Chinese Journal of Microsurgery 2000;0(03):-
Objective To explore clinical curative effect of microsurgery of petroclival tumors by transpetrosal presigmoid approach. Methods A series of 23 patients of petroclival tumors resected by microneurosurgery were analyzed retrospectively, including tumors histological types, clinical and preoperative MRI features, operative approaches, surgical technique and common postoperative complications. Results Total resection of the tumor was achieved microsurgically in 16 cases, subtotal resection in 4 cases, partial resection in 3 cases. No patient died in this series. 8 patients presented worsening crainal nerves palsies postoperatively. CSF leaks were uncommon, occuring in 2 patients. Conclusion The tumors in petroclival region can be totally resected by transpetrosal presigmoid approach. The advantages were offered by this approach such as the temporal lobes and cerebellum are minimally retracted; the operative distance of petroclival tumors is shortened with exposure of clivus and juxta-clival region.

Objective:To investigate the treatment failure sites, risk factors, and survival rates of patients with persistent or recur-rent squamous cell carcinoma (SCC) of the cervix within one year after concurrent chemoradiotherapy (CCRT). Methods:Clinical data of 30 patients with persistent or recurrent SCC of the cervix within one year after CCRT between June 2006 and June 2011 were ana-lyzed retrospectively. These data were compared with those of another 35 SCC cases without recurrence after complete remission. These 35 patients were treated homeochronously (from 2006 to 2011) and randomized in the control group. Results:Among the 30 pa-tients, 25 exhibited distant metastases;14 of these 25 patients were observed within six months after CCRT. Univariate analysis showed a higher incidence of pelvic or para-aortic lymphadenectasis and SCC-ag>10 ng/mL in the group with persistent or recurrent disease be-fore treatment (P<0.01). Multivariate analysis by logistic regression revealed that the pre-therapeutic pelvic or para-aortic lymph node enlargement and SCC-ag>10 ng/mL were found as the independent risk factors. Palliative chemotherapy ranked as the first method used to treat patients. The two-year survival rate was 21.7%, and the median survival time was 17 months. Conclusion:Patients with persistent or recurrent SCC of the cervix after CCRT possibly exhibited a high rate of distant metastasis with poor prognosis. The pre-therapeutic pelvic or para-aortic lymph node enlargement and SCC-ag>10 ng/mL were found as the independent risk factors of per-sistent or recurrent SCC within one year after CCRT.

Purpose To explore the value of low tube voltage, original data iterative reconstruction (SAFIRE) and intelligent optimum tube voltage scanning (CARE kV) technology in reducing radiation dose of high resolution CT (HRCT) temporal bone scanning and its impact on image quality. Materials and Methods Ninety patients with ear problem underwent clinical HRCT scan. All the patients were randomly divided into group A (conventional scanning), group B (CARE kV) and group C (low tube voltage) according to chronological order, with 30 cases in each group. Image quality was double-blind scored by two qualified radiologists. CT value, noise, contrast to noise ratio (CNR), signal to noise ratio (SNR), CT dose index volume (CTDIvol), dose-length product (DLP) and effective dose (ED) were compared among three groups. Results Image quality and the average CT value had no statistical difference among three groups (F=3.70, 3.61;P>0.05);noise, CNR, SNR, CTDIvol, DLP and ED had statistical difference among three groups (F=1.23, 1.42, 2.05, 2.13, 1.79, 1.32; P<0.05). Compared with group B, noise reduced about 14.1%in group C, while CNR and SNR increased about 32.1%and 26.3%, respectively (q=3.64, 3.45, 4.36; P<0.05). CTDIvol, DLP and ED were significantly different between group B and group C (q=3.13, 4.24, 4.24; P<0.05). Conclusion Scanning with low tube voltage, SAFIRE and CARE kV technology not only effectively reduce radiation dose, but also obtain satisfactory image quality. It should be widely utilized.

Journal of Pharmaceutical Analysis 2001;13(2):110-112
Objective To explore expression of endogenous ouabain(EO) in multiple adrenal tumors. Methods Thirty-one cases of adrenal tumors and 6 cases of healthy adrenal tissues were selected. The expression of EO in the adrenal tiss ue was detected with immunohistochemical streptavidin peroxadase conjugated(SP) method. Results Most of EO positive products were localized in cy toplasm of the zona reticularis of human adrenal cortex, and positive products s howed to be fine granular. There was no positive signal in the medulla. EO showe d on diffused positive in patients with pheochromocytoma accompanied high blood pressure[SBP:(165.22±7.61) mmHg, DBP:(105.52±4.26) mmHg], but there were neg ative in ones with normative blood pressure[SBP:(118.52±4.58) mmHg, DBP:(83±3.60) m mHg]. The expression of EO was positive in all adrenocortical hyperplasic, aden oma an d carcinoma, no matter its high or normative blood pressure. The degree of expre ssion of EO in adrenal tissues was related to the level of BP.Conclusion Expression of endogenous ouabain(EO) in health y adrenal tissue and adrenal tumors was a valuable morphological and pathophysio logical clue for the research on ouabain.

Objective To investigate whether area under the curve (AUC) of oral glucose tolerance test (OGTT) could work as a predictor of outcomes of gestational diabetes mellitus (GDM) on condition that blood glucose is controlled. Methods A total of 1 796 women who had a standard antenatal care in Peking University First Hospital and gave single live births from July 1, 2011 to December 31,2 013 were included. They should be diagnosed of GDM by the diagnosis criteria of gestational diabetes published by the Ministry of Health of PRC and diabetes pre-pregnancy excluded. Data were analyzed with SPSS 17.0, grouping by AUC. Results (1)Women with higher AUC had a rising trend of age and a downward trend of gestational weight gain, however, not statistically significant [specifically, in the four group of less than 15.00 mmol·L-1·h-1, 15.00 to 16.79 mmol · L-1 · h-1, 16.80 to 17.99 mmol · L-1 · h-1 and 18.00 mmol · L-1 · h-1 or more, gestational weight gain was (15.3±5.2), (14.1±4.8), (13.5±4.7) and (13.1±4.8) kg]. The prevalence of macrosomia raised while AUC increased. Those with an AUC of lower than 15.00 (mmol·L-1·h-1) had a lower risk of macrosomia (P=0.04) . But those with an AUC of 18.00 (mmol·L-1·h-1) or more had a higher risk of macrosomia (P=0.02). There was a rising trend in premature birth and preeclampsia with AUC increasing but not significant (the prevalence of premature birth was 4.38%, 5.36%, 7.71%and 7.94%while that of preeclampsia was 2.85%, 4.69%, 4.67% and 5.08% in these four groups).(2)The prevalence of macrosomia was 12.76% (54/423) when overweight pre-pregnancy ,significantly higher compared with 5.87%(65/1 107) in normal group. The prevalence of preeclampsia was 5.91%(25/423) and 3.34%(37/1 107) in those two groups, which was also significantly different. The obese group had a statistically highest prevalence of preeclampsia of 9.23%(12/130). (3)AUC (P<0.05, OR=1.113, 95%CI:1.008-1.218), as well as gestational weight gain (P<0.05, OR=1.520, 95%CI:1.279-1.806) and pre-pregnancy BMI (P<0.05, OR=1.183, 95%CI:1.125-1.243) made a difference in the prevalence of macrosomia. Meanwhile, pre-pregnancy BMI made sense in the prevalence of premature labor (P<0.05, OR=1.059, 95%CI:1.003-1.119) and preeclampsia (P<0.01, OR=1.202, 95%CI:1.123-1.286). Conclusions AUC, as well as pre-pregnancy BMI and gestational weight gain have a significant impact on outcomes of GDM, macrosomia especially, though blood glucose is controlled. Meanwhile, AUC might be considered as a predictor of macrosomia.
