1.Construction of evaluation index system for postgraduate tutors in medical universities

Objective To build a scientific,rational and practical evaluation index system for medical tutors.Methods Evaluation framework was initially constructed on the basis of literature analysis.Two rounds questionnaires investigation were conducted soon using Delphi method and the data were analyzed using descriptive analysis,independent samples of non-parametric test (Kreskas-Wallis test),expert reliability analysis,analytic hierarchy process and other methods.Results The results of two rounds of expert questionnaires showed that both recovery rate and acceptance rate of the questionnaires were more than 90% and overall expert coefficient was 0.8957.The coordinate coefficient of the second-class index was 0.181,x2 =27.148,P < 0.001,and that of the third-class index was 0.157,x2 =127.03,P < 0.001.Experts' opinions were highly consistent.Conclusions Evaluation index system including three first-class indexes,seven second-class indexes and twenty-eight third-class indexes was established for medical tutors.The evaluation index system is of great importance in safeguarding postgraduate culturing quality and strengthening teaching staff construction.
2.The clinical study on ankle joint fracture treated by open reduction and internal fixation

Objective To explore the surgical treatment of ankle fracture.Methods 326 cases with ankle joint fracture in our hospital treated by surgical treatment were followed up.There were 182 males and 43 females,aged from 7 ~ 75 years.The patients with medial malleolus fracture were cut through postero-medial process,and the lagged screw,absorbed screw,or tension band wire were used.Patients with bimalleolus and trimalleolus fracture were cut through medial and lateral two incision.When the fracture line of the fibula is high,the triangular needle,kirschner wire,plate and the screw,remember alloy were used,when the fracture line of the fibula was high,1 ~ 2 cortical or cancellous bone lagged screw were used.The patients with diastasis of inferior tibiofibular syndesmosis were cut through lateral process,1 lagged screw was used through 3 layer cortical bone.After the operation,the plaster or brace was used according to the stability of the fracture,then systemic functional exercises.Results All the patients were followed up from 4 ~ 120 months,according to the ankle-hind foot score of American Orthopedic Foot and Ankle Society Score(AOFAS),their clinical treatment results were assessed.In 225 cases,excellent and good rate was 86.7%.Connclusion Proper approach,anatomy reduction,articular surface recovery to smooth,stable internal fixation,necessary external fixation and systemic rehabilitation were the key aspects to the surgical treatment.Good result of most of the patients with fracture of the ankle joint can be obtained.
3.Studies on the exponentially declining infusion and the program infusion of theophyl ine in rabbits

Constant infusion, exponentially declining infusion and program infusion of theophlline were administered to rabbits byadopting the rabbit population pharmacokinetic parameters in this laboratory, and the plasma concentrations of the drug was measured by ul- traviolet spectrophotometry. The results showed that the plasma drug concentrations following exponentially declining infusion and program infusion attained the desired steady-state level at only 30 min, though the T1/2?= 6. 08h, The median absolute value of performance error in pro-gram infusion and exponentially declining infusion was 7. 8% and 14% respectively.
4.Treatment principle for the patients with maxillofacial injuries occurred in earthquake disaster

Objective To investigate and analyze the state of injury and treatment for the patients with maxillofacial injuries in Wenchuan earthquake, and search for more effective methods for treating maxillofacial injury. Methods One hundred and sixty-five patients wounded in Wenchuan earthquake were admitted to authors' hospital, and they received specialized diagnosis and treatment. In this paper, the emphasis was put on those patients with severe ear-nose-throat and maxillofacial injuries. Based on the analysis of the state of injuries, the patients were treated with debridement and suture, reshaping and suture, reduction and fixation of fracture, or local or systematic anti-infection therapy. All the treated patients were followed up 2 weeks after treatment. Results Thirty-three of the 165 admitted patients suffered from various degrees of maxillofacial injury (20.0%), including 10 cases of ear injuries (6.1%), 8 cases of nasal trauma (4.8%) and 2 cases of lip laceration (1.2%). All the wounds healed by primary intention after the treatment with no disfiguration. 8 cases of nasal sinus and zygomatic fractures (4.8%) and 5 cases of mandibular fracture (3.0%) recovered nicely after the treatment. No trauma was found in throat, neck, trachea and esophagus. 6 patients (3.6%) were complicated with infection after injury (onset of infection outside of hospital). There was no death. Conclusion Patients with maxillofacial injury should receive early and appropriate treatment. Besides restoration of function, it is also important to restore the physiognomy

This paper reviewed and analyzed the 72 fulfilled projects in the discipline of Otorhinolar-yngology supported in division Ⅱ of basic research of clinical medicine by the National Natural Science Foun-dation of China (NSFC) from year 2002 to 2006, and gave a brief introduction to the part of research a-chievements. The science foundation promotes Otorhinolaryngology to a quick development, and produces some outstanding research achievements with international recognition. Thus, it plays an important role in el-evating the level of discipline and bringing up specialists. According to the retrospection and analysis of the fulfilled projects and their research achievements, we hope it can objective and overall reflect research pro-gress in the field of Otorhinolaryngology, and offer as a reference to the relevant researchers and scientific re-search managers.
6.Meta-analysis on autogenous fat injection for unilateral vocal cord paralysis.

OBJECTIVE:
This study conduct a qualitative synthesis and quantitative meta-analysis of VFAFI, aimed to study whether it is a useful treatment for UVCP.
METHOD:
Electronic databases PubMed, YZ365. COM, WANFANG DATA, CMJD, CHKD,CNKI were searched using relevant keywords. Reported treatment outcomes were clustered into three categories,i. e. subjective, perceptual,acoustic,aerodynamic,and stroboscopic. Meta-analyses were performed on studies with numerical results using random effects model.
RESULT:
Five articles were identified with a total of 404 patients. All the studies reported significant improvements or decrease after VFAFI in each category of outcome measurements. Meta-analysis demonstrated a significant increase or decrease in all categories. Adverse effects include slight inflammatory reponse can resolve spontaneously within 1 month. The recurrence rate after VFAFI was high due to the self absorption. NNE and Jitter of post-operation is lower than pre-operation,there is no significantly change between the control group and experimental group; F0, Shimmer and MPT of post-operation is higher than pre-operation, there is no significantly change between the control group and experimental group.
CONCLUSION
The invasiveness and morbidity of VFAFI are low and the side effects are self-limited. Meta-analyses demonstrated significant improvements or decreased from both objective and subjective measurements. Further controlled studies with longer follow-up periods and more person were included may evaluate the effectiveness of VFAFI more reliably.

7.Meta-analysis on effectiveness of prelingually deaf patients at different ages following cochlear implantation.

OBJECTIVE:
To assess the clinical effeetiveness of prelingually deaf children after cochlear implantation at different ages so as to provide reasonable expectations for the patients and guidance for the clinical treatment.
METHOD:
Electronic databases PubMed, YZ365. COM, WANFANG DATA, CMJD, CHKD, CNKI were searched using relevant keywords. Extracted data included author, year of publication, diagnosis, et al. Reported treatment outcomes were clustered into speech discrimination and hearing abilities. Meta-analyses were performed on studies with numerical results using random or fixed effects model.
RESULT:
There were eight randomized control studies including 442 patients. Comparing speech perception of prelingually deaf children after cochlear implantation younger than three years old (experimental group) and 3-6 years old (control group), three and six months after operation showed that experimental group performed significantly worse than control group; 12 months after operation showed that experimental group performed significantly better than control group. Comparing hearing abilities, three and six months after operation showed that experimental group performed significantly worse than control group; 12 months after operation showed showed that experimental group performed significantly better than control group. Comparing speech perception of younger or older than 4. 5 years old children showed that after 1.5-2 years of operation children implanted younger than 4.5 years of age performed significantly better than children implanted older than 4.5 years old. Comparing speech perception of 7-12 years old children showed that after 3, 6, 12 months of operation patients of 7-12 years old performed significantly better than those children older than 12 years old. Comparing speech perception of implantation younger or older than 18 years old (7-14 yeas old was group A, > 14-18 yeas old was group B, older than 18 yeas old was group C) showed that after one and four years of operation A > B > C, and there were significant differences among them. Comparing warble tone threshold average (WTA) showed that after one year of operation A < B < C, and there were significant differences among them. However, after four years of operation, there was no significant difference among them.
CONCLUSION
Prelinguistically deafened patients younger than three years old with cochlear implantation, insisting on scienctific rehabilitation training for a long period of time can receive the optimal recovery effect. The older patients are suggested as early as possible receiving cochlear implantation. The longer they are implanted, the better results they will receive. Moreover, the younger age they are implanted, the faster postoperative language progress they will receive. Further controlled studies with longer follow-up periods and more person included may make the effectiveness of cochlear implantaion more reliable.

8.Meta-analysis of the efficacy of cochlear implantation in deaf patients with inner ear malformation.

OBJECTIVE:
Assess and compare the clinical efficacy and safety of cochlear implantation in deaf patients with inner ear malformation and in the ones with normal inner ear structure, so that to clarify whether it is effective to restore hearing for the deaf patients with inner ear malformation.
METHOD:
The literature with relevant key words were retrieved in the databases including PubMed, YZ365. com, WANFANG data, CMJD, CHKD and CNKI with language limited to Chinese and English. Extracted data included author, year of publication, diagnosis, treatment, et al. The clinical efficacy of cochlear implantation was assessed by the complications, electrode impedance, behavior T-level, hearing abilities and speech discrimination. Meta-analysis was performed using random or fixed effects model according to the heterogeneity of data.
RESULT:
There were 11 randomized control studies involving 655 patients included in this study. There was no statistically significant difference among the deaf patients in mixed inner ear-malformation group, Mondini group and large vestibular aqueduct syndrome group in the aspects of postoperative complications, electrode impedance, behavior T-level, hearing abilities and speech discrimination.
CONCLUSION
Cochlear implantation could be the way of treatment and rehabilitation for deaf patients with inner ear malformation. Further controlled studies with longer follow-up periods and more multiracial cases included may help to evaluate the efficacy of cochlear implantation for deaf patients with inner ear malformation more reliably.

9.Alfentanil protects the isolated rat heart against ischemia and reperfusion injury via opioid receptors and NO linked mechanisms
