Invasion of enteric bacteria, such as Salmonella and invasive E. coli, into intestinal epithelial cells induces proinflammatory gene responses and finally epithelial cell apoptosis. In this study, we asked whether invasive bacterial infection of human intestinal epithelial cells could upregulate cyclooxygenase-2 (COX-2) gene expression and whether increased COX-2 expression could influence intestinal epithelial cell apoptosis. Expression of COX-2 mRNA and prostaglandin (PG) E production were upregulated in HT-29 colon epithelial cells which were infected with S. dublin or invasive E. coli, as examined by quantitative RT-PCR and radioimmunoassay. Inhibition of COX-2 expression and PGE2 production using NS-398, a specific COX-2 inhibitor, showed a significant increase af epithelial cell apoptosis and caspase-3 activation in HT-29 cells infected with invasive bacteria. However, the addition of valerylsalicylate, a specific COX-1 inhibitor, did not change apoptosis in S. dublin-infected HT-29 cells. These results suggest that upregulated COX-2 expression and PGE2 production in response to invasive bacterial infection could contribute to host defense by inhibiting apoptosis of intestinal epithelial cells.

BACKGROUND: Doppler echocardiography is a non-invasive technique that has been used to evaluate LV diastolic dysfunction. Impaired left ventricular diastolic filling is known to occur in patients with coronary artery disease. Compared with those in normal subjects, Doppler-derived transmitral blood flow velocities have been reported to be reduced during early diastolic filling and to be compensatory elevated subsequent to atrial systole in patinets with coronary artery disease. But stiffness of myocardium normalize the E/A ratio, and normal E/A ratio may reveal increased ventricular filling pressure. We tried to investigate left ventricular filling parameters by Doppler echocardiography in patients with early myocardial infarction, and to compare left ventricular diastolic function regarding infarct location on EKG, one or multivessel disease on coronary angiography, and treatment modality. METHODS: From September 1993 to August 1995, Pulsed wave Doppler echocardiography was performed in patients with early acute myocardial infarction(N=95) and control group(N=20) within 5 days after admission, and parameters of diastolic function was evaluated. RESULTS: Echocardiographic data showed significant differences in mean ejection fraction, mean left ventricular mass, and mean left ventricular mass index between two groups. There was no significant difference in E/A ratio, deceleration time, and isovolumetric relaxation time between two groups. Neither, there was significant difference in each diastolic parameter for infarct related wall on EKG. And there was no significant difference in deceleration time for one or multi vessel disease on coronary angiography, treatment modality(conservative treatment, thrombolytic therapy, or primary PTCA). CONCLUSION: In patients with early acute myocardial infarction, left ventricular diastolic dysfunction was absent. And there was no significant correlation between the presence of diastolic dysfunction and the location of infarct related wall on EKG, or one or multi vessel disease, or treatment modality.

PURPOSE: This study was aimed at providing basic data for developing a nursing intervention program which enables systematic and correct visual acuity care by discovering out visual acuity conditions, prevalence rate of myopia, and the factors related to myopia with Preschool children. METHOD: The subjects of this study consisted of 519 children between 3 and 6 years of age from 12 kindergartens in Seoul which were selected through multiple cluster sampling. Myopia was defined as the spherical equivalent (SE) of more than -0.5 diopters (D) inthe right eye. The data was analyzed by t-test, X2-test, ANOVA, and logistic regression by using the SAS program. RESULT: The prevalence rate of myopia was 8.7%. the odds ratio of child myopia when one parent had myopia was 2.2 times higher than when neither parent had myopia. The odds ratio of child myopia when reading more than three books per week was 0.27 times higher than reading less than three books. CONCLUSION: Myopia should be continuously and intensively managed from the age of 3.

Social support was found to have buffering effects on the stress response of stroke patients. Especially, the aspects of social support found to be most influential were support from a close, personal source, and overall satisfaction with support. The purpose of this study was to identify the current social network and its characteristics according to gender difference in post-hospitalized stroke patients in Korea. The sample consisted of a convenience sample of 254 patients was recruited 129 men and 125 women who were receiving follow-up care at outpatients clinics. Four aspects of social support-source, quantity, quality & type-were measured using the modified Social Support Inventory for Stroke Survivors(SSISS) which was developed by McColl & Friedland(1989). Regarding sources of social support, 61.4% reported for 'spouse' as primary caregiver and 31.9%, 'children'. But the distribution of sources of personal support were related to gender; 82.2% of male patients had support from their spouses, while only 40% of female patients reported from 'spouse' but 51.4% from 'children'. Among the children, daughters and sons were more significant support persons than daughters-in-law. The percentages for sources of these significant other support persons were 55.5% for 'children', 8.7% for 'spouse', and 8.3% for 'brothers'. The physician at the outpatient department was the main source of professional support. For the quantity and quality of social support, the primary caregiver's support was more significant than support by significant other persons. Male patients reported that primary the caregiver' support was greater than that of significant other persons, while female patients perceived significant other persons as giving greater support. Regarding the type of perceived social support, the stroke patients were highly satisfied with the primary caregiver's support in aspects of instrumental, emotional, and informational support. They also reported high satisfaction with support from significant others' support in the aspect of emotional support, while emotional and informational support from professionals was reported as satisfactory. In conclusion, gender difference in the social support network was found in that male patients perceived more support from their spouses, while female patients perceived more support from their children as compared to their spouses.

We observed the patients admitted to the department of pediatrics of Seoul Red Cross Hospital during the past twenty years form Jan. 1971 to Dec. 1990. The results were as follows: 1) The total number of pediatric inpatients during 20 years was 15,800, of which 9,683 were male, 6,117 were female and male to female ratio was 1.58:1. 2) On age distribution, neonates and infants less than 1 year of age were 6,224 cases (39.4%). 3) Monthly distribution of the inpatients showed the highest incidence in October. 4) Infectious and parasitic diseases were 4,759 cases (30.1%) and respiratory tract diseases, 3,556 cases (22.5%) and disease of these two group occupied more than one half of total admission. 5) Major 10 leading causes of hospitalization were diarrheal disease (14.2%), convulsion (7.0%), acute bronchiolitis (6.8%), pneumonia, tuberculosis, URI, neonatal hyperbilirubinemia, prematurity and low birth weight infant, acute glomerulonephritis and laryngitis in the order of frequency. 6) There was marked decrease in the number of the admitted patients with typhoid fever, bacillary dysentery, tuberculous meningitis, hepatitis, pyogenic meningitis, rheumatic fever, and acute glomerulonephritis. But the patients with URI, bronchial asthma, neonatal hyperbilirubinemia and Kawasaki disease have increased.

Lichen striatus is an uncommon nonspecific inflammatory cutaneous disease of unknown etiology occuring as unilateral and linear distribution on extremities and healing spontaneously within a few months to a year. Clinically it is composed of small licbenoid papules which tend to be grouped and coalesced to form either a continuous or an interrupted hypopigmented band and is not usually accompanied by any subjective symptoms. The principal histologic features are an infiltrate composed of lymphocytes and histiocytes around papillary vessels and usually also around, some of the deeper vessels and skin appendages and consequent secondary changes of the epidermis. We present the results of clinical and histopathologic observation in 19 patients with lichen striatus observed at Department of Dermatology, National Medical Center and Seoul National University Hospital during 4 years period from l972 to 1976 which would be the first recorded report on this rare disease in Korea. 1. Clinieal Observation.' Among the 19 patients, 10 were female and 9 were male; 15 were under 10 years of age and 4 were over 10 years of age. Duration of the eruption varied from 1 week to 4 years and average duration was 9.6 months at the time of the first visit. It was unilateral in all cases and affected the upper extremities in 9 and the lower extremities in 8 instances. The eruption extended onto the upper part of the trunk in 2 cases. Only 3 patients had a history of mild intermittent itching sensation. Characteristically, the eruption consisted of grouped, linear, slightly scaling flat polygonal papules with varying degrees of hypopigmentation, extending at least onethird of the length of an extremity. . Histopathologic observation: Skin biopsy was performed in 13 cases. The epiderrnal changes consist of mild diffuse hyperkeratosis (13 cases), focal parakeratosis (5 cases), varying degree of atrophy of malpighian layer (7 cases), mild acanthosis (2 cases), intercellular and intracellular edema (8 cases), exocytosih of lymphocytes (7 cases), focal basal cell disruption (11 cases), and dyskeratotic cells (colloid body) (2 cases). Dermal changes include mild to moderate perivaacular lymphocytic and histiocytic infiltrate rnostly on the level of papillary dermis. Also, there was chronic infIammatory cell infiltrate around hair follicle and sweat glands and ducts in 7 cases respectively, which seems to be peculiar in this disease. Overall histologic findings in our cases are generally consistent with those of the primary phase of the so-called lichenoid tissue reaction of Pinkus, The possible pathogenetic mechanism of the linear eruption and immunobiologic interpretation of the lichenoid tissue reaction were literally reviewed.

Fibromatosis can occur at various sites, but intracranial fibromatosis is exceptionally rare. Here, we report a case of intracranial fibromatosis arising in the suprasellar area of a 52-year-old woman who had undergone a surgery at that site. A computed tomography scan revealed a heavily calcified, highly enhancing, poorly demarcated mass in the left sellar area that extended into the left suprasellar, parasellar areas, and orbital apex and completely encased the left distal inferior cerebral artery. Histologic and immunohistochemical features were compatible with those of fibromatosis, although the cellularity was focally higher than usual. The etiology of extra-abdominal fibromatosis is unknown, but physical injuries such as trauma and irradiation have been reported to be associated with its occurrence. Although fibromatosis is rare in the intracranial area, it should be considered as a differential diagnosis when an intracranial mass occurs at a previously injured site.
