1.Study on quality standard for Zangfukang Suppository

AIM: To establish the method for quality control of Zangfukang Supposit ory (Radix Przewalskii tanguticae, Pericarpium Zanthoxyli, Herba Oxytropis falca tae, etc.). METHODS: Radix Przewalskii tanguticae, Pericarpium Zanthoxyli, Her ba Oxytropis falcatae were identified by TLC, and the content of hyoscyamine in Radix Przewalskii tanguticae was determined by HPLC. RESULTS: The ethyl acetate-methanol-ammonia(17∶2∶1) was applied as the developer for the identification of Radix Przewalskii tanguticae on silica gel G coating plate. The cyclohexane-ethyl acetate-formic acid (30∶15∶1) was applied as the developer for the identification of Pericarpium Zanthoxyli on s ilic a gel G coating plate. The cyclohexane-ethyl acetate-formic acid (50∶10∶1) w as taken as the developer for the identification of Herba Oxytropis falcatae als o on silica gel G coating plate. Hyoscyamine showed a good linear relationship a t a range of 2.5~25?g,r=0.9999. The average recovery was 99.59%, and RSD was 1.34%. CONCLUSION: The methods are accurate and reliable, and can control the quality of this preparation effectively.
2.A new triterpenoid saponin from the fruits of Polygonum orientale

To study the chemical constituents of the fruits of Polygonum orientale L. , silica gel and ODS column chromatography methods were used to separate and purify the constituents. Their structures were elucidated on the basis of physicochemical properties and spectral analysis. Three compounds were identified as 28-O-β-D-glucopyranosyl-3β,7β-dihydroxy-lup-20 (29) -en-28-oate ( 1 ), 5,7-dihydroxychromone (2) and naringenin (3). Compound 1 is a new triterpenoid saponin and others were isolated from the fruits of this plant for the first time.
3.Determination of Ginsenoside Rb_1,Ginsenoside Re and Ginsenoside Rg_1 in Yixin Fumai Granula by HPLC

OBJECTIVE: To determine Ginsenoside Rg1,Ginsenoside Re and Ginsenoside Rb1 in Yixin fumai granula by HPLC.METHODS: Samples were determined on a Diamond C18(150 mm?4.6 mm,5 ?m).The mobile phase consisted of accetonitrile-water by gradient elution with flow rate at 1 mL?min-1,UV detection wavelength at 203 nm,column temperature at 25 ℃ and sample size at 10 ?L.RESULTS: The linear ranges of Ginsenoside Rg1,Ginsenoside Re and Ginsenoside Rb1 were 0.645~6.450 ?g(r=0.999 8),0.54~5.40 ?g(r=0.999 7) and 0.605~6.050 ?g(r=0.999 9),respectively;the average recoveries were 100.59%(RSD=2.03%),98.70%(RSD=1.46%)and 98.99%(RSD=1.19%),respectively.CONCLUSION: The method is sensitive,simple and accurate,and it can be used for the quality control of Yixin fumai granula.
4.Quality standard for Gutongning Liniment

AIM: To establish the method for quality control of Gutongning Liniment(Herba Epimedii,Semen Strychni,Herba Ephedrae,etc.) METHODS: Herba Ephedrae,Radix Notoginseng,Ramulus Cinnamomi,Flos Caryophylli,Flos Daturae were identified by TLC,icariin content in Herba Epimedii and strychnine content in Semen Strychni were determined by HPLC. RESULTS: The identified characteristics of chromatography were distinct and the spots were clear.Icariin showed a good linear relationship at a range of 0.2-1.2 ?g,r=0.999 8.The(average) recovery was 10.3%,and RSD was 1.5%.Strychnine showed a good linear relationship at a range of 0.8-4.8 ?g,r=0.999 7.The average recovery was 99.17%,and RSD was 1.4%. CONCLUSION: The methods are accurate and reliable,and can be used to control the quality of this preparation effectively.

6.Correlation of left ventricular hypertrophy detected by electrocardiogram and main cardiovascular accidents in hypertension patients

Objective To analyze the predicting value of Cornell-QRS standard and Sokolow-Lyon voltage to left ventricular hypertrophy in hypertensive patients.Methods One hundred and seventy-four patients with primary hypertension were enrolled, who were divided into left ventricular hypertrophy group (LVH group,n=50)and non-LVH group(n=124).The blood pressure, Cornell-QRS standard and Sokolow-Lyon voltage were collected and compared in the following-up period.Results Compared with non-LVH group,in LVH group the history of hypertrophy was longer(P <0.05),percent of grade 3 hypertrophy was higher(P <0.05),and 24 h SBP was higher(P <0.05).During the following-up of 6 months,1 year,2 year,the SBP (systolic blood pressure), DBP (diastolic blood pressure),Cornell-QRS standard and Sokolow-Lyon voltage in LVH patients were all significantly decreased (P <0.01).Compared with non-LVH group,the incidence of non-fatal myocardial infarction and stroke was higher in LVH group[ 3(6.0%)vs .1(0.8%),P <0.05;6 (12.0%)vs .2 (1.6%),P <0.01 ].By Cox analysis,Cornell-QRS standard and Sokolow-Lyon voltage were respectively independent predictors to non-fatal myocardial infarction and also stroke.Conclusion Cornell-QRS stand-ard and Sokolow-Lyon voltage may be independent predictors to main cardiovascular accident in hypertensive patients.
7.Different dose and effects of dexmedetomidine in preventing shivering after general anesthesia for laparoscopic surgery

Objective The aim of this study was to investigate different dosages and effects of dexmedetomidine for prevention of postanesthetic shivering. Methods One-hundred twenty patients scheduled for laparoscopic surgery were randomly allocated in four groups: before the operation, slowly injected 0.9% normal saline (group S, dexmedetomidine 0.5 μg/kg (group D0.5), dexmedetomidine 0.75 μg/kg (group D0.75), dexmedetomidine 1.0 μg/kg(group D1.0). HR and rectal temperature[C2] were continually monitered during and after operation, time to extubation was measured. Grades of shivering were recorded. Pain evaluation was assessed by a visual analogue scale, sedation was evaluated by Modified Observer′s Assessment of Alertness/Sedation scale. Results The patients in group S showed a significantly higher HR and postoperative incidence of shivering than those in group D0.75 and group D1.0, (P < 0.05). but the extubation time in groupd D0.75 and group D1.0 were longer than patients in group S (P<0.05). Conclusion Slowly injected dexmedetomidine 0.75 μg/kg or 1.0 μg/kg can prevent postanesthetic shivering in laparoscopic surgery effectively.
8.Effects of Honokiol on Proliferation and Apoptosis on U937 Cells

[Objective] To investigate the anti-proliferative and apoptosis effect induced by Honokiol (HNK) on human myeloid leukemia cell line U937 cells in vitro.[Methods] After treated with different concentration of HNK,Hoechst33342 fluorescent staining was used to detect cell apoptosis;the growth inhibition ration of U937 cells and PBMCs were analyzed by MTT assay;the apoptosis ration was detected by flow cytometry;mitochondrial membrane potential was explored by rhodamine 123 stain;Caspase3/7 protein activity kit was used to test the Caspase3/7 activity;the Caspase-3 and Caspase-7 mRNA levels were detected by real-time fluorescent relative-quantification reverse transcriptional PCR (FQ-PCR).[Results] Honokiol could significantly inhibit the proliferation of U937 cells in terms of the indexes of IC50/U937 11.8 μg/mL and IC50/PBMCs 40.3 μg/mL,and the anti-proliferative effect was in a time and concentration dependent manner;Flow cytometry analysis manifested that Honokiol could induce U937cells apoptosis by Annexin V/PI double Annexin V/PI fluorescein stain;Honokiol significantly inhibited the mitochondrial membrane potential of U937 cells and enhanced the ability of Caspase3/7 and the mRNA expression levels,but not the PBMCs.[Conclusion] HNK can inhibit U937 cells proliferation and induce cells apoptosis via activating Caspase 3/7.
9.Impact of different hemoglobin on postoperative cognitive functions in female patients undergoing laparo- scopic surgery

Objective To explore the impact of different hemoglobin on postoperative cognitive functions in female patients underwent laparoscopic surgery. Methods 215 female patients who were underwent laparoscopic total hysterectomy or laparoscopic myomectomy from January 2016 to December 2016 in our hospital were selected. The patients were divided into 3 groups according to their hemoglobin:Group N with normal hemoglobin ,Group A with hemoglobin concentration of 9 ~ 11 g/L ,and Group B with hemoglobin concentration of 6 ~ 9 g/L. Same anesthesia and recovery plan were performed in all patients. Basic and operative information were recorded. The score of MoCA on the day before surgry ,the day after surgry ,3 days after surgry and 30 days after surgry were recorded. Blood samples were collected on the day before surgry and the day after surgry for the determina-tion of serum S-100β protein and IL-6 concentrations. Results No significant difference was found in basic and operative information among three groups. The recovery time was much longer in Group B than that in Group N. No difference were found among three group in the score of MoCA on the day before surgry and 30 days after surgry ,while the score in the day after surgry and 3 days after surgry were lower in Group B com-pared with Group N. The incidence of POCD were higher in Group B compared with Group N. Compared with the day before surgry ,the concentration of serum S-100β protein and IL-6 were much higher. And compared with Group N ,the concentration of serum S-100β protein was higher in Group A and B in the day after surgry , while IL-6 was higher in Group B. The concentration of serum S-100βprotein and IL-6 were much higher in Group B compared with Group A in the day after surgry. Conclusion Female patients with hemoglobin concen-tration of 6 ~ 9 g/L are susceptible to POCD ,which might be contributed by operative central nerve system damage induced by excessive inflammation.
10.The characteristics of optical coherence tomography angiography and its relationship with visual field defect in nonarteritic anterior ischemic optic neuropathy

Objective To observe the blood perfusion of optic nerve and macular areas and investigate its relationship with visual field defect in nonarteritic anterior ischemic optic neuropathy (NAION). Methods Twelve consecutive unilateral NAION patients (course of disease <3 months) and 12 healthy Chinese adults were enrolled in the study. The affected eyes and fellow eyes from 12 NAION patients were defined as group A and group B; 12 eyes from 12 healthy adults were defined as group C. Best corrected visual acuity (BCVA), intraocular pressure (IOP), indirect ophthalmoscope and computer optometry were performed on all of the three groups of patients. Visual field (VF) and optical coherence tomography (OCT) were performed on NAION patients. Logarithm of the minimum angle of resolution (logMAR) was used to calculate visual acuity. Compared to group B, logMAR BCVA, mean deviation (MD) and pattern standard deviation (PSD) in group A were significant decreased (t=3.278, -4.909, 4.130, P<0.05). There was no significant difference in spherical equivalent, IOP, peripapillary retinal nerve fibre layer (pRNFL) between group A and group B (t=0.000, 0.890, 1.215; P>0.05). OCT angiography (OCTA) was used to measure the flow area (FA) at optic disc, flow area at radial peripapillary capillaries (RCFA) and FA, non-perfusion area (NFA), parafoveal vessel density (PVD) and parafoveal vascular index (PVI) in macular area. Pearson correlations between the deficiency of optic blood flow and visual field were analyzed. Results The differences of FA at optic disc and peripapillary RCFA among 3 groups were significant (F=4.162, 3.357; P<0.050). Compared to group B (t=-5.822, -7.467; P<0.001) and C (t=9.435, 4.615, P<0.05), FA at optic disc and peripapillary RCFA in group A was significantly reduced. There is several NAION showed quadrantal FA decreased in optic nerve. However, there was no significant difference in optic disc FA and peripapillar RCFA between group B and C (F=0.004, 0.030; P>0.050). There was no differences of FA, NFA, PVD and PVI among 3 groups (F=0.488, 1.107, 0.493, 1.086, 1.098, 0.093, 1.093, 1.221; P>0.05). Positive correlation between optic disc FA, peripapillary RCFA and MD (r=0.542, 0.585;P<0.05) were observed. However, there was no significant correlation between optic disc FA, peripapillary RCFA and PSD (r=-0.404, -0.430; P>0.05), and negatively correlated to BCVA (r=-0.617, -0.596; P<0.05). PRNFL was negatively correlated to optic disc FA (r=-0.643, P<0.05), but not correlated to peripapillary RCFA (r=-0.377, P>0.05). Conclusions The optic disc blood flow reduced in affected eyes of unilateral NAION whose disease course was less than 3 months, while the macular perfusion was normal. There was a positive correlation between optic disc flow and visual field.
