1.Recanalization of Chronic Peripheral Arterial Occlusions Using Local Low dose Thrombolysis:in Combination with Angioplasty and Stent Implantation

Local low-dose thrombolysis, combined with angioplasty or stent implantation, was performed in 5 patients with chronic occlusions of iliac and femoropopliteal arteries. The obstructed segments were found in the external iliac artery (n=2), common iliac and external artery (n=1), femoral artery (n=1), popliteal artery (n=1) and femoropopliteal artery (n=1). The occluded length ranged from 8~25 (mean, 15.3)cm. The estimated duration of occlusions were from 1~24(mean, 9.4) months. Local thrombolysis was performed by using urokinase 56, 000 IU/h, accompanied by heparin treatment (1,000 IU/h) in all patients. The infusion time ranged from 5.5~58(mean, 26) hours. PTA (n=3) and intravascular stent placement (n=1) were immediately performed after thrombolysis. The initial success rate was 100%. Follow-up lasted 2~9 months (mean 5.3 months). This approach was even succesful in a patient with occluded duration up to 2 years. This therapy represents a true altermative to vascular surgery and as first-choice treatment for segmental peripheral occlusions. Thrombolytic procedure dose, interventional indications, other factors and long-term effects were discussed.
2.Application percutaneous transluminal angioplasty and stent in the treatment of subclavian artery stenosis

Objective To evaluate and compare the therapeutic results of subclavian artery angioplasty and stenting in patients with aortoarerteritis and atherosclerosis. Methods 13 cases (9 males and 4 females; age, 28-69 years) of subclavian artery stenotic occlusive disease were performed with PTA and stent. Results Six patients had aortoarerteritis, 7 had atherosclerosis lesions. Twelve of 13 stenoses could be successfully angioplasty ( n =6) and stenting ( n =6) . The one failure was due to the inability to cross the lesion with the guidewrie. Compared with atherosclerosis, higher balloon inflation pressure was required to dilate the lesions of aortoarerteritis. 12 patients were asymptomatic after PTA and stent implatation. Patients were follow up for 3-24 months and clinical examination demonstrated patency of the vessels with PTA and stents. Conclusion Subclavian PTA and stent implatation is safe and good clinical result as in aortoarerteritis as in atherosclerosis.
3.Clinical application of combined detection of three indicators in pulmonary tuberculosis patients accompanied with infection

Objective To explore the clinical significance of the three indicators combined detection that plasma endotoxin,pro-calcitonin (PCT)and (1,3)-β-D-glucan(BG)in pulmonary tuberculosis patients complicated with bacterial,fungal infection.Methods Retrospective investigation was conducted in 240 pulmonary tuberculosis patients,who were divided into gram negative bacteria (G- )group(39cases),gram positive bacteria (G+ )group(45 cases)including fungal,germ-free group(156 cases).Other 45 healthy people were selected into control group.The levels of plasma endotoxin,PCT and BG in the four groups were compared.Results The levels of plasma endotoxin(0.682±0.418)EU/mL,PCT(2.93±0.87)μg/L in the G- group were significant higher than those of the G+ group (0.063±0.034)EU/mL,(0.85±0.52)μg/L,the difference was significant (P <0.05).The levels of the plasma endotoxin,PCT in the G- group were higher than those of the germ-free group.The levels of plasma endotoxin,PCT and BG in the control group had no significant difference with those of the germ-free group.Conclusion The combined detection of plasma endo-toxin,PCT and BG have some clinical value on the early diagnosis of the patients with pulmonary tuberculosis complicated with G-fungal infection for the advantages of fast and sensitiveness.
4.Correlation between serum uric acid and cardiovascular risk factors in Shandong coastal inhabitants

Objective To elucidate the relationship between serum uric acid (SUA) and cardiovascular risk factors in Shandong coastal inhabitants. Methods A random, stratified cluster sampling method was conducted. A population.based cross-sectional survey for hyperuricemia was performed and 4 988 dwellers of Shandong coast were investigated. Fasting SUA,lipid profiles,insulin concentrations,etc.,were determined by automatic biochemical analysis device and electrochemiluminescence analyzer.The investigation of general,healthy and nutritional conditions was carried out by questionnaire. Results In male and female subjects, the cardiovascular risk factors such as systolic blood pressure,diastolic blood pressure, selum ereatinine,total cholesterol.triglvceride,waist circumference and body mass index all increased with rising SUA,and the level of senlm high densitv lipoprotein fell accordingly.Homeostasis model assessment insulin resistance index (HOMA- IR) in female subjects increased by the quartile group,not in male subjects.Compared with SUA<280 μmol/L group.odds ratio(OR) values of hypertension in SUA 280-319,320-349,≥350 μmol/L groups were 1.54,1.88 and 2.70 respectively in female,and 2.29,4.15 and 4.96 for metabolic syndrome (MS).As for male,compared with SUA<280 μmol/L group OR value of hypertension in SUA ≥420 μmol/L group was 1.52,and OR values of MS of 1.41,1.87 and 3.27 occurred in SUA 320~379,380~419,≥420 μmol/L groups.After being adjusted for confounding factors,logistic stepwise regression revealed that elevated SUA was an independent risk factor for hypertension in both genders.Conclusion Elevated SUA level is closely related to the incidence and development of cardiovascular diseases,and should not be ignored in clinical practice.
5.Electrocardiographic Characteristics in Patients With Cardiac Amyloidosis

Objective: To study the diagnostic characteristics of cardiac amyloidosis (CA) by non-invasive electrocardiography (ECG) in relevant patients. Methods: We retrospectively analyzed 60 CA patients diagnosed in our hospital from 2008-08 to 2013-12 for their clinical and ECG characteristics. Results: There were 48 male and 12 female patients with the ratio of 4: 1. The ifrst time diagnosis rate was low and the average age for conifrmed diagnosis was at (54. 5±14. 2) years.①There were 32 (53. 3%) cases combining heart failure, 12 (20%) with pleural effusion, 20 (33. 3%) with atrial arrhythmia, 8 (13. 3%)with ventricular arrhythmia, 4 (6. 7%)with sino-atrial block, 15 (25%)with atrio-ventricular block, 4 (6. 7%) with left bundle branch block (LBBB), 5 (8. 3%)with RBBB and 8 (13. 3%)with intra-ventricular block.②There were 32 (53. 3%) cases with low voltage on limb leads, 52 (86. 7%) with pseudo-infarct pattern, 48 (60%) with ST-T abnormality and 30 (50%) combining low voltage on limb leads with pseudo-infarct pattern.③The patients combining pleural effusion and with pseudo-infarct pattern had the increased ratio of low voltage on limb leads, while there were still 22 (45. 8%) cases without pleural effusion had low voltage on limb leads.④ ECG characteristics for 60 CA patients were as follows: QRS duration (104±26) ms, QT interval (404±34) ms, QTc (462±35) ms; the R wave of avR 0. 17 mV, QRS wave 0.30 mV; the R wave of limb leads and V1-3 were all<0.5mV, the S wave of V1-3 were 0. 62mV, 1. 61mV, 1. 56mV; the R/S ratio of V1-3 were 0. 19, 0. 12, 0. 20 respectively. Conclusion: CA patients had the highest incidence of pseudo-infarct pattern; meanwhile, combining with low voltage on limb leads, pseudo-infarct with long Q or S wave and ST-T abnormality but normal QRS duration was helpful for differential diagnosis of CA in clinical practice.
6.Myocardial perfusion abnormalities in patients with isolated left ventricular noncompaction

Objective To investigate the myocardial perfusion abnormalities in patients with isolated left ventricular noncompaction (ILVNC) and analyze the correlation between MPI and MRI.Methods Nineteen patients (14 males,5 females,age range:15-76 years) with ILVNC diagnosed by cardiac MRI were recruited.All patients underwent 99Tcm-MIBI MPI.Both MPI and cardiac MRI were analyzed using a 17-segment model.The thickness ratio of the non-compacted to compacted layers of myocardium (NC/C) was calculated,and segments with NC/C>2.3 were considered as noncompaction.The incidences of delayed enhancement (DE) and myocardial perfusion abnormalities in non-compacted segments and compacted segments were calculated.x2 test was used for categorical data.The Pearson and Spearman correlation coefficient were used to assess the relationship between the numbers of myocardial segments with myocardial perfusion abnormalities/noncompaction/DE and lgLVEF.Results Of 19 patients,myocardial perfusion abnormality was found in 16 (84.2%) patients.The incidences of perfusion abnormality were 33.6% (36/107) in non-compacted segments and 31.9% (69/216) in compacted segments,respectively (x2=0.09,P>0.05).There were 31 segments with DE.The incidences of DE were 5.6% (6/107) in non-compacted segments and 11.6% (25/216) in compacted segments,respectively (x2 =2.94,P>0.05).The incidence of reduced perfusion was higher in segments with DE than those in segments without DE (54.8% (17/31) vs 30.1% (88/292) ; x2 =7.80,P<0.01).The lgLVEF and the numbers of myocardial segments with noncompaction/DE/myocardial perfusion abnormalities were not correlated(r=-0.35,0.15,-0.34,all P>0.05).Conclusion Most patients with ILVNC have myocardial perfusion abnormality,which can be observed both in non-compacted and compacted myocardium.Further research is required to elucidate the role of myocardial perfusion abnormality in ILVNC.

8.Differential expression of microRNA-125b in the neuronal differentiation of adipose-derived Flk1~+ mesenchymal stem cells

BACKGROUND:The differentiation of mesenchymal stem cells(MSCs)is strictly regulated by both genetic and epigenetic Factors .Emerging evidences have demonstrated that microRNA also plays an important role in this process.OBJECTIVE:To explore the differential expression of microRNA-125b during neuronal dliferentiation of Flk1~+ adipose-derived MSCs(AD-MSCs).MATERIALS:The fat samples were provided bv healthy female volunteers aged 15-35 years and recruited from the Plastic Surgery Hospital affiliated to the Chinese Academy of Medical Sciences.METHODS:Adult adipose tissues were obtained from healthy voluntears with age of 15-35 years .Using adherence method,Flk1~+ MSCs were obtained and the 3~(rd) passage cells were taken in the experiment.Cultured in neuronalinduction medium.these MSC were induced to differentiate towards neuronal lineage.The expression of microRNA-125b was examined at days 0,4,8 and 12.To explore its role in neuronal differentiation,we need to change its expression.RT-PCR and Taqman real-time PCR were carried out to explore the difierentially expression of microRNA-125b during neuronal differentiation of AD-MSCs.The effect of inhibitor on the expression of microRNA-125b was detected.RESULTS AND CONCLUTION:①The Flk-1~+MSCs were successfully induced into neuronal differentiation and displayed typical morphological changes 12 days after induction:Most cells retracted their cytoplasm ,fomling spherical cell body and emitted cellular protrusions.RT-PCR and immunocytochemistry studies confirmed their phenotype with expression of known neuronal cell markers including neurofilament,glial fibrillary acidic protein.②The expression of microRNA-125b was significant up-regulated during neuronal differentiation .Results of RT-PCR and Taqman real-time PCR were concordarlce with that of microRNA chip technology.③Inhibitor could down-regulate microRNA-125b.The results implied that microRNA-125b may play an important role in neuronal differentiation.
9.Comparison of 99Tcm-MIBI SPECT/18F-FDG PET imaging and cardiac MRI in assessment of myocardial injury cases with idiopathic dilated cardiomyopathy

Objective To evaluate the relationship of myocardial MR contrast delay-enhancement and nuclear perfusion-metabolism pattern in patients with idiopathic dilated cardiomyopathy (IDCM).Methods Forty-two consecutive patients (29 men and 13 women,age:(53 ±12) years) diagnosed clinically with IDCM were enrolled.All patients underwent 99Tcm-MIBI SPECT,18F-FDG PET imaging and MR contrast delay-enhancement imaging within 3-7 d.The myocardial perfusion-metabolism segment analysis was performed using a 17-segment model.Segmental 99Tcm-MIBI and 18F-FDG uptakes were scored visually using a 4-grade scoring system (0 =normal uptake,1 =mildly reduced uptake,2 =moderately reduced uptake,3 =severely reduced uptake).Patterns of perfusiorn/metabolism were classified as normal,mismatch,mild-to-moderate match and severe match.Myocardial MR contrast delay-enhancement was classified into 3 categories (non,mid-wall and transmural delay-enhancement).x2 test was used to analyze the differences of perfusion/metabolism patterns among non,mid-wall and transmural delay-enhancement groups and the myocardial MR contrast delay-enhancement incidence among four perfusion/metabolism groups.Resuits Among the 42 patients,myocardial delay-enhancement was present in 18 patients,of which 94.4% (17/18) showed abnormal myocardial perfusion/metabolism patterns and only 33.3 % (8/24) patients without abnormal myocardial delay-enhancement had abnormal myocardial perfusion/metabolism patterns (x2 =15.944,P < 0.001).Perfusion/metabolism patterns varied in three different categories of non,mid-wall and transmural delay-enhancement (x2 =14.276,P < 0.001).The normal peffusion/metabolism pattern proportions in the non,mid-wall and transmural delay-enhancement groups were 86.2% (526/610),71.0% (44/62) and 28.6% (12/42),respectively.The incidence of transmural delay-enhancement (44.4% (12/27)) was significantly higher in segments with severe match than that in the other 3 groups (normal:2.1% (12/582) ; mismatch:18.1% (15/83) ; mild-to-moderate match:13.6% (3/22) ; x2 =112.530,P < 0.001).Conclusions MR contrast delay-enhancement is much more sensitive in detecting moderate fibrosis,while nuclear perfusion-metabolism imaging can detect more impaired but viable myocardium.Combining the two imaging modalities is useful for providing comprehensive evaluations of myocardial injury in patients with IDCM.
10.Quality in fruits of Lycium barbarum and its relationship with soil fertility factors

Objective The contents of total sugar,polysaccharide,amino acid,betaine,carotenoid,flavone,hundred-seed weight of Lycium barbarum fruits from eight different habitats in first stubble,prosperous time,and autumn were determined and the ratios of total sugar to betaine were analyzed,in order to classify these indexes,and investigate the relationship with the soil fertility factors.MethodsHPLC and UV-spectrophotometry methods were used to determine the contents and the results were analyzed adopting the clustering,correlation,and variance analysis.Results All the indexes were divided into three parts which were total sugar,betaine,and carotenoid.Various indexes were affected in different degrees by the soil fertility factors.Conclusion The contents of total sugar,betaine,carotenoid,and the ratios of total sugar to betaine could be used to assay the quality of L.barbarum fruits reasonably;The accumulation of each ingredient in L.barbarum fruits is affected by the synergistic effects of all the soil fertility factors,not a single role.It would be beneficial to the fertilization and improvement for the quality of L.barbarum.
