ObjectiveTo compare the clinical effectiveness and adverse effects following low doses versus traditional doses of amphotericin B liposome (L-AmB) in the treatment of patients with invasive pulmonary fungal infections (IPFI) after renal transplantation.MethodsA total of 26 postrenal transplantation patients with IPFI between Jan. 2005 and Mar. 2011in Zhujiang hospital received L-AmB treatment identified low doses group (0.2-0.5 mg·kg-1·d-1,n =19) or traditional doses group (1-5 mg· kg-1,d-1,n =7) were reviewed.ResultsThe treatment duration in low doses group and traditional doses group was 20.3 +12.7 and19.3 ±13.2 days respectively (P＞0.05).The effective rate in low doses group and traditional doses group was 84.2％ and 57.1％ respectively (P＞0.05).The overall dosage was significantly less in the low doses group (414.7 ± 241.7 mg) than in the traditional doses group (1158.8 ± 928.0 mg) (P＜0.05).The incidence of adverse effect was significantly lower in the low doses group than in the traditional doses group (21.1％ vs.85.7％,P＜0.05).ConclusionThe effectiveness of low doses of L-AmB protocol in the treatment of IPFI postrenal transplantation patients was similar to that of traditional doses of L-AmB protocol,but the incidence of adverse effects in low doses of L-AmB protocol was significantly lower.

Objective To systematically evaluate the application of problem-based learning (PBL)in the teaching process of health management major in China.Methods Databases including CNKI (1979 to December the 2015),VIP (1989 to December the 2015),Wanfang (1982 to December the 2015) and PubMed were systematically retrieved.Any literature about PBL of health management major was included.Seffdeveloped data extraction form was used for collecting the information.Data were input and analyzed using Excel 2007.Results 15 papers were included in the analysis.The first author mainly came from the northeast region of China (7).Most (10) papers did not get the funding support.No paper was published in the journals contained in the Guide to the Core Journals of China.6 papers of experimental studies compared the performance difference of students of the PBL and lecture-based learning (LBL),which was statistically significant.Conclusion PBL is superior to the LBL.However,due to the large difference in the quality of literature,more studies were needed to determine the effect of PBL.Moreover,we should pay attention to the combination of PBL and LBL.

Objective SELDI-TOF-MS technology was used to contrastly analyse the changes of protein expression profiles of gastric cancer cells after TGF-?1 stimulation,and provide theoretical and experimental foundation for screening different significant proteins. Methods Serial subcultivation gastric cancer cells BGC823,MKN45,and SGC7901 cultured in vitro were grouped into test and control groups.TGF-?1 was added to the test group,but not to control group.Cell culture fluid was collected and centrifuged after cultured 24h,and crossing with WCX2 protein chip to detect protein differences. Results When the test group was compared with control group,we found:(1) thirteen different proteins in BGC823 cells after TGF-?1 stimulaton,and their M/Z were M4294,M4932,M4945,M4972,M4991,M5015,5036,M5060,M5153,M5180,M5197,M8577,and M8784,respectively;(2) eighteen different proteins in MKN45 cells after TGF-?1 stimulaton,and their M/Z were M4292,M4931,M4945,M4972,M4990,M5014,M5152,M5178,M7055,M8190,M8570,M8652,M8670,M8780,M9963,M10098,M10523,and M11653,respectively;(3) eight difference proteins in SGC7901 cells after TGF-?1 stimulation,and their M/Z were M4945,M4972,M4992,M5015,M5180,M7056,M8573,and M8604,respectively. By comparing three protein expression profiles of gastric cancer cells after TGF-?1 treatment,we found two significcant proteins with common differences,that had M/Z of M4945 and M4972,respectively. Conclusions The biological markers whose M/Z is M4945,M4972 with gastric cancer characteristic and associated with TGF-?1 have been screened,which can be as the basis for early prediction and clinical diagnosis research on metastasis and invasiveness of gastric cancer.

The mRNA and protein expression of thymidylate synthase (TS), thymidine phosphorylase (TP) and dihydropyrimidine dehydrogenase (DPD) and their relationship with prognosis were investigated. Real-time quantitative RT-PCR (Taqman) was used to detect the mRNA expression of TS, TP and DPD in formalin-fixed and paraffin-embedded 106 samples of epithelial ovarian cancer and 29 normal ovaries. A TATA box-binding protein (TBP) was used as an endogenous reference gene. A relationship between TS, TP, DPD expression and clinicopathologic features was investigated. The protein location and expression of TS, TP and DPD was examined in the same patients by an avidin-biotin-peroxidase immunohistochemistry. TS and TP mRNA expression levels were significantly higher in tumor group than in normal controls, with the average value of TS and TP mRNA being 6.14+/-0.62 and 0.59+/-0.06 in tumor tissue, and 0.71+/-0.14 and 0.16+/-0.04 in normal tissue, respectively. DPD mRNA expression levels were significantly lower in tumor group (0.11+/-0.02) than in normal controls (0.38+/-0.05). There was statistically significant difference in TS and TP mRNA expression levels among different pathological grades and clinical stages (P<0.05), but histological subtype was not significantly associated with TS and TP mRNA expression. DPD gene expression was not significantly associated with any clinicopathological parameters. Immunohistochemistry revealed that TP protein was mainly distributed in nucleus, and TS and DPD mainly in cytoplasm. The protein expression intensity of TS, TP and DPD was coincided with the mRNA expression levels. It was concluded that TS, TP mRNA and protein expression levels were significantly higher in epithelial ovarian cancer, and DPD mRNA and protein expression levels were significantly lower. The expression levels of TS and DPD were related to the patients' prognosis and survival. Combined gene expression levels of TS, TP and DPD represent a new variable to predict the clinical outcome in ovarian cancer. The association of TS, TP and DPD expression levels with survival suggests an importance of these genes for tumor occurrence and progression.

Background The potential protective effect of ischemia postconditiomng for hypoxia has been determined in many tissues and organs.But its mechanism in retina is still lack.Objective The aim of this study was to investigate the protective effect of ischemia postconditioning on retinal ischemia reperfusion injury.Methods Thirty-six health male Wistar rats were randomized into the normal control group,sham group,ischemia reperfusion group and ischemic postconditioning group.Retinal ischemia reperfusion models were induced by elevating the intraocular pressure to 110 mmHg for 60 minutes,and then the ischemic postconditioning was performed right away,1 minute and 10 minutes after ischemia in the rats of the ischemic postconditioning Ⅰ,Ⅱ,Ⅲ group,respectively.Electroretinogram (ERG) was recorded in the rats 7 days after operation,and the rats were sacrificed by excessive anesthesia.Retinal specimens were prepared for the histopathological examination.The retinal thickness and the recovery rates of ERG a,b waves and oscillation potentials (OPs) were compared among the groups using ANOVA of SPSS software.Results One day after experiment,the structure of rat retinas was nearly normal.However,retinal edema and vacuolar degeneration were seen in the rats of the ischemia reperfusion group and ischemic postconditioning Ⅰ,Ⅱ,Ⅲ groups primarily in the inner plexiform layer (IPL),inner nuclear layer (INL).The retinal thickness values of the entire layer,IPL and INL were significantly higher in the ischemia reperfusion group and ischemic postconditioning Ⅰ,Ⅱ,Ⅲ groups compared with the normal control group (all at P＜0.05).Seven days after experiment,the retinas were thinner in the rats of the ischemia reperfusion group than those of the rats in the sham group,but the retinal thickness values in the ischemic postconditioning Ⅰ,Ⅱ,Ⅲ groups were significantly increased in comparison with the ischemia reperfusion group (all at P＜0.05).In addition,the recovery rates of ERG a,b,OPs amplitudes were significantly lower in the ischemia reperfusion group and ischemic postconditioning Ⅰ,Ⅱ,Ⅲ groups compared with the normal control group (all at P＜0.05).However,the recovery rates of ERG a,b,OPs amplitudes evidently elevated in the ischemic postconditioning Ⅰ,Ⅱ,Ⅲ groups in comparison with the ischemia reperfusion group (all at P＜0.05).Conclusions Ischemia postconditioning can protect the structure and function of retina from ischemia reperfusion injury.Ischemia postconditioning plays the maximal effect immediately to 1 minute after ischemia reperfusion injury in Wistar rat.

Objective To establish a method for culturing neonatal mice cardiomyocytes, and construct an in vitro model of cardiomyocytes for assessing the cardiac toxicity of chemicals. Methods Hearts of neonatal mice of 1 day old were digested with enzyme mixture of trypsin/collagenase type Ⅱ/dispase and the cell suspensions were pre-plated to flask for a short time and then seeded on coated dishes. The cultured cells were treated with 5-fiuorine-deoxy-uridine(15 μg/ml)and uridine(35 μg/ml)to enrich cardiomyocytes that were identified according to the morphology and immunocytochemistry of α-actin antigen. Cardiac myocytes were incubated with 0-64 μg/ml of a fusarium mycotoxin butenolide(BUT) for 12 h. Cell viability was then evaluated by MTT assay. Microscopic observation showed that BUT induced significant morphological changes including cellular swelling, vacuolation and breakage of muscle fibers. Results It was found that 96% of the cultured cells were cardiomyocytes and the myocytes kept beating after 90 days of culture. Concentration-dependent decreases in cell viability following exposure of cardiac myocytes to BUT were observed. Conclusion The results indicated that relatively pure primary culture of neonatal mice cardiomyocytes is successfully established. BUT possesses the potential to induce myocardial toxicity.

ObjectiveTo prospectively compare the effects of Prestige LP cervical disc replacement with those of anterior cervical discectomy and fusion (ACDF) in treatment of cervical disc herniation.MethodsFrom January 2008 to March 2010,a total of 87 patients were enrolled at our site as study investigating ACDF versus Prestige LP cervical disc prosthesis.Forty-four patients received the investigational device (arthroplasty group) and 43 underwent a single-level ACDF (fusion group).Visual analogue scale(VAS) neck/arm pain,Japanese Orthopedics Association (JOA) score,Short Form-36 (SF-36) both physical and mental,flexion-extension range of motion of operative and adjacent segments were evaluated preoperatively and 1 week and 3,6,12,24 months postoperatively.ResultsA total of 78 patients (89.7％) were followed up for an average 12.4 months(range,6-24).Both groups have favorably demonstrated improved functional outcomes for JOA,VAS scores and SF-36.The improvement in the VAS and JOA scores were equivalent at each follow-up point between the two groups(P＞0.05).However,arthroplasty group had statistically significant improvement as assessed by the SF-36 scores compared with the fusion group at some follow-up points (P＜0.05).In arthroplasty group,the postoperative range of motion of operative and adjacent segments showed no statistical difference with the preoperative counterpart(P＞0.05),and fusion was successful in 92.1％ of control patients.ConclusionPrestige LP cervical disc replacement is a feasible alternative to ACDF for patients with persistently symptomatic cervical disc disease and matching the inclusion criteria.

A total of 40 patients with COPD (excluding those with correlated/relevant diseases)were measured for inflammation parameters of erythrocyte sedimentation rate (ESR) and c-reactive protein (CRP) after hospital admission and some coagulation/fibrinolysis parameters including D-dimer,thrombinantithrombin (TAT),prothrombin fragment 1 + 2,(tissue plasminogen activator) tPA,plasminogen activator inhibitor 1 (PAI-1),von Willebrand factor (von WF),endothelin receptor A,thromboxane B2,P-selectin and pulmonary arterial pressure (PAP) by ultrasonic cardiography after the settling of the symptons of acute period.All patients were then divided into 2 groups according to PAP [＜ 40 mm Hg (1 mmHg=0.133 kPa) (n=24),＞40 mm Hg (n=16)].The values of CRP and ESRin the group with PAP ＞ 40 mm Hg were significantly higher than those in another group (P =0.044 and P =0.002respectively) while tPA was lower (P =0.04).A moderate positive correlation existed between tPA and TXB2 (r =0.547).Moreover,a highly positive correlation was found between TXB2 and PAl-1 (r =0.929).The results indicated that the COPD patients with pulmonary arterial hypertension (PAH) tend to have a higher level of inflammation,and their fibrinolysis becomes impaired leading to a prothrombotic state.

In industrial glutamate fermentation, intermitted feeding glucose with the help of off-line glucose measurement is generally necessary. This kind of feeding strategy could cause large variations in glucose concentration so that it is not favorable for the achievement of efficient and stable glutamate fermentation. Glutamate fermentation is characterized with typical non-growth association behavior, and during glutamate production phase glucose consumption is closely correlated with ammonia consumption. In this study, glucose concentration was controlled at various pre-determined levels by predicting glucose consumption amount and thus its concentration with the aid of on-line monitoring ammonia consumption. When glucose concentration was controlled around a lower level of 5 g/L~10 g/L, the final glutamate concentration could reach a relatively higher level of 80 g/L. In this way, the huge osmotic stress change due to the large glucose concentration variation with the intermitted feeding method could be avoided and the glutamate fermentation performance enhancement be expected.

Objective To evaluate the quality status of recombinant human interferon α1b injection and find out some quality problems.Methods Totally 31 batches of recombinant human interferon α1b for injection and 11 batches of recombinant human interferon α1b injection from two enterprises were examined according to Chinese Pharmacopoeia Volume Ⅲ (2010),and the quality status of recombinant human interferon α1b injection was evaluated by statistical analysis of the results.Results All 42 batches of samples were qualified.The production process of each enterprise was steady.Conclusion At present the quality of recombinant human interferon αlb injection is generally good.The current standards are feasible,but the specified standard of osmolality needs to be improved.
