6.Comparison of Gene Expression Profile in Eutopic Endometria with or without Endometriosis: A Microarray Study.

OBJECTIVE: Pathogenesis of the endometriosis is very complex and the etiology is still unclear. Our hypothesis is that there may be some difference in gene expression patterns between eutopic endometriums with or without endometriosis. In this study, we analyzed the difference of gene expression profile with cDNA microarray. METHODS: Endometrial tissues were gathered from patients with endometriosis or other benign gynecologic diseases. cDNA microarray technique was applied to screen the different gene expression profiles from early and late secretory phase endometria of those two groups. Each three mRNA samples isolated from early and late secretory phase of endometrial tissues of control were pooled and used as master controls and labeled with Cy3-dUTP. Then the differences of gene expression pattern were screened by comparing eutopic endometria with endometriosis, which were labeled with Cy5-dUTP. Fluorescent labeled probes were hybridized on a microarray of 4,800 human genes. RESULTS: Twelve genes were consistently overexpressed in the endometrium of endometriosis such as ATP synthase H transporting F1 (ATP5B), eukaryotic translation elongation factor 1, isocitrate dehydrogenase 1 (NADP+), mitochondrial ribosomal protein L3, ATP synthase H+ transporting (ATP5C1) and TNF alpha factor. Eleven genes were consistently down-regulated in the endometriosis samples. Many extracellular matrix protein genes (decorin, lumican, EGF-containing fibulin-like extracellular matrix protein 1, fibulin 5, and matrix Gla protein) and protease/protease inhibitors (serine proteinase inhibitor, matrix metalloproteinase 2, tissue inhibitor of metalloproteinase 1), and insulin like growth factor II associated protein were included. Expression patterns of selected eight genes from the cDNA microarray were confirmed by quantitative RT-PCR or real time RT-PCR. CONCLUSION: The result of this analysis supports the hypothesis that the endometrium from patients with endometriosis has distinct gene expression profile from control endometrium without endometriosis.

7.Survey on Sedation Training for Pediatric Residents in Training Hospitals

The purpose of this study was to investigate current status of sedation training for the residents in pediatric dentistry training institutions and opinions about continuing education after the residency program.
Surveys were sent to 18 pediatric dentistry training institutions by e-mail, and the responses were collected and analyzed. Most of the sedation education period for the residents were the 1st-year education (61.1%) and 1 - 3 years of integrated education (55.6%). In terms of an externship, 5 institutions (27.8%) sent their residents to the department of anesthesiology. Second half of the 1st year (50%) was the highest for a resident to use sedation for the first time. The period of supervisor participation varied from not participating at all to whole time throughout the residency program. The sedation training is conducted at all training institutions, but there were variations in the experience that a resident can gain.
All training institutions agreed on the necessity of continuing education of the sedation, but there were various opinions regarding time, method, and the period of review course. Overall, this study suggested that continuing education should be consisted of 1 - 2 hours of didactic education every year and clinical skills and simulation training in every 2 - 3 years.
8.The current status and implications of the medical insurance fee schedule in Japan

Korea has the most rapidly aging population in the world. Medical costs for the elderly are quickly increasing, which raises concerns about the sustainability of health insurance finances. Accordingly, the need to allocate limited medical resources efficiently has increased, with improving the fee schedule seen as an effective way to achieve this. Japan has experienced a super-aging society for at least 20 years prior to Korea and has been successful in improving its fee schedule. Korea’s fee schedule, however, needs to overcome the challenge of being relatively limited in type and simplicity.Current Concepts: The new patient consultation fee in Japan is the same regardless of the type of medical institution and is approximately 40% higher than found in Korea. In the case of established patient consultations, the fee for medical institutions with fewer than 200 beds are substantially higher than for large medical institutions, thereby suppressing re-examination at large hospitals. Japan’s additional points consist of 71 types of items, which is more diverse than Korea’s 28 types, and the additional points for children under six years of age, holidays, and late nights are set significantly higher than that found in Korea.Discussion and Conclusion: Japan designed its fee schedule to structurally reduce the consumption of medical resources and to effectively compensate for the behavior of medical providers. This policy is a standard which Korea’s fee schedule should aim for. In the future, it will be necessary to prepare measures to respond to the super-aging environment through detailed research concerning Japan’s fee schedule.
9.Effects of Nitric Oxide Synthase Inhibitor on Urinary Bladder and Proximal Urethral Function: Comparison between Mature Male and Female Rat.

PURPOSE: The present study was undertaken to compare the function of the urinary bladder, reflex changes of the urinary bladder, and bladder outlet, during micturition between mature male and female rats. MATERIALS AND METHODS: Mature male(n=6) and female(n=6) Sprague-Dawley rats weighing 250 to 300gm were used. After each rat was anesthetized with urethane(1g/kga simultaneous recording of the intravesical pressure and voided urinary volume during micturition were recorded. And then, the urethra was ligated placing 4-0 silk tie just distal to the bladder neck. Isovolumetric bladder contractions and urethral pressure were recorded independently. RESULTS: The voided volume and intravesical pressure during micturltion showed no significant differences between mature fema1e and male rats. During reflex bladder contraction, isovolumetric bladder contractions and urethral pressure also showed no significant differences between mature female and male rats. Administration of N-nitro-L-arginine(L-NOARG, 15mg/kg), a nitric oxide synthase inhibitor, intravenously in female rats, reversibly decreased the magnitude(42.5%, p<0.05) and duration(35.4%, p<0.05) of reflex urethral relaxation. There was no effect on reflex urethral relaxation in male rats. Administration of L-NOARG 20mg/kgintravenously in male rats reversibly decreased the magnitude(49.8%, p<0.05) and duration(38.7%, p<0.05) of reflex urethral relaxation. CONCLUSIONS: From the results, it appears that the function of the urinary bladder and the reflex change of the urinary bladder and bladder outlet during micturition showed no significant difference between male and female rats of similar weight. Howerever, the dosage of L-NOARG showed difference of the decreasing level of the magnitude and duration of the urethral relaxation.

10.Polycythemia as a Complication of Long-acting Injectable Testosterone Undecanoate.

Polycythemia is a condition in which the red blood cell count is increased due to an inherited or acquired mutation, a physiologic response to hypoxia, autonomous erythropoietin production, or deliberate erythropoietin administration. Higher testosterone levels appear to act as a stimulus for erythropoiesis and testosterone replacement therapies have rarely been reported as causes of polycythemia. We report here a case of a 51-year-old man with polycythemia that was caused by long-acting testosterone undecanoate (Nebido(R)).
