Objective To study the clinical effect of remifentanil intravenous patient-controlled analgesia pump combined with psychological intervention in labor analgesia.Methods 100 cases of maternal in our hospital from April 2015 to July 2016 were selected as the research object in this study.They were randomly divided into the control group and the experimental group, each group had a total of 50 cases.Patients in the control group were treated with intravenous patient-controlled analgesia pump, while the experimental group received psychological intervention on the basis of remifentanil treatment.The anxiety, pain and cesarean section were compared between the experimental group and the control group.Results After the corresponding treatment, the experimental group of 50 patients, the number of cases of cesarean section in 6 cases, the rate of cesarean section was 12%, in the control group of patients with a total of 50 cases, the number of cases of cesarean section in patients with cesarean section rate of 28%.But the control group was significantly higher than that of the experimental group, the difference was statistically significant(P<0.05).The visual analogue score of the experimental group was significantly lower than that of the control group, with statistical difference(P<0.05).The incidence of adverse reactions in the experimental group was 18%, and the incidence of adverse reactions in the control group was 14%.The number of patients with moderate anxiety in the control group was significantly higher than that in the experimental group, with statistical difference(P<0.05).Conclusion Remifentanil intravenous analgesia pump combined with psychological intervention for clinical effect of labor analgesia is better, can effectively eliminate the anxiety of patients, relieve pain, and high safety, is further applied in clinical significance.

Phloroglucinol is an antineoplastic effective compound from Dryopteria Crassirhizoma Nakai. In this study, mice implanted with leukemia P388 were examined under an electron microscope. It' was found that the leukemia bearing mice, which had been injected with phloroglucinol exhibited degeneration and swelling of mitochondria in the P38'8 cells. Measurement with an oxygen electrode showed that the drug treatment of P388 cells markedly lowered their oxygen consumption in comparison with the control cells .The results demonstrate that phloroglucinol inhibits respiration of tumor cells. Probably, the mitochondria of tumor cells is a target that the drug attacks.

AIM: To investigate the antidysmsenorrhea pharmacologic effects of Compound Yimucao Paste (Herba Leonuri, Rhizoma chuanxiong, Radix Angelicae Sinensis, Flos Carthami) METHODS: The methods of microcirculation and isolated uterus smooth muscle of rat were used. RESULTS: Compound Yimucao Paste could improve the microcirculation disturbance on the aspects of the diameter, the velocity of mice auricle microarterioles and microveins, inhibit the isolated uterus smooth muscle contraction and mouse twister induced by oxytocin. CONCLUSION:Compound Yimucao Paste has an obvious action of blood quickening and stasis transforming and inhibit the isolate and uterus smooth muscle contraction induced by oxytocin, which are contributed to its antidysmenorrhea effect.

Objective To investigate how build a comprehensive basic theory training curriculum for residents.Methods Random questionnaire survey of residents,along with a forum of experts to explore an optimal basic theory training curriculum for residents.Results 71.9％ of the residents surveyed were satisfied with the basic theory training curriculum,14.9％ proposing training time be adjusted to evenings or weekends,and 8.8％ proposing to adjust the training content.Conclusion A diversified reform on the training content,training time,training teachers,management and teaching evaluation aspects.These will help building a comprehensive and practical,standardized and reasonable basic theory curriculum,in order to effectively improve the quality of residency training.

Objective To explore the clinical significance of early invasive mechanical ventilation for acute respiratory distress syndrome (ARDS) caused by thoracic trauma.Methods We retrospectively analyzed the clinical data of 93 patients of invasive mechanical ventilation in those with ARDS caused by thoracic trauma,and divided them into delayed invasive mechanical ventilation group (n =45) and timely invasive ventilation group (n =48) by the time of invasive mechanical ventilation.We compared the blood gas analysis,oxygen index,heart rate,lactic acid,muscle use score of the two groups before and after invasive mechanical ventilation and the differences of mechanical ventilation time,incidence of complications,mortality and length of hospital stay between the two groups.Results Each index of the delayed invasive mechanical ventilation group and the timely invasive ventilation group was calculated:PaO2 (63.2 ± 12.3,74.2 ± 12.1)mm Hg,PaCO2 (39.4 ±8.7,38.9 +7.6) mm Hg,PaO2/FiO2 (196.2 ± 17.8,231.4 ± 18.2),respiratory rate (27.4 ±3.7,21.1 ± 3.4) beats/min,heart rate (117.2 ± 26.8,96.0 ± 25.1) beats/min,accessory muscle use score (2.69 ± 0.15,1.67 ± 0.10),lactic acid (4.23 ± 1.70,2.12 ± 0.80) mmol/L Except for PaCO2,the differences of each index were statistically significant (t values were respectively 4.346,9.420,3.949,5.436,38.809,7.736; P ＜ 0.05).The duration of mechanical ventilation treatment (10.21 ± 1.50,6.47 ± 1.20) d and the length of hospital stay (21.53 ± 1.90,16.32 ±2.10) d of the delayed group and the timely group were significantly different (t values were respectively 12.518,13.318; P ＜ 0.01).The timely group had significantly lower mortality rate (26.7％ (12/45),14.6％ (7/48)),VAP (46.7％ (21/45),22.9％(11/48)),acute gastric mucosal lesions (33.3％ (15/45),12.5％ (6/48)),MODS (40.0％ (18/45),16.7％ (8/48)) than the delayed group (x2 values were respectively 3.86,5.81,4.49,5.70; P ＜ 0.05).There is no significant difference on the incidence of pneumothorax between the two groups (11.1％ (5/45),8.3％ (4/48)).Conclusion Invasive mechanical ventilation is an effective treatment of ARDS caused by chest trauma.Early invasive mechanical ventilation can reduce the mortality rate and shorten the duration of mechanical ventilation and hospital stay of the patients with ARDS after severe thoracic trauma.

Objective To observe the molecular weight distribution of soybean peptides prepared by complex enzyme hydrolysation and tumor inhibitory effect in vivo or in vitro. Method The soybean protein was enzymolyzed to soybean peptides by Flavourzyme, Alealase and neutral protease, at(50-55)℃, followed by separation, absorption,affination, ultra centrifuge and spray drying. The soybean peptides were passed Sephadex G25 column at 0.5ml/min with 0.1mol/L Tris-HCl buffer. The fragment of fluid collected was determined by UV at 280nm and HPLC. Then the molecular weight distribution was calculated. The inhibitory effect on ansplant glioma (G422) at dose 1.25, 2.5 and 5g/kg bw (ig daily for 11 d) was tested in mice and in Hep-2 in vitro. BACKGROUND: Mild cognitive impairment (MCI) in elderly people is becoming increasingly a new hot spot for the clinical study of Alzheimer disease at present, and it is still unknown how many cases of MCI will convert into Alzheimer disease. OBJECTIVE: To analyze the incidence rate of mild cognitive impairment and the conversion rates into dementia and Alzheimer disease among elderly people.DESIGN: A prospective cohort study based on diagnosis. SETTING: Second Department of Cadre's Ward, Bethune International Peace Hospital of Chinese PLA.PARTICIPANTS: The subjects were 216 MCI patients and 2 302 subjects with normal cognition selected in the baseline investigation to the retired cadres aged 60 years and above, in 26 military cadres' sanatoriums of Shijiazhuang city between August and September 2001.METHODS: The MCI diagnosis was according to the MCI diagnostic standard in Diagnostic and Statistical Manual of Mental Disorders - Fourth Edition (DSM-Ⅳ), and the diagnosis of dementia was according to the standard in DSM-Ⅳ revised by American Psychiatric Association. Alzheimer disease was diagnosed according to the standards of National Institute of Neurological and Communicative Disorders and Stroke/the Alzheimer Disease and Related Disorders Association (NINCDS-ADRDA). All the 216 MCIpatients and 2 302 subjects with normal cognition were enrolled in the 3-year cohort study. MAIN OUTCOME MEASURES: The incidences of MCI among subjects with normal cognition; the annual conversion rates into Alzheimer disease in MCI patients and subjects with normal cognition; the relative risks (RR) and 95% confidence interval (95% CI) of MCI and normal cognition developing into Alzheimer disease. RESULTS: The MCI group and normal cognition group were followed up once every year for 3 years. Of the 216 MCI patients, 7 died, and 209 were followed up actually, and the follow-up rate was 96.8%. Of the 2 302 subjects with normal cognition, 36 died, and 2 266 were followed up actually,and the follow-up rate was 98.4%. ① In the elderly subjects with normal cognition, the annual incidence of MCI was 4.8%, and those of dementia and Alzheimer disease were 1.3% and 0.8 respectively. ② The annual in cidence rates of dementia and Alzheimer disease in MCI patients were 8.1% and 5.6% respectively, which were insignificantly different between males and females (P ＞ 0.05). ③ The incidence rates of dementia and Alzheimer disease showed a descending trend with the elevation of educational level in both the MCI patients (trend x2 =5.57, P=0.02; trend x2 =4.92, P=0.03) and the subjects with normal cognition (trend x2 =23.1, P＜ 0.001; trend x2 =18.0, P ＜ 0.001). ④ The annual incidence rates of dementia and Alzheimer disease showed an ascending trend with aging in both the MCI patients (trend x2 =14.6, P ＜ 0.01; trend x2 =13.9, P ＜ 0.01) and the subjects with normal cognition (trend x2 =32.3, P ＜ 0.01; trend x2=23.8, P ＜ 0.01). ⑤ The relative risks of the conversion into dementia and Alz heimer disease in MCI patients were 6.4 and 7.4 times as many as those in the subjects with normal cognition.CONCLUSION: The risks of conversion into dementia and Alzheimer disease in elderly MCI patients are far higher than those in elderly people with normal cognition. It should be reinforced to monitor elderly MCI patients, who are the of high risk group of dementia.
