% (7/327), χ(2) = 5.59, P = 0.045) , the difference of detection rate analysis on neurological sign was not statistically significant in the different concentration group on the rest of the age and gender groups (P > 0.05).
CONCLUSIONWithin the concentration range of the object of study contact actual, different concentrations of carbon disulfide in addition to individual neurological signs of individual ages influential, it has no significant effect on the various signs of nervous system of workers of most age and gender groups, expect the age below the 25 years old group.

3.System review of the Chinese medicine bushenhuoxue for treating benign prostatic hyperplasia.

OBJECTIVETo make a system review of the effects of Bushenhuoxue (kidney-tonifying and blood-activating prescription), a category of compound Chinese medicines, on benign prostatic hyperplasia (BPH) and its side effects.
METHODSAll the research articles about Chinese medicines treating BPH from January 1978 to February 2003 were retrieved using the methods of international evidence-based medicine (EBM), and their qualities were evaluated based on JADAD standard and concealment of research allocation. Then the included articles went through META-analysis with Revman 4.2 software.
RESULTSThe efficacy of Bushenhuoxue on BPH was better than Qianliekang but not statistically different from finasteride. The combined use of Bushenhuoxue with surgery had no statistical difference from mere surgical treatment. Four articles reported the side effects of this compound Chinese medicine including upset and pain in the abdomen, nausea, diarrhea and dryness of the nose.
CONCLUSIONMassive, multi-center and randomized controlled clinical trials should be conducted to find out more effective methods for treating BPH with Chinese medicines based on the improvement of measurable symptom evaluation method and for evaluating their side effects.

4.Prognosis analysis of 117 nasopharyngeal carcinoma patients treated by intensity modulated radiotherapy

Objective To analyse the prognosis of 117 newly diagnosed nasopharyngeal carcinoma (NPC) patients underwent intensity modulated radiotherapy (IMRT). Methods From Jan to Nov 2005, 117 NPC patients who were treated by IMRT were enrolled. There were 81 males and 36 females with a median age of 42 years (range 18-76 years). According to Chinese Fuzhou Staging system(1992), 11 cases were Stage I , 15 Stage Ⅱ, 54 Stage Ⅲ and 37 Stage ⅣA. IMRT was carried out with Peacock plan. The prescription dose to the gross target volume(GTVnx) of nasopharyngeal tumor was 68 Gy, that of positive neck lymph nodes (GTVnd) was 60-66 Gy, clinical target volume 1 (CTV1) was 60 Gy, and CTV2 was 54 Gy. Results After a median follow-up time of 48 months (range 10.5-59.5 months), the 3-and 5-year overall survival (OS) rates were 95.7 % and 89.7 %, the disease-free survival (DFS) rates were 91.5 % and 87.2%, and the local-regional control rates were 94.0 % and 91.5 %. Univariate analysis showed the KPS, stage, Fuzhou clinical stage, status of blood platelet before treatment and uric acid after treatment were correlated with OS rate. T stage was the only independent factor of prognosis in the COX stepwise regression model. Conclusion Radical IMRT significantly prolongs the survival of NPC patients. T stage is the only independent prognostic factor for NPC patients.
5.Correlation between Intra-abdominal Fat Thickness and Components of Metabolic Syndrome with Regard to Gender Differences

Objective To investigate gender differences in the prevalence of metabolic syndrome (MS) and to explore the relationship between intra-abdominal fat thickness and MS and its components with respect to gender differences. Methods Based on gender and IDF AHA/NHLBI criteria used to diagnose MS,205 middle-aged and elderly urban residents were divided into a male MS group (124 patients) and a female MS group (81 patients). The relative indices and the thickness of intra-abdominal fat were measured. Results The prevalence of MS was observed to be higher in men (P < 0.01). A greater number of men than women showed waist circumference and blood glucose abnormalities,but fewer men than women showed high-density lipoprotein cholesterol (HDL-C) abnormalities (P < 0.05). Multivariate logistic regression analysis showed that after adjusting for potential confounding factors,intra-abdominal fat thickness was an independent predictor in men,but not in women (men:OR = 1.304,95%CI:1.014-1.676,P = 0.039,women:OR = 1.257,95%CI:0.984-1.605, P = 0.067). Intra-abdominal fat thickness was linearly correlated with age,waist circumference,and the 2-hour insulin levels in men,but only linearly correlated with waist circumference in women. Conclusion Prevalence of MS and MS components varies between genders. Intra-abdominal fat thickness was an independent predictor of MS in men. The role of intra-abdominal fat thickness as a determinant of MS in men can not be replaced by the waistline circumference alone as determined in women.
6.Expression profiling of long noncoding RNAs and mRNAs in infantile hemangioma

Objective:To investigate differentially expressed long noncoding RNAs (lncRNAs) and mRNAs between proliferating and involuting infantile hemangioma (IH) .Methods:Eight IH specimens were surgically resected from 4 children with proliferating IH and 4 with involuting IH in Department of Pediatric Surgery, West China Hospital, Sichuan University from January to March in 2019. Differentially expressed lncRNAs and mRNAs between proliferating and involuting IH (a P value < 0.05 and a fold change ≥ 2.0) were screened by microarray analysis, and verified by real-time fluorescence-based quantitative PCR (qRT-PCR) . Bioinformatics analysis was further performed, including GO and KEGG pathway analyses of differentially expressed mRNAs, construction of a lncRNA-mRNA co-expression network, and prediction of cis-acting target genes of differentially expressed lncRNAs. Results:A total of 405 differentially expressed lncRNAs and 772 differentially expressed mRNAs were identified between the proliferating and involuting IH specimens by using microarray technology. Of them, 108 lncRNAs and 107 mRNAs were downregulated, 297 lncRNAs and 665 mRNAs were upregulated in the proliferating IH specimens. Four lncRNAs (n335248, ENST00000450864, n333319, and n335185) and 4 mRNAs (EDNRA, IFI6, HK2, and ITGA1) were verified by qRT-PCR, and the results were consistent with those of microarray analysis. GO and KEGG enrichment analyses showed that differentially expressed mRNAs were mainly involved in the biological processes blood coagulation, axon guidance, angiogenesis and cell adhesion. Besides, differentially expressed mRNAs were mostly enriched in the metabolic pathways focal cell adhesion, regulation of actin cytoskeleton, leukocyte transendothelial migration and phosphatidylinositol 3-kinase/serine-threonine protein kinase and other signaling pathways. In addition, a lncRNA-mRNA co-expression network was constructed with 23 mRNAs and 58 lncRNAs with the degree ≥ 15.Conclusion:Lots of differentially expressed lncRNAs and mRNAs were identified between proliferating and involuting IH tissues, and these lncRNAs may play important roles in the development of IH by regulating corresponding target mRNAs.
7.Research progress on the effect of parathyroidectomy on the quality of life in patients with secondary hyperparathyroidism

Secondary hyperparathyroidism (SHPT) is a common and serious complication in long-term dialysis patients, who often experience multiple symptoms and a poor quality of life. Parathyroidectomy (PTX) is the best choice for severe SHPT patients with poor drug treatment. At present, the follow-up after PTX has not received sufficient attention, and the assessment of patients' clinical symptoms and quality of life is not yet adequate. In view of this, we systematically combed the changes and influencing factors of patients' quality of life and related symptoms after surgery, in order to provide reference for the practice and research of symptom management of such patients in the future.
8.The Timing of Return to Sports after Anterior Cruciate Ligament Reconstruction in Elite Athletes: A Study Based on Isokinetic Knee Muscle Strength and Multiple Hop Tests

Objective To compare the knee function recovery at different times of returning to sport after anterior cruciate ligament reconstruction(ACLR) among elite athletes using knee isokinetic muscle strength test and various hop test.Methods Forty-one elite athletes(14 males,27 females,mean age 22.6 ± 4.1 years) undergoing ACLR between January 2013 and September 2014 were chosen from the database of the National Institute of Sports Medicine and Shanghai Huashan Hospital.Rehabilitation was performed using the same protocol by professional physiotherapists and trainers,who recorded the time of returning to sport of each athlete.One week prior to the scheduled return,bilateral knee isokinetic muscle strength test(test value:peak torque;angle velocity:60°/s,180°/s;motion:flexion,extension) and four hop tests(single hop for distance,side-to-side hop,up-down hop and 8 hop) were applied with the limb symmetry index(LSI) calculated.The athletes were then grouped by their returntime referring to surgery into the premature group(6~8 months),timely return group(9~12 months) and delayed group(over 12 months).The tests results were recorded and compared among the three groups.Results Fourteen athletes were selected into the premature group,with 19 in the timely group and 8 in the delayed group.The average LSI of 60°/s flexion peak torque of the premature group (87.4% ± 7.5%) was significantly lower than the timely group(95.8% ± 6.6%) and the delayed group(96.0% ± 2.4%) (P<0.01).Significant differences were observed between the premature group and delayed group regarding the 60° extension peak torque(85.8% ± 9.4% and 94.8% ± 4.8%,P<0.05),180°/s flexion peak torque(90.7% ± 8.7% and 101.4% ± 6.8%,P<0.05),and 180°/s extension peak torque (90.6% ± 5.2% and 97.8% ± 5.6%,P<0.05).The average LSL of the premature group at single hop for distance,side-to-side hop and up-down hop(93.A% ± 8.5%,84.7% ± 7.3% and 112.5% ± 5.7%) was significantly lower than that of the timely group(95.7% ± 6.0%,104.2% ± 4.3% and 105.3% ± 7.9%) and the delayed group regarding(98.1% ± 1.9%,104.7% ± 4.0% and 106.3% ± 7.4%) (P<0.01 for all).The relative peak torque of 60°/s extension of the premature group(2.48 ± 0.58 Nm/kg) was significantly lower than the delayed group(3.21 ± 0.51 Nm/kg) (P<0.01).Conclusions For elite athletes,returning to sport within 9 months after ACLR results in insufficient restoration of the knee function.Delayed return to sport doesn't improve the outcomes of hop tests,but can enhance the maximum extension torque peak,which needs further study.
9.Prenatal diagnosis for a pedigree affected with Wolf-Hirschhorn syndrome due to a subtle chromosomal translocation.

OBJECTIVE:
To make molecular diagnosis of an infant affected with severe developmental delay and multiple birth defects, assisting prenatal diagnosis for the second pregnancy.
METHODS:
Standard G-banded karyotyping was performed for the fetus and his parents. Single nucleotide polymorphism array (SNP array) was used to detect submicroscopic chromosomal aberration. Fluorescence in situ hybridization (FISH) was employed to determine the parental origin of the aberration.
RESULTS:
Both the proband and the fetus harbored a 5.4 Mb distal 4p deletion and a 6.9 Mb distal 6q duplication. FISH confirmed that the mother has carried a balanced translocation involving 4p and 6q.
CONCLUSION
The unbalanced chromosomal aberration in the proband and the fetus were both derived from the mother. Both patients showed a Wolf-Hirschhorn syndrom phenotype and partial phenotype of 6q trisomy. SNP array combined with FISH are essential for the detection of cryptic chromosomal aberrations which may be missed by coventional karyotyping analysis.

10.Sirolimus combined with prednisone in the treatment of Kaposiform hemangioendothelioma complicated by Kasabach-Merritt phenomenon: a preliminary study

Objective:To preliminarily assess the clinical value of sirolimus combined with prednisone in the treatment of Kaposiform hemangioendothelioma (KHE) complicated by Kasabach-Merritt phenomenon (KMP) .Methods:A retrospective study was conducted. General clinical data and relevant data on efficacy and adverse reactions were collected from 36 patients with KHE complicated by KMP, who received oral sirolimus combined with prednisone in Department of Pediatric Surgery, West China Hospital, Sichuan University from January 2011 to January 2018.Results:Among the 36 patients with KHE complicated by KMP, the male-to-female ratio was 1∶0.8; their average age was 15.0 months (range, 4.0 - 60.0 months) , and the average age of onset was 6.3 months (range, 0.8 - 48.0 months) ; 32 (88.9%) patients were diagnosed with mixed KHE, and 4 (11.1%) with deep KHE; the tumor size ranged from 2.5 cm × 4.0 cm to 20.0 cm × 24.0 cm. During the combined treatment, the average duration of prednisone administration was 6.4 weeks (range, 5.0 - 9.0 weeks) , and that of sirolimus administration was 19.3 months (range, 13.0 - 27.0 months) . After 1-5 weeks of combined treatment, platelet counts and fibrinogen levels of the 36 patients gradually returned to normal. After short-term prednisone combined with long-term sirolimus treatment, the average disease severity score decreased from 4.0 before treatment to 2.4 at 6 months and 1.6 at 12 months. After 12 months of the combined treatment, tumors mostly regressed in 32 (88.9%) patients, partially regressed in 3 (8.3%) , showed no obvious change in 1 (2.8%) . During the treatment, common adverse reactions included gastrointestinal discomfort and oral ulcers, no patient discontinued the treatment due to severe adverse reactions, and no drug toxicity-related death occurred.Conclusion:Sirolimus combined with prednisone is effective and safe for the treatment of KHE complicated by KMP.
