4.Fiber Choledochoscopic Plasma Shock Wave Lithotripsy with Inverse Flush for Difficult Bile Duct Residual Stones
Chinese Journal of Minimally Invasive Surgery 2015;(2):170-172
[Summary] The article reported 107 cases of bile duct residual stones treated by plasma shock wave lithotripsy with inverse flush under fiber choledochoscope via T-tube fistula.Of the 107 patients, stones were completely removed in 98 patients, and residual stones remained in 9.Intraoperative biliary hemorrhage occurred in 5 cases, which were stopped after spraying the norepinephrine . There were 4 cases of arhythmia , 13 cases of subcostal or upper abdominal pain , and 11 cases of abdominal distention and diarrhea , all of which were cured by symptomatic treatment .Postoperative biliary infection occurred in 7 cases, which were cured by anti-infection and biliary drainage.Except for 1 patient of lost of follow-up, 106 patients were followed for 6 months.Ultrasound and CT examinations showed free of calculus in 101 cases, and intrahepatic bile duct residual stones in 5 cases. We deem that plasma shock wave lithotripsy combined with reverse flushing under fiber choledochoscope is a minimally invasive , safe, and effective treatment for difficult biliary residual stones .

Objective To investigate the influence of Tangshenbao Decoction on oxidation irritable reaction and pathological changes of renal cortex in diabetic rats. Methods The diabetic models were made by the intraperitoneal injection of streptozotocin. Then the rats were treated with Tangshenbao Decoction (Chinese herbs group) and Ramipril (western medicine group) respectively for eight weeks. Observe the oxidation irritable reaction and pathological changes of renal cortex in diabetic rats. Results The Chinese herbs group can decrease the urine microalbumin, improve the activity of SOD in renal cortex, reduce the content of MDA, cut down the MGA of renal glomerulus, and has obvious effects in mitigating the oxidation damages. Conclusion Tangshenbao Decoction can decrease the excretion of urine albumin in 24 hours, mitigate the oxidation damages of kidney tissue, lighten the pathological damages of kidney, while the oxidation damages in kidney tissue may play an important role in the genesis and development of diabetic nephropathy.

Objective To explore the possibility of bone marrow mesenchymal stem cells (MSCs) differentiating into active melanocytes in vitro. Methods Bone marrow stromal cells were harvested from femoral marrow of 6-week-old black male C57BL/6 mice, and subjected to a primary culture. After 6-passage subculture,an induction medium containing hydrocortisone, recombinant human insulin, transferrin and fibroblast growth factor was used to induce the differentiation of MSCs into melanocytes.Inverted light microscopy was applied to observe the process of cell differentiation, transmission electron microscopy to observe melanosome formation and maturation, and immunocytochemistry to determine the expression of melanocyte-associated epitopes, and flow cytometry to analyze cell cycles and yield of differentiated melanocytes. Results The total number of MSCs was close to 109 after 6 passages of subculture, and immunofluorescent studies showed an expression rate of 94.3% for CD44 and 82.3% for CD105 in these MSCs.After 180-day cultivation in the induction medium, the MSCs showed a morphological similarity to melanocytes with an increase in dendrites, formation of melanosome-like structures, and cell growth cycle was shortened to 3-4 days.Brown/black cell sediments were visualized by naked eyes.Electron microscopy revealed that intracellular melanosomes were mainly in Ⅳ phase.Immunofluorescence studies of the differentiated melanocytes showed a positive staining for tyrosinase related protein-1 (TRP-1),TRP-2,and microphthalmia-associated transcription factor (MITF).Flow cytometric analysis showed that most of the melanocytes differentiated from the MSCs were in G1 and S phases, and TRP-1-positive melanocytes amounted to 80% of gate cells. Conclusions Bone marrow MSCs can be largely differentiated into melanocytes with a close similarity to normal melanocytes in morphology, ultrastructure and specific epitopes and a certain degree of proliferative activity.

ObjectiveTo analysis the subsets of the lymphocyte and dendritic cells (DC) in peripheral blood in patients with neurosyphilis and investigate the relationship between these cells and onset of neurosyphilis.MethodsThe subsets of CD4+T cells as well as DCs and the counts of CD4+T and CD8+T were analyzed by flow cytometry with immunofluorescent staining in 8 patients with neurosyphilis and 10 healthy controls.ResultsIn peripheral blood of neurosyphilis patients the proportion of CD4+T cell was significantly higher than that in the normal controls ( P<0.01); the proportion of CD8+T cell was significantly lower than that in the normal controls ( P<0.05). The proportion of Th1, Th2 and the ratio of Th1/Th2 in neurosyphilis patients was (14.12±5.12)%, (1.52±0.88)% and (12.05±5.62), respectively. In normal controls, it was (26.10±4.98)%, (0.99±0.35)% and (31.62±16.62) respectively. The expression of Th1 and the ratio of Th1/Th2 in neurosyphilis patients was significantly lower than the normal controls (P<0.05). The proportion of DC1 and DC2 in neurosyphilis patients was not significantly different from that in normal controls ( P>0.05). There was a significant correlation between DC1 and Th1, DC2 and Th2 in the neurosyphilis patients ( P<0.051), but no correlation between the proportion of DCs and the ratio of Th1/Th2 ( P>0.05).ConclusionThe cellular immune function of neurosyphilis patients may be inhibited, and there is no relationship between the proportion of DCs subset and unbalance of Th1/Th2.

To use Agrobacterium rhizogenes-induced hairy roots to create new germplasm of Dianthus caryophyllus, we transformed D. caryophyllus with A. rhizogenes by leaf disc for plant regeneration from hairy roots. The white hairy roots could be induced from the basal surface of leaf explants of D. caryophyllus 12 days after inoculation with A. rhizogenes ATCC15834. The percentage of the rooting leaf explants was about 90% 21 days after inoculation. The hairy roots could grow rapidly and autonomously in liquid or solid phytohormone-free MS medium. The transformation was confirmed by PCR amplification of rol gene of Ri plasmid and silica gel thin-layer chromatography of opines from D. caryophyllus hairy roots. Hairy roots could form light green callus after cultured on MS+6-BA 1.0-3.0 mg/L + NAA 0.1-0.2 mg/L for 15 days. The optimum medium for adventitious shoots formation was MS + 6-BA 2.0 mg/L + NAA 0.02 mg/L, where the rate of adventitious shoot induction was 100% after cultured for 6 weeks. The mean number of adventitious shoot per callus was 30-40. The adventitious shoots can form roots when cultured on phytohormone-free 1/2 MS or 1/2 MS +0.5 mg/L NAA for 10 days. When the rooted plantlets transplanted in the substrate mixed with perlite sand and peat (volume ratio of 1:2), the survival rate was above 95%.
