Purpose To explore the value of low tube voltage, original data iterative reconstruction (SAFIRE) and intelligent optimum tube voltage scanning (CARE kV) technology in reducing radiation dose of high resolution CT (HRCT) temporal bone scanning and its impact on image quality. Materials and Methods Ninety patients with ear problem underwent clinical HRCT scan. All the patients were randomly divided into group A (conventional scanning), group B (CARE kV) and group C (low tube voltage) according to chronological order, with 30 cases in each group. Image quality was double-blind scored by two qualified radiologists. CT value, noise, contrast to noise ratio (CNR), signal to noise ratio (SNR), CT dose index volume (CTDIvol), dose-length product (DLP) and effective dose (ED) were compared among three groups. Results Image quality and the average CT value had no statistical difference among three groups (F=3.70, 3.61;P>0.05);noise, CNR, SNR, CTDIvol, DLP and ED had statistical difference among three groups (F=1.23, 1.42, 2.05, 2.13, 1.79, 1.32; P<0.05). Compared with group B, noise reduced about 14.1%in group C, while CNR and SNR increased about 32.1%and 26.3%, respectively (q=3.64, 3.45, 4.36; P<0.05). CTDIvol, DLP and ED were significantly different between group B and group C (q=3.13, 4.24, 4.24; P<0.05). Conclusion Scanning with low tube voltage, SAFIRE and CARE kV technology not only effectively reduce radiation dose, but also obtain satisfactory image quality. It should be widely utilized.

Purpose To assess the feasibility of applying dual-energy dual-source CT virtual non-contrast (VNC) imaging in the diagnosis of colorectal diseases. Materials and Methods Eighty-ifve patients with clinically suspected colorectal lesions underwent abdominal CT scan as well as arterial and venous phase dual-energy enhanced scan, VCN images of arterial and venous phase were obtained using the dual-energy software, the differences of image quality, radiation dose and diagnostic coincidence rate between the true non-contrast scan and VNC images were compared. Results The radiation dose of two-phase dual-energy scan was 34.8%lower when compared with the conventional three-phase scans. The CT values of the intestinal lesions, metastasis lymph nodes and intestinal fat in VNC were lower than the true unenhanced scan (P<0.05 or P<0.01), whereas were higher than the real scan in liver and liver metastases (P<0.01), there was no signiifcant difference of VNC between the arterial and venous phase. Differences of intestinal wall thickness, metastasis lymph node size, peripheral invasion and liver metastases observed from real non-contrast scan and VNC were not statistically significant (P>0.05), and neither was the diagnostic coincidence rate for intestinal diseases (P>0.05). The noise level of images obtained from VNC was lower than that of the real non-contrast scan (P<0.05), with higher SNR and CNR, there was no signiifcant difference of image scoring among the three groups (P>0.05). Conclusion For colorectal lesions, the virtual non-contrast images from the dual-energy dual-source CT scan can be used to reduce the radiation dose without effecting image quality and diagnosis accuracy.

Objective To investigate the effect of meridian-unblocking and mind-regulating acupuncture pretreatment on neurobehavioral functions and cerebral water content and explore its mechanism for modulating miRNA664 and MMP9 expressions in cerebral ischemia/reperfusion rats. Methods One hundred and thirty Wistar rats were randomized into group A (electroacupuncture pretreatment) of 30 rats, group B (moxibustion pretreatment) of 30 rats, group C (aspirin pretreatment) of 30 rats, group D (model) of 30 rats and group E (blank control) of 10 rats. Group A received electroacupuncture; group B, suspended moxibustion with moxa sticks; group C, an oral gavage of aspirin 10 mg/kg. A model of cerebral ischemia/reperfusion was made in every group after seven days. Rat neurobehavioral functions were observed and cerebral water content and relative miRNA664 and MMP9 expressions in the cortical region were determined at 24 hours after reperfusion. Results There was a statistically significant difference in the neurobehavioral score between group A, B, C or D and group E (P<0.01), between group A or B and group C (P<0.01) and between groups A and B (P<0.01). There were statistically significant differences in cerebral water content and relative miRNA664 and MMP9 expressions between group A, B, C or D and group E (P<0.01,P<0.05), between group A, B or C and group D (P<0.01), between group A or B and group C (P<0.01,P<0.05) and between groups A and B (P<0.01,P<0.05).Conclusions Meridian-unblocking and mind-regulating acupuncture pretreatment can effectively decrease the neurobehavioral score and cerebral water content and reduce relative MMP9 expression through modulating miRNA664 expression in cerebral ischemia/reperfusion rats. One of the mechanisms of Du meridian-unblocking and mind-regulating acupuncture pretreatment for protecting the brain may be reducing relative MMP9 expression, inducing tolerance to cerebral ischemia and relieving cerebral edema by the modulation of miRNA664 expression.

Objective:To investigate the current situation of the use of transjugular intrahepatic portosystemic shunt (TIPS) for portal hypertension, which should aid the development of TIPS in China. Methods:The China Portal Hypertension Alliance (CHESS) initiated this study that comprehensively investigated the basic situation of TIPS for portal hypertension in China through network research. The survey included the following: the number of surgical cases, main indications, the development of Early-TIPS, TIPS for portal vein cavernous transformation, collateral circulation embolization, intraoperative portal pressure gradient measurement, commonly used stent types, conventional anticoagulation and time, postoperative follow-up, obstacles, and the application of domestic instruments. Results:According to the survey, a total of 13 527 TIPS operations were carried out in 545 hospitals participating in the survey in 2021, and 94.1% of the hospital had the habit of routine follow-up after TIPS. Most hospitals believed that the main indications of TIPS were the control of acute bleeding (42.6%) and the prevention of rebleeding (40.7%). 48.1% of the teams carried out early or priority TIPS, 53.0% of the teams carried out TIPS for the cavernous transformation of the portal vein, and 81.0% chose routine embolization of collateral circulation during operation. Most of them used coils and biological glue as embolic materials, and 78.5% of the team routinely performed intraoperative portal pressure gradient measurements. In selecting TIPS stents, 57.1% of the hospitals woulel choose Viator-specific stents, 57.2% woulel choose conventional anticoagulation after TIPS, and the duration of anticoagulation was between 3-6 months (55.4%). The limitation of TIPS surgery was mainly due to cost (72.3%) and insufficient understanding of doctors in related departments (77.4%). Most teams accepted the domestic instruments used in TIPS (92.7%).Conclusions:This survey shows that TIPS treatment is an essential part of treating portal hypertension in China. The total number of TIPS cases is far from that of patients with portal hypertension. In the future, it is still necessary to popularize TIPS technology and further standardize surgical indications, routine operations, and instrument application.
