1.Community-acquired pneumonia treated by ticarcillin sodium and davulanate potassium in elderly patients with cerebrovascular sequelae

Eighty-six patients with community-acquired pneumonia who suffered from cerebrovascular sequelae were admitted to hospital from 2005 to 2007.Patients were randomized into ticarcillin sodium/clavulanate potassium group and cefuroxime Sodium group.Sputum culture was performed for each case before and after treatment.Gram-negative bacilli accounted for 74%of infection,and the most common pathogens were klebsiella pneumoniae(26%)and Escherichia coli(14%).The response rates in two groups were 93%and 72%,respectively(χ2=6.54,P<0.05).Bacteria eradication rates in two groups were 90%and 62%,respectively(χ2=6.00,P<0.05).Ticarcillin sodium and clavulanate potassium is an effective antibiotic for community-acquired pneumonia in elderly patients with cerebrovascular sequelae.
2.Research advances in induced pluripotent stem cells in Alzheimer's disease

BACKGROUND:Induced pluripotent stem cell (iPSC) technique is a newborn technology reprogramming human somatic cells into pluripotent stem cells, which can be used in disease modeling, drug screening and cell therapy. OBJECTIVE: To analyze the feasibility and advances of iPSC application in Alzheimer's disease, and conclude the insufficient and future of this technology. METHODS:CNKI and PubMed were searched for relevant articles concerning Alzheimer's disease and iPSCs, and the retrieval results were screened by relativity and repeatability. Finally, 30 articles were included in result analysis. RESULTS AND CONCLUSION: This review concludes the superiority of iPSC technology in the treatment of neurodegenerative diseases and in vivo harvest of nerve cells. The use of inducible pluripotent stem cells differentiated from patient's own cells is more practicable, but there are still many problems to solve at this stage, such as high-efficient and stable directional differentiation, safety of induced pluripotent stem cells, the most appropriate donor cells, and ethical disputes.
3.The relationship between extravascular lung water and the prognosis of sepsis acute lung injury/acute respiratory distress syndrome: a Meta analysis

Objective To investigate the value of extravascular lung water (EVLW) in predicting the prognosis of sepsis acute lung injury (ALI) / acute respiratory distress syndrome (ARDS).Methods Relevant published articles concerned with EVLW both in Chinese and in English from 1991-2011 were searched,and Meta-analysis was carried out with Stata software. Results Seven articles were selected for Meta-analysis according to the inclusion and exclusion criteria.The results indicated EVLW of dead patients group kept in a high level,while EVLW showed a down-trend in the survival group.The difference in level of EVLW between the two groups became more obvious with the day ( SMDd1< SMDd2 < SMDd3 ) ( the 1st day:SMDd1=0.29,95%CI:0.047-0.532; the2nd day:SMDd2=1.64,95%CI:0.14-3.13; the 3rd day:SMDd3 =1.83,95% CI:0.56-3.10).Conclusions EVLW and its dynamic change in the early stage of sepsis ALI/ARDS could predict the prognosis.This suggested the sustained high level would lead to high mortality.
4.The expression of von Willebrand factor and ADAMTS13 in aneurysmal subarachnoid hemorrhage

Objective To study the clinical values of dynamic changes of yon Willebrand factor (vWF) and ADAMTS13 (a disintegrin and metalloprotease with thrombospondin repeats-13 ) in aneurysmal subarachnoid hemorrhage. Methods Twenty-nine patients with aneurysmal subarachnoid hemorrhage admitted to Department of Neurosurgery from April 2010 through April 2011 were enrolled for retrospective study.They could be categorized into 3 sets of grouping:delayed cerebral ischemia group ( DCI group) and non-delayed cerebral ischemia group ( no DCI group ),cerebral vasospasm group ( CVS group ) and no vasospasm group (no CVS group),and good prognosis group and poor prognosis group,and another 20 healthy subjects as control group.All patients were examined with CT,DSA,or/and CTA to identify the intracranial subarachnoid hemorrhage resulted from aneurysm rupture.The exclusion criteria included:(1)the time from onset to admission was longer than 72 hours or patient was in imminent danger of death; (2)patients had surgery,interventiona] or conservative treatment outside the hospital; (3) patients were under the treatment of antiplatelet medicine such as aspirin,clopidogrel,or other anticoagulants such as warfarin,etc ; (4) patients had blood diseases,impaired kidney or liver function,pregnant,or with recent infections.Venous blood were taken one day,4 days and 10 days after SAH to determine plasma concentrations of ADAMTS13 and vWF by using enzyme-linked immunosorbent assay (ELISA). Transcranial Doppler ultrasonography (TCD) was used to measure mean blood flow velocity of middle cerebral artery (VMCA).Glasgow outcome scale (GOS) score was measured before discharge. Data were analyzed by using SPSS version 13.0 software. Results The levels of vWF were significantly higher in DCI group,CVS group and poor prognosis group compared with those in the control group 1 day,4 days and 10 days after SAH.There were differences in vWF between DCI group and no DCI group 1 day and 4 days after SAH ( P < 0.05 ).There were significantly differences in vWF between CVS group and no CVS group,and between good prognosis group and poor prognosis group 4 days and 10 days after SAH ( P < 0.01 ).In DCI group and poor prognosis group,the level of plasma ADAMTS13 was significantly lower 1 day after SAH than that in the normal control group (P <0.01) and in the no DCI group (P <0.O1 ); and there were no differences in ADAMTS13 between CVS group and no CVS group.Conclusions In the early stage,the increase in plasma vWF and decrease in ADAMTS13 activity are associated with DCI,and the decrease in ADAMTS13 activity can be used to predict the outcome.
5.Doxycycline inhibits corneal angiogenesis after keratoplasty

BACKROUND:Corneal hemangiogenesis occurs in 40%-60%patients after keratoplasty.Blood vessel is one of the high risk factors for corneal immunological rejection.To inhibit corneal hemangiogenesis would prolong the survival time of the grafts and promote the successful rate of the keratoplasty.OBJECTIVE:To explore the inhibitive effects of doxycycline on corneal angiogenesis after keratoplasty.DESIGN,TIME AND SETTING:A randomized controlled animal experiment was performed at the State Key Laboratory of Ophthalmology(No.2006DA105054),Zhongshan Ophthalmic Center,Sun Yat-sen University from March to August 2007.MATERIALS:A total of 48 healthy dean Sprague Dawley rats served as recipients(right eye)and 24 Wistar rats as donors(both eyes).CD31-PEfluorescent antibody was obtained from Sigma,USA.Sandwich enzyme-linked immunosorbent assay(ELISA)kit for vascular endothelial growth factor(VEGF)was brought from RapidBio,USA.METHODS:Corneal allogenic transplantation models were established in rats.Recipients were equally and randomly divided into 2 groups:saline control group and doxycycline group.Twenty minutes prior to surgery,mydriasis was performed using 1%atropine,with a diameter of 2.75 mm of implant and 2.5 mm of implant bed.In the saline control group,conjunctiva of the right eye received saline,three times a day,following surgery.In the doxycycline group,conjunctiva of the right eye received 1%doxycycline,three times a day,till 30 days following surgery.MAIN OUTCOME MEASURES:The following parameters were measured:corneal angiogenesis using immunofluorescence,expression of VEGF protein by using ELISA.RESULTS:Compared with the survival time of saline control group[(9.67±2.73)days],the mean survival time of doxycycline group[(20.67±3.01)days]was significantly prolonged(P<0.01).The mean percentages of neovascularized corneal area in the saline control group were(4.00±1.00)%,(14.33±4.04)%,(31.33±3.51)%at 3,7 and 14 days after keratoplasty,respectively.The mean percentages of neovascularized corneal area in the doxycycline group were(1.67±1.15)%,(4.67±1.53)%,(18.33±1.53)%at the same time point respectively.Compared with the saline control group,the mean percentages of neovascularized corneal area of the doxycycline group was significantly reduced at 7 and 14 days after keratoplasty(P <0.05).The expression of VEGF in the saline control group was(541.00±75.44)pg/mg,(960.00±90.14)pg/mg,(976.00±130.41)pg/mg at 3,7 and 14 days after keratoplasty,respectively,while expression of VEGF in the doxycycline group was(115.33±9.29)pg/mg,(239.00±41.62)pg/mg,(361.00±65.20)pg/mg,respectively.The difference of VEGF expression at all time points between the two groups was significant (P<0.05).CONCLUSION:Doxycycline has a significant effect in inhibiting corneal angiogenesis and prolonging survival time of implants after keratoplasty.
6.A Study of Hemorrheological Changes in Patients with Severe Viral Hepatitis

This paper is to report the study of the microscopic and macroscopic he-morrheological changes and the conditions of the nail-fold microcirculation in 30 patients with severe viral hepatitis. Normal 'individuals, patients with acute icteric hepatitis, and those with chronic active hepatitis were employed as the controls.The changes observed in the patients with severe viral hepatitis were as follows: The reduced viscosity of the whole blood and the plasma viscosity increased markedly, the dispersion rate of the blood viscosity values increased as the clinical state of the patient became more severe, and the factors increasing the blood viscosity (namely, increase of the amount of immune globulin, pro-longation of the electrophoretic time of RBC, increase of RBC sedimentation rate), and the factors decreasing the blood viscosity (namely, decreases of he-matocrit reading, fibrinogen, and the aggregation ratio of platelets) existed simultaneously. The changes of the flow condition and velocity of the nail-fold microcirculation were in close correlation with the increase of blood viscosity and the severity of the disease.The causes and the mechanism of the above-mentioned changes were discussed.
7.Plasma soluble triggering receptor expressed on myeloid cells-1 as a marker of sepsis: a meta-analysis

Objective To investigate the value of plasma soluble triggering receptor expressed on myeloid cells-1 as a diagnosis marker of sepsis.Methods Articles on plasma soluble triggering receptor expressed on myeloid cells-1 as a marker of sepsis which were public published in the PubMed,Ovid,Springer,Wanfang database from 1991-2012 were searched and conducted a meta-analysis by MetaDiSc and Stata.Results Seven articles were selected to the meta-analysis according to the inclusion criteria,of which cut-off values varied signicantly from studies.Due to the data heterogeneity (I2 > 50%,P <0.05),random model was used to pool the effect sizes.The overall combined effect sizes:sensitivity =81% (95%CI:0.76-0.86); specificity =81% (95% CI:0.76-0.86); DOR =30.03 (95% CI:7.89-114.37) ; AUC of SROC =0.905 9; Q*-0.837 6.Deek' s funnel plot showed little publication bias.Conclusions Plasma soluble triggering receptor expressed on myeloid cells-1 may be a useful adjunctive tool for the diagnosis of sepsis.However,further studies are needed in order to identify the best cut-off value in the diagnosis of sepsis.
8.Operative treatment for the elderly patients with pituitary adenomas

Objective To evaluate the overall effectiveness of microsurgery for elderly pituitary adenomas,and to select the optimal surgical approaches for this type of tumor. Methods Microsurgical operation was performed on 190 patients with elderly pituitary ademomas,and these patients were retrospectively analysed. The 190 cases were divided into transsphenoidal group and transcranial group with a comparison analysis of respective removal percentage,postoperative complication and follow-up. Results In the transsphenoidal group with 158 cases,total removal were performed in 112 cases (71.0%),subtotal removal in 30 cases,partial removal in 16 cases. In the transcranial group with 32 cases,total removal were performed in 8 cases (25.0%),subtotal removal in 16 cases,partial removal in 8 cases. Conclusions Transsphenoidal approach was characterized by shorter hospitalization period,less therapeutic charges and better postoperative quality of life and it should be of choice for the treatment of the elderly pituitary adenomas.

10.Effects of human fetal hepatic stimulator substance on D-galac-tosamine-induced liver necrosis in rats

The effects of human fetal hepatic stimulator substance (h-HSS) and liver cytosol (h-FLC) on the survival rate,the intensity of liver damage,and liver regeneration in rats with liver necrosis induced with D-galactosamine were observed.The physico-chemical properties of h-HSS were preliminarily determined.It was found that both h-HSS and H-FLC could markedly increase the.survival rate of the rats with toxic liver necrosis,significantly decrease the serum level of ornithine carbamoyl transferase and mitochondria aspartic trans-aminase,improve the hepaplastin activity,and elevate the incorporation rate of 3H-thymidine with liver DNA.h-HSS was stable after heated to 95℃ for 30 minutes,and sensitive to trypsin treatment.It demonstrated one major band at 15 000 daltons and 3 minor ones at 24 000,34 000,and 40 000 daltons respectively on sodium dodecyl sulfate-polyacrylamide gel electrophoresis.The relative content of the major band was 47.57%.
