Journal of Southern Medical University 2007;27(12):1941-1943
OBJECTIVETo observe the effect of the extracts from rabbit skin inflamed by vaccinia virus for injection, a commercially available agent (Analgecine), in the treatment of postherpetic neuralgia (PHN) in aged patients and its influence on serum interleukin-6 (IL-6) level.
METHODSA total of 44 elderly patients with PHN were randomly divided into two groups. In addition to the conventional antiviral therapy, patients in group A were given analgecine, and those in group B given indomethacin tablet. After the completion of the 14-day treatment course, the therapeutic effects were evaluated using visual analogue scale (VAS), and the changes in serum IL-6 levels after treatment were measured.
RESULTSThe score of VAS decreased significantly after treatment in group A (P<0.01), but not in group B (P>0.05). In both groups, the serum IL-6 level before treatment was significantly higher than that in normal control subjects (P<0.001), and lowered significantly after treatment (P<0.01) in group A, but not in group B (P>0.05).
CONCLUSIONThe extracts from rabbit skin inflamed by vaccinia virus have definite effects for rapid pain relief, and can effectively decrease the serum IL-6 level in elderly patients with PHN.

Objective To compare the bio-medical parameters in SIV infected Chinese rhesus monkeys with diverse disease progression,by which the pathogenesis of simian AIDS were to be investigated. Methods Sixteen Chinese rhesus monkeys were inoculated intravenously with SIVmac239 and followed-up for 18 months.Based on their progression patterns and plasma viral loads, animals were divided into 3 groups, including 1 rapid progressor( RP),13 normal progressors(NP),and 2 elite controllor(EC).Their parameters of haematology, virology, immunology and pathology were examined and compared. Results Compared with other animals,RM449(RP) showed higher viral load, unresponsive humoral immunity, and higher level of auto-antibodies against lymph node, thymus, and spleen.Additionally, its effector memory CD4 count was lower, with the transformation progress being blocked-like from naive/central memory subsets to effector memory subset,as the flow-cytometry assay showed.Notable decrease in its peripheral B cell was also observed, especially to the sub-population of tissue-like memory B cells and activated memory B cells.Pathological examination showed the depletion of lymphoid tissue, atrophy of spleen and loss of thymus. Moreover, most of these parameters of RM450 and RM453 (EC) changed opposite to that of RP.Conclusion The hallmarks of RM449 were higher viraemia and lower SIV specific IgG level, which may due to the disturbance of T cells and B cells development and differentiation. Moreover, destructions of organs of the immune system may contribute to the disturbance. Our study suggest that the change of micro-environments of thymus induced by SIV infection, which is necessary in T cell and B cell development and differentiation, may contribute at least partially to the AIDS pathogenesis.

Objective:To analyze the changes in the number and workload of pediatricians, for evidences in further optimizing the allocation of pediatrician resources and formulating relevant policies in China. Methods:According to data from the National Bureau of Statistics and the " China Health and Health Statistics Yearbook", descriptive analysis was made to investigate the changes in the number and workload of pediatricians in China from 2010 to 2019.Results:During 2010 and 2019, the number of pediatric practitioners(assistants) increased from 115 800 to 158 500, and the growth rate was faster than that of the children aged 0-14 years, but slower than that of overall medical practitioners(assistants). The number of pediatric practitioners(assistants) per thousand children in China had grown steadily from 0.52 to 0.67, compared with the burden and the workload of medical practitioners nationwide, the workload of pediatricians was still relatively heavy. Conclusions:The number of pediatricians had increased gradually between 2010 and 2019, and the effects of various policies began to work, while the workload was still heavy. It is recommended to further develop a salary system and supporting policies in line with the characteristics of pediatrics sector, and focus on such problems as unbalanced and inadequate distribution of pediatricians.

OBJECTIVE To investigate the effects of individualized dosing regimen on blood trough concentration of vancomycin and renal function in critically ill patients. METHODS According to relevant guidelines and the results of Vancomycin Calculator, clinical pharmacists formulated an individualized dosing regimen of vancomycin including loading dose and maintenance dose for critically ill patients based on the two independent variables of body weight and creatinine clearance rate. Using the method of retrospective study, patients who were admitted to the department of intensive care unit (ICU) of the Second Affiliated Hospital of Guangzhou Medical University and used the regimen from July 2018 to December 2021 were selected as the trial group, and patients who were treated with vancomycin and received blood drug concentration monitoring in ICU from January 2015 to June 2018 were recruited in the control group. The difference in trough concentration distribution and the incidence of acute kidney injury (AKI) after medication were compared between the two groups, the change of serum creatinine before and after medication in the trial group was analyzed. RESULTS Totally 197 patients were included in the trial group and 144 patients were in the control group. There was no significant difference between the two groups in the clinical information (gender, age, body weight, acute physiology and chronic health evaluation Ⅱ score, the proportion of patients with renal insufficiency, etc.) (P>0.05). The proportions of major infection sites (including lung, urinary, abdominal, blood and central nervous system) and treatment type (target or empirical treatment) also had no significant difference between the two groups (P>0.05). There was no significant difference in the attainment rate of ideal trough concentration (15-20 μg/mL) and the proportion of patients with trough concentration >20 μg/mL between the two groups (P>0.05), while the attainment rate of target trough concentration (10-20 μg/mL) and the proportion of patients with trough concentration <10 μg/mL were significantly different between the two groups (P<0.05). The attainment rate of target trough concentration in patients with chronic renal insufficiency in trial group was significantly higher than that in control group (P<0.05). There was no significant difference in the incidence of AKI and vancomycin-associated AKI between the two groups (P>0.05). In the trial group with medication duration ≥7 days , the level of serum creatinine on the 7th day of treatment was increased significantly, compared with that on the 3rd day of treatment (P<0.05). CONCLUSIONS This individualized dosing regimen can improve the attainment rate of target trough concentration of vancomycin in critically ill patients, especially those with chronic renal insufficiency, during the first standardized monitoring, and not increase the risk of renal injury compared with previous empirical medication.
