1.Chronic insufficiency following kidney transplantation in 22 cases

Due to influnce of various factors, the transplanted kidney will appear the chronic renal insufficiency symptom at a certain period after transplantation. How to prolong the function of transplanted kidney and life of the patients is a key subject in clinical research. This study based on the retrospective analysis of 22 cases kidney transplant of chronic kidney failure patients enrolled in the Department of Blood Purification, Hainan SanYa Nongken Hospital. According to the different stages of the chronic renal failure of patients, tacrolimus (FK506) was used instead of cyclosporine A (CsA) combined with blood purification treatment (hemodialysis or/and hemoadsorption), the abnormal biochemical markers decreased significantly in all patients after treatment, with 7 cases are clinical cured (7/22, 31.8%), 10 cases are ameliorated (10/22, 45.5%) and 5 cases gave up treating (5/22, 22.7%). The result indicates that this therapy can ameliorate the transplanted kidney function, and improve the life quality of the patients.

OBJECTIVE:To observe clinical efficacy of epinephrine combined with vasopressin and naloxone for cardiopulmonary resuscitation(CPR)and to investigate its mechanism. METHODS:64 patients with sudden cardiac and respiratory arrest were randomly divided into control group(epinephrine group),treatment group Ⅰ(epinephrine+vasopressin group)and treatment group Ⅱ(vasopressin+epinephrine+naloxone group). 4.The study on molecular evolution of influenza virus B isolated in Shenzhen from 1994 to 2006

Objective To study the prevalence and variation of influenza B viruses of Shenzhen. Methods Fifty strains influenza B viruses in Shenzhen from 1994 to 2006 were selected. HA1 gene were amplified by RT-PCR and sequenced. Phylogenetic analysis of HA1 was conducted by MEGA program. Results The influenza B viruses of Shenzhen were divided into Yamagata and Victoria lineage. The two lineages prevailed respectively in different years from 1994 to 2006. The variance of glycosylation site and some mutations of antigenic determinants were detected in the two lineages. Conclusion The viruses of Yamagata and Victoria lineage prevailed respectively in different years in Shenzhen but the mutation rates of the two lineages were slowly.
5.Surveillance for neuraminidase inhibitor resistance of seasonal H1N1 influenza A viruses isolated in Shenzhen during 2008 to 2009

Objective To analyze neuraminidase(NA) inhibitor resistance of seasonal H1N1 influenza A viruses isolated in Shenzhen during 2008 to 2009. Methods The NA gene of these viruses were sequenced. Phylogenetic analysis of the sequences was performed with Mega3. 1 software. Results In 2008, most isolates of the seasonal H1 N1 virus were susceptible to neuraminidase inhibitors, but the H275Y mutation in the neuraminidase gene region associated with high-level oseltamivir resistance had been detected in 92.6% of the strains isolated in 2009. Furthermore, a strain with Q136K was found, which showed the resistance to Zanamivir. Conclusion In the light of emerging resistance, close monitoring and understanding of the nature and dynamics of resistance mutations in influenza virus should be a priority.
6.Relationship between nuclear factor-κB as well as p53 up-regulated modulator of apoptosis and lung injury induced by severe acute pancreatitis and therapeutic effect of proline dithiocarbamate

Objective To investigate the expression of nuclear factor-κB (NF-κB) and p53 up-regulated modulator of apoptosis (PUMA) in acute lung injury (ALI) induced by severe acute pancreatitis (SAP), and the therapeutic role of proline dithiocarbamate (PDTC). Method SD rats weighed 200~ 250 g were randomly(random number) divided into sham operation group (A group, n = 18), ALI group (B group, n = 18) and PDTC treatment group (C group, n = 18). The model of SAP was eastablished by injecting 1 mL/kg of sodium tauarocholate into the pancreatic capsule of the rats in B group and C group. The model rats in C group were treated with PDTC one hour after modeling. Six rats of each group were sacrificed 6 h,12 h, and 24 hours after modeling. The histopathological changes in lung and pancreas were observed. The levels of NF-κB p65 and PUMA in lung were detected by using Western blotting, and the expressions of bcl-2, bax and caspase-3 mRNA in the lung were detected by using RT-PCR. The lung tissue was taken for examination under transmission electron microscope. TUNEL was used for detection of apoptotic alveolar epithelial cells. Results Six to 24 hours after modeling, the pathological scores in lung of ALI group were significantly higher than those of control group and PDTC group after sodium taurocholate injection ( P < 0.05). The levels of NF-κB p65 and PUMA, and the expressions of bax and caspase3 mRNA in ALI group at different intervals were higher than those in control group and PDTC group ( P < 0.05),whereas the expression of bcl-2 mRNA in ALI group was lower than that in control group and PDTC group ( P <0.05). The NF-κB p65 was correlated closely and positively with PUMA ( r= 0.987, P < 0.01). Higher activity of caspase-3 acrtive units was seen in ALI group than that in control group and PDTC group ( P < 0.05). The microvilli disappeared in ALI group 24 hours later. The apoptosis index in ALI group was higher than that in control group and PDTC group ( P < 0.05). Conclusions The apoptosis of alveolar epithelial cells of rats in ALI group is caused by PUMA activated by NF-κB. PDTC treatment can inhibit apoptosis of alveolar epithelial cells of rats in ALI group by inhibiting the activation of NF-κB.

OBJECTIVETo report a new method of coracoid transposition for the treatment of complete dislocation of acromioclavicular joint and to evaluate its efficacy.
METHODSWe modified Dewar's surgical method as follows: (1) Two small incisions, a transversal incision on the acromioclavicular joint and a longitudinal incision on the coracoid, were made instead of a conventional large arc incision from the acromion to coracoid. (2) The foreign body in the acromioclavicular joint was cleared out. The chondral surface at the lateral segment of clavicle was resected to form a pseudarthrosis and meanwhile the residual joint capsule and ligaments were repaired. (3) The coracoid was moved to the anteroinferior edge of the clavicle instead of the anterior margin and (4) the coracoid was moved to the lateral border of the clavicle instead of the superior border of the coracoclavicular ligament.
RESULTSThe follow-up duration in 30 patients of the series was from 6 to 72 months (mean 41 months). Functional assessment was carried out by the criteria delineated previously by Karkson, in which Grade A was in 24 cases, Grade B in 4 cases, and C in 2.
CONCLUSIONSThis modified technique, having less postoperative complications and less injuries to tissues and according well with the requirement of biomechanics, can achieve a stable reduction of acromioclavicular joint with a good functional and cosmetic result and therefore is preferable to use clinically on a large scale.

10.Congenital tracheal stenosis complicated with communicating bronchopulmonary foregut malformation in 2 children and literature review

Objective:To explore the clinical typing, diagnostic method and treatment plan of congenital tracheal stenosis (CTS) combined with communicating bronchopulmonary foregut malformation (CBPFM) in children.Methods:The clinical data of 2 children with CTS and CBPFM who were treated in the Center for Respiratory Intervention of Children′s Hospital Affiliated to Shandong University in May 2021 and January 2022 were retrospectively analyzed.Studies were retrieved from domestic and foreign databases, so as to summarize the clinical characteristics of CTS complicated with CBPFM and investigate the typing method of CBPFM.Results:One patient was a 4-year-old girl, who sought the medical advice due to " recurrent cough and asthma for more than 4 years" . She was diagnosed with typeⅡ CBPFM at the right side and CTS.Surgical thoracoscopic right pneumonectomy plus oesophageal repair was performed.The other patient was a 7-month-and-2-day-old female, who visited the hospital for " difficult eating, dyspnea and purple lip cyanosis for 7 months" . This patient was diagnosed with typeⅡ CBPFM at the left side and CTS.Slide tracheoplasty and left pneumonectomy+ oesophageal repair were performed successively.Eight English and one Chinese studies were collected.Twenty-one children with CBPFM and 12 children with CTS and CBPFM were included.Eleven CTS cases with sufficient diagnostic evidence were complicated with typeⅠA and typeⅡCBPFM.Conclusions:CTS and CBPFM can lead to severe wheezing and dyspnea.Clinicians should enhance their awareness and be more cautious.There may be a potential link between CTS and typeⅠA and typeⅡCBPFM, and further investigation is required.
