The model of lymphocyte differentiation described for Lukes-Collins' classification of non-Hodgkin's lymphoma is related particularly to morphological alterations of nuclei of follicular center lymphocytes by antigenic stimulation. The authors carried out ultrastructural and morphometric studies to investigate morphological alterations during lymphocyte transformation on the nuclear profiles of follicular center, parafollicular and mantle zone lymphocytes in ten tonsillectomy cases of chronic hypertrophic tonsillitis. The nuclear parameters measured included nuclear area, contour index, frequency invagination and cleft, depth of invagination and nuclear diameters. Follicular centers contained a mixed population of lymphocytes consisting of untransformed (type 1), partially transformed (type 2) and fully transformed (type 3) lymphocytes. During lymphocyte transformation in both follicular, and parafollicular and mantle zones, the nuclei had a gradual and progressive increase in size. The nuclear contour index of type 2 nuclei of both follicular and parafollicular and mantle zones tended to be higher than those of type 1 and 3, indicating a greater degree of irregularity and variability of nuclear profiles. Invaginated and cleaved lymphocytes were not confined to me transformed lymphocytes. A considerable portion of lymphocytes had invaginations and clefts in parafollicular and mantle zone as well as follicular center. No difference on the depth of invagination was noted in type 1, type 2 and type 3 lymphocytes. The results indicate that some promise of the Lukes-Collins conepts of follicular center cells and the process of lymphocyte transformation in follicular centers may be necessary to revise

Serial changes of serum IgE, IgG, eosinophils were observed in 25 patients with acute myocaridial infarction and 20 ischemic heart disease without evidence of acute myocardial infarction and evaluated in terms of several parameters and its clinical significance. The results observed were as follows : 1) Serum IgE levels were propgressively elevated from the first hospital day(259+/-3IU/ml) up to peak level of the fifth hospital day(415+/-2IU/ml) and progressively lowered and returned to almost same level as the first hospital day on the twenty first hospital day. On the other hand control group showed significantly lower IgE levels throughout all hospital day and also did not showed serial change. 2) In the patient group with the initial serum IgE level above 200IU/m; showed significantly lower level of serum SGOT, CPK level than the group of below 200IU/ml group. This suggests the initial serum IgE level might have some correlation of the extent of myocardial necrosis. 3) In patients of acute myocardial infarction, ejection fraction was checked at discharge. Initial serum IgE level above 200IU/ml group showed significantly higher ejection fraction than below 200IU/ml group(59.4+/-13.5% vs 38.4+/-13.7%). 4) Serum IgE was checked concomittantly with serum IgE. It showed slightly decreasing tendency at third hospital day but not statistically significant. Eosinophil changed similar pattern as serum IgE but it was also not statistically significant. In conclusion, serial checking of serum IgE level in patient of acute myocardial infarction may give some help in prediction the clinical course and prognosis.

The objective of cancer screening is to reduce the risk of death, i.e., mortality from cancer among individuals subjected to screening. For cervical and colorectal cancer, the screening test is aimed at detection of preinvasive lesions. Therefore, reduction in the incidence of invasive disease also results from screening. In a national cancer control program, the programs should be orgainzed to ensure that a large proportion of the target group is screened and that those individuals in whom abnormalities are abserved receive appropriate diagnosis and therapy. Agreement should be reached on guidelines to be applied in the national cancer control program. When establishing appropriate cancer screening recommendations several factors should be considered. ① For establishing the starting age, age-specific incidence and mortality rate, life lost person-years, and cost-effectiveness can be considered. ② For frequency of screening, it is necessary to understand the natural history of disease. ③ For choosing the best test tool, sensitivity and specificity, compliance, cost, and adverse effects can be considered. It seems that doctors' willingness to participate is essential for a successful cancer screening program. They should understand the rationale of cancer secreening, its effectiveness, and the risk.

To investigate the heterogeneity of fibroblast proliferation rate depending on the depth of dermis in the psoriatic skin lesions, fibroblasts were obtained from the upper and lower dermis of the forearm skin lesions in 9 psoriatir, patients and of the comparable sites in 7 healthy persons respectively, and were cultured. The in vitro proliferation rates of the fibroblasts were calculated by measuring the efficiency af cell attachment, cell numbers on varying days of culture and the population doubling time during the 3rd passage of subculture. The results were as follows: 1, The efficiencies of cell attachment at 24 hours after seeding were not statistically different between the upper derrnal fibroblasts and the lower ones in both psoriatic skin lesions and normal skins. 2. By rneasuring the cell numbers and the population doubling times, the prolifeatior rate of the upper der mal fibroblasts was greater than that of the lower ones in both psoriatic skin lesions and normal skins and that the fibroblastic proliferation rate was significantly inereased in the psoriatic skin lesion compared to the normal skin.
