1.A Case of Deep Vein Thrombosis Associated with Pulmonary and Renal Thromboembolism.

The deep vein thrombosis had been described as the reaction of vascular inflammatory change by John Hunter in 1773, and Virchow described the pathophysiology of etiologic factors for the first time in 1856. Deep Vein thrombosis and pulmonary embolism is common disease in the North American and European countries. It was reported that deep Vein thrombosis and pulmonary embolism were associated with 300,000 to 600,000 hospitalizations for each year and that as many as 50,000 individuals die each year as a result of pulmonary embolism in the United States. In Korea, deep vein thrombosis is uncommon, complicating pulmonary embolism & renal thromboembolism are also rare, and many cases had not specific history & precipitating factor. The report is for one case of deep vein thrombosis associated with pulmonary & renal thromboembolism, who was 34-years-old, admitted to Masan Koryo hospital because of pain and edema in left lower extremity, had not specific history & precipitating factor. The thrombosis(or thromboembolism) involed his left lung & kidney, and the patient present with severe swelling of affected limb and oliguria. He underwent leg elevation under absolute bed rest and anticoagulant therapy. The significant improvement was done under medical treatment. We report one case of deep vein thrombosis with a brief of literatures.

2.Comparative study of eosinophil counts and eosinophil catonic protein(ECP), soluble vascular cell adhesion molecule-l(VCAM-1) in tears and sera in children with allergic conjunctivitis.

The eye is a common target organ of the allergy, and allergic conjunctivitis is the most common of eye diseases. Clinical manifestations of allergic conjunctivitis are acute bilateral red, itchy, and watery eyes. The presence of conjuncitival eosinophilia may be consided to be a diagnostic indicator of allergic conjunctivitis. ECP is a quantifiable toxic product secreted by activated cosinc phils. VCAM-1 promotes adhesion of leukocytes to endothelium in vitro and may promote imflammation in vivo. The objective of this study is to measure eosinophil count, ECP and sVCAM-1 levels of tears and sera in patients with acute allergic conjunctivitis and normal subjects and to assess the correlation of these mediators with the severity of the disease and the clinical usefulness. Seventeen subjects were selected on the basis of clinical manifestations, history, skin prick test, total IgE. A microcapillary tube was used to collect the tears from the inner canthus, conjunctival epithelia were obtained for eosinophil count by scraping the upper tarsal conjunctiva. The level of ECP was measured by CAP system (Kabi-Pharmacia, Sweden), sVCAM-1 was measured by ELISA (R&D, USA). Serum IgE and eosinophil count were in creased in 10 patients, allergic skin prick test were positive in 11 subjects (D.p: 9, D.f: 8), eosinophilia in conjunctival epithelium were present in 11 subjects (4 patients: > 3/HPF, 7 patients: 1-3/HPF). ECP in tears were increased in patients significantly (12.0+8.0 vs 3.9+3.8 ng/ml, p=0.01), but not in serum (52.5+43.1 vs 28.3+25.9 ng/ml). There is no significant correlation between eosinophil count and ECP in serum and tears (p>0.05, r-=0.19). Serum sVCA-M-1 level is significant different between patients and controls (1916.5+756.0 vs 1147.2+146.1 micro gram/ml, p=0.01), sVCAM-1 is significantly correlated to eosinophil count (p=0.01, r=0.56) and ECP (p<0.05, r=0.65). In conclusion, eosinophil and ECP in tears may be very important role in allergic conjunctivitis and are useful indicators of the disease. The elevation of sVCAM-1 in serum may be interpreted simply as marker of the presence of non-specific inflammation.

3.The Outcome of Renal Transplantation Using Exchange Donor Program.

PURPOSE: The availability of living donors is the major limiting factor in living related kidney transplantion. In 1997, Ross et al. proposed that the living unrelated kidney transplantation through exchange arrangement of ABO-incompatible donors-recipient pairs to expand donor pool is ethically acceptable as living related kidney transplantation. We analyzed exchange donor program whether this could expand the kidney donor pool in living donor renal transplantation. METHODS: We retrospectively reviewed the records of the 121 exchange donor renal transplantations performed from 1991 to 2005 in our center. Recipient's and donor's mean age were 38.8 (15~64) and 40.4 (21~60) year-old, respectively. Mean follow up period is 81 months (1~170). The reasons of exchanging donor were ABO incompatibility (92/121, 76%), positive HLA cross matching (20/121, 16%) and patients who received kidneys from unknown volunteer donors (9/121, 7%). We compared graft survival and acute rejection rate of exchange donor group and living related donor group. RESULTS: The graft survival rate (1, 5, 10 year) of exchange donor group (92%, 80.6%, 72%) was similar to living related donor groups (95%, 77%, 66%). The acute rejection rate were 34% (42/121 ) and 24%, respectively. We have observed acute rejections during the same time in both groups from 1991 to 2005. Although acute rejection rate of exchange donor group is higher, we see the similar survival rate of transplanted kidneys in exchange donor group. CONCLUSION: We suggest that exchange donor program could expand the donor pool in living donor renal transplantation, when recipient cannot receive kidney from healthy donor because of ABO incompatibility or positive HLA cross matching.

4.An Epidemiologic Study on an Outbreak of Dermatosis Associated with Moths at a Factory.

An outbreak of dermatosis occurred at a machinery manufacture factory in kyongju on Aug 1995. Authors conducted a questionnaire survey among 92 workers in the factory and a dermatologist examined their skin lesions. Authors also collected moths and identified them. The results are as follows; 1. Forty-two cases of dermatosis were identified with attack rate of 45.7%. Attack rate was not different by company, age, sex, educational level and duration of employment. Attack rate was 47.8% among productive workers, 25.0% among clerical workers and 100.0% among patrolmen. 2. Two cases among 42 dermatosis cases and three subjects among 50 non-cases had a history of same dermatosis last year. Four of dermatosis cases had a history of dermatosis among their family members. 3. skin lesions of the cases were scattered or grouped rice-sized erythematous papules or vesicles. Duration of dermatosis from the onset to the time of survey was from one day to more than 30 days. The most frequent site of skin lesion was the arms(81.0%), and it was also observed at the neck(47.6%), abdomen(45.2%), chest(42.9%), legs(38.1%) and back(35.7%). skin lesion was aggravated with sweating(31.0%), after shower(16.7%) and with scratching(l1.9%). 4. Moths were collected and identified as Euproetis subflava(Bremer). Many poisons stings were observed on their wngs which is harmful to human. 5. Contact with moths' wing droppings to two authors on their forearms for provocation resulted in severe immediate pain followed by prolonged pruritus. Erythematous macules quickly appeared and suoceeded by firm wheal-like red papules. Authors thought that the outbreak of dermatosis was brought about by Euproetix subflava(Bremer). Further studies on the factors contributing to the prosperity of the moths in this area are needed.

5.The Outcome and Risk Factors of Renal Retransplantation.

PURPOSE: The aims of this study are to compare the results of renal retransplantation and the first renal transplantation in our center, and to analyze the risk factor affecting second graft survival rate. METHODS: We analyzed 37 cases of second or third renal transplantations performed in our center from 1981 to 2005, retrospectively. We compared the second graft survival rate with the first graft survival rate. And risk factors affecting long-term second graft survival were also analyzed. RESULTS: 37 cases of retransplantation out of 277 cases of first graft loss were performed in our center (5%). Second graft survival rates (1, 5, 10 year) were 83.7, 66.5, 49.9%, while first graft survival rates were 90.0, 74.8, 59.1%, repectively. Risk factors which shows significance were high HLA-AB matching, living donor and absence of DGF. CONCLUSION: The graft survival rates of second renal transplantation were similar to that of first renal transplantantation.

6.The analysis of the present status of the medical internet sites in Korea.

BACKGROUND: After the World-Wide-Web(www) appeared, history has abruptly fallen in the new era of the Internet. In Korea, after the first medical internet site appeared, there was a major development in the WWW. But the quality and quantity of the web-page remains in infancy. Therefore, through the analysis of the Korean medical internet sites on present status, directions for development of Korean medical internet sites can be made. METHODS: From April 1st, 1999 to June 30th, 1999 in the Korean web sites, we visited 1044 sites. They included "Medmark Korea" representing medical search engine, "Eulji medical library" in the Eulji Medical College Hospital, and the medical sites of the "Yahoo Korea", "naver.com", and "simmani.com". Through this internet search, the information about locations, first publication year, classification by characteristics of homepage, speciality, web service form was obtained. Among these sites, acquireing from insufficient source further, questions were sent to the webmasters of 143 websites by e-mail. The questions included 7 items on the characteristic of webmasters, 8 items on the general characteristics of internet sites, 8 items on the contents, and their own comments. RESULTS: The occupation of webmasters was as follows: were physicians 94.4%, were male. According to age distribution, those in their thirties was 48.2%, and forties, 33.3%. Amone the 521 websites confirmed, there were 44 sites in 199 151 in 1996, 237 in 1997, and 79 in 1999. According to the distribution of location, metropolitan areas including Seoul, Inchon, Kyonggi occupied 68.9% among total. The main portion of the intentional visitors was ordinary people comprising 39.2%, members 9.8%, and physicians 7.7%. Concerning the purposes of publication, 38.7% of websites intended for health education to citizens, 31.0% for publicity of hospital or organizations, and 15.5% for exchange of information between members. For the most significant contents, 26.6% focused on the common-sense health issues, 35.7% on exchange of special medical information, 8.4% on education data for medical students, and 13.3% on publicity of hospitals. For the update interval, 22.0% renewed their sites within 1 week, 22.7% in 1 month, 23.4% in 3 months, and 20.6% in 6 months. For the effectiveness of homepage, the rate of `good' and `very good' response was 52.8%. CONCLUSION: Our investigation, showed that medical internet sites are lead by those in their thirties and forties. Web sites was concentrated to metropolitan areas where informational infrastructure was well developed. It is emphasized that efforts to informationalization by are needed by, governmental medical database systems should be established, and that the guidelines which enables validity of medical information should be set forth.

7.Effect of Rhodiola Sachalinensis Administration and Endurance Exercise on Insulin Sensitivity and Expression of Proteins Related with Glucose Transport in Skeletal Muscle of Obese Zucker Rat.

Peripheral insulin resistance in obese/type II diabetes animals results from an impairment of insulin-stimulated glucose uptake into skeletal muscle. Insulin stimulate the translocation of GLUT4 from intracellular location to the plasma membrane. Soluble N-ethylmaleimide-sensitive factor attachment protein receptors (SNAREs) is implicated in mediation of fusion of GLUT4-containing vesicle with the plasma membrane. Present study investigated regulatory effects of Rhodiola sachalinensis administration and exercise training on the expression of GLUT4 protein and SNAREs protein in skeletal muscles of obese Zucker rats. Experimental animals were randomly assigned into one of five groups ; lean control (LN), obese control (OB), exercise-treated (EXE), Rhodiola sachalinensis-treated (Rho), combine of Rho & EXE (Rho-EXE). All animals of exercise training (EXE, Rho-EXE) performed treadmill running for 8 weeks, and animals of Rho groups (Rho, Rho-EXE) were dosed daily by gastric gavage during the same period. After experiment, blood were taken for analyses of glucose, insulin, and lipids levels. Mitochondrial oxidative enzyme (citrate synthase, CS ; beta-hydroxyacyl-CoA dehydrogenase, beta-HAD) activity were analysed. Skeletal muscles were dissected out for analyses of proteins (GLUT4, VAMP2, syntaxin4, SNAP23). Results are as follows. Exercise and/or Rhodiola sachalinensis administration significantly reduced body weight and improved blood lipids (TG, FFA), and increased insulin sensitivity. Endurance exercise significantly increased the activity of mitochondrial enzymes and the expression of GLUT4 protein, however, administration of Rhodiola sachalinensis did not affect them. The effect of exercise and/or Rhodiola sachalinensis administration on the expression of SNARE proteins was unclear. Our study suggested that improvement insulin sensitivity by exercise and/or Rhodiola sachalinensis administration in obese Zucker rats is independent of expression of SNARE proteins.

8.Plasmid Profile and B - Lactamase Type of Multidrug - Resistant Salmonella typhi Isolated from Korea, 1997.

Eight strains of multidrug-resistant (MDR) Salmonella typhi were isolated from Kyonggi area during January-February,1997. They were resistant to ampiciUin, amoxicillin, carbeniciillin, tetracycline, chloramphenicol, trimethoprim/sulfamethoxazole, trimethoprim. Eight strains had one plasmid respectively which size was approximately M.W 220 kb and showed same restriction pattern by endonuclease HindIII. The plasmid was similar to the plasmid in size that was related to multidrug resistant S. typhi isolated from southeast Asia. It were transferred by conjugation to recipient E, coli K-12 in frequency of 2.43 x10-4 - 1.73 x 10-2 and transconjugant showed same drug-resistant pattem with donor cells. All of 8 strains produced B-lactamase that was assummed to TEM-1 type by isoelectric focusing and PCR.

9.Effect of Atrial Natriuretic Peptide and Sodium Chloride on the Pressor Response to Endothelin in Rats.

BACKGROUND: To explore an interaction(s) of endothelin with other blood pressure regulating systems, effects of either a high concentration of sodium chloride vehicle or pretreatment with atrial natriuretic peptide (ANP) on the pressor response to endothelin were investigated in rats. METHOD: Sprague-Dawley rats were anesthetized with thiopental (50mg/kg, IP) and were cannulated into left lateral cerebral ventricle. The left femoral artery and vein were also catheterized to measure blood pressure and to serve as an infusion route, respectively. Endothelin (10pmol) dissolved in either normal (0.15M) or high concentration (3.0M) of NaCl vehicle with ANP (1 nmol, ICV). For intravenous (IV) infusion, endothelin(1pmol/min) dissolved either in normal or high concentration of NaCl vehicle was infused for 20min. RESULT: ICV endothelin caused an increase of arterial pressure, in which the NaCl did not affect but ANP-pretreatment significantly attenuated the pressor magnitude. IV endothelin also increased the arterial pressure, which was potentiated by NaCl, ie, the maximum increase caused by the IV endothelin in the high NaCl vehicle was greater than the sum of that induced by endothelin (infused in normal NaCl vehicle) and that induced by high NaCl vehicle only. CONCLUSION: These results suggest that endothelin has separate central and peripheral pressor actions, in which the former is not affected by high NaCl but is attenuated by the ANP system and the latter is potentiated by high NaCl.

10.A Cytogenetic Study of Recurrent Spontaneous Abortion.

OBJECTIVE: The purpose of this investigation is to determine the frequency of chromosomal or genetic causes of recurrent spontaneous abortion. METHODS : A cytogenetic study was made in of 921 couples for 13 years from January 1984 to December 1997 in which the woman was ascertained to have had two or more spontaneous abortions at our Cytogenetic Laboratory, Institute of Reproductive Medicine and Population, Seoul National University. RESULTS : The overall incidence of chromosome anomaly was 80 out of 921 (8.7%). There were 34 cases (3.69%) of reciprocal balanced translocation and 13 cases (1.41%) of Robertsonian translocation. Also 17 cases (1.85%) of inversion and 5 cases (0.54%) of X chromosome mosiacism was observed. In the case of reciprocal balanced translocation, chromosome 8,6,7,13 were preferentially involved over others. And in the case of Robertsonian translocation, chromosome 13 was preferentially involved. CONCLUSION: Our study demonstrates that cytogenetic analysis is indicated in couples with 2 or more spontaneous abortion and about half of these disorders are reciprocal balanced or Robertsonian translocations.
