Journal of Shanghai Jiaotong University(Medical Science) 2000;20(5):439-441,449
Objective To study the relationship among p53 gene mutation, apoptosis and cellploidy as well as to explore the role of mutated p53 gene in tumorigenesis. MethodsA total of 85 speci-mens (20 normal ovarian specimens, 20 ovarian benign tumors, 20 non - metastatic carcinomas and 25metastatic carcinomas) were chosen. p53 gene mutations were detected by PCR- SSCP (single strand con-formation polymorphism). Cell ploidy, apoptosis role of carcinoma cells and cell distribution in each cellstage were detected by flow cytometry. Resultsp53 gene mutation and non- mutation of ovarian carci-noma was 60% and 40% respectively ( P >0.05). The rate of apoptosis of carcinoma cells with mutationof p53 gene (20.25%) was much lower than that without p53 mutation (41.68%, P <0.05). Aneuploidcarcinoma cells had been found in 17/23 carcinoma specimens with p53 gene mutation and 8/22 carcinomaspecimens without p53 mutation ( P < 0.05). Aneuploid carcinoma cells metastasize more easily thandiploid carcinoma cells ( P < 0.05). The rate of apoptosis was not different between aneuploid and diploidpatients. ConclusionMutation of p53 gene decreases the apoptosis rate of ovarian carcinoma and maypromote tumorigenesis. Cell ploidy had a linkage with p53 gene mutation, but not with cell apoptosis.

Objective To investigate the effect of breast milk and some other kinds of infant foods on the adhesion of EPEC to Hela cells and intestinal mucosa so as to understand the possible protective mechanism of breast-feeding in the intestine of infants. Methods The adhesion of EPEC to Hela cells and intestinal mucosa were examined by cell culture, bacterial adherence test, light and electron microscopy in the mediums that contain breast milk, cow's milk and formula milk or contain ecal filtrate of the infants fed by breast milk and formula milk respectively. Results Adhesive index for EPEC to Hela cells was significantly lower in the medium containing breast milk than in control and so did in the fecal medium containing breast fed infants than artificial feeding infants. Conclusion Breast milk may inhibit the adhesion of EPEC to infant intestine.

Objective To explore the mechanisms of individual, social and environmental factors influencing physical activity and the relationships among the factors in patients with type 2 diabetes. Methods A survey was conducted from January to July 2015 among 225 patients with type 2 diabetes using convenience sampling. The questionnaire included 9- item Patient Health Questionnaire, the International Physical Activity Questionnaire,Perceived Social Support Scale, Chinese Walkable Environment Scale, Bandura′s Exercise Self- efficacy Scale. Results The effective questionnaire recovery rate was 98.7%(222/225). Exercise self- efficacy was a mediator of factors influencing physical activity. Exercise self- efficacy and physical activity were positively correlated (β=0.24, P<0.05). Walkable environment only affected physical activity indirectly medicated by exercise self- efficacy (β=-0.26, P<0.05). Perceived social support directly associated with physical activity (β=0.14, P<0.05). Depression not only affected physical activity directly (β=-0.28, P<0.05), but also associated with physical activity indirectly medicated by exercise self-efficacy (β=-0.32, P<0.05). Conclusions Interventions should be taken to increase the perceived social support, improve depression and walkable environment, especially to enhance exercise self- efficacy, so as to improve the physical activity level of patients with type 2 diabetes.

Objective To investigate the relationship between the hemostatic coagulation markers of pr ethrombosis state and pregnancy induced hypertension(PIH). Methods Forty-five PIH patients and 20 control patients were studied. P-selectin, pro thrombin fragments 1+2 (F1+2), D-dimers(D-D) and plasmin-antiplasmin complex (PAP) were measured by enzyme linked immunosorbent assay. Antithrombin activity (AT: A) was measured by chromogenic peptide substrate assay. Results (1) The P-selectin level of pre-delivery in moderate and severe PIH patients w as (66?24)?g/L and (80?30)?g/L, it was (49?15)?g/L in the control group ( both P

Objective To study the methods and skill of vena cava filter placement via the antecubital access. Methods Six patients with DVT (4 males and 2 females, mean age of 62) underwent vena cava filter placement via the antecubital access in Huashan Hospital from Oct. 2004 to May. 2006. The right basilic vein was punctured with the use of micropuncture technique. SNF (Simon nitinol filter, Bard)was inserted through its carrier into the 90-cm-long sheath. The filter was then deployed with a standard fashion in the IVC, 5 cm inferior to the renal vein. Results The filter was once placed successfully in all six patients within average time of 25 min without complications, but with good healing, exclusion of bleeding and no phlebitis. The position of filter was accurate without deviation and no occurrence of pulmonary embolism. Conclusions vena cava filter placement via antecubital access is easy, minimal invasive, no need of lying in bed postoperatively. It is beneficial for DVT patients as an alternative for the contra-indication to femoral venous access.

Objective To analyze the prevalence of peripheral arterial disease (PAD) in type 2 diabetic patients and its association with traditional and non-traditional risk factors. Methods A total of 420 type 2 diabetic patients with duration exceeding 1 year and without acute myocardial infarction, unstable angina, and infection were evaluated. PAD was diagnosed by ankle-brachial index ≤0.90. Risk factors included age, gender, body mass index, smoking, hypertension, diabetes duration, glycated hemoglobin A[C ( HbA_1C ), uric acid, triglyceride, total cholesterol, low density lipoprotein cholesterol (LDL-C), high density lipoprotein cholesterol(HDL-C),high sensitive C reactive protein(hs-CRP),fibrinogen,and urinary albumin excretion rate (UAER). Results The prevalence rate of PAD was 16.4%(69/420). Older age, longer diabetes duration, lower HDL-C, higher UAER, hs-CRP and fibrinogen were associated with PAD. Multiple Logistic regression revealed that only age, UAER, hs-CRP and fibrinogen levels were independent risk factors. The prevalence rate of PAD was 4.7%(2/43), 12.9%(27/209) and 23.8%(40/168) in patients of less than 50 years old,50-70 years old and more than 70 years old, and was 7.9%( 14/177),9.3%( 11/118) and 35.2% (44/125) in different hs-CRP group (hs-CRP < 1 mg/L,l-3 mg/L, >3 mg/L), and was 9.7% (30/310) ,34.4%(33/96) and 42.9%(6/14) in normoalbuminuria, microalbuminuria and macroalbuminuria, and was 12.1%(43/356) and 40.6%(26/64) in different fibrinogen levels group (≤4 g/L and > 4 g/L). Conclusions The prevalence rate of PAD is higher in type 2 diabetic patients. Non-traditional cardiovascular risk factors may be involved in the development of PAD.

Objective To evaluate the result of manipulative reduction and percutaneous cannulated screws fixation for the triplane fracture of the distal tibia in children. Methods From September 2004 to March 2009, 15 cases of triplane fracture of the distal tibia were treated with manipulative reduction and percutaneous cannulated screws fixation. This study included 9 boys with an average age of 13.2 years (range, 12-14 years) and 6 girls with an average age of 12.4 years (range, 11-14 years). Preoperative diagnoses were confirmed by the X-ray films or CT examination. If plain radiographs showed the fracture gap of epiphysis board and articular surface were greater than 2 mm after closed reduction, the operation was needed. Under "C"-arm X-ray machine, anatomical reduction was achieved by manipulative reduction according to the bone fracture type. The fractures were fixed by percutaneous cannulated screw. Postoperative CT scanning confirmed anatomical reduction. All patients had been immobilized with cast for 8 weeks. The ankle joint function and growth were evaluated by modified Weber protocol. Results All patients were followed up for 15 to 68 months with an average of 26 months. There were excellent in 13 cases, good in 2 cases. X-ray films showed all fractures were healed. There were not bone bridge and uneven articular surface, or rotating and shortening deformity. All patients could participate in the normal physical activities. The 2 cases with good outcome were related to weight-bearing too early. The main problem was that pain occurred in ankle joint with excess activity. Conclusion The method of manipulative reduction and percutaneous cannulated screw fixation is relatively reliable treatment for closed triplane fractures of the distal tibia.
