6.Intraoperative Normovolemic Hemodilution in Patients Undergonig Posterolateral Spinal Fusion: Safety and Efficacy.

BACKGROUND: The implementation of intraoperative normovolemic hemodilution is a strategy used in an attempt to diminish the need for or obviate allogeneic transfusion and to avert the potential complications. The goal of this study was to evaluate the safety and efficacy of moderate intraoperative normovolemic hemodilution. METHODS: Fifteen patients scheduled for posterolateral spinal fusion underwent intraoperative normovolemic hemodilution with 10% pentastarch to a target hematocrit level of 25% (hemodilution group). All units of blood procured by hemodilution and additional allogeneic blood was transfused in the perioperative period to maintain the hematocrit level of>25%. We investigated the effect of hemodilution on whole blood coagulation as measured by the thromboelastography and, evaluated its efficacy of decreasing the need for allogeneic blood transfusion as compared to the nonhemodilution group, retrospectively. RESULTS: Hemodilution with pentastarch caused a decrease in response and coagulation time (p<0.01) but did not influence on the alpha angle and maximum amplitude on thromboelstogram. Amount of transfusion of allogeneic blood was 4.1 1.7 units for the hemodilution group and 5.0 1.5 units for the nonhemodilution group. Net red blood cell volume ""saved"" from hemodilution was about 120 ml. CONCLUSION: Moderate intraoperative normovolemic hemodilution with pentastarch does not affect the coagulability of whole blood. But its efficacy of decreasing the need for allogeneic blood transfusion is minimal.

7.Development of Metabolic Syndrome in Children Who were Surgically Treated for Craniopharyngioma.

PURPOSE:Obesity is a common complication in children who were surgically treated for craniopharyngioma. We analysed body composition, metabolic parameters and evaluated for correlation between adiponectin and metabolic parameters in children who were surgically treated for craniopharyngioma. METHODS:Sixteen patients with craniopharyngioma (9 males, 7 females) were included in this study (patient group). The data of patient group were compared to those of children who visited the clinic for normal short stature with appropriate gestational age (14 males, 4 females) (control group). Height SDS, weight SDS, BMI were calculated. Total body fat, abdominal fat distribution were measured by bioelectrical impedance analysis. Plasma levels of total cholesterol, HDL-cholesterol, triglycerides, adiponectin, and insulin were measured after overnight fasting. The degree of insulin resistance was calculated by HOMA-IR. RESULTS:Four of 16 patients were obese. and three were overweight. Patient group had higher total body fat percentage (27.5+/-6.8 vs 17.6+/-6.8, P=0.01), triglycerides (207.9+/-241.6 vs 76.7+/-30.8, P=0.001), lower HDL-cholesterol (37.5+/-14.6 vs 61.4+/-15.4, P=0.000) and adiponectin levels (7.1+/-4.2 vs 10.7+/-4.3, P=0.012). Adiponectin levels were correlated negatively with total body fat percentage (r=-0.631, P= 0.009), abdominal fat distribution (r=-0.547, P=0.03), triglycerides (r=-0.518, P=0.04), insulin (r=-0.525, P=0.037) and correlated positively with HDL-cholesterol (r=0.572, P=0.021). CONCLUSION: These results suggest that children who were surgically treated for craniopharyngioma, had tendency to develop obesity, dyslipidemia and Insulin resistance. Therefore, patients who were surgically treated, should be closely monitored to minimize the effect of metabolic syndrome on their health and well being.

OBJECTIVE: To assess the reliability of triple marker screening test in midtrimester pregnancy for fetal Down syndrome. METHODS: From October 1, 1996 to May 31, 1998 at Nowon Eulji Hospital, 3700 Pregnant women underwent serum tiple marker screening for Down syndrome during 15-20weeks of gestational age. The results of serum triple marker screenig tests for Down syndrome and the outcomes of pregnancies were retrospectively assesed. RESULTS: Sixty seven of 3700 cases(1.81%) were positive in screening test, and 3633(98.18%) cases were negative. Among 67 cases of positive screening test, 1 case(1.49%) was diagnosed as Down syndrome. Among 3633 cases of negative screening test, 4 cases(0.1%) were diagnosed as chromosomal abnormalies postnatally. Two of these 4 cases of chromosomal abnormalies were Down syndrome. CONCLUSION: With this results, sensitivity of triple marker screeing test for Down syndrome is very low as 33.3%. In order to increase the sensitivity, some compensatory adjustment is required in triple marker screening test.
