1.Clinical analysis of 23 cases of histiocytic necrotizing lymphadenitis presented as fever of unknown origin

We retrospectively analyzed 23 cases of histiocytic necrotizing lymphadenitis (HNL) admitted to our hospital as fever of unknown origin (FUO).Fever,lymphadenopathy and skin rash were the most common clinical manifestation.The major laboratory features included cytopenia,abnormal liver enzyme and elevated inflammatory markers.Three of the 23 cases were finally diagnosed as infectious disease,5 as autoimmune disease,while the left 15 remained as HNL during the follow-up.Four of the 15 HNL patients resolved spontaneously,while the other 11 relieved by steroid use.Two patients relapsed during the follow-up.HNL was one of the rare causes of FUO.Since it would be accompanied with infectious,autoimmune or malignant diseases,long follow-up is necessary.
2.The clinical analysis of three methods in the treatment of intracranial bacterial infection

Objective To analyze the effect of three therapeutic methods to find an optimal approach to the treatment of intracranial bacterial infection by retrospectively reviewing 33 intracranial bacterial infection patients who were admitted from 1995 to 2008 in oar hospital. Methods The treatments by intermittent lumbar puncture,continuous lumbar subarachnoid space drainage, and embedment of Ommaya cyst for continuous drainage from the ventricles were performed in 15 cases, 12 cases, and 6 cases respectively along with intravenous application of full dose of antibiotics. Results Nineteen cases were cured and the best prognosis was from the group of Ommaya cyst embedment and continuous drainage from the ventricles. Conclusion Management goals are prompt recognition of the central nervous system (CNS) infection, rapid identification of causative organisms and initiation of treatment with the optimal management methods for complications. Embedment of Ommaya cyst for continuous drainage from the ventricle is a safe and effective treatment for intracranial bacterial infection.
4.Noticeable problem in diagnosis and treatment of drug-induced keratopathy

Ocular surface disorders can be caused by the drug-derived toxicity,adverse effect and abuse of eye drops,and usually these disorders present with unspecific and untypical clinical findings.So the eye abnormality induced by topical administration of eye drops often is confused with some corneal inflammatory diseases.In addition,pathogenic mechanisms vary widely due to the different types of eye drops.Therefore,difficulty of diagnosis and poor curative effect often is a challenge for us.Ophthalmologist should be aware of the toxicity derived eye drops on ocular surface.Several noticeable problems of diagnosis and treatment of drug-derived keratopathy were elaborated in this paper.
5.Primary evaluation of collateral flow of anterior circulation cerebralvascular atherosclerosis under neurovascular ultrasound examination

9.Effect of enteral nutrition treatment on metabolism and serum adiponectin level in elderly patients with metabolic syndrome

Objective To observe the effect of enteral nutrition therapy on metabolic status and adiponectin levels in elderly patients with metabolic syndrome (MS) complicated with nonalcoholic fatty liver disease (NAFLD).Methods 92 elderly hospitalized patients with mild to moderate non-alcoholic fatty liver disease underwent enteral nutrition (EN,n=46) and total parenteral nutrition (TPN,n=46) for 2 months.Body mass index,triceps skin-fold thickness,waist hip ratio,serum high-molecular weight (HMW) form of adiponectin,fasting blood glucose,postprandial 2-hour blood glucose,glycosylated hemoglobin,plasma insulin,alanine aminotransferase,aspartate aminotransferase,γ-glutamyltransferase,total bilirubin,direct bilirubin,total cholesterol,triglyceride,low density lipoprotein cholesterol,high density lipoprotein cholesterol,the blood pressure and liver ultrasound test were detected.The insulin resistance (HOMA-IR) was used to assess insulin resistance.Results In pre-versus post-treatment,serum level of HMW adiponectin [(6.8 ± 4.0) μg/L vs.(7.1 ± 3.9) μg/L,P > 0.05 in enteral nutrition],and [(6.8 ± 3.5) μg/L vs.(5.0 ± 1.1)μg/L(P<0.05)] in parenteral nutrition were found.The significantly decreased body mass index in the obese patients (P<0.05),significantly improved values of 2-hour blood glucose,glycated hemoglobin,liver function,triglycerides,low-density lipoprotein cholesterol levels (all P<0.05),and no obvious change in HOMA-IR were found after two months of enteral nutrition treatment.There were no significant changes in indicators mentioned above in TPN group after 2 months of treatment.Conclusions Enteral nutrition therapy can improve the glucose metabolism,lipid metabolism,the non-alcoholic fatty liver disease and body mass index,affect the level of adiponectin in the elderly patients with MS and NAFLD.It is vital to body metabolism.
10.Responsiveness and precision of Gross Motor Function Measure(66 items version)

ObjectiveTo compare the responsiveness and precision between the 66 items version of Gross Motor Function Measure(GMFM-66) and 88 items version(GMFM-88).Methods173 children with cerebral palsy(male 126,female 47) who have received twice assessments were involved.All the second assessment were performed 3.8±4.3 months later.112 children received the third assessment 3.2±2.2 months after the second assessment.The effect size and relative precision between the GMFM-66 and GMFM-88 were analyzed.ResultsThe effect size of GMFM-66 between every two assessments was nearly the same as those of GMFM-88.The precision of GMFM-66 was not lower than that of GMFM-88.ConclusionThe interval-scale GMFM-66 has satisfying responsiveness and precision.It is more useful than GMFM-88 in measuring the treating effect of children with cerebral palsy.
