1.Otolaryngeal mucoepidermoid carcinoma:a clinicopathological study of 15 cases

Purpose:To study the clinicopathological features of otolaryngeal mucoepidermoid carcinoma, and to elucidate its diagnosis and differential diagnosis. Methods:The clinical data were reviewed retrospectively and immunohistochemical stains were performed in 15 cases of mucoepidermiod carcinoma.Results:The 15 patients were all male with average age of 55 years. Two histological types were found: well differentiated (14 cases), poorly differentiated (1 case). The tumor cells were positive for CK,CK8,EMA,CEA.Conclusions:Mucoepidermoid carcinoma of otolaryngeal tumor is a rare tumor. It can be differentiated from other otolaryngeal tumors based on the histopathologic features and immunohistochemical findings. Histological type of the tumor is important for clinical therapy and prognostic evaluation.

3.THE RELATIONSHIP OF SERUM CALCIUM, MAGNESIUM, ZINC, AND COPPER BETWEEN MATERNAL AND CORD BLOOD

Serum calcium, magnesium, zinc, and copper were determined in 117 healthy pregnant women and their infants. Maternal and cord blood were collected at delivery and assayed by flame atomic absorption spectropho-tometry, and the data obtained were evaluated by regression analysis.The results suggested that the serum concentrations of calcium, magnesium, and zinc of maternal blood were significantly lower than that of cord blood. The concentrations of serum calcium and magnesium between maternal and cord blood were highly correlated, but not of serum zinc. The serum concentration of copper in maternal blood was significantly higher than that in cord blood, but there was nos ignificant correlation between them. The serum calcium of infants always maintained over 2.0mmol/L(8mg/dl), even when the serum calcium of pregnant women was lower than 2.0mmol/L (8mg/dl). This fact indicates that the fetus draw much calcium from mother for development, despite calcium in the mother being depleted.
4.Significance of histologic molecular markers for prognosis of the ⅢA (N2) phase non-small cell lung cancer

Prognosis factors of Ⅲ A (N2) phase non-small cell lung cancer are various.The histological molecular markers are mainly about multi-resistant gene,growth factors,mucin 4,insulin-like growth factor-2,Ki-67,PTEN and so on.Different expressions of these genes may divide the N2 non-small-cell lung cancer into different molecular subtypes,which is more effective to guide clinical therapy.
5.Measurement of complement receptor 1 on neutrophils in bacterial and viral pneumonia

Objective To study the different expression of complement receptors on neutrophils between bacterial and viral pneumonia. Methods Fifty-nine patients with CAP were studied prospectively. CR1 ,CR3 were measured by,using flow cytometer. ESR,CRP and WBC were also measured. Results Thirteen patients had pneumococcal pneumonia; 13 patients, A type of infuenza virus pneumonia ;5 patients, atypical pneumonia, and 28 patients, aetiologically undefined pneumonia. The mean expression of complement receptor 1 ( CR1 ) on neutrophils and aetiologically undefined pneumonia was( 19.4 ± 9.8 ) and ( 19.3 ± 8.8), which were significantly higher than those with A type of infuenza virus( t = 6.13,5.09 ,P < 0.01 ). Conclusion The expression of CR1 may be of value as an additional rapid tool in the aetiological diagnosis, bacterial or viral infection of CAP.
6.ASSESSMENT OF VITAMIN A STATUS BY CONJUNCTIVAL IMPRESSION CYTOLOGY

Ocular signs and serum vitamin A concentration are commonly used tor the diagnosis of vitamin A deficiency. Recently, impression cytology was suggested as a method for early detection of vitamin A status. Twenty patients with normal eyes and digestive disease, and 111 children with normal eyes were evaluated by serum vitamin A concentration, relative-dose-response and conjunctival impression cytology test. About 90% of the patients with normal conjunctival impression cytology has normal vitamin A status. About 70% of the patients with abnormal conjunctival impression cytology has abnormal vitamin A status despite their seemingly normal clinical ocular examination, and so did the children. These results suggest that impression cytology can identify individuals with preclinical vitamin A deficiency.
7.Tip-apex distance and calcar-referenced tip-apex distance used as a predictor of screws cutout from the femoral head

Intertrochanteric fracture,a common kind of injury in orthopaedic surgery,often requires surgical intervention,especially the unstable ones.Since far back in 1995,Baumgaertner et al.recommended using the distance between the screw tip and femoral head apex (tip-apex distance,TAD) as a predictor of the risk of lag screw cutout,the importance of TAD has been recognized by orthopaedic surgeons.This concept has been widely accepted in practice,and even used as a standard in determining the helical blade position.It is not clear,however,whether the helical blade system is equal to lag screws in terms of TAD.After the limitations of TAD had been reported,Kuzyk et al.proposed a tip-apex distance referenced from the calcar (Cal-TAD) as a predictor of lag screw cutout to improve the concept further.In this article,these concepts and the latest clinical applications of TAD and Cal-TAD are reviewed based on the publications available.

10.Flexible laser endotracheal intubation optical device used for oral and maxillofacial intubation operations in 42 patients

Objective To explore the effect of flexible laser endotracheal intubation optical device (patent number 201520785723.7) on oral and maxillofacial intubation operations. Methods Forty-two patients with the oral and maxillofacial intubation optical device for general anesthesia were included in this study. The patients were from 16 to 73 years old and ASAⅠ-Ⅱ. Surface anesthesia was prepared for airway and nasal cavity. There were 27 patients whose mouth opening were smaller than 3 cm. After intravenous injection of midazolam 0.05 mg/kg and sufentanil 0.2μg/kg, reinforced endotracheal tube was inserted into throat 13-15 cm, then the probe shape to 120° and optical device were placed into the tube respectively, guided the tube according to the light spot in front of neck. There were 15 patients whose mouth opening were not smaller than 3 cm. The quick guidance optical device and larygoscope were adopted to help the intubation. After intravenous injection of midazolam 0.05 mg/kg, sufentanil 0.5 μg/kg, propofol 2 mg/kg, cisatracurium 0.2 mg/kg, the reinforced endotracheal tube was inserted into the oral cavity nasally, and completed intubation with laryngoscope. The success rate of intubation, time of intubation, the real-time blood pressure before and after intubation, heart rate and related complications were recorded before intubation and after intubation. Results In the 42 patients, 41 patients were successful for guidance intubation, and the success rate of intubation was about 97.6%. The average intubation time was(124.5±38.2) seconds. The shortest intubation time was 12 seconds and the longest was 276 seconds. During intubation, the increases of blood pressure and heart rate were in the normal range (<30%). Levels of SpO2 were maintained at 0.92-1.00. All the patients were without agitation. There were no serious complications such as hoarseness and pharyngalgia after operation.Conclusion The flexible laser endotracheal intubation optical device can be used quickly for oral and maxillofacial intubation, and the 45° bevel design for optical device can form two kinds of zones with bright light upward and downward. It is a new tool for nasal intubation that is more convenient for location of light guidance, and has high success rate and small stimulation.
