1.Distribution of Mosquito Larvae on Kosrae Island, Kosrae State, the Federated States of Micronesia

Surveys of mosquito larvae were carried out in six areas of Kosrae Island, Kosrae State, the Federated States of Micronesia in December 2009 and June 2012. A total of 962 larvae of six species were collected from 106 natural and artificial habitats. They were identified as Aedes aegypti, Ae. albopictus, Ae. marshallensis, Culex quinquefasciatus, Cx. annulirostris, and Cx. kusaiensis. This is the first report from Kosrae Island for three of these species—Ae. marshallensis, Cx. quinquefasciatus, and Cx. annulirostris. The most abundant species was Ae. albopictus, followed by Ae. marshallensis, and these two species were found in all areas. Relatively large numbers of Cx. quinquefasciatus and Cx. kusaiensis were found in five areas. Fewer Cx. annulirostris were found, and only in three areas. Aedes aegypti larvae were collected from a single habitat at Tafunsak in 2009. To prevent the outbreak of dengue fever, environmental management should focus on the destruction, alteration, disposal and recycling of containers that produce larger numbers of adult Aedes mosquitoes.
2.Development of ADL index for older community people.

The purposes of this study were to examine reliability and validity of ADL index for older people at home, and relationships between ADL score and various factors (ex. self-assessment of health status and physical fitness level, and exercise frequency/week), and to examine the screening basis for execution of physical fitness test based on distribution of ADL score. A questionnaire consisting of 12 ADL items and a physical fitness test of Ministry of Education, Science and Clture were administered to 5, 715 subjects 65 years or more age (male: 2, 745; female: 2, 970) . As a result of examining test-retest reliability of the ADL index, significant high correlations were shown among 12 ADL items (0.674≤r≤0.886), and in overall score (r=0.943) . The ADL score was significantly related to age and physical fitness test scores, and tended to be higher in subjects with a higher self-assessment of health status and physical fitness level, and with more frequency. It was suggested that these results prove the utility of the present ADL index. Further, as the result of examining relationships between distribution of overall ADL score and physical fitness test scores, the following screening bases were considered to be valid for judgement whether the physical fitness tests could be executed. The three screening bases using overall ADL score were: 1) 12 or under; 2) over 13 to under 24; 3) 24 and over. The four bases using item score were: 1) a category 1 response for items 1, 5 and 6; 2) a category 2 or 3 response for items 1, 5 and 6, and a category 1 response for items 3 and 4; 3) a category 3 response for items 2 and 14; 4) a category 1 response for items 10 and 12.
3.Parasite egg contamination of water and air in a suburban area of Hanoi, Vietnam

Contamination of water and air by soil-transmitted helminth eggs was investigated in a small village located in the suburbs of Hanoi, Vietnam. Water samples were collected from 29 households, two schools, two kindergartens, one restaurant, three ponds and 23 ditch sites during the rainy season. Water samples were also collected at the same places, except for one household and one restaurant, during the dry season. The water samples collected from households, schools, kindergartens and restaurant were comprised of well-water and rain-water. These samples included both water filtrated with sand and gravel and non-filtrated water. Two-liter water samples were examined for helminth eggs by either a centrifugation or flotation method. The contamination of air by helminth eggs was assessed by the method of Kroeger et al. (1992). Eggs in air were trapped on adhesive tapes hanging in rooms and in the area around 29 houses, two schools, two kindergartens, one restaurant and 18 utility poles.
Out of 63 water samples collected from households in the rainy season, helminth eggs were found in four water samples; one from non-filtrated well-water and three from filtrated well-water samples. The one non-filtrated water sample contained six eggs of Toxocara sp., while the three filtrated water samples contained one egg each of Trichuris sp., Trichiuris sp. and Taenia sp. No eggs were found in the water samples collected from schools, kindergartens or the restaurant in the rainy season. All water samples collected from ponds and ditches in the rainy season contained many helminth eggs. The eggs found were Ascaris sp., Trichuris sp., Toxocara sp., Ascaridia galli, hookworm, Taenia sp. and Fasciola sp. Examination of the adhesive tapes hanging in the air showed that four sites were contaminated by helminth eggs, i. e. one site near the house, two sites near the school and one site at a utility pole. The species of eggs found were Trichuris sp., Ascaridia galli and Taenia sp. The number of eggs found on tapes was only one or two. In the dry season, a few samples of well-water and rain-water collected from the residential area were contaminated with helminth eggs, and all samples collected from ponds and ditches contained many eggs of various species similar to those collected in the rainy season. The present study clearly indicates that, in our study area, the villagers were subject to infection from soil-transmitted helminthes directly and indirectly through water.
4.The correlation between contamination of soil with Ascaris sp. eggs in school yards and ascariasis among primary school children in Mai Trung Commune, Northern Vietnam

Four hundred and six soil samples collected from five schoolyards in Mai Trung Commune, Bac Giang Province, Northern Vietnam, from 2005 to 2007 were examined for helminth eggs using a sucrose centrifugal flotation method. The soils around latrines was found to be heavily contaminated with Ascaris sp. eggs. The contamination rate of Ascaris sp. eggs at each school and prevalence of ascariasis among the school children showed a positive correlation (P&It;0.05). This indicated that examination of soil from the area around latrines for Ascaris sp. eggs is a useful method to estimate the prevalence of ascariasis among school children.

The purpose of this study was to propose useful ADLs (activities of daily living) and scales to estimate ADL ability on community older people from the viewpoints of aging, physical fitness level, and interrelationship among ADL. The survey constructing of 15 ADL items and physical fitness test of Ministry of Education, Science and Culture were carried out on 458 older people. Each ADL item was constructed with 3 level scales.
As the results of this study, the following 12 ADLs were proposed as useful ADL items; in the locomotion domain, jumping across a ditch, walking, running, up and down stairs, and transfer; in the manipulation domain, taking bedding in and out, buttoning a shirt; in stability domain, standing in the bus or train, dressing while standing (trousers), standing on one foot with eyes open; in the posture-change domain, sitting up, standing up from the floor. These items were significantly related to age and physical fitness elements contributed largely to achievement of each ADL. Since significant high correlation was found between the index using 12 ADL items and that using 15 items, it is considered that the 12 items scale can account for the variance corresponded enough to that of 15 items scale, and that the new index is practical and simple.
In further research, it will be required to examine validity of each ADL item and index by using more large sample, and to clarify the relation to various factors influenced to ADL ability level.
6.The effect of an intervention of a regional palliative care intervention program on home hospice utilization and hospital staff’s perceptions about home care: an observation from the OPTIM-study

This study aimed to clarify whether a regional palliative care intervention program, the OPTIM project, increased home hospice utilization, and explore the potential association between the home hospice utilization and the hospital staff's perceptions on home care. A questionnaire survey was conducted involving 154 physicians and 469 nurses. The rate of patients who made the transition to home-based care increased 967% in A Hospital, 295% in B Hospital, and 221% in C Hospital in 2010 compared to 2007, which was assumed to be 100. Staff of a hospital where many patients made the transition to home-based care were more likely to agree with the following statements concerning home care perspectives: “I started to consider that even cancer patients can be treated at home until the last moment of their life”, “I usually ask patients whether they wish to receive home-based care”, “We decided on coping strategies for sudden changes in the course of disease and a place to contact in advance”, and “I started to simplify treatment procedures, such as prescriptions during hospitalization for patients and their families to prepare for home-based care“.
7.High-Grade, Advanced Tongue Cancer Treated with Arterial Injection Chemoradiotherapy by Multidisciplinary Medical Teams

In recent years, favorable therapeutic outcomes have been reported for arterial injection chemoradiotherapy for tongue cancer. The present case involves an 80-year-old woman in our palliative care department who had high-grade, advanced tongue cancer. Because there was a request for surgery to prevent airway occlusion due to growth of the tumor, she was referred to our department in April 2009. As a treatment policy for controlling tumor growth in high-grade, advanced tongue cancer, arterial injection chemoradiotherapy was carried out through the superficial temporal artery, with a tongue artery catheter in place on both sides. Therapeutic effect was obtained, and it was possible to avoid airway occlusion through tumor regression. Dysphagia and dysphemia were improved, which in turn improved quality of life. In this case, there was an opportunity to carry out multidisciplinary team medicine, including support from the oral care and palliative care teams as part of the process of cancer therapy. Here, we present our findings in this case.
8.Associations of education and income with hazardous drinking among postpartum women in Japan: results from the TMM BirThree Cohort Study.

BACKGROUND:
Although the postpartum period is suggested to provide an ideal opportunity for interventions to prevent hazardous drinking, evidence on the associations of education and income with hazardous drinking during this period is limited, including in Japan.
METHODS:
We analyzed data from 11,031 women who participated in the Tohoku Medical Megabank Project Birth and Three-Generation Cohort Study in Japan. Hazardous drinking was defined as ethanol intake of ≥20 g/day 1 year after delivery. We conducted multiple logistic regression analyses to examine whether educational attainment or equivalent household income was associated with hazardous drinking, adjusting for age, parity, drinking status during pregnancy, work status, postpartum depression, breastfeeding, and income/education. We also conducted stratified analyses by income and education groups.
RESULTS:
The prevalence of hazardous drinking 1 year after delivery was 3.6%. Lower education was associated with hazardous drinking; the odds ratio (95% confidence interval) of high school education or lower compared with university education or higher was 2.17 (1.59-2.98). Lower income was also associated with hazardous drinking, but this association disappeared after further adjustments for education; the odds ratios (95% confidence intervals) of the lowest compared with highest level of income were 1.42 (1.04-1.94) and 1.12 (0.81-1.54), respectively. A significant interaction was detected; lower education and lower income were associated with increased risks of hazardous drinking only in a lower income group and lower education group, respectively.
CONCLUSIONS
Postpartum women with lower education and lower income had higher risks of hazardous drinking in Japan.
