1.A Clinical Observation on 55 Cases of Neonatal Sepsis.

Neonatal Sepsis is a frequent and important cause of morbidity and mortality in the neonatal period. This study was undertaken to observe the 55 cases of proven neonatal sepsis among 6,717 newborn infants under 4 weeks of age, admitted to the nursery of Yeungnam University Hospital from May 1, 1983 to April 30, 1988. We observed following results: 1. The morbidity rate of male (1.12%) was higher than that of female (0.44%) significantly (p<0.05). 2. The morbidity rate of prematurity (2.65%) was higher than that of full term (0.76%) significantly (p<0.05). 3. The incidence of perinatal obstetric complications in early onset neonatal sepsis was higher than that of late onset neonatal sepsis (38.5% vs 10.3%, p<0.05). 4. The common clinical manifestations were poor feeding (52.7%), jaundice (45.5%), diarrhea (30.9%) and irritability (30.9%). 5. Among the causative organism, gram positive organisms were predominated and Staphylococcus epidermidis was the most common organism. There was no difference in the causative organisms between early onset and late onset sepsis.

3.In Situ Aortic Reconstruction with Femoral Vein after Post-EVAR Infection.

A 78-year-old male presented with early gastric cancer and a 5.5 cm-sized infrarenal abdominal aortic aneurysm, detected during regular screening. Endovascular aneurysm repair (EVAR) was performed first, followed by laparoscopic distal gastrectomy. After gastrectomy, the patient underwent computed tomography (CT) scan due to persistent fever, which showed increased perigraft fluid collection around the right iliac limb graft. Echocardiography also revealed mitral valve vegetation, consistent with infective endocarditis. Despite intensive antibiotic treatment, the patient had persistent fever and showed fluid extension to the psoas muscle on CT scan. On the 49th post operative day (POD) after EVAR, stent graft explantation and aortic reconstruction with the left superficial femoral vein was performed. Ligation of the right iliac artery for infection control and simultaneous femoro-femoral bypass was also performed. The patient was discharged on the 46th POD after graft removal without any events.

4.Clear Cell Meningioma arising from Lumbar Nerve Root in a Child: A case report.

Meningioma of unusual age of onset, location, histogenesis and histologic type is reported. The patient, 4 year-old girl, had an intradural spinal meningioma arising from lumbar nerve root with no dural attachement. The meningioma revealed glycogen-rich, clear cell type with extensive and blocky hyalinization of the stroma. The tumor shared common fibrous sheath with attached lumbar nerve, and nerve fibers were scattered within the tumor. Ultrastructurally, the tumor cells had abundant glycogen particles, intermediate filaments and intercellular desmosomes. Hyalinized material revealed large amianthoid collagen fibers.

5.Hypofractionated Radiation Therapy for Early Glottic Cancer: Preliminary Results.

PURPOSE: This study was performed for the evaluation of the feasibility and toxicity of hypofractionated radiation therapy for early glottic cancer METHODS AND MATERIALS: From February 1999 to February 2000, 20 patients with histologically confirmed Stage I, II glottic cancer were enrolled into this study. There were 18 males and 2 females, the median age of the patients was 59 years. The distribution of stage distribution was as follows; T1aN0-16 patients, T1bN0-1 patient, T2N0-3 patients. Eighteen patients underwent laryngomicroscopic biopsy only, and two patients underwent laser cordectomy. All patients received radical radiation therapy (2.5 Gy per fraction, 24 fractions, total 60 Gy). Median duration of treatment was 36 days (range 31-45 days). RESULTS: Radiation therapy were well tolerated. Most common acute reactions were odynophagia and hoarseness, and these reactions resolved after radiation therapy. There were one case of RTOG grade 3 odynophagia (5%), six cases of grade 3 hoarseness (30%). Response of radiation therapy was evaluated one month after completion of treatment. All patients revealed complete response. During follow up, total three cases of treatment failure were detected. two cases were local recurrence in 10 and 13 months of radiation therapy and one case was local recurrence and distant metastasis in 2 months of radiation therapy CONCLUSION: This hypofractionated radiation therapy schedule was feasible and effective for control of early glottic cancer. But longer follow up time would be required to assess the long-term disease control and the late complication by shortening radiation therapy duration.

6.Concentrations of Major Minerals and Trace Elements in Sera of The Breast-fed and Formula-fed Infants.

The serum concentrations of the major minerals(calcium, phosphorus, magnesium, sodium, potassium) and trace elements(iron, Zinc, copper manganese)were datermined in 23 breast fed infants living in Eumsung, Choong-buk region. The results obtained are summarized as follows: 1)The mean levels of calcium, phosphorus, magnesium, sodium and potassium of the serum of total subjects were 8.15+/-0.33mg/dI, 11.06+/-0.16mg/dI, 2.00+/-0.14mg/dI, 3.4476+/-17.99mg/dI and 9.06+/-2.04mg/dI respectively. 2)The serum concentrations of iron, zinc, copper and manganese in total subjects averaged 95.83+/-0.33mg/dI, 93.79+/-7.06 microgram/dI and 98.57+/-7.06 microgram/dI and 4.93+/-0.62 microgram/dI respecitively. 3)Breast fed infants had significantly higher serum calcim, magnesium, sodium and iron concentrations than the formula fed groups. Otherwise, serum potassium, copper and manganese levels in breast milk fed infants were significantly lower than those in formula fed infants. 4) In formula fed infants, serum potassium and copper levels increased increased significantly with months after birth.

8.Ecological Studies of Maternal-Infant Nutrition and Feeding in Urban Low Income Areas - II. Anthropometric Measurements, Dietary Intakes, Breast Milk Components and Serum Lipid/Fatty Acid Composition of Lactating Women.

This study was aimed at evaluating the nutritional status of Korean lactating women, who were attending peripheral community clinics in low-income areas, by anthropometric measurement, estimating dietary intakes and analyzing the contents of breast milk components, serum lipid content and fatty acid composition. Regarding anghropometric measurements, maternal circumferences decreased significantly at the trunk but not at the limbs. The skinfold thickness on all regions had the tendency to decrease during lactation. It was found that intakes of energy, protein, lipid and carbohydrates were desirable while calcium, zinc, copper and vitamin A were low and sodium was excessive. The energy % of fat was 22.41% of the total energy which is higher than the RDA for adults but it is proper for lactating women, but energy % from each fatty acid was usually low and an intake ratio of omega6/omega3 fatty acids was desirable. Accordingly, it is thought that dietary fat intake was balanced in quality. Concentrations of the nutrients in matured human milk showed the same range as that reported form different regions with the exception of calcium and magnesium which were relatively low. Serum lipid contents were decreased with the time postpartum and the average serum lipid content of lactating women were similar to the normal range of adults. Serum polyunsaturated fatty acids. especially omega6 fatty acids have gradually increased with the time postpartum, but the composition were not greatly influenced by dietary intakes.

9.Body image perception and self-reported weight control activities in adolescent girls.

BACKGROUND: Adolescent girls are interested in weight reduction, but effective and adequate education for weight reduction is not done in Korea. And so much information is accepted via mass media. We investigated body image perception, weight control activities and their side effects among adolescent girls. METHODS: We used questionnaire about body image and weight control activities of middle and high school girls in Seoul in Sep. 1995. RESULTS: 88.6% of girls were not satisfied with their own height and 85.1% of girls with their own weight. 66.3% underweight students(BMI<20kg/m2) thought they were too fat and 98% acceptable weight students(20kg/m2<_KBMI<25kg/m2) thought they were too fat. So most girls had body image disturbance. 42.4% of girls were trying to lose weight and 85.1% were trying to lose weight reason for attractiveness and 16.6% were trying to lose for health. Self reported weight reduction methods were exercise(31.6%), fasting(21.9%), diet(21.5%), monodiet(10.8 %), laxatives(2.3%), very low calorie diet(1.8%) etc. Students who tr ied to lose weight for attractiveness selected more undesirable methods than who tried to lose weight for health(P<0.01). Most common route is via magazine, TV and newspaper. 43.4% of girls who tried to lose weight experienced side effects after weight reduction, those were dizziness(71.3%), general weakness(51%), apathy(26.S%) etc. CONCLUSIONS: Most girls had body image distortions. Useless weight reduction activities were done or planned and most of information was accepted via mass media. It is important to educate the girls to get adequate body image and effective weight reduction program is needed for health in adolescent girls.

10.Treatment of Gap Nonunion of the Tibia by the Ilizarov Method

Twenty four patients with gap nonunions of the tibia were treated with the Ilizarov intercalary bone transport method. The purpose of this retrospective study is to evaluate the results and complications in gap nonunions of the tibia treated using the Ilizarov method. There were 22 males and two females with an average age of the 35.8 years(range, 23-63 years). The mean follow up period was 30.2 months(range, 17-70 months). Ten of the 24 patients had no external shortening with bone gap, 13 external shortening with gap and one hemicircumferential defect. On average, the size of bone loss measured 8.2 cm(range, 3-18 cm). Twenty-one of 24 patients were treated by whole segment internal transport(fifteen were proximal fragment transport, five were distal fragment transport, one was proximal and distal fragment transport), one patient by anterior hemicircumferential corticotomy and partial bone fragment internal transport and tow patients by fibula transfer. All patients healed with solid bony union, although twelve patients required bone grafts, eleven at the docking site and one at the distraction site. On average, the healing index was 49 days/cm(range, 18-82 days/cm). There was on relationship between the percentage transport(size of the distraction gap/the transporting fragment length x 100) and healing index. Numerous complications were encountered, most commonly delayed union and pin site infection, Conclusively, the application of Ilizarov techniques to gap nonunions of the tibia was very effective, but required correct technique and careful follow-up examination.
