3.A clinical study on indomethacin for prevention of heterotopic ossification following surgical treatment of acetabular fractures

Objective To investigate the effect of indomethacin on prevention of heterotopic ossification (HO)after operative treatment of acetabular fractures.Methods Fifty patients with acetabular fractures received in our department operative treatment through Kocher-langenbeck(K-L)approach and oral administration of in- domethacin afer operation from February 2001 to August 2003.Forty-eight of them were successfully followed up for incidence of HO and their clinical functions were assessed.The results were compared with those of 40 patients who had been treated with the same operative procedures but without oral administration of indomethacin in our depart- ment from March 1993 to May 1998.The patients who could not tolerate the drug were not included.Results The follow-ups averaged 22.8 months(range,6 to 39 months).HO occurred in eight cases.The incidence of HO was 16.7%(8/48).According to Brooker evaluation of HO,five cases were rated as degreeⅠ,three as degreeⅡ, and none as degreeⅢorⅣ.The incidence of severe HO was 0.In the control group,the incidence of HO was 35.0%(14/40)and the incidence of severe HO was 10.0%(4/40).The differences were statistically significant (P<0.05).Conclusion Oral administration of indomethacin after operative treatment of acetabular fractures can inhibit HO.
4.Effects of quercetin on H2 O2 - induced oxidative damage in human retina pigment epithelium cells

AIM: To discuss the protective effects and possible mechanisms of quercetin in oxidative damage of human retina pigment epithelium ( RPE) cells induced by H2 O2 .METHODS: RPE cells were subculture, and they were divided into negative control group: cultured with normal culture medium;oxidative injury group:100 μmol/L H2 O2 treated for 12h; quercetin low dose group: 100 μmol/L quercetin incubated for 24h then treated with 100 μmol/L H2 O2 for 12h; and quercetin high dose group:100 μmol/L quercetin incubated for 24h then treated with 100 μmol/L H2 O2 for 12h. Cell viability were tested by MTT colorimetric detection, apoptosis rate was detected by flow cytometry, apoptotic cell morphology was observed by Hochest33258 staining, expression of catalase (CAT), superoxide dismutase ( SOD) and glutathione peroxidase ( GSH-Px) were tested by colorimetric detection.
RESULTS: Quercetin inhibited H2 O2 - induced cell viability decreased in RPE cells, after treated with different concentrations of quercetin, RPE cells activity increased to (79. 67±4. 98)% and (83. 00±3. 60)%, which had statistical significance difference compared with oxidative damage group (48. 93±3. 39)% (P<0. 05). After treated with different concentrations of quercetin, the apoptosis rate of RPE cells decreased to (23. 23±3. 29)% and (16. 23±1. 94)%, respectively, which had statistical significance difference compared with oxidative damage group (38. 03±4. 76)%(P < 0. 05 ). In addition, quercetin also increased the expression of CAT、SOD、GSH-Px in RPE cells, which had statistical significance difference compared with oxidative damage group.
CONCLUSION:Quercetin effectively inhibited H2 O2 -induced RPE cells damage by improving cell antioxidant enzyme activity, which provide reliable experimental basis for the treatment of injuries in RPE cells.
5.Comparative analysis of laser confocal microscopy and corneal smear in the diagnosis of fungal keratitis

AIM: To comparatively analyze the application of laser confocal microscopy and corneal smear in the diagnosis of fungal keratitis.METHODS: Totally 77 patients (77 eyes) diagnosed as fungal keratitis were selected.Laser confocal microscopy and corneal smear examination were performed to observe the characteristics of the images, and the detection rate of fungus were compared between the two methods.RESULTS: Of the 77 patients, 66 eyes (86%) were positive and 11 eyes were negative detected by laser confocal microscopy;51 eyes (66%) were positive and 26 eyes were negative detected by corneal smear examination, the difference was statistically significant compared between two group (P<0.05).CONCLUSION: Laser corneal confocal microscopy is relatively safe and sensitive.If combined the application of laser corneal confocal microscopy and corneal smear examination in clinical, the diagnosis rate of fungal keratitis may be improved.
6.Blood-saving effect of acute plateletpheresis and back-transfusion in patients undergoing open heart surgery with CPB

ObjectiveTo assess the blood-saving effect of acute plateletpheresis (APP) and back-transfusion in patients undergoing open.heart surgery with CPB.MethodsThirty ASA Ⅱ or Ⅲ patients aged 41-63 yr weighing 52-72 kg undergoing open heart surgery with CPB were randomly divided into 2 groups (n =15 each):control group (group C) and APP group.In APP group,platelet-rich plasma was harvested immediately after induction of anesthesia and transfused back to the patient after termination of CPB and neutralization of heparin with protamine.In group C plateletpheresis was not performed.Hb,Plt,PT,APTT and Fib were measured before induction and at 1,24 and 48 h after operation.CPB time,aortic crossclamping time,postoperative chest tube drainage and blood product transfusion requirements were recorded.ResultsIn APP group the volume of the whole blood processed for plateletpheresis was (1285 ± 185) ml and ( 192 ± 38) ml of platelet-rich plasma was sequestered.The platelet count of the sequestered plasma was(817 ± 282) × 109/L,accounting for (21 ± 3) % of the total number of Pit in the whole blood volume.The plateletpheresis took (35 ± 10) min.The platelet count at 1 h after operation was significantly higher in APP group than in group C.The volume of postoperative chest tube drainage at 24 h after operation was significantly lower in APP group than in group C.Less allogeneic RBC and Plt were transfused in APP group than in group C.There was no significant difference in other variables between the 2 groups.ConclusionAPP has blood-saving effect in patients undergoing open heart surgery with CPB.

8.Perutaneous pedicle screw fixation in treatment of thoraco-lumbar fractures

Objective To study the feasibility of percutaneous pedicle screw fixation in treatment of thoraco-lumbar fractures.Methods Twenty-two patients with thoraeo-lumbar fractures were treated with the posterior percutaneous or open pedicle screw fixation respectively.Operation time,the bleeding volume,the height of the anterior border,the cobb angles of seoliosis,the late loss of correction,and complications were compared.Results All patients were followed up for mean 19 months(13 to 22 months).In percutaneous group,the mean time of operation was 102 minutes,the anterior height of compressed vertebral bodies was restored from 52% to 95% of the normal height,and the Cobb angle was corrected from 16?to 6.3?,the mean late loss of correction was 13% in percutaneous group,and there were no significant differences between the two groups(P>0.05).The mean bleeding volume in the percutaneous group and open group was 102 ml and 290 ml respectively,which showed significant difference(P<0.05).Conclusion Posterior percutaneous pedicle screw fixation in treatment of thoracolumar fractures,which didn't need vertebral canal decompression,was a perfect and effective method.Compared with open method,it had such strongpoints as quicker recovery and less invasive.
9.Clinical observation on influence of vertebral fixation through or across the affected vertebra on vertebral morphology.

OBJECTIVETo compare the long-term influence of vertebral fixation through or across the affected vertebra on vertebral morphology.
METHODSClinical data of 48 patients with simple thoracic and lumbar spinal fractures who were admitted between Jan. 2008 and Dec. 2010 were analyzed retrospectively. Among them 36 cases (28 males and 8 females) were fixed through the injured vertebra (group A) and 12 cases (8 males and 4 females) were fixed across the injured vertebra (group B). All patients were followed up for 6-36 months (mean 11.5 months). The vertebral body height, endplate angle and neurofunction were compared between the two groups before surgery, a week after surgery and at the end of the follow-up period.
RESULTSThere was no statistically significant difference in vertebral body height,endplate angle and neurofunction before operation between group A and B (P > 0.05). Vertebral body height and endplate angle improved in both groups a week after operation and at the end of the follow-up period as compared with those before operation (P < 0.05), and the efficacy in group B was significantly better than that in group A (P < 0.05). There was no significant difference in neurofunction between the two groups (P > 0.05).
CONCLUSIONThe fixation method through the injured vertebra had a better reduction effect, more stable fixation, and a better long-term effect on vertebral morphology than that across the injured vertebra in the treatment of thoracolumbar vertebral fractures.

10.Evaluation on outcome of iodine deficiency disorders control in Hubei Provinvce in 2007

Objective To investigate the current situation of the prevention and contml of iodine deficiency disorders(IDD)in Hubei Province,SO as to provide a policy.making basis for controlling work of IDD. Methods Using the method of proportional population sampling(PPS),30 eounties were seiected in Hubei Province.In each selected county,1 primary school was chosen.In every primary school,40 pupil8 aged 8~10 years were selected to examine thyroid size,intelligent quotient(IQ),and salt iodine contents at their home.In the selected pupils,2 boys and 2 girls were chosen to determine their urinary iodine contents in each age grouD.Twenty pupils in the above school and 5 housewives ileal"to this school were tested in health education questionnaire. Results The median of salt iodine was 30.1 mg/kg,and the rate of comsuming qualified iodized salt was 96.2% (1154/1200).The rates ofchild goiter were 6.5%(78/1200)by palpation and 4.1%(49/1200)by B ultrasound.The median of urinary iodine Was 358.4μg/L and mean of the IQ was 105.3±14.4.The rate of qualified scores of both students and housewives Was 25.5%(153/600)、90.7%(136/150).Conclusions The current Bituation of iedine nutrition is good.The goal of eliminating IDD has been achieved in Hubei Province.
