1.Are new resuscitation guidelines better? Experience of an Asian metropolitan hospital.

INTRODUCTIONCardiopulmonary resuscitation (CPR) guidelines were revised in 2005 based on new evidence and expert consensus. However, the benefits of the new guidelines remain undetermined and their influence has not been published in Asia. This study aimed to evaluate the impact of implementing the new resuscitation guidelines and identify factors that influence the discharge survival of out-of-hospital cardiac arrest (OHCA) patients in an Asian metropolitan city.
MATERIALS AND METHODSThis was an observational cohort study of all OHCA patients seen by the emergency medical service during the period before (Nov 2003 to Oct 2005) and after (May 2006 to Oct 2008) implementing the new resuscitation guidelines. Detailed clinical information was recorded using the Ustein style template. Statistical analysis was done using X2 test or t-test for univariate analysis and the logistic regression model for multivariate analysis.
RESULTSThere were 463 patients before and 430 patients after the new guidelines who received resuscitation. The rate of recovery of spontaneous circulation (ROSC), survival-to-intensive care unit (ICU) admission, and survival-to-hospital discharge all showed no benefits regarding the new resuscitation guidelines (ROSC: 42% vs 39%, P = 0.32; Survival-to-ICU admission: 33% vs 30%, P = 0.27; survival-to-hospital discharge: 10% vs 7%, P = 0.09). The rate of ventricular fibrillation/pulseless ventricular tachycardia (VF/pulseless VT), rate of witnessed arrest, and rate of bystander CPR were much lower than in Western studies. After multivariate logistic regression, factors related to discharge survival were witnessed arrest and initial rhythm with VF/pulseless VT. The new resuscitation guidelines did not significantly influence the discharge survival.
CONCLUSIONSWe did not observe any improvement in survival after implementing the new guidelines. Independent factors of survival-to-hospital discharge are witnessed arrest and initial rhythm with VF/pulseless VT. Because the rates of VF/pulseless VT and bystander CPR in Asia are low, popularising CPR training programmes and increasing the rate of bystander CPR may be more important for improving OHCA survival rates than frequent guideline changes.

2.Accuracy of three diagnostic tests used alone and in combination for detecting Helicobacter pylori infection in patients with bleeding gastric ulcers.

OBJECTIVEAccuracy of diagnostic methods for detecting Helicobacter pylori (H. pylori) infection among patients with bleeding peptic ulcers has not been thoroughly investigated. The aim of this study was to compare the diagnostic tests and their combined usage in detection of H. pylori infection in patients with bleeding gastric ulcers and without the use of nonsteroidal anti-inflammatory drugs.
METHODSA total of 57 patients who presented with bleeding gastric ulcers by endoscopy were enrolled. The status of H. pylori was identified by performing the rapid urease test (RUT), histology and (13)C-labeled urea breath test (UBT). The criteria for having H. pylori infection was a minimum of two positive tests.
RESULTSThe prevalence of H. pylori infection in our patient group was 80.7%. Among the three tests used: RUT, histology, and UBT, sensitivities were 56.5%, 97.8% and 100%, while specificities were 100%, 45.5% and 81.8%, respectively. The overall accuracies of the tests were 78.3%, 71.6% and 90.9%, respectively. Although UBT obtained significantly higher accuracy than histology (P = 0.02) as opposed to RUT (P = 0.11), UBT had significantly higher sensitivity than RUT (P < 0.001). In terms of combining any two of the three tests, more accuracy (98.9%) was achieved when both UBT and histology were used to confirm the diagnosis of the other. Conversely, failure to use combined tests generated the potential of missing a proper H. pylori diagnosis.
CONCLUSIONSUBT is superior to the other two tests in bleeding gastric ulcers. RUT lacks sensitivity for detection of H. pylori infection. However, the concomitant use of UBT and histology seems to be more accurate when gastric ulcers present with bleeding.

3.Doxorubicin Promotes Migration and Invasion of Breast Cancer Cells through the Upregulation of the RhoA/MLC Pathway

PURPOSE: Cancer cells develop acquired resistance induced by chemotherapeutic drugs. In this study, we investigated the effects of brief treatment with cytotoxic drugs on the phenotype of breast cancer cells. METHODS: Breast cancer cells MCF7 and BT-474 were briefly treated with paclitaxel or doxorubicin. Clonogenic, migration, and invasion assays were performed on the treated cells. Western blot analysis and RhoA activity assay were also performed. RESULTS: Breast cancer cells when briefly treated with paclitaxel or doxorubicin showed reduced clonogenic ability. Doxorubicin, but not paclitaxel, augmented cell migration and invasion. The invasion-promoting effects of doxorubicin were lost when the two drugs were sequentially used in combination. Myosin light chain (MLC) 2 phosphorylation and RhoA activity were upregulated by doxorubicin and downregulated by paclitaxel. Pretreatment with RhoA inhibitors abolished the migration- and invasion-promoting effects of doxorubicin. CONCLUSION: Doxorubicin activates the RhoA/MLC pathway and enhances breast cancer cell migration and invasion. Therefore, this pathway might be explored as a therapeutic target to suppress anthracycline-enhanced tumor progression.

4.Indoxyl sulfate, homocysteine, and antioxidant capacities in patients at different stages of chronic kidney disease

BACKGROUND/OBJECTIVES:
Increased levels of uremic toxins and decreased antioxidant capacity have a significant impact on the progression of chronic kidney disease (CKD). However, it remains unclear whether they interact with each other to mediate the damage of kidney function. The purpose of this study was to investigate whether uremic toxins (i.e., homocysteine and indoxyl sulfate [IS]), as well as glutathione-dependent antioxidant enzyme activities are dependently or independently associated with kidney function during different stages of CKD patients.
SUBJECTS/METHODS:
One hundred thirty-two patients diagnosed with CKD at stages 1 to 5 participated in this cross-sectional study.
RESULTS:
Patients who had reached an advanced CKD stage experienced an increase in plasma uremic toxin levels, along with decreased glutathione peroxidase (GSH-Px) activity.Plasma homocysteine, cysteine, and IS concentrations were all positively associated with each other, but negatively correlated to GSH-Px activity levels after adjusting for potential confounders in all CKD patients. Although plasma homocysteine, cysteine, IS, and GSHPx levels were significantly associated with kidney function, only plasma IS levels still had a significant association with kidney function after these parameters were simultaneously adjusted. In addition, plasma IS could interact with GSH-Px activity to be associated with kidney function.
CONCLUSIONS
IS plays a more dominant role than homocysteine and GSH-Px activity in relation to kidney function.
5.Perforated Sigmoid Colon Cancer within an Irreducible Inguinal Hernia: a Case Report.

A perforated sigmoid colon cancer within an inguinal hernia is extremely rare. This unexpected finding is usually discovered during surgery and causes an unavoidable septic evolution. Here, we describe the case of an 84-year-old man who presented with fever, abdominal distension, and a painful, enlarged, left scrotum. A CT showed a left, incarcerated, inguinal hernia containing a perforated sigmoid adenocarcinoma (which was confirmed by histopathology). The possibility of an irreducible inguinal hernia in association with perforated sigmoid colon cancer should be considered in the array of diagnoses. A pre-operative CT scan would be helpful in facilitating an accurate diagnosis.

6.01-3 Time-course changes in local and systemic vasomotor activities during a hand warming in young individuals

Objectives: The early local vasodilator response to local warming is predominantly dependent on neural reflexes. However, it is suggested that systemic vasomotor activities are unaffected by a local warming at early stage. The purposes of this study were to assess the hypothesis that systemic vasomotor activities might make an adjustment at early stage of hand warming. Materials and Methods: Thirty-nine young volunteers who were healthy were recruited. Each participant received a right hand bathing at 40°C for 10 minutes. Doppler ultrasound technique was used to monitor the brachial artery mean blood velocity (aMBV) at the heated arm, and changes in aMBV were used to evaluate local vasodilator response to hand warming. Photoplethysmographic technique was used to monitor digital volume pulse (DVP) at the unheated finger, and changes in the DVP derived peak amplitude (DVPampl), reflection index (DVPRI), stiffness index (DVPSI), and heart rate (HR) were used to evaluate vascular distensibility of the unheated upper limb, small artery tone in the lower body, large artery stiffness, and cardiac regulation respectively. The data collected in each minute were averaged as an interval. Comparisons of aMBV, DVPRI, DVPSI, and HR at each interval with their respective baseline values were performed using one-way ANOVA. Results: Plots of one-minute intervals versus aMBV, DVPampl, DVPRI, DVPSI, and HR were shown in Figure. aMBV values presented significant increases with a early peak at the third minute. DVPampl presented significant decrease in the first minute and then reversed to a significant increase at the sixth minute. DVPRI presented a significant increase in the first three minutes, and then returned to the level of baseline. DVPSI and HR did not present any significant changes. Conclusion: At the period of early local vasodilator response to hand warming, systemic vasomotor activities did make a significant adjustment by decreasing vascular distensibility of the unheated upper limb and increasing small artery tone of the lower body, though the activities in heart rate and large artery tone were unaffected. Later, vascular distensibility of the unheated upper limb made a reverse adjustment and reached a significant increase.
7.14-1 Gender differences in seasonal variation of risk factors for acute myocardial infarction in eastern Taiwan

Objectives: The purposes of this study were to determine whether men and women differ in risk factors for acute myocardial infarction (AMI) during the four seasons of the year. Materials and methods: Medical records of 2,086 (women: 693) patients hospitalized with a confirmed AMI were reviewed retrospectively from the region’s only medical center in eastern Taiwan. The onset date of 544 patients (women: 178) was in spring, 493 patients (women: 165) in summer, 474 patients (women: 155) in autumn, and 575 patients (women: 195) in winter. Risk factors of age, percentage of smoking (smoking%), percentage of diabetes (diabetes%), percentage of hypertension (hypertension%), total cholesterol, and body mass index were assessed. In each season, logistic regression model was used to calculate the odds ratio (OR) and 95% conference interval (CI) of women compared to men by risk factors. Results: In spring, women presented significantly higher values in age (OR 1.022; 95%CI 1.012-1.031 ) and diabetes% (OR2.373;95%CI 1.554-3.625), significantly lower in smoking% (OR 0.187;95%CI 0.145-0.240). In summer, women presented significantly lower values in body mass index (OR 0.907;95%CI 0.856-0.960) and smoking% (OR 0.222; 95%CI 0.134-0.367). In autumn, women presented significantly higher values in age (OR 1.033;95%CI 1.012-1.053) and total cholesterol (OR 1.009;95%CI 1.004-1.013), significantly lower in smoking% (OR 0.168;95%CI 0.098-0.289). In winter, women presented significantly higher values in diabetes% (OR 1.845;95%CI 1.250-2.725), hypertension% (OR1.550;95%CI 1.001-2.402), and total cholesterol (OR 1.008;95%CI 1.004-1.012), significantly lower in smoking% (OR 0.188;95%CI 0.119-0.297). Conclusion: The differences between women and men in risk factors for AMI did present seasonal variation in eastern Taiwan. This finding would provide further insight into medical climatology in preventing serious cardiovascular events.

Objectives:
This study aimed to present the Asia-Pacific consensus on long-term and sequential therapy for osteoporosis, offering evidence-based recommendations for the effective management of this chronic condition.The primary focus is on achieving optimal fracture prevention through a comprehensive, individualized approach.
Methods:
A panel of experts convened to develop consensus statements by synthesizing the current literature and leveraging clinical expertise. The review encompassed long-term anti-osteoporosis medication goals, first-line treatments for individuals at very high fracture risk, and the strategic integration of anabolic and anti resorptive agents in sequential therapy approaches.
Results:
The panelists reached a consensus on 12 statements. Key recommendations included advocating for anabolic agents as the first-line treatment for individuals at very high fracture risk and transitioning to anti resorptive agents following the completion of anabolic therapy. Anabolic therapy remains an option for in dividuals experiencing new fractures or persistent high fracture risk despite antiresorptive treatment. In cases of inadequate response, the consensus recommended considering a switch to more potent medications. The consensus also addressed the management of medication-related complications, proposing alternatives instead of discontinuation of treatment.
Conclusions
This consensus provides a comprehensive, cost-effective strategy for fracture prevention with an emphasis on shared decision-making and the incorporation of country-specific case management systems, such as fracture liaison services. It serves as a valuable guide for healthcare professionals in the Asia-Pacific region, contributing to the ongoing evolution of osteoporosis management.
