1.The clinical effect of radio frequency catheter ablation guiding by B ultrasound combined with CT in the treatment of primary liver cancer

Objective To explore the clinical effect of radio frequency catheter ablation( RFCA) guiding by B ultrasound combined with CT on treating primary hepatic carcinoma. Methods Eighty-two patients with primary liver cancer were treated with transcatheter arterial chemoembolization therapy. One week after the operation,41 patients were treated with RFCA guiding by B ultrasound combined with RFA therapy and served as treatment group. Another 41 cases were given RFA treatment guiding by CT as control group. The indices including tumor shrinkage rate,survival rate of 0. 5,1 and 2 years. Results The tumor shrinkage and total effective rates in the treatment and control group were 95. 12%(39 / 41)and 80. 49%(33 / 41),and there was statistically significant difference between the two groups(χ2 = 5. 33,P < 0. 05). After treatment,the survival rates of 0. 5,1and 2 years in treatment group were 100%(41 / 41),82. 9%(34 / 41)and 73. 2%(30 / 41),and that of the control group were 100%(41 / 41),70. 7%(29 / 41)and 51. 2%(21 / 41). There was significant differences between groups in terms of the survival rates of 1and 2 years( χ2 = 3. 96,5. 07,P < 0. 05). Conclusion The curative effect of RFA guiding by B ultrasound combined with CT is significantly superior to that guiding by CT.
2.Quality of life of AIDS patients following Chinese traditional medicine plus HIV treatment regular drug

Objective To investigate and evaluate the improvement of the quality of life of pulmonary infection in AIDS patients treated by Chinese traditional treatment combined Western medicines treatment.Methods Sixty cases with HIV infection combined with pulmonary infection were selected as our subjects.They were randomly divided into two groups and each group was 30 cases.Patients in western medicine group were given medicine plus placebo,and in Chinese medicine group were given west medicine combined with traditional Chinese medicine.Patients hospitalized for 4 weeks.WHO QOL-HIV plus AIDS-related quality of life scale were used to collect the information about quality of life.Results There were no significant difference between two groups in terms of quality of life,life activity,health status,sleep status,ability to work,leisure opportunities scores at before treatment (P > 0.05).After treatment,the score of quality of life,life activity,health status,sleep status,ability to work,leisure opportunities in patients of Chinese group were (3.78 ± 0.69),(3.55 ± 0.67),(3.55 ± 0.67),(3.18 ± 0.72),(3.66 ± 0.70),(3.03 ± 0.73) respectively,higher than that of western medicine group ((2.90 ± 0.77),(2.75 ± 0.85),(2.75 ± 0.85),(2.85 ± 0.68),(3.35 ± 0.57),(2.95 ± 0.85) respectively,t =-2.791,-2.791,-4.28,-3.08,-4.08,-2.56,P < 0.05).There were no significant differences between the two groups in the terms of the score of satisfaction,sex ability,HIV impaction on the body,the meaning of life,the energy aspects of patients at before treatment (P > 0.05),but after treatment,these scores in Chinese medicine group were higher that in the control group ((3.51 ± 0.67) vs.(2.93 ±0.71),(3.13±0.46) vs.(2.86±0.74),(3.96±0.71) vs.(3.45 ±0.98),(3.66±0.72) vs.(3.23 ± 0.88),(3.23 ± 0.67) vs.(2.90 ± 0.65)),and the differences were statistically significant (t =-4.38,-2.34,-3.75,-2.76,-2.38,P < 0.05).There was no significant difference in the term of the mental effect of HIV within groups at before treatment,but after treatment,the score in Chinese group was lower than the Western group ((3.05 ± 0.90) vs.(3.46 ± 0.87),t =2.31,P < 0.05).Conclusion Chinese medicine combined western medicine can significant improve the quality of life of HIV patients with pulmonary infection.

4.Clinical research on advanced liver cancer treated with percutaneous RFA cool-tip electrode under ultrasound guidance

Objective To evaluate the clinical efficacy and complications in the treatment of advance liver cancer under ultrasound guidance with percutaneous RFTI-1 TM-cold cycle RF tumor ablation apparatus produced by Nangjing TianMa high-tech Company limited. Methods 25 patients of advance liver cancer, including 8 cases of metastatic liver cancer, 17 of primary hepatic carcinoma, were treated with percutaneous RFA cool-tip electrode under Ultrasound guidance 1 to 2 times. Eight of the patients prior to RFA were treated with TACE of individually three times for each. The postoperative efficacy was evaluated by enhanced CT. Results 10 lesions were completely necrotized and the majority parts of another 15 lesions were also under necrosis. Follow-up of six months, all patients are still alive with marked improvement of life quality. Conclusion The short-term efficacy of percutaneous Ultrasound-guided RFA with cool-tip electrode for treating advance liver cancer is quite satisfied, worthy to be recommended.
5.Clinical features of ocular symptom in patients with allergic rhinitis.

OBJECTIVE:
To investigate the clinical characteristics of ocular symptoms in patients with allergic rhinitis (AR) and the impact of the ocular symptoms on patients' quality of life.
METHOD:
AR patients' history and clinical data were collected and analyzed. One hundred cases were extracted from adult patients with and without ocular symptoms in each group and their quality of life were evaluated using rhino-conjunctivitis quality of life questionnaire (RQLQ).
RESULT:
Totally 1119 cases were collected and 859 cases had ocular symptoms. Of the patients with ocular symptoms, 582 cases were mild, 234 cases were moderate, 43 cases were severe. Eye itching was the most common symptom, followed by tears, hyperemia and swelling. Patients with ocular symptoms had longer disease history than non ocular symptom group (P < 0.05). There was a positive correlation between ocular and nasal symptoms (P < 0.01); patients with ocular symptoms had more severe nasal symptoms than patients without ocular problems; while patients with moderate to severe AR had more severe ocular symptoms than mild AR patients (P < 0.01). Female patients had higher incidence (P < 0.05) and ocular symptoms score (P < 0.05) than male. Children less than 10 years old had a relative lower incidence and score of ocular symptoms. While the incidence and score had an increasing trend for patients older than fifty. There was no difference regarding the type and number of allergen in the patients with and without ocular symptom. Moreover, patients with ocular symptoms had higher scores in the domains of non-nose/eye symptoms, practical problems, ocular symptoms, emotional function and total score in RQLQ than patients without ocular symptom.
CONCLUSION
It was common for the AR patients to have ocular symptoms, and ocular symptoms had obvious influence on the patients' quality of life. So controlling of the ocular symptoms should not be ignored during the diagnosis and treatment of AR.

6.The Clinical Effect of Recombinant Human Epidermal Growth Factor in Treatment of Chronic Tympanic Membrane Perforation

Objective To evaluate the clinical effect of recombinant human epidermal growth factor (rhEGF) on the treatment of chronic tympanic membrane perforation and the function of inner ear. Methods From 2005 to 2008, the outpatients were randomly divided into three groups (A:control group, B:kangfuxin group, C:rhEGF group) . The patients were also divided into three groups (small, medium and large) . The hearing level and the function of inner ear in three groups were compared. Results The hearing level of rhEGF group was 4.5, which was significantly than that of other groups (7.3 and 9.4 respectively, <0.05) . There was no significant difference in the function of inner ear among the three groups ( >0.05) . Conclusion rhEGF can decrease the healing time significantly and increase the healing rates in the treatment of chronic tympanic membrane perforation, and it has no obvious adverse effects on the inner ear.
7.Treatment of inverted papilloma of nasal cavity and sinuses by endoscopic transnasal approach

OBJECTIVE To explore the outcome of endoscopic transnasal management of inverted papilloma of nasal cavity and sinuses. METHODS The clinical data of 28 cases with inverted papillomas of nasal cavity and sinuses treated via nasal endoscope were studied retrospectively. RESULTS All of the patients were followed up from 7 months to 5 years. Only 6 cases(21%) recurred. Of the 6 cases, ethmoid and sphenoid sinuses were involved in 2 cases and maxillary sinus was involved in 4 cases. CONCLUSION Endoscopic transnasal resection of inverted papilloma is a good approach for the treatment of patients with inverted papilloma of nasal cavity and sinuses.
9.The reformation of electrocardiogram teaching mode under clinical pathophisiology and therapy mode

Obejective To reform the electrocardiogram teaching mode under clinical pathophisioligy and therapy(CPPT) mode.Methods The junior students of Grade 2010 in Second Clinical Medical College of Wuhan University were selected as subjects.The students were randomly divided into experimental group (44 students) and control group(52 students).Control group was taught by using traditional methods for electrocardiogram(ECG) diagnostics theory,and by using Diagnostics department's own Atlas,while experimental group was taught by reformed teaching methods,and ECG reformed textbook was used in their theoretical lessons,strengthening electrocardiogram and electrophysiology,cardiovascular disease combined.Meanwhile,the practice of teaching was taught by using preview-speaking Figure-Picture stories-Case-Read figure,a way to strengthen the theory-practice-theory of the cycle.The questionnaire was carried out in two groups after ECG teaching.After the end of the course two groups were given packet electrocardiogram atlas test and ECG theoretical knowledge examination and the above evaluation results were statistically analyzed by using SPSS 10.Results The statistically significant differences between experimental group and the control group were in the right ventricular hypertrophy,ventricular fibrillation,WPW syndrome,et al(P<0.05).The ratio of the experimental group students' liking electrocardiogram course and their recognition of electrocardiogram teaching mode was higher than that of the control group while the ratio of their thinking ECG difficult was lower than that of the control group and the difference was statistically significant difference(P<0.05).The excellent rate and good rate of the experimental group students' theory knowledge examination results were higher than the control group,two groups of results distribution difference being statistically significant(P=-0.043).Conclusions CPPT teaching model can effectively help students overcome the difficulties in reading electrocardiogram and deepen their understanding of theoretical knowledge and memory.
10.The clinic anatomy of operation on pterygopalatine fossa through nasal cavity under endoscope.

OBJECTIVE:
To provide anatomic data of pterygopalatine fossa(PPF) for endoscopic PPF surgery.
METHOD:
Fifteen wet adult skull specimen fixed in 10% formaldehyde were studies. From center the skulls were sawed, the middle and inferior turbinate were resected, the anterior and posterior ethmoid sinuses were resected. Then after punch out perpendicular part of palatine bone and posterior wall of maxillary,the pterygopalatine fossa were showed and some related measurements were made.
RESULT:
This pathway can lead to a good view of pterygopalatine fossa. The main soft structures in PPF were maxillary artery and nerve with their branches. The diameter of artery branches was smaller than 3 mm.
CONCLUSION
The endoscopic PPF surgery is safe and practical from the anatomic data.
