Chinese Journal of Hospital Administration 1996;0(10):-
Based on the lessons and experience from the country's fight against SARS, the paper analyzes the problems and shortcomings in its public health emergency reaction. It also describes the pubhc health systems set up in America, Britain and Japan in response to emergent events. With regard to the building up of a public health emergency system that conforms to the situation in China, the following proposals are put forward: (1)establishing effective command networks, such as setting up a national crisis management commission and formulating corresponding laws and regulations and crash programs; (2)establishing unimpeded information networks, such as setting up a national disease monitoring and reporting system and urban symptom monitoring systems; and (3)establishing tight prevention and treatment networks, with the network of prevention being composed of the headquarters, disease prevention and control institutions, medical institutions, health supervision institutions, experts commissions, and cooperation centers for tackling key problems in science and technology.

Evaluation of medical assistance capability plays a significant role in improving medical assistance capability and accomplishing a diverse array of military tasks.Based on gray theory and fuzzy mathematics,we put forward a new evaluation model for disaster medical assistance capability, integrating the AHP,the gray cluster analysis and fuzzy evaluation. This model not only reflects the ambiguity of medical assistance capability evaluation but also makes full use of the information, which avoids the flaws caused by single method of fuzzy mathematics. It provides a new way to evaluate disaster medical assistance capability comprehensively and systematically.

Objective To compare the clinical outcomes of modified and traditional posterior lumbar interbody fusion (PLIF) in treatment of lumbar spinal stenosis. Methods A retrospective review was made on 47 patients who sustained monosegmental L4.5 stenosis due to lumbar degenerative revise and characterized by typical intermittent claudication and single or both limb numbness and pain. The patients were randomly allocated to Group A and Group B using the lottery method.Group A (n =24) consisted of 10 males and 14 females at mean age of 52 years (range,47-66 years) and the course of disease averaged 13 months (range,9-23 months).Group B (n =23) contained 8 males and 15 females at mean age of 53 years (range,49-67 years) and the course of disease averaged 11.5 months (range,6-22months).Modified PLIF including spinous process replantation combined with canal H-shaped bone grafting for posterior column reconstruction and interbody fusion was performed in Group A.Traditional PLIF,namely posterior spinal decompression and interbody fusion, was performed in Group B.Waist function rehabilitation after surgery of the patients in the two groups was assessed using the Oswestry disability index (ODI) and postoperative results were analyzed. Results There were no significant differences between Groups A and B regarding the operation time [(1 90 ± 15.66) min vs (170 ± 11.32) min] and intraoperative blood loss [(980 ± 35.58) ml vs (879 ± 21.25) ml] (P > 0.05).Mean period of follow-up was 20 months (range,12-28 months).Postoperative results in Group A were graded as excellent in 19cases,good in three, fair in two and poor in zero, with excellence rate of 88%.While in Group B,the results were excellent in 13 cases, good in four, fair in four and poor in two, with excellence rate of 74%(P < 0.05).ODI score and symptom improvement rate after operation were better in Group A than in Group B (P < 0.05).Conclusion The modified PLIF including spinous process replantation combined with canal H-shaped bone grafting for posterior column reconstruction is relatively an ideal surgical technique for treatment of lumbar spinal stenosis and deserves wide promotion.

Aim To investigate the effect of ASIC1 a ( acid-sensing ion channel 1 a ) on the pathological change of diabetes complication liver fibrosis and the proliferation and activation of hepatic stellate cell ( HSC-T6 ) stimulated by PDGF-BB under hyperglyce-mia. Methods Diabetes rats model was established by streptozotocin ( STZ) , and liver fibrosis rats model was induced by carbon tetrachloride ( CCl4 ) . Then, the liver extent of damage and the expression of ASIC1 a were observed in the diabetic rats, liver fibrosis rats and diabetes complication liver fibrosis rats. In vitro, after pretreated with amiloride, HSC-T6 was treated with high glucose for 24 h and then stimulated with PDGF-BB for another 24 h. The proliferation and acti-vation of HSC-T6 were observed, and the expression of ASIC1a, α-SMA and collagen Ⅰ were detected by Western blot. Results Compared with the control group, rats from diabetic group induced by STZ, liver fibrosis group induced by CCl4 , and the diabetes com-plication liver fibrosis rats co-induced by STZ and CCl4 were all observed with liver damage at different levels, and tissue injury of complication group was most seri-ous. However, the expression of ASIC1a in the three model groups was significantly increased compared to the control group. ASIC1a level was most obvious in the diabetes complication liver fibrosis rats. Amiloride pretreatment significantly decreased ASIC1 a expression and inhibited PDGF-BB mediated proliferation and the expression ofα-SMA and collagenⅠin HSC-T6 under high glucose environment. Conclusion High ambient glucose aggravates HSC activation and hepatic fibrosis, and this may be related with the increasing expression of ASIC1a.

Objective To evaluate the advantages of transrectal contrast-enhanced ultrasound assis-ted prostate biopsy by comparing with systemic prostate biopsy . Methods From Jan.2010 to Dec.2011, 213 cases were selected randomly.Patient's mean age was 71.0±8.9 (54-83) years.Their PSA level was 22.5±19.4 (4.1-150.0) μg/L.The positive rates of DRE and transrectal ultrasound were 55.9%( 119/213) and 37.6%(80/213).This group of patients were biopsied according to transrectal contrast-enhanced ultrasound assisted prostate biopsies ( experimental group ) .During the same period , another 218 cases were selected randomly.Patient′s mean age was 65.0±9.1 (36-94) years.Their PSA level was 23.3±18.9 (4.6-147.0) μg/L.The positive rates of DRE and transrectal ultrasound were 57.3%( 125/218 ) and 38.1%(83/213).This group of patients were biopsied according to systemic prostate biopsy (control group). The data of two groups were compared to find out the advantages and disadvantages of transrectal contrast -en-hanced ultrasound assisted prostate biopsy . Results Comparing these two groups , the positive rates of prostate cancer in experimental group and control group were 38.0%(81/213) and 34.9%(76/218), with no statistically significance ( P>0.05); the numbers of cores were 9.5 and 12.0, with statistically signifi-cance (P<0.05);the positive rates of single needle were 18.2%(369/2 025) and 11.5%(301/2 616), with statistically significance (P<0.05); and the mean Gleason scores were 7.1 and 6.6, with statistically significance (P<0.05). Conclusions The efficiency of transrectal contrast-enhanced ultrasound assisted prostate biopsy is higher than systemic prostate biopsy .Transrectal contrast-enhanced ultrasound assisted prostate biopsy can be considered as a new option for prostate biopsy .

Journal of Cerebrovascular and Endovascular Neurosurgery 2024;26(3):298-303
Partial trisomy of the long arm of chromosome 17 (17q) is a rare but clinically recognized syndrome that involves facial dysmorphisms, skeletal abnormalities, and global developmental delay, as well as various reports of cardiovascular, renal, and central nervous system abnormalities. This report presents a novel neuroradiologic finding of diffuse enlarged, tortuous cortical veins with physiological antegrade flow in a child with a microduplication of the distal end of 17q. To our knowledge, this finding has not been described previously. Although the exact cause for the cortical vascular anomaly is currently unknown, this duplicated region contains genes of interest for future studies that focus on normal and abnormal angiogenesis
