4.Study on the therapeutic effects of interferon and gamma-globulin in experimental Pneumocystis carinii pneumonia.

This study was performed to observe the therapeutic effects of interferon-gamma(IFN-gamma) and gamma-globulin(gamma-globulin) in experimental Pneumocystis carinii pneumonia of immune suppressed mice. After 9 weeks, trimethoprim-sulfamethoxazole(TMP-SMZ; 10-50 mg/mouse/day), mouse IFN-gamma(5 x 10(4) units/mouse/day) and mouse gamma-globulin(20 mg/mouse/day) were administered to the mice for 3 weeks by the experimental group. The therapeutic efficacy was evaluated by body weights, histopathologic and electron microscopic findings of the lungs, and number of P. carinii cysts by Gomori's methenamine silver stain. Body weights of the mice were significantly increased in the group of combination therapy of TMP-SMZ with IFN-gamma or gamma-globulin, and in the group of TMP-SMZ treatment (p < 0.05), however, little effect was found in the group of gamma-globulin alone. Histopathologic findings of P. carinii pneumonia were much improved in the group of combination therapy of TMP-SMZ with IFN-gamma. Treatment with either TMP-SMZ or IFN-gamma significantly reduced the number of cysts in the P. carinii pneumonia, but gamma-globulin alone was ineffective. In electron microscopic findings of P. carinii pneumonia, the number of trophozoites and cysts were reduced by treatment with either TMP-SMZ or IFN-gamma, and most of the cysts were empty or containing one or two intracystic bodies. The present results suggested, that combination therapy of TMP-SMZ with IFN-gamma had synergistic effects in treatment of P. carinii pneumonia in experimental mice.

5.A Case of Allergic Contact Dermatitis due to Lanolin.

Lanolin is a complex material consisting of a mixture of numerous esters and polyesters of high-moleculer-weight alcohols and fatty acids, Lanolin allergy is important because lanolin is widely used as an emulsifier in topical medicaments and in numerous cosmetic preparations. We report a case of allergic contact dermatitis in a 27-year-old female. An erythematous pruritic rash develgood around the eyelid developed after applying a new eye cream and was aggravated after applying Cambison ophthalmic ointment. Patch testing showed positive reactions to Cambison ophthalmic ointment, 30% wool alcohol, Amerchol L101 and Nivea cream. A use test with 100% Cambison ophthalmic ointment base was positive

6.A Preliminary Study for the Perceived Need and Use of Child Mental Health Services.

OBJECTIVES: We aimed to identify the degree of perceived need and use of child mental health services, and to examine the factors associated with the perceived need and use of the child mental health services. METHODS: The sample consisted of 3,477 children aged 8 to 13 years. The parents completed the sociodemographic data, the Korean version of the Child Behavior Checklist (K-CBCL), and the Korean version of DuPaul's Attention Deficit Hyperactivity Disorder Rating Scale (K-ARS). RESULTS: Overall, 10% of children demonstrated the need for mental health services, and 2.36% utilized the child mental health services. All subcategory scores of K-CBCL and K-ARS were significantly low in the no help needed group. Factors associated with the actual use of child mental health services included social problems and thought problems. CONCLUSION: The demand for child mental health services is high, but only a small number of children are using mental health services. To increase actual use, it is necessary to educate and campaign for various child mental health.

7.Extended distal pancreatectomy for advanced pancreatic neck cancer.

BACKGROUNDS/AIMS: We investigated the clinical application of extended distal pancreatectomy in patients with pancreatic neck cancer accompanied by distal pancreatic atrophy. In this study, we have emphasized on the technical aspects of using the linear stapling device for a bulky target organ. METHODS: From March 2010 to September 2013, 46 patients with pancreatic adenocarcinoma, who underwent pancreatic resection with radical intent at our institute, were reviewed retrospectively. Among them, three patients (6.5%) underwent extended distal pancreatectomy. A linear stapling device and vise-grip locking pliers were used for en bloc resection of the distal pancreas, first duodenal portion, and distal common bile duct. The results were compared with those after standard pancreatectomy. RESULTS: All three patients presented with jaundice, and the ratio of pancreatic duct to parenchymal thickness of the pancreatic body was greater than 0.5. Grade A pancreatic fistula developed in all of the cases, but none of these fistulae were lethal. Pathological staging was T3N1M0 in all of the patients. The postoperative daily serum glucose fluctuations and insulin requirements were comparable to those in patients who received pancreaticoduodenectomy or distal pancreatectomy. At the last follow-up, two patients were alive with liver metastasis at 4 and 10 months postoperatively, respectively, and one patient died of liver metastasis at 5 months postoperatively. CONCLUSIONS: While the prognosis of advanced pancreatic neck adenocarcinoma is still dismal, extended distal pancreatectomy is a valid treatment option, especially when there is atrophy of the distal pancreas. Also, the procedure is technically feasible, and further refinement is necessary to improve patient survival.

9.Development of Korean-Peer Nomination Inventory(K-PNI):An Inventory to Evaluate School Bullying.

OBJECTIVES: School bullying is known to be related to behavioral, emotional and social problems both in victims and perpetrators. The aims of this study are to develop a Korean-Peer Nomination Inventory(K-PNI), which can identify victims and perpetrators of school bullying in an objective and effective way, and to examine its reliability and validity. METHODS: Fourth to 6 th graders of two elementary schools in An Yang city completed K-PNI, Peer Acceptance Scale and Self-Perception Scale. Varimax Rotation Factor Analysis, Cronbach's Alpha Reliability Analysis and Pearson's Correlational Analysis were performed for statistical analyses. RESULTS: Total of 532 students(male:female=1:1;4 th:5 th:6 th grader=1:1:1.2) participate in this study. Factor analysis showed that K-PNI identified victims and perpetrators of school bullying effectively with excellent reliability(Chrobach's alpha;0.90-0.94). Victims were further divided into two categories;passive bullying including exclusion and verbal abuse, and active bullying including physical abuse and being forced. K-PNI, Peer acceptance scale and Self-perception scale were significantly correlated, reflecting its validity identifying victims and perpetrators of school bullying. CONCLUSION: K-PNI is an effective tool with excellent reliability and validity to identify victims and perpetrators of school bullying. K-PNI is expected to be a useful tool in future studies of school bullying and school violence.

10.A Case of Neurofibromatosis with Bilateral Pheochromocytoma.

Pheochromocytoma is originated from chromaffin cell of sympathetic nervous system and associated with other disease, such as neurofibromatosis, duodenal carcinoid, medullary thyroid cancer and parathyroid adenoma. Especially, pheochromocytoma is developed more than 50% in neurofibromatosis associated with hypertension. In such cases, several clinical features documented as more frequent bilateral phochromocytoma, more associated with other neuroendocrine tumors and thus more poor prognosis. We can observe the sustained hypertension despite of surgical resection of tumors in pheochromocytoma cases. One of the possible reason of post operative sustained hypertension is the pheochromocytoma originated from minor organ of Zukerkandl that was not resected during operation. Untreated or delayed treated cases with pheochromocytoma were often expired by complication of hypertension such as cerebrovascular hemorrhage, myocardial infarction, etc. Thus, in neurofibromatosis with hypertension, screening of pheochromocytorna is very important for the early detection of tumor and more favorable prognosis. Recently, We experienced a case of neurofibromatosis associated with bilateral pheochromocy-toma expired by cerebral hemorrhage during operation, so we report the case with literature review.
