1.Preliminary study on the plasmids containing genes to encode antibiotic resistance of Shigella Flexneri strains isolated in Vietnam

Recently, treatment for bacillary dysentery have meet many difficulty due to Shigella which antibiotic resistance. Most of Shigella strains also resistant to many antibiotics at the same time (multi-antibiotics). The aim of the research is finding out genes that are encoded antibiotic resistance, features about structure, activity mechanism as well as transmitable of the genes in community. Material and method: researched strains: 100 S.flexneri strains, standard strain: E.coli K12-J5-3. Tecnology: diffused paper slice. The result chosen Sflexneri strains that resistant multi-antibiotic from 237 primary strains. Among them, 100 strains have major resistant type is resistant to 5 kinds of antibiotic. The strains are chosen to receive to E.coli K12J5-3. The percentage of receipt are 56%. Most of receive type don’t get enough resistance to the 5 antibiotics.

3.Infection of Extended-Spectrum β-Lactamase Producing Shigella flexneri in Children Attending a Childcare Center in Korea.

Shigella is a common cause of bacterial enteritis worldwide. Shigella sonnei accounts for 90% of Shigella infections and Shigella flexneri is rarely reported in Korea. Although the incidence of Shigella infection has decreased, the incidence of organisms with antibiotic resistance has gradually increased in Korea. An outbreak of extended-spectrum β-lactamase (ESBL)-producing S. sonnei in children was reported in Korea; however, ESBL-producing S. flexneri has not yet been reported. We report the first two cases of multidrug-resistant CTX-M-14-producing S. flexneri infections in Korean children.

4.Analysis on incidence of bacillary dysentery in Jinan municipality from 1951 to 2005.

OBJECTIVETo explore the epidemic characteristics of bacterial dysentery in Jinan municipality, and to provide scientific basis for effective strategy for bacterial dysentery control.
METHODSThe epidemiological characteristics of bacillary dysentery in Jinan from 1951 to 2005 were analyzed. A total of 485,333 cases in the span of 50 years were recorded, while the population-based case distribution was less than the total cases due to the data incompleteness during the Cultural Revolution.
RESULTSThe incidence of bacillary dysentery in Jinan has been decreasing by years with average incidence rate of 283.10/100,000. The significant differences were observed among the incidence rates of various ages(chi2 = 14.99, P < 0.05). There were four epidemic peaks, and all the incidence rates were about 1000/100,000. Age of onset mainly concentrated in the 0-4 years old, 20-years old and 30-years old. In terms of occupational distribution, workers accounted for 30.31%, the living-scattered children accounted for 22.71%, and the farmers accounted for 17.90%. The incidence focus was from July to September, which accounted for 71.57%. The peak of incidence emerged in August. The highest incidence in urban was 550.94/100,000.
CONCLUSIONThrough the efforts of several generations of health workers, the incidence of bacillary dysentery in Jinan has been basically brought under control. Further step should be taken for the control of bacterial dysentery in urban areas and the management of bacterial dysentery in rural areas. Moreover, the biological characteristics of F2a should be a focus for the future study.

5.Effects of Non-steroidal Anti-inflammatory Drug on Anterior Uveitis.

To evaluate effects of the topical administration of non-steroidal antiinflammatory drug on anterior uveitis, suprofen eye drop was instilled into the left eye of 10 pigmented rabbits and then experimental anterior uveitis was induced by injecting 750 mocro gram/kg endotoxin of Shigella flexneri serotype 1A into their peritoneal cavities. The pupillary diameters were measured, and cell and flare gradings were recorded in 20 eyes of 10 rabbits for one week at an interval of 12 hours for the first day and then daily. Differences between the treatment and control groups were investigated. All the above parameters showed greatest changes at 12 or 24 hours after the injection of endotoxin and became normal by one week. The treatment and the control groups demonstrated statistically significant difference at 12 hours, day 1, and day 2 as for pupillary diameter and at day 1 and day 2 as for cell and at 12 hours and day 1 as for flare. Thus, it is concluded that prostaglandin plays a role in miosis and the appearance of inflammatory cells and flare in anterior uveitis and the topical administration of non-steroidal anti-inflammatory drug can alleviate signs of anterior uveitis. The specific relationship between leucotriene B4 and polymorphonuclear leucocytes influx was not demonstrated.

6.Genome Sequencing Analysis of Atypical Shigella flexneri Isolated in Korea.

OBJECTIVES: An atypical Shigella flexneri strain with a plural agglutination pattern [i.e., reacting not only with serum samples containing type antigen II but also with serum samples containing group antigens (3)4 and 7(8)] was selected for genome sequencing, with the aim of obtaining additional comparative information about such strains. METHODS: The genomic DNA of atypical S. flexneri strain NCCP 15744 was sequenced using an Ion Torrent PGM sequencing machine (Life Technologies, USA). The raw sequence data were preprocessed and reference-assembled in the CLC Assembly Cell software (version 4.0.6; CLC bio, USA). RESULTS: Ion Torrent sequencing produced 1,450,025 single reads with an average length of 144 bp, totaling ~209 Mbp. The NCCP 15744 genome is composed of one chromosome and four plasmids and contains a gtrX gene. Among the published genome sequences of S. flexneri strains, including 2457T, Sf301, and 2002017, strain NCCP 15744 showed high similarity with strain 2002017. The differences between NCCP 15744 and 2002017 are as follows: i) NCCP 15744 carries four plasmids whereas 2002017 carries five; ii) 19 genes (including CI, CII, and cro) were lost in the SHI-O genomic island of NCCP 15744 and six genes were gained as compared with strain 2002017. CONCLUSION: Strain NCCP 15744 is genetically similar to 2002017, but these two strains have different multilocus sequence types and serotypes. The exact reason is unclear, but the 19 lost genes may be responsible for the atypical seroconversion of strain NCCP 15744.

7.Experimental study on the antibacterial effect of origanum volatile oil on dysentery bacilli in vivo and in vitro.

To observe the germistatic and germicidal effects of origanum volatile oil (OVI) on the dysentery bacteria, the abdominal cavity of mice was infected with Shigella sonne (Sh. sonnei) and Shigella flexneri (Sh. flexneri) F2a. After OVI was given to the mice via gastric lavage, the effects of OVI on the infected mice were observed. The minimal inhibitory concentration (MIC) and minimal bactericidal concentration (MBC) for dysentery bacteria were determined in vitro. The results showed that origanum volatile oil showed obvious protective effect on mice infected with Sh. sonnei and Sh. flexneri F2a and it had germistatic and germicidal effects on dysentry bacteria. We are led to conclude that origanum volatile oil is an effective medicine against the infection of dysentery bacteria.

8.Shigella flexneri Inhibits Intestinal Inflammation by Modulation of Host Sphingosine-1-Phosphate in Mice.

Infection with invasive Shigella species results in intestinal inflammation in humans but no symptoms in adult mice. To investigate why adult mice are resistant to invasive shigellae, 6~8-week-old mice were infected orally with S. flexneri 5a. Shigellae successfully colonized the small and large intestines. Mild cell death was seen but no inflammation. The infected bacteria were cleared 24 hours later. Microarray analysis of infected intestinal tissue showed that several genes that are involved with the sphingosine-1-phosphate (S1P) signaling pathway, a lipid mediator which mediates immune responses, were altered significantly. Shigella infection of a human intestinal cell line modulated host S1P-related genes to reduce S1P levels. In addition, co-administration of S1P with shigellae could induce inflammatory responses in the gut. Here we propose that Shigella species have evasion mechanisms that dampen host inflammatory responses by lowering host S1P levels in the gut of adult mice.

9.Characterization of Extended-Spectrum β-Lactamase Genes of Shigella flexneri Isolates With Fosfomycin Resistance From Patients in China.

BACKGROUND: The emergence of fosfomycin resistance and extended-spectrum β-lactamase (ESBL) genes is a serious threat to public health and a new challenge in shigellosis treatment. The purpose of this study was to identify fosfomycin resistance and characterize β-lactamase genes in fos-carrying isolates of Shigella flexneri from patients in China. METHODS: A total of 263 S. flexneri isolates were collected from 34 hospitals in the Anhui Province of China during September 2012-September 2015 and screened for fosA3, fosA, and fosC2 by PCR amplification and sequencing. The fos-carrying isolates were then screened for β-lactamase genes. The clonal relationships between fosA3-carrying isolates, the transmissibility of fosfomycin resistance, replicon types of plasmids carrying fosfomycin resistance genes and other associated resistance genes were investigated. RESULTS: Twenty-five of the 263 isolates (9.5%) showed resistance to fosfomycin, and 18 (6.8%) were positive for fosA3. None of the isolates was positive for fosA or fosC2. Seventeen of the isolates carrying fosA3 (94%) were CTX-M producers (seven CTX-M-55, five CTX-M-14, and five CTX-M-123), while three (16.7%) were TEM producers (TEM-1).Sixteen (88.9%) fosA3-carrying isolates exhibited multi-drug resistance. The replicon types of the 13 fosA3-carrying plasmids were IncF (n=13), IncHI2 (n=3), IncIl-Ir (n=2), and IncN (n=1). CONCLUSIONS: Our results indicated that fosA3 could spread through plasmids in S. flexneri isolates, along with the bla(CTX-M) and bla(TEM), which facilitate its quick dispersal. To the best of our knowledge, this is the first report of CTX-M-123-type ESBLs in S. flexneri isolates from patients in China.

10.Shigella flexneri bacteremia: A case report.

Shigella bacteremia occurs so rarely that blood culture is useless for the laboratory diagnosis of dysentery. S flexneri type 2 was isolated from a blood culture of a 3-year-old boy with clinical diagnosis of dysentery. A stool culture was negative for not only shigella but also other pathogenic bacteria. This was the only shigella-positive blood culture during the last 12 1/2 years although more than 1,200 cases of bacteriologically proven dysentery were encountered. One of the 4 bottles inoculated with 2 blood samples drawn on the 4th day of illness yielded numerous shigella and few Klebsiella pneumoniae colonies on subculture. On admission the patient was a moderately nourished boy with body temperature of 38 degrees C. The leukocyte count was 10,200/microliter with 29% neutrophils. No evidence of septicemia was noted. He was placed on antibiotics and fluid replacement. The patient was discharged in 6 days after full recovery.
