1.Application of totally implantable venous access ports in chemotherapy for young children with malig-nant tumors

Objective To summarize our experiences of applying totally implantable venous access ports ( TIVAP) in chemotherapy for young children with malignant tumors.Methods Between December 2011 and May 2015, 50 young children with malignant tumors were implanted with TIVAP in Anhui Provincial Children's Hospital for chemotherapy.A retrospective review was conducted with their clinical records, focusing on success rate, clinical effect, and complications of TIVAP.Results The technical success rate was 100%with no intrao-perative complication such as puncture-related injury.In one case the device had to be adjusted through reopera-tion due to malposition of catheter tip.The port was removed in one case after 1 year due to infection.15 patients had the ports removed after the completion of chemotherapy.5 patients died due to the relapse and metastasis of tumor.29 patients are still using the port with no complications.Conclusion TIVAP is a safe device in young children, ensuring reliable vascular access for chemotherapy and improving patients'quality of life.
2.Skin characteristics of patients with melasma under VISIA detector

Objective:To compare the skin characteristics of melasma patients with general population by the VISIA skin detector.Methods:In July to September 2018, the skin spots, wrinkles, texture, pore, UV spots, porphyrins, brown spots, the absolute scores of red areas, and the age difference were analyzed in patients with melasma and general population in the Laser Center of Dermatology, the Wuxi Second Hospital.Results:Compared with general population the spots (35.62±8.90, 27.82±4.98), wrinkles (24.36±6.93, 19.89±1.10), texture (12.92±5.37, 6.63±4.74), pore (12.35±4.68, 17.56±10.69), UV spots (23.11±4.55, 17.71±7.26), brown spots (56.34±5.74, 52.81±3.16) and the age difference (-0.59±3.35, 3.96±3.60) of melasma patients had the statistically significant difference ( t=3.63, 2.26, 4.07, 2.64, 3.03, 2.56, 4.85, P<0.05); Porphyrin (7.81±6.83, 7.67±5.60) and red areas (40.44±6.73, 38.50±9.87) had no statistical difference ( t=0.08, 0.89, P>0.05). Conclusions:Compared with general population, the skin of patients with melasma has less splashes, wrinkles and the poorer skin smoothness and looks older, but it is more delicate.
3.Laparoscopic surgery for treatment of hepaticojejunostomy strictures after cholangiectasis surgery in children

Objective:To study the safety and feasibility of laparoscopic surgery for treatment of hepaticojejunostomy strictures after cholangiectasis surgery in children.Methods:The clinical data of 12 children was retrospectively analysed. There were 5 males and 7 females, aged 4 (range 0.45 to 9.00) years old, who developed hepaticojejunostomy strictures after cholangiectasis surgery and underwent reoperative treatment at Anhui Provincial Children's Hospital from January 2013 to January 2021. These patients were divided into the laparoscopic surgery group ( n=5) and the open surgery group ( n=7) based on the mode of reoperation. The children were followed-up by outpatient review and the relevant clinical data of the children in the 2 groups was analyzed. Results:The reoperations were completed successfully in the 2 groups. The maximum preoperative dilated common hepatic duct diameter was significantly larger in the laparoscopic group (1.26±0.23) cm than the open group (0.64±0.19) cm ( P<0.05). The alkaline phosphatase and glutamyltransferase levels in the laparoscopic surgery group were significantly lower before the operation (all P<0.05), and the total bilirubin, direct bilirubin, alkaline phosphatase, and glutamyltransferase levels were significantly lower in the laparotomy group than before the operations (all P<0.05). In the laparoscopic group, the time of the reoperations, postoperative hospital stay, and blood loss were 268(117, 340) min, (9.0±2.9) d and (14.0±5.5) ml, respectively, while those in the open group were 180(150, 205) min, (9.7±3.4) d and (13.3±2.6) ml, respectively. There were no significant differences between the two groups (all P>0.05). On follow-up, all children were well except for one child who showed mild elevation levels of alanine aminotransferase and aspartate aminotransferase. Conclusion:Laparoscopic surgery for hepaticojejunostomy strictures after cholangiectasis surgery in children was safe and feasible. Its curative effect was no less than that of open surgery.
4.Prevalence situation and risk factors related to acne in adult woman

Objective:To determine the prevalence situation of adult female acne (AFA) patients and to explore the risk factors related to AFA.Methods:From September 2019 to June 2020, 290 female acne patients aged from 25 to 48 (29.57±4.50) years were surveyed with a questionnaire of risk factors and the prevalence situation of acne in the acne clinic of the Department of Dermatology, Wuxi No.2 Hospital Affiliated with Nanjing Medical University.Results:AFA occured more frequently in the jaw (95.17%), cheek (93.79%) and forehead (89.66%). Recurrent acne (38.62%) and comedonal acne (50.34%) more commonly occured. Cosmetics, endocrine, diet, genetics and other factors aggravated AFA. Age ( H=7.286, P>0.05; F=0.122, P>0.05), gonadal hormone concentrations ( Z=-0.365, P>0.05; χ 2=0.276, P>0.05), menstrual cycle ( Z=-0.274, P>0.05; χ 2=0.217, P>0.05), genetics ( Z=-1.244, P>0.05; χ 2=1.771, P>0.05) made no difference to acne grading and types. Excessive use of cosmetics could lead to increased comedo (χ 2=7.097, P<0.05). Cosmetics had no difference to acne grading ( Z=-0.065, P>0.05). Gonadal hormone concentrations were uncorrelated with menstrual cycle (χ 2=1.397, P>0.05). Conclusions:The pathogenesis of AFA is related to a variety of factors, which affect the skin barrier function and require comprehensive treatment.
5. Molecular mechanism and treatment of pulmonary fibrosis

Pulmonary fibrosis (PF) is a chronic progressive interstitial lung disease. The pathogenesis of PF is not yet clear. The two anti fibrosis drugs approved for IPF treatment, nidanib and pirfenidone, have been proved to reduce the decline of pulmonary function of pF, but both have side effects. So far, there is no obvious and effective treatment to prevent the progress of pF. Therefore, this review focuses on the different cells, molecular mechanisms involved in PF and the current treatment progress of PF, so as to provide theoretical support for a better understanding of these cells, molecular mechanisms and drug development and application in PF.
6. Research status of dialectical prevention and treatment of acute lung injury based on the theory of "Wei qi and Ying Xue"

Acute lung injury (ALI) is a common clinical critical respiratory disease. At present, the mechanism of the disease has not been fully elucidated, there is a lack of specific drugs in clinical practice and the mortality rate is high, which is a difficult problem in the medical field. In recent years, traditional Chinese medicine has exerted its unique advantages and efficacy in the prevention and treatment of ALI, which has aroused the attention of domestic and foreign scholars. Based on the theory of "Wei Qi Ying Xue", this paper discusses the current research status of prevention and treatment of ALI by traditional Chinese medicine, and analyzes its pathogenesis, clinical manifestations and corresponding analysis with TCM syndrome. According to the angle of "Wei Qi Ying Xue", the progress of syndrome differentiation and treatment is highly consistent with immune response, inflammatory response, oxidative stress and apoptosis, in order to find new ideas and medication for the prevention and treatment of ALI with integrated traditional Chinese and Western medicine.
