1.Research in relationship between storage method and pollution degree for medical devices after cleaning

Objective The study aimed to explore the relationship between storage methods and pollution degrees of two different storage methods for medical devices which can not be packed promptly after cleaning.Methods 300 pieces of surgical devices were randomly divided into the experimental group and the control group according to random digit table,150 pieces in each group.Each group was washed with two cleaning machines of the same brand and model number using the same cleaning protocol,and after cleaning,the experimental group was stored in the cleaning machine,while the control group was exposed in the packaging area.Ten hours later,each surgical instrument was detected using ATP biological fluorescence detector.Results The percent of pass in the experimental group was higher than that of the control group [98% (147/150) vs.86% (129/150)],x2=12.7,P< 0.05.Conclusions Medical devices should better be stored in the washing machine than exposure to the air after cleaning if they could not be packaged and sterilized immediately.
2.Prognostic analysis of primary gastrointestinal diffuse large B-cell lymphoma

Objective To assess the clinical characteristics of primary gastrointestinal diffuse large B-cell lymphoma (PGI-DLBCL) and its treatment and prognosis. Methods Twenty patients diagnosed with PGI-DLBCL were admitted to hospital between 2003 and 2007. The clinical characteristics and tumor molecular model of PGI-DLBCL as well as therapeutic methods were retrospectively studied. The factors that related to survival and prognosis were statistically analysed.Results The overall survival (OS) time of the patients were from 42 months to 52 months,while the progression-free survival (PFS) time were from 37months to 47 months. The International Prognostic Index (IPI) score (0~2 or >2) played a reminding role in prognosis of the disease. Tumor molecular model was no effect on prognosis [that was no significant difference between germinal center B-cell-like (GCB) type and non-GCB type]. The efficacy of Rituximab in combination with CHOP chemotherapy (R-CHOP) in treatment of PGI-DLBCL was similar to CHOP chemotherapy alone, whereas surgical intervention might prolong survival period. Conclusions The biological characteristics of PGI-DLBCL is so particular that the most therapeutic method, which need to be further studied, would be different from DLBCL in other position.
3.Influence of Mixed Decoction of Cortex Phellodendri,Radix Glycyrrhizae and Ramulus Loranthi on Ex-traction Rate of Berberine

OBJECTIVE:To study the infuence of mixed decoction of Cortex Phellodendri,Radix Glycyrrhizae and Ramu?lus Loranthi on extraction rate of berberine.METHODS:The extraction amount from Cortex Phellodendri,Radix Glycyrrhizae and Ramulus Loranthi mixed decoction was determined by HPLC.The HPLC conditions were:Hypersil BDS C 18 column;mobile phase,acetonitrile-33mmol/L KH 2 PO 4 -triethylamine(20∶72∶0.1);detecting wavelength,345nm;column temperature25℃;flow fate1.0ml/min.RESULTS:Decoction of Cortex Phellodendri,Radix Glycyrrhizae and Ramulus Loranthi in combi?nation obviously decreased the extraction rate of berberine.CONCLUSION:Cortex Phellodendri should be extracted separately in formulating extract technic for preparations containing above-mentioned herbs.
4.Clinical study on wound dressing of sustained-release alginate fiber

Objective To research and develop a novel-style wound dressing.Methods 80 patients treated with the new wound dressing of sustained-release alginate fiber were clinically observed.Result Among 90 wounds in 80 patients treated with the dressing,68 wounds healed in grade A,16 in grade B and 6 in grade C.Mean heal-time was 10.6 days.Conclusion The wound dressing of sustained-release alginate fiber is of sound biocompatibility and has many functions in speeding wound healing.
5.Prognosis discussion and complication of total hip arthroplasty in the treatment of ankylosing spondylitis hip joint ankylosis

Objective To explore the efficacy and complication of total hip arthroplasty in the treatment of ankylosing spondylitis hip joint ankylosis,and analyze its prognosis.Methods Eighty-two patients with ankylosing spondylitis hip joint ankylosis were divided into observation group and control group by random digits table method,the 42 patients in observation group were given total hip arthroplasty after the diagnosis of early,the 40 patients in control group were given delayed total hip arthroplasty.In the 2 groups,the recovery of hip joint function were observed,postoperative complication were recorded,and the 5-year survival rate situation were followed up.Results The activity of hip joint,degree of hip joint flexion and Harris score after operation in the 2 groups were significantly better than those before operation,there were statistical differences (P < 0.05),the indexes after operation in observation group were significantly better than those in control group [activity of hip joint:175.25° ± 15.52° vs.122.95° ± 16.26° ; degree of hip joint flexion:85.65° ± 5.33° vs.70.78° ± 4.97° ;Harris score:(86.08 ± 5.41) scores vs.(73.92 ± 6.62)scores],there were statistical differences (P < 0.05).Infection occurred in 1 case,dislocation in 0 case,prosthesis loosening in 2 cases in observation group,the rate of complication was 7.14%(3/42).Infection occurred in 5 cases,dislocation in 2 cases,prosthesis loosening in 4 cases in control group,the rate of complication was 27.50% (11/40).The rate of complication in observation group was significantly lower than that in control group,there was statistical difference (x2 =8.34,P< 0.01).The 1,3-year survival rate in observation group was higher than that in control group,but there was no statistical differences (P > 0.05).The 5-year survival rate in observation group was significantly higher than that in control group [90.48%(38/42) vs.70.00% (28/40)],there was statistical difference (x2 =7.57,P < 0.01).Conclusion Early total hip arthroplasty has significantly clinical effects in the treatment of patients with ankylosing spondylitis hip joint ankylosis,and its early operation curative effect is better than that of delayed operation.
6.The bionic artificial joint capsule study (1)--mechanics simulation.

In this paper, a three-dimensional finite element analysis (FEA) model was created for bionic artificial joint with joint capsule. Finite element method (FEM) was used to calculate and simulate mechanics distribution of the joint capsule under different thickness of the joint capsule, different loading, and different angular displacements. The results of the simulation show that the maximum stress is created in the joint area between artificial joint capsule. And the effect of the thickness of the artificial joint capsule on the stress magnitude and distribution is depend on motion model. On standing situation, the maximum stress decreases with the increase of the thickness of joint capsule. However, on walking situation, the maximum stress increases with the increase of the thickness of joint capsule. Whatever conditions simulated, the maximum stress of the artificial joint capsule is not over the limit of the material strength (9.97 megapascals). All the large stress, which gained from the simulation under different situations, locates at the interface between the capsule and the artificial joint. This is because the artificial joint and the capsule transfer loading each other at the interface. At the same time, supporting area of the capsule at the location of the interface is minimum for the whole vesicle. The stress concentration is inevitable at the interface due to the model structure. This result will offer guidance for the optimum joint structure of the capsule and the artificial joint.

7.Determination of Blood Concentration of Pioglitazone Hydrochloride by HPLC

OBJECTIVE:To establish the method of determining the blood concentration of pioglitazone hydrochloride. 8.Experimental study on apoptosis induced by pcDNA3-survivin-mutant in gastric cancer cell lines

Objective Survivin is overexpressed in gastric cancer. However it not expressed in normal gastric mucosa. The expression of survivin is tightly related to the prognosis of gastric cancer.By gene reconstruction we generated pcDNA3 survivin mutant(Cys84Ala) plasmid, and observed its effect on the gastric carcinoma cell lines. Methods The survivin mRNA and protein expression levels were determined by reverse transcription polymerase chain reaction(RT PCR) analysis,Western blot and immunohistochemical staining respectively . Flowcytometry and acridine orange staning were employed to detect apoptosis. Results Overexpression of survivin mRNA and protein were detected in the gastric cancer cell lines. Inhibition of survivin by survivin mutant cDNA induced apoptosis,activated caspase 3 activity,cleaved PARP and promoted cytochrome C releasing in gastric cancer cells,and effectively sensitized gastric cancer cells to chemotherapeutic agents. Conclusion Inhibition of survivin may induce apoptosis in gastic cancer and sensitize gastric cancer cells to chemotherapeutic agents.Survivin targeted therapeutic protocol may potentially benefit gastric cancer therapy.
9.Effects of gantry acceleration limitations on VMAT plans

Objective:To study the effects of gantry acceleration limitations of a linear accelerator (linac) on the dosimetry of volumetric modulated arc therapy (VMAT) plans, machine efficiency, and dose verification result of VMAT plans and to explore the optimal selection of gantry motion models in the Pinnacle treatment planning system.Methods:Ten cases of nasopharyngeal carcinoma, non-small cell lung cancer, sigmoid adenocarcinoma with retroperitoneal lymph node metastasis, and invasive ductal carcinoma of the breast were each selected for this study. Then two models were set up in the Pinnacle v9.10 treatment planning system, namely the one allowing gantry acceleration and the one limiting gantry acceleration. The same field arrangement, optimized target parameters, and optimized weights of VMAT plans were adopted in the two models, in order to analyze the dosimetric variations in targets and organs at risk (OARs) and compare the differences in treatment time and gamma passing rates.Results:The treatment time of the enrolled patients under the model allowing gantry acceleration was significantly lower than that of the patients under the model limiting gantry acceleration was adopted ( t=-6.751, -0.209, -19.523, -28.999; P< 0.05) and decreased by 15.27%, 18.07%, 19.71%, and 28.75%, respectively. Meanwhile, the conformity and uniformity of target areas were affected, while there was no statistical significance in the gamma passing rates in the validation of VMAT plans ( P>0.05). For the cases of nasopharyngeal carcinoma (NPC), the maximum dose to brainstem PRV increased by 1.25%. For the cases of lung cancer, the maximum dose to the spinal cord and lung V20 increased by 1.19% and 1.21%, respectively, while lung V5 decreased by 1.21%. For the cases of sigmoid adenocarcinoma with retroperitoneal lymph node metastasis, the mean doses to bilateral kidneys, livers, small intestine, and colon all increased. For the cases of breast cancer, lung V10 on the opposite side of cancer increased by 1.66% and the mean dose to the lungs on the same side of cancer decreased by 7.45%. Conclusions:The model allowing gantry acceleration allows the treatment time to be significantly shortened and the treatment efficiency improved. Although this model had the shortcomings such as affecting the conformity and uniformity of target areas to a certain extent and increasing the doses to some OARs, clinical requirements for dosimetry were still met. Therefore, it is recommended to use the model allowing gantry acceleration in the Pinnacle planning system.
