4.A Case of Darier's Disease.

A case of Dariers disease was presented and the literatures were reviewed. The patient was 20 year old male whose skin lesions revealed normal skin colored, dirty, scaly, keratotic papules with mild itching on face, anterior chest, back, abdomen, axillae, groins and flexural surfaces of extremities. They had a tendency to aggrevate in summer and improve in winter. Histopathologically, suprabasal lacunae, hyperkeratosis, papiIIomatosis, moderate degree of acanthosis and corps ronds in Iacuna were demonstrated in the epidermis. Dermal papillae surrounded with single layer of basal cells proliferated inta the lacunae. Upper dermis revealed mild nonspecific inflammatory cell infiltrates.

6.A Case of Allergic Contact Dermatitis due to Lanolin.

Lanolin is a complex material consisting of a mixture of numerous esters and polyesters of high-moleculer-weight alcohols and fatty acids, Lanolin allergy is important because lanolin is widely used as an emulsifier in topical medicaments and in numerous cosmetic preparations. We report a case of allergic contact dermatitis in a 27-year-old female. An erythematous pruritic rash develgood around the eyelid developed after applying a new eye cream and was aggravated after applying Cambison ophthalmic ointment. Patch testing showed positive reactions to Cambison ophthalmic ointment, 30% wool alcohol, Amerchol L101 and Nivea cream. A use test with 100% Cambison ophthalmic ointment base was positive

7.A Case of Neurofibromatosis with Bilateral Pheochromocytoma.

Pheochromocytoma is originated from chromaffin cell of sympathetic nervous system and associated with other disease, such as neurofibromatosis, duodenal carcinoid, medullary thyroid cancer and parathyroid adenoma. Especially, pheochromocytoma is developed more than 50% in neurofibromatosis associated with hypertension. In such cases, several clinical features documented as more frequent bilateral phochromocytoma, more associated with other neuroendocrine tumors and thus more poor prognosis. We can observe the sustained hypertension despite of surgical resection of tumors in pheochromocytoma cases. One of the possible reason of post operative sustained hypertension is the pheochromocytoma originated from minor organ of Zukerkandl that was not resected during operation. Untreated or delayed treated cases with pheochromocytoma were often expired by complication of hypertension such as cerebrovascular hemorrhage, myocardial infarction, etc. Thus, in neurofibromatosis with hypertension, screening of pheochromocytorna is very important for the early detection of tumor and more favorable prognosis. Recently, We experienced a case of neurofibromatosis associated with bilateral pheochromocy-toma expired by cerebral hemorrhage during operation, so we report the case with literature review.

8.Coronary Flow Doppler Profile in No-Reflex Phenomenon after Direct PTCA in Acute Myocardial Infarction.

Profound reduction of anterograde coronary flow with concomitant ischemia is seen occasionally during percutaneous coronary intervention despite technically successful procedure. We found interesting coronary flow pattern in a patient with acute myocardial infarction, showing angiographic no reflow phenomenon after direct PTCA. The coronary blood flow pattern of the angiographic no-reflow phenomenon in this case was characterized by minimal systolic flow and sharp deceleration of diastolic flow. Coronary flow reserve calculated by the ratio of adenosine induced maximal hyperemic velocity and basal velocity was reduced. The Dopplertipped guide wire was useful for observation of phasic coronary flow pattern of angiographic no-reflow phenomenon.

10.A Case of Heterotopic Pregnancy in a Natural Cycle.

Heterotopic pregnancy is the coexistency of intrauterine and extrauterine pregnancy. The incidence of heterotopic pregnancy is about 1 to 30,000 pregnancy in a natural cycle. However, the frequency of heterotopic pregnancy has steadily increased because of rising incidence of pelvic inflammatory disease, pelvic surgery and the development of ovulation induction and assisted reproduction. Because heterotopic pregnancy is difficult to diagnose and it has high morbidity and mortality rate, one should always take this into consideration and should conduct careful and thorough gynecologic evaluation. We have experienced a case of heterotopic pregnancy in a 29-year old woman who presented with acute abdominal pain in a natural cycle and report this case with a brief review of literature.
