1.Practice of Post-marketing Safety Reevaluation of Salvianolate and Other TCM Injections in Our Hospital

OBJECTIVE:To explore the idea and method of post-marketing safety reevaluation of TCM injection in our hospi-tal. METHODS:In case-control study,salvianolate for injection was chosen for practice. 1 000 patients with stable angina receiv-ing Salvianolate for injection in our hospital during May 2012-Oct. 2013 were selected as subjects. The occurrence of ADR/ADE were observed and Logistic regression analysis was conducted by using SPSS 20.0 software. RESULTS:The incidence of ADR/ADE was 0.6%,and rare(0.1%-1%). The ADR/ADE were general and mainly manifested as erythra(66.67%). Skin and its ap-pendants were main organs involved in ADR(66.67%). Logistic regression analysis showed that gender,drug combination and age are risk factors of ADR/ADE. CONCLUSIONS:The post-marketing safety reevaluation of TCM injection not only make up for the deficiency of pre-marketing safety evaluation of TCM injection and find out potential delayed and severe ADR/ADE and drug inter-action,but also provide reference for rational drug use in the clinic and guarantee the safe and rational drug use.
2.Construction of lentivirus vectors carrying alphastatin gene and its secretion expression in human umbilical vein endothelia cells

Objective To construct lentivirus vectors carrying alphastatin gene, test its secretion expression in human umbilical vein endothelia cells (HUVECs) and observe its effects on growth, migration and tube formation of HUVECs. Methods We constructed recombinant lentivirus vectors of NT4-alphastatin fusion gene containing neurotrophin-4 signal peptide, pro-region sequences and alphastatin, then transfected the recombinant lentivirus vectors into HUVECs to obtain secretory protein alphastatin and test its anti-angiogenic activities in vitro. Results Our data showed that recombinant self-inactivating lentivirus vectors of NT4-alphastatin were successfully constructed, and stable NT4-alphastatin transduced HUVECs were capable of sustainably secreting alphastatin which significantly suppressed HUVECs migration and differentiation but not VEGF-induced proliferation. Conclusion This report represents the first time on the use of lentivirus-based vectors to deliver alphastatin, the endogenous angiogenesis inhibitor, and reveals the potential utility of anti-angiogenic gene therapy with lentivirus vectors for treating cancer.
3.Determination of Chromium in Human Hair by Spectrophotometry

Objective To study the determination of chromium in human hair by spectrophotometry using starch as color agent. Methods The total chromium(Cr)in human hair was oxidized by (NH4?雪2S2O3 into Cr6+,which could oxidize KI to form I-3 in acid medium. I-3 reacted with starch to form a blue colored complex,which revealed the maximum absorption peak at 590 nm. Results The contents of Cr in human hair were 8.46-29.56 ?g/g.The lowest detection limit and linear range were 0.02 and 0.02-1.0 ?g/ml respectively.The coefficient of variation and standard material-added recovery rates were 3.3% and 94.8%-104.5% respectively. Conclusion This methods was rapid,accurate and convenient for the determination of chromium in human hair.
4.Ethical thought on patients of head injury

The head injury patients is very common in clinic and it is complicated to cure them.Furthermore,many ethical questions are involved during the treatment,which are discussed in the article.The aim is to be helpful to protect both patients'and medical staff's right,reduce medical disputes and form a good relationship between doctors and patients from the view of ethics.Accordingly,the patients of head injury will acquire better treatment.
5.Blood pressure circadian rhythm and heart rate turbulence in hypertensive patients: relationship with left ventricular hypertrophy

Objective To investigate the relationship of blood pressure circadian rhythm with myocardial hypertrophy and the changes of autonomic nerve function in patients with essential hypertension (EH). Methods Eighty-two female patients with essential hypertension (EH) underwent 24-hours ambulatory blood pressure monitorings (ABPM), dynamic electrocardiogram (Holter) and echocardiography examination. Patients were classified into non-dipping group (n=40) and dipping group (n=42) according to the result of ABPM. Left ventricular mass index (LVMI), heart rate variability (HRV) in time domain (including SDNN, SDANN, rMSSD, PNN50) and heart rate turbulence (HRT) parameters (including turbulence onset [TO] and turbulence slope [TS]) were measured. Results Compared with those in dipping group, patients in non-dipping group have higher incidence of LVH (19.0% vs 52.5%, P<0.01), greater mean LVMI (112.39±12.79 g/m2 vs 121.98±13.35 g/m2, P<0.01), decreased PNN50 and rMSSD. TS value was decreased while TO was increased in non-dipping group compared with those in dipping group (both P <0.01); patients with LVH showed decreased TS and increased TO, compared with those without LVH. Conclusion In female patients with EH, non-dipping blood pressure circadian is associated with higher incidence of LVH. The HRV and HRT were more remarkably blunted in non-dipping patients, as well as those with LVH.
6.Establishment of Hospital Intravenous Drug Use Management Model Based on Index System Construction

Objective: To construct the management model for intravenous drug use based on index system in a hospital.Methods: A three-level organizational framework was established, the KPI assessment method was used to set control index and observational index respectively for outpatients and inpatients.The completion of the clinical department indicators were monitored and analyzed with the help of the hospital information systems, and regulated by pharmacy and management tools.Results: After the application of the hospital intravenous drug use management model, the qualification rate of intravenous medication orders was improved in December 2015 (89%) when compared with that in December 2014 (68%), and the difference was statistically significant(P<0.05).The indices of intravenous drug use of inpatients and outpatients in 2015 were significantly improved when compared with those in 2014.Conclusion: The hospital intravenous drug use management model based on index system construction can effectively reduce unnecessary intravenous medication and promote the rationality of intravenous medication.
7.Reversible Posterior Leukoencephalopathy Syndrome during Gestation Period: 4 Cases Report

ObjectiveTo explore the clinical and imageological features of reversible posterior leukoencephalopathy syndrome(RPLS) during gestation period.MethodsClinical and imageological data of 4 pregnant women with RPLS were analyzed retrospectively.ResultsHeadache, seizure, confusion and visual loss were the mainly clinical manifestations. At MR imaging, the brain typically demonstrated focal regions of symmetric hemispheric edema which may disappear in several weeks. The parietal and occipital lobes were most commonly affected, followed by the frontal lobe cerebral vasoconstriction syndrome. Three of the four patients suffered eclampsia which happened in two days after delivery.ConclusionThere are distinctively clinical features in posterior leukoencephalopathy syndrome during gestation period. Most of them are invovled in patients with eclampsia.
8.Effects of infrasound on Ca2 +-activated-K + channel of the bovine retinal microvascular endothelial cells

BACKGROUND: The permeability of blood-retinal barrier in rats can be increased due to the exposure under infrasound. There is rare research on ionic mechanism of such damage to barrier because of lacking the sources of retinal microvascular endothelial cells.OBJECTIVE: To investigate the impact of infrasound on calcium-activated potassium channel(BKca) of bovine retinal microvascular endothelial cells (BRECs).DESIGN: A completely randomized controlled opening study.SETTING and MATERIALS: The research was conducted in the Laboratory for patch-clamp, Department of Clinical Aerospace Medicine, Fourth Military Medical University of Chinese PLA. Experimental subjects were BRECs cultured.INTERVENTIONS: The cultured BRECs were exposed to the infrasound of 8 Hz, 130 dB for 30 minutes.MAIN OUTCOME MEASURES: The activity of BKCa in BRECs was observed.RESULTS: The activity of BKCa channel in BRECs increased after the exposure of infrasound of 8 Hz, 130 dB for 30 minutes. BRECs were cultured in an incubation cabin for 30 minutes after exposure for another detection by patch-clamp ionic current, and the increased BKCa channel activity slightly decresed.CONCLUSION: The breakdown of blood-retinal barrier under infrasound is partly due to the stimulatory effect of infrasound on Ca2 +-activated-K +channel, which will cause membranous depolarization providing the driving force of Ca2+ influx on BRECs.
9.Protective effect of polysaccharides from Lycium ruthenicum Murray against ultraviolet B radiation-induced photodamage in HaCaT cells

Objective To evaluate preventive effect of polysaccharides from Lycium ruthenicum Murray against photodamage in HaCaT cells,and to explore its possible mechanism.Methods Ultrasoundassisted extraction was used to extract polysaccharides from Lycium ruthenicum Murray in Qaidam Basin.In vitro cultured HaCaT cells were randomly divided into 3 groups:control group receiving no treatment,ultraviolet B (UVB) group irradiated with 30 nJ/cm2 UVB alone for 1 hour,experimental group pretreated with 2 g/L Lyeium ruthenicum polysaccharide solution followed 6 hours later by 30 mJ/cm2 UVB radiation for 1 hour.At twelve hours after UVB radiation,an inverted microscope was used to observe cell morphology.Then,MTS assay was performed to estimate cell proliferation,flow cytometry to detect cell apoptosis,an enzyme-labeled antigen method to detect levels of malondialdehyde (MDA) and glutathione peroxidase (GSH-Px),as well as to evaluate the activity of superoxide dismutase (SOD) and catalase (CAT),and enzyme-linked immunosorbent assay (ELISA) to measure levels of intedeukin-1 (IL-1) and tumor necrosis factor-alpha (TNF-α) in HaCaT cells and their culture supernatant.Results Compared with the control group,the UVB group showed obscure cell morphology,cell death and floating phenomenon,while cells became swollen but renained morphologically distinct in the experimental group.MTS assay revealed that the cell proliferative activity significantly differed among the above 3 groups (F =48.88,P < 0.01),and the cell proliferative activity was significantly lower in the UVB group (1.72 ± 0.12) than in the control group (2.34 ± 0.11,P < 0.05) and experimental group (2.11 ± 0.10,P < 0.05).Moreover,the apoptosis rate was significantly higher in the UVB group (82.41% ± 2.49%) than in the control group (3.98% ± 0.19%,P < 0.05) and experimental group (22.79% ± 0.97%,P < 0.05),as well as higher in the experimental group than in the control group (P < 0.05).Compared with the control group,the UVB group showed significantly higher levels of MDA,supernatant levels of IL-1 and TNF-α,and intracellular levels of TNF-α,but significantly lower GSH-Px levels and activity of SOl and CAT (all P < 0.05).However,there was no signifi-cant difference in the intracellular level of TNF-α between the UVB group and control group (P > 0.05).Conclusion Lycium ruthenicum polysaccharide has protective effects against photodamage in HaCaT cells,likely by reducing the synthesis and secretion of inflammatory substances as well as free radicals.
10.Management Effect of Clinical Pharmacist on Antibiotics Use during Perioperative Period of Great Saphe-nous Varicose Veins Surgery

Objective:To evaluate the management effect of clinical pharmacist on antibiotics use during perioperative period of great saphenous varicose veins surgery. Methods:The medical records of all the patients undergoing simple great saphenous vein strip-ping, great saphenous vein high ligation plus simple stripping and saphenous vein ligation plus endovenous laser treatment between June and September in 2010 ( before the intervention) , 2011 ( after the intervention) and 2014 ( consolidation period) were selected, and the rationality of prophylactic use of antibiotics during perioperative period was evaluated. Results: After the management of clinical pharmacists, the preventive application of antibiotics during perioperative period was markedly improved in the indication, medicine va-riety choice,application and dosage, administration route, solvent and solvent dose, administration time, course of treatment and drug combination when compared with that before the intervention. Antibiotics use ratio, average hospitalization days, antibiotics ratio, drug ratio and infusion ratio were reduced obviously, which could be consolidated. Conclusion: The intervention of clinical pharmacists in antibiotics use during perioperative period of great saphenous varicose veins surgery improves the reasonable use ratio of antibiotics, and makes certain significance in the cost reduction for patients and the specification for hospital management.
